Moderate and Severe Blood Pressure elevation Associated with Stroke in the Mexican Hispanic Population

Background: Stroke is the fourth leading cause of death in US. Amongst other factors such as age, sex, race, genetics, obesity, diabetes etc., hypertension continues to be the leading contributing factor towards stroke. Studies regarding stroke in Hispanics are sparse and inconclusive. Objectives: The objective of the present study is to investigate the potential association between blood pressure elevation and risk of ischemic stroke among the Mexican Hispanic population. Methods: A retrospective data analysis was carried out for a planned case-control study with case-control ratios of 1:2. Mexican Hispanic cases were from the elPasoStroke database with diagnosed hypertension that had sustained an ischemic stroke (n = 505) and Mexican Hispanics diagnosed with hypertension who were stroke-free as controls from the 2005-2010 NHANES databases (n = 1010). In this analysis, we included subjects who had data on systolic, diastolic or mean arterial blood pressures for cases (327) and controls (772). In cases, blood pressure was determined by the initial admission measurement, and in controls, the first measured blood pressure was used. The unadjusted and adjusted effects of continuous measurements of systolic, diastolic and mean arterial blood pressure on stroke were determined using logistic regression analyses. Subjects were further classified into groups based on prehypertension and hypertension ranges, as established by the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure (JNC7). Unadjusted and adjusted logistic regression models were also used to determine the effect of categorized blood pressures. Results: Our data indicate that per unit increase in systolic, diastolic or mean arterial blood pressure elevates the odds of stroke among the Mexican Hispanic population. Characterization and Optimization of Bacteriocin from <i>Lactobacillus plantarum</i>Isolated from Fermented Beef (Shermout)

Many lactic acid bacteria (LAB) were isolated from “Shermout”, a popular Sudanese fermented beef product intended for long storage. An isolate that demonstrated significant antibacterial activity was identified as Lactobacillus plantarum PM4 based on phenotypic, physiological and biochemical characteristics and carbohydrate utilization patterns. The inhibitory activity of the partially purified bacteriocin was completely arrested by the proteolytic enzymes proteinase-k and pepsin but not by α-amylase, asserting its proteinaceous nature. The activity was not due to H2O2 as similar inhibition was obtained by cell-free supernatant (CFS) produced under anaerobic conditions. The bacteriocin showed a molecular weight in the range of 3 - 5 kDa and had a bactericidal mode of action. No significant reduction in activity was observed on heating to 60℃ for 60 min, but activity was lost on heating to 100℃ or autoclaving. Highest inhibitory activity was at pH 5.5 and there was appreciable reduction in activity at pH 3, 7 or 9. There was no drop in activity at –80℃ or –20℃ up to four weeks of storage. However, at 4℃ and 35℃, a gradual decline in activity was observed. Lb. plantarum PM4 exhibited bactericidal activity against Staphylococcus aureus, Bacillus subtilis, Enterococcus faecalis, Escherichia coli ATCC25922, Klebsiella pneumoniae and Proteus vulgaris. Bacteriocin production generally coincided with the phase of maximum growth and the best combination for maximum production of inhibitory activity was at pH 5.5 for 48 h whether incubated at 25℃, 30℃ or 37℃. Lb. plantarum PM4 showed promise as a starter culture in the fermentation of preserved meat products.

Epidemiological Approach to the Use of Psychotropic Medications at Moulay el Hassan Hospital, in the Gharb Region of Morocco

The use of psychotropic drugs is important in Morocco related to the large amount of sufferers by psychiatric disorders, according to a survey of the Moroccan Ministry of Health (2010). A prevalence of 26.5% of depressive disorders, and 9% for generalized anxiety disorder, and 5.6% of psychotic disorders was recorded within population, hence the importance to describe the use and assess the prevalence of psychotropic and socio-economic factors was associated with their use. Our research is a retrospective study, carried out between February 2010-December 2013, holding a sample of 5618 patients aged between 1 and 90 years old. The study is concerned with drawing social and clinical data from patients’ records. The different classes of psychotropic drugs prescribed to patients in the psychiatric department Moulay el Hassan Hospital, vary between antidepressants (36%), antipsychotics (32%), anxiolytics (10%), anti-epileptics (9%), the class of diverse “sulpiride” (7%), anti-parkinsonian (6%), and hypnotics were prescribed in rare cases. The relationship between socio-economic factors and the take of a psychotropic treatment was very strong in Morocco. In contrast, the patients support showed by close families and the health care staff remain a challenge for Moroccan population.

Liver Segmentation Technique for CT Images Based on Statistical Analysis

This article presented the automatic diagnosis of liver pathologies and its 3D volume rendering. The first and fundamental step in all these studies is the automatic liver segmentation, which is still an open problem. In this thesis, two automatic methods are described to segment the liver from abdominal CT image data. The first step is image enhancement. Secondly, texture analysis is using standard statistical measures, finally it is a rough mask to segment the liver.

