COVID-19 Patients with Hepatic Complications During the Third Wave of Pandemic in Egypt

COVID-19 is a respiratory infection that has lately begun to affect other vital organs,including the heart,kidney,and *** purpose of this study was to investigate the hepatic complications in COVID-19 patients and the risk of being admitted to ICU or facing ***:Comorbidities(hypertension and diabetes),COVID-19 symptoms,laboratory findings(ALT level,AST level,and albumin),complications during hospitalization,treatment protocol used,and survival outcomes were all studied in 200 COVID-19 infected Egyptian patients who had virological symptoms and were followed up until they recovered or *** was found that older people and those with higher blood glucose levels have a higher risk of developing liver-associated COVID-19 ***,the majority of patients who developed liver complications in the course of the infection had high mortality *** with diabetes,hypertension,or hepatic disease are at higher risk of ICU admission or ***,it is important to pay attention to these problems in the diagnosis and treatment of COVID-19 to develop a suitable individualized treatment *** was also a correlation between the mortality in COVID-19 patients and both,high blood glucose and liver enzyme *** can be attributed to the correlation between diabetes and liver disease as every disease may be a complication to the other;moreover,COVID-19 may lead to increased blood sugar levels in addition to ALT and AST *** theory is that COVID-19 may afiect the liver and hence people with chronic liver disease.

Risk factors for liver fibrosis among human immunodeficiency virus monoinfected patients using the FIB4 index in Morocco

AIM: To study the prevalence and risk factors of significant hepatic fibrosis in Moroccan human immunodeficiency virus(HIV) monoinfected ***: We conducted a cross-sectional study among HIV monoinfected patients(negative for hepatitis B surface antigen and hepatitis C antibody). Clinical and laboratory data were collected from the data base of the Infectious Diseases Unit in Ibn Rochd Hospital Center [age, gender, duration of HIV infection, CD4 T lymphocyte count, HIV viral load, glycemia and current or prior use of antiretroviral and antiretroviral therapy(ART) duration]. The primary outcome was a FIB4score > 1.45. Multivariable logistic regression identifiedindependent risk factors for FIB4 > ***: A FIB4 score > 1.45 was identified in 96among 619(15.5%). HIV monoinfected patients followed up between September 1990 and September2012. Multivariate analysis showed that only a viral load > 75(OR = 2.23, 95%CI: 1.36-3.67), CD4 > 200cells/mm3(OR = 0.39, 95%CI: 0.21-0.72) and age at FIB4 index calculation(OR = 1.10, 95%CI: 1.07-1.13)were independently associated with the occurrence of FIB4 index(> 1.45). Gender, duration of HIV infection,glycemia, use of antiretroviral therapy and ART duration were not associated with significant fibrosis by ***: FIB4 score > 1.45 was found in 15.5%of Moroccan HIV monoinfected patients. Age, HIV viremia > 75 copies/mL and CD4 count > 200 cells/mm3are associated with liver fibrosis. Further studies are needed to explore mechanisms for fibrosis in HIV monoinfected patients.

Evaluation of Geological Hazards for Landuse Planning in Nabq Protectorate, Southeastern Sinai Using GIS Techniques

Nabq protectorate is one of wonderful natural places in Egypt. It is characterized by diversity of bio-lives such as mangrove forests, coral colonies, wild life plants and migratory birds. Ongoing growth of tourism industries at Sharm el Sheikh northward into the Nabq protectorate causes severe hazards on its natural resources. The aim of the present study is to assess the present geo-environmental hazards in the Nabq protectorate. Assessment includes the analysis of satellite images, topographical, geological and other ancillary geological data using GIS technology. GIS data analyses indicate that the area is under threat from some of geo-hazards. Rough topography and mass wasting with high probability of flash flooding threaten different constructions in this area. The mobilization of coastal sand dunes, wave action and tidal currents are natural impacts on Nabq ecosystems, where moved dunes leave clay soils that are removed in some places by tropical storms increasing sea water turbidity that threaten the corals and other living organisms in the tidal flat region. The seismic activity hazard in the study area is usually active on lineaments extending parallel to the trend of the Gulf of Aqaba-Dead Sea transform fault where the Nabq protectorate occupies its southern segment. Unwise planning activities destroy the natural environmental resources in Nabq area by construction of new resorts on mangrove forests, coral colonies and raised beaches. Hazard assessment identifies land suitability and land use maps that are clearly exhibit models of traditional dams and buffer strips on coastal zone and highways as well as around the Bedouin communities which are worked on tourism and fishing. These maps are urgent in need of an assessment and rehabilitation program to mitigate geo-hazard.

