Techniques for colorectal anastomosis

Colorectal anastomotic leak remains one of the most feared post-operative complications, particularly after anterior resection of the rectum with, the shift from abdomino-peritoneal resections to total mesorectal excision and primary anastomosis. The literature fails to demonstrate superiority of stapled over hand-sewn techniques in colorectal anastomosis, regardless of the level of anastomosis, although a high stricture rate was noted in the former technique. Thus, improvements in safety aspects of anastomosis and alternatives to handsewn and stapled techniques are being sought. Here, we review alternative anastomotic techniques used to fashion bowel anastomosis. Compression anastomosis using compression anastomotic clips, endoluminal compression anastomotic rings, AKA-2, biofragmental anastomotic rings, or Magnamosis all involve the concept of creating a sutureless end-to-end anastomosis by compressing two bowel ends together, leading to a simultaneous necrosis and healing process that joins the two lumens. Staple line reinforcement is a new approach that reduce the drawbacks of staplers used in colorectal practice, i.e. leakage, bleeding, misfi ring, and inadequate tissue approximation. Various nonabsorbable, semi or fully absorbable materials are now available. Two other techniques can provide alternative anastomotic support to the suture line: a colorectal drain and a polyester stent, which can be utilized in ultra-low rectal excision and can negate the formation of a defunctioning stoma. Doxycycline coated sutures have been used to overcome the post-operative weakness in anastomosis secondary to rapid matrix degradation mediated by matrix metalloproteinase. Another novel technique, the electric welding system, showed promising results in construction of a safe, neat, smooth sutureless bowel anastomosis. Controlling of Fusarium Wilt of Cucumber by Antagonistic Bacteria

Studying about the effect of four pseudomonas and two serratia isolates on growth of Fusarium oxysporum showed that, Pseudomonasfluorescens No.2 ＆ No.3 and Serratia marcescens No.2 gave highest inhibition zones which were 37.33, 35,00 and 31.33 mm, respectively. Evaluating about the effect ofpseudomonas and serratia culture filtrates at three concentrations （10, 25, 50%） on the linear growth and spore germination ofFusarium oxysporum revealed that, all filtrates of the tested isolates reduced the mycelial growth and spore germination of F. oxysporum. All filtrates of the tested isolates at 50% concentration completely inhabited spore germination ofF. oxysporurn. Culture filtrates of Pseudomonasputida and Serratia marcensens No.2 at 50% concentration reduced the mycelial growth of F. oxysporurn by 80.74 and 80.37% respectively. In this respect, all tested isolates made lysis to mycelial of F. oxysporum except Pseudornonasfluorescens No.3. Effect of treating cucumber seeds with cell suspension of pseudomonas and serratia isolates on incidence of Fusarium wilt disease revealed that all isolates were effective in reducing disease incidence and disease severity and increasing the percentage of healthy plants compared to the control. Pseudornonasfluorescens No.3 and Serratia marcensens No.2 were the best isolates and completely prevented the disease incidence.

Effect of Zirconium Oxide Nano-Fillers Addition on the Flexural Strength, Fracture Toughness, and Hardness of Heat-Polymerized Acrylic Resin

Purpose: The mechanical strength of polymethyl methacrylate (PMMA) remains far from ideal for maintaining the longevity of denture. The purpose of this study was to evaluate the effect of Zirconium oxide (ZrO2) nanofillers powder with different concentration (1.5%, 3%, 5% and 7%) on the flexural strength, fracture toughness, and hardness of heat-polymerized acrylic resin. Materials and methods: Zirconium oxide powders with different concentrations (1.5%, 3%, 5% and 7%) were incorporated into heat-cure acrylic resin (PMMA) and processed with optimal condition (2.5:1 Powder/monomer ratio, conventional packing method and water bath curing for 2 hours at 95。C) to fabricate test specimens of PMMA of dimensions (50 × 30 × 30 mm) for the flexural strength, fracture toughness, and (50 × 30 × 30 mm) were fabricated for measuring hardness. PMMA without additives was prepared as a test control. Three types of mechanical tests;flexural strength, fracture toughness and hardness were carried out on the samples. The recorded values of flexural strength in (MPa), fracture toughness in (MPa.m1/2), and hardness (VHN) were collected, tabulated and statistically analyzed. One way analysis of variance (ANOVA) and Tukey’s tests were used for testing the significance between the means of tested groups which are statistically significant when the P value ≤ 0.05. Results: Addition of Zirconium oxide nanofillers to PMMA significantly increased the flexural strength, fracture toughness and hardness. Conclusion: These results indicate that Zirconium oxide nanofillers added to PMMA has a potential as a reliable denture base material with increased flexural strength, fracture toughness, and hardness. According to the results of the present study, the best mechanical properties were achieved by adding 7%wt ZrO2 concentration.