AIM:To investigate the clinicopathological features and the significance of different prognostic factors which predict surgical overall survival in patients with gastric ***:This retrospective study includes 80 patients diagnosed and treated at gastroenterology surgical center,Mansoura University,Egypt between February 2009 to February *** factors were assessed by cox proportional hazard ***:There were 57 male and 23 *** median age was 57 years(24-83).One,3 and 5 years survival rates were 71%,69% and 46% *** median survival was 69.96 *** the follow-up period,13 patients died(16%).Hospital morbidity was reported in 10 patients(12.5%).The median number of lymph nodes removed was 22(4-41).Lymph node(LN) involvement was found in 91% of *** R0 resection,depth of wall invasion,LN involvement and the number(> 15) of retrieved LN,LN ratio and tumor differentiation predict *** multivariable analysis,tumor differentiation,curability of resection and a number of resected LN superior to 15 were found to beindependent prognostic ***:Surgery remains the cornerstone of *** differentiation,curability of resection and a number of resected LN superior to 15 were found to be independent prognostic *** LN dissection does not increase the morbidity or mortality rate but markedly improves long term survival.

Day case paediatric microlaryngobronchosocopy:A prospective study

AIM To assess the feasibility and parental acceptance of diagnostic microlaryngobronchoscopy(MLB) as day case *** A prospective study was performed over a 26 moperiod at a tertiary paediatric ENT centre. Patients were selected in clinic using set criteria. All MLBs were performed using a standardised anaesthetic protocol and patients monitored post-operatively. Six weeks following surgery, parents underwent questionnaire *** Ninety-four out of 101 MLBs was successfully performed as day case surgery over the set period. Seven patients required an overnight stay for further observation. Fiftyseven parents took part in the questionnaire of which 68.4% were highly satisfied with same day discharge. CONCLUSION MLB is feasible, safe and acceptable as day case surgery in carefully selected patients.

The Expression of miRNAs in Sudanese Patients with Systemic Lupus Erythematosus in Khartoum State

Background: MicroRNAs (miRs) are noncoding gene regulators that may have a role as diagnostic or prognostic biomarkers in systemic lupus erythematosus (SLE). Aim: To measure the blood levels of miR-146a, miR-126 and miR-30a in Sudanese SLE patients and to investigate their potential role in disease pathogenesis and utility as biomarkers for SLE. Material and Methods: A total of 48 SLE patients and 20 matched healthy individuals were enrolled in this study. SLE disease activity index (SLEDAI) was assessed. The blood levels of miR-146a, miR-126 and miR-30a were determined by Real-time polymerase chain reaction (PCR) in all participants. Γ-INF and IL-2 were analyzed by elISA, and CD markers were used in flow cytometry. Results: The mean age of the patients was 31.5 ± 8.5 years with disease duration > 5 years. In SLE patients, the mean blood level fold changes of miR-146a (0.33 ± 0.277;P 0.305) compared to controls. Down regulation of miR-146a increase expression of γ-INF (P ely, miR-30a was insignificantly associated with SLE disease (P value > 0.05) as no differences between the SLE patients and healthy control. Conclusion: Circulating miR-146a and miR-126 could be a potential noninvasive biomarker in SLE. This study provides an overview of the current state of research on the role of miRNAs in the immune pathogenesis and regulation of SLE. Further studies are needed in miRNAs profiling expressions of SLE diseases.

Objective:To investigate the chemical composition,antioxidant and antimicrobial activities of essential oil of Santolina chamaecyparissus *** in ***:The chemical composition of hydrodistilled essential oil from flowering aerial parts has been analyzed by gas chromatograph and gas chromatograph-mass spectrometer *** antioxidant activities conducted by 2,2-diphenyl-1-picrylhydrazyl scavenging test andβ-carotene bleaching *** antimicrobial activity was realised by agar disc diffusion method and cornell medical index was determined by agar dilution method in solid ***:About 36 components accounting more than 82%of the total oil were *** monoterpenes was the main fraction(54.66%)and was represented by artemisia ketone(40.33%)as major component of this *** other major constituents were(Z)-thujone(9.82%),(2Z,6E)-farnesol(7.30%)and limonene(6.87%)and concerning antiradical scavenging test(2,2-diphenyl-1-picrylhydrazyl)had demonstrated a weak activity obtained with an EC_(50) of(43.01±8.04)and moderate activity for β-carotene bleaching test(47.00±3.13)at 120 *** the other hand,this oil was found effective against all tested strains except Pseudomonas aeruginosa and Aspergillus *** activity was ranging from 10 mm to 24 mm with the lowest minimum inhibitory concentration value between under 0.070μg/mL to upper 9μg/***:The results provided evidence that the studied plant might indeed be potential sources of natural antioxidant and antimicrobial agents.

当前,分数槽永磁同步电机,尤其是多相电机,因为宽弱磁调速范围和出色的容错能力,在牵引领域中得到越来越多的关注.本文主要确定一台5相分数槽双层绕组永磁同步电机的线圈节距因数.提出的确定方法由解析计算和数值计算相结合,从电枢磁动势的空间分布出发,考虑由正弦电流供电的表贴式永磁电机.通过对电枢磁动势谐波成分的分析,获得总体谐波含量与线圈节距因数之间的关系.之后,给出了齿槽转矩的解析表达式,探究齿槽转矩和线圈节距因数之间的联系.数值计算部分主要针对电磁转矩和除切向力以外的电磁力和线圈节距之间的关系,利用有限元进行辅助分析.数值计算和有限元计算的结果得到了线圈节距因数选择的一些关键要素,之后,基于这些关键要素,用有限元进行了验证,证明了方法的可靠性.