Altered Levels of Blood Glucose and Serum Lipids in Sudanese Patients with Ovarian Cancer

Background: The etiology of ovarian cancer is not well-understood;numerous metabolomics profiling, epidemiological, and hospital-based case control studies have associated abnormal levels of blood glucose and serum lipids with the risk and the prognosis of various types of cancers including ovarian cancer. The association between the risk of the incidence of ovarian cancer and the alterations in the levels of blood glucose and serum lipids is not well defined. Objective: In this study we aimed to compare the levels of blood glucose, triacylglycerols, low-density lipoprotein cholesterol, and high-density lipoprotein cholesterol in patients with different stages of ovarian cancer and healthy controls to determine how they relate to the risk and prognosis of ovarian cancer. Methodology: In a case-control cross sectional study, we enrolled ninety-nine Sudanese women, diagnosed with ovarian cancer but had not received any kind of treatment as the study group, and a control group of forty-one age-matched, apparently healthy women. The patients were classified according to the International Federation of Obstetricians and Gynecologists staging system into two groups: early stages (stage I & II) and late stages (stages III & IV). Blood glucose and serum lipids;triacylglycerols, total cholesterol, low-density lipoprotein cholesterol and high-density lipoprotein cholesterol were determined by enzymatic colorimetric methods using commercially available analytical kits. The IBM SPSS version 20 software was used for statistical analysis. A Mann-Whitney U test was used for comparison of the median concentrations of blood glucose, triacylglycerols, total cholesterol, low-density lipoprotein cholesterol and high-density lipoprotein cholesterol in the study groups. Logistic regression model was used to estimate the relative risk of ovarian cancer in relation to levels of blood glucose and serum lipids. P value of 0.05 was considered significant. Study of Radionuclides and Radon Exhalation Rate in Soil and Sand Samples from Tiba, Luxor, Governorate

In this study, the natural radionuclides in soil and sand have been measured by using high purity germanium (HPGe) detector. While, radon exhalation rate has been measured by Alpha GUARD. The data analysis is performed to determine 226Ra, 232Th, and 40K activity concentrations in addition to 222Rn exhalation rate. The values of radium equivalent activity (Raeq), external hazard index (Hex), internal hazard index (Hin), and absorbed dose rate were ranged from 46.46 to 124.16 Bq⋅kg−1, 0.07 to 0.33 Bq⋅kg−1, 0.09 to 0.42 Bq⋅kg−1, and 13.24 to 58.37 nGy⋅h−1 respectively in all samples. The area and mass exhalation rates were increased from 9.16 ±2.83 to 16.18 ±2.83 Bq⋅m−2⋅h−1 and 1.8 ±1.34 to 11.35 ±0.98 Bq⋅kg−1⋅h−1 respectively.

Dual Band BPF Based on Parallel Coupled Lines Loaded by Open Stubs for Ku-Band Satellite Applications

This paper presents a dual band Band Pass Filter (BPF) operating at both the downlink and uplink frequency bands for Ku-band satellite applications. The commonly used frequency band in mobile communications satellites is the Ku-band. These mobile satellite systems help connect remote regions, vehicles, ships, people and aircraft to other parts of the world and/or other mobile or stationary communications units, in addition to serving as navigation systems. The structure of the proposed filter is based on parallel coupled microstrip lines and four sections are used. Tuning the two operational bands can be achieved using two open-circuited stubs at the first and last sections of the parallel coupled microstrip lines. The proposed filter is adjusted to operate at 12.54 GHz and 14.14 GHz for downlink and uplink bands, respectively. The proposed dual band BPF is fabricated, measured, and good agreement is obtained between simulated and measured results.

Clinical Outcome of Low Dose Contrast during Percutaneous Coronary Intervention in Patients with Moderate to Severe Kidney Impairment

Background: Chronic kidney disease patients are at a greater risk for nephropathy requiring dialysis after percutaneous coronary intervention. Such patients are usually deferred due to fear of “Renalism”.?Objectives This study assesses the outcome of Low dose contrast protocol during PCI in CKD patients whose e-GFR 60 ml/min/1.72 m and investigates a safety margin for contrast use in these high-risk categories.?Methods: Patients were into three groups according to CV/e-GFR ratio: Group (A) low-dose: CV/e-GFR ratio 2.0 Group (B) medium-dose: CV/e-GFR ratio > 2.0 and × bodyweight\***). Group (C) high-dose: CV/e-GFR ratio > MACD. Results: A total of 73 patients were enrolled. Average age was 54 ± 8 years,81.4% were male and 18.6% were females and 52% were diabetic. Mean baseline e-GFR was 40 ± 8.0 ml/min/1.73m2. Contrast Volume used in group A was (58.26 ± 15.05) (n = 24), in group B (109.42 ± 17.11) (n = 26) and in group C (304.5 ± 60.30) (n = 23), respectively. The incidences of CI-AKI in the 3 groups were 0%, 11.5% and 35%, respectively (p = 0.02). All-cause death 0%, 17% and introduction of maintenance hemo dialysis was 0%, 11.5% and 26%, respectively (p Conclusion: Low dose contrast protocol is safe, effective and easily applicable technique without CI-AKI or death.

Reconstruction of Post Burn Neck Contracture Using Full Thickness Skin Graft

Background: Neck contracture after burns is a major complication that affects function and cosmesis. The aim of covering the raw area and defects is through using good quality pliable skin. Full thickness skin graft allows a large dimension sheet of good quality skin with low donor-site morbidity. Also it provides similar skin quality to the recipient areas with much less cosmetic difference. Methods: Four men and eight women underwent neck contracture release and reconstruction from December of 2015 to August of 2016. Mean patient age was 29 years (range from 12 to 46 years). Burn scar contracture releases were performed and cervicoplasty was added for optimal neck appearance. Uniformly full thickness skin grafts were applied. Both lateral ends of these grafts (release incisions) were designed with a fishtail shape for sufficient release and to minimize linear scar band formation in the most lateral region of the neck. Results: Full thickness skin grafts as large as 24 ± 12 cm (in length) and 10 - 15 cm (in width) were used. All grafts were taken without significant complications. Range of neck motion increased, and the cervico-mental angle was regained in all patients. A highly natural neck contour was universally obtained without a secondary debulking procedure. Conclusions: Full thickness skin grafts for treatment of post burn neck contraction give good functional and cosmetic results. They give similar color match and good skin quality, also help in regaining of cervico-mental angle.

Evaluation of the Microbiological Quality of Different Moroccan Drinking Water in the Beni Mellal Region

Drinking water is water that does not contain pathogens or chemicals at levels that may be harmful to health. Beni Mellal region, one of the major rich areas in ground and surface water in Morocco (the rivers like Oum Errabia, Darna, ...;Bin el-Ouidane and martyr ahmed el Hansali dams and sources like Ain Asserdoune, …) is located in the North-Centre of Morocco. The main objective of the present study was to evaluate the microbiological quality of different drinking waters originated from sources, faucets, fountains and wells in the Beni Mellal city and its agricultural surrounding areas. For this, hygienic quality indicator parameters of water quality were determined by assessing the analysis of Total Coliforms Count, Fecal Coliforms Count, streptococcus D and revivable germs while germs such as Anaerobic Sulphite-reducing Bacteria, S. aureus and Salmonella sp, were carried out according to national and international standards. The analysis of the microbiological results found for all the different samples waters investigated in this study revealed that the waters originated from fountains, faucets and sources were proper for main consumption at microbiological point of view while the wells waters were improper for consumption according to norms. Finally, it appears, from the results obtained, that the potential risk of infection of water consumers especially (groundwater) calls for prompt intervention in order to alleviate the potential health impact of water-borne diseases in the community. From this, a proper sanitary survey and implementation of water and sanitation projects in the community is recommended.