Analysis of Surface Roughness Using Laser Optical Imaging Techniques

Speckle phenomena were produced by using interference of scattered laser beams from certain rough object. Digital speckle images were recorded for different rougher and smoother surfaces, using an optical imaging system in two and three dimensions with a high resolution CCD camera. The obtained speckle images were transformed to equivalent binary images. The values of surface roughness depend on the degree of agglomeration of the speckle images. The optical density was calculated and it was found that it depends on the different conditions for the optical imaging system. The back projection technique was used to reconstruct 3-dimensional surface roughness profiles from multi-directional projection data. Also, interference microscope was used for the reconstruction of surface topography for different rough surfaces.

Effect of Nanoparticles Reinforced Adhesive Layers on Microleakage of Tooth Restorations

Adhesive layer is an essential part of tooth colored restorations which play an important role in decreasing the microleakage between the tooth and restoration material after polymerization shrinkage. The purpose of this study was to evaluate the effect of deferent adhesive system of bonding agent on microleakage of nanocomposite resin in class II cavities. Two different types of adhesive systems: universal adhesive (ExciTE) and a newly developed adhesive (Nano-Bond) and one type of light-cured resin restorative material (nanocomposite resin) were used in this study. These adhesives were applied to prepared tooth cavities by either manufactures’ instructions or by an experimental method (single or double application). Nanocomposite resin was then placed and light-cured for 40 seconds. Teeth were subjected to 500 thermal cycles between 5°C and 55°C and were immersed in 3% methylene blue solution for 24 hours;microleakage was observed microscopically. The data were analyzed by a two-way ANOVA. For comparison between groups, Tukey’s post-hoc test was used. Nanoparticles reinforced adhesive system shows low microleakage in compare with universal adhesive system, and application of two adhesive layers also can decrease the microleakage. The ability of stress absorption by adhesive layer after polymerization shrinkage of restoration material will reduce the microleakage.

First Cycle Neutrophil Count and Subsequent Chemotherapy Dose Delivery in Breast Cancer Patients Receiving Adjuvant Chemotherapy

Background: Chemotherapy-induced neutropenia is a major dose-limiting toxicity of systemic cancer chemotherapy, and it is associated with substantial morbidity, mortality, and costs. The ability to predict the group of patients who are at high risk for neutropenic complications might help to reduce the morbidity due to febrile neutropenia and increase the likelihood of delivering full chemotherapy dose on time. In addition, hematopoietic growth factors could be administered to patients who need them and allow more efficient use of medical resources. Methods: The study included 120 female patients with breast cancer, treated in adjuvant setting with FAC chemotherapy regimen. Chemotherapy was delayed if the ANC was less than 1500 cells/μL on the planned first day of the cycle. Dose delays would be for 1 week. The chemotherapy dose was reduced if the patient experienced an episode of febrile neutropenia. Dose reductions usually consist of a 20% reduction in dosage of all chemotherapy drugs. Results: Out of the 120 patients in the study, 58 patients experienced neutropenic complications (febrile neutropenia or dose delayed) accounting for 48.3% of total sample size. In univariate analysis, there was statistically significant association between first cycle ANC nadir and neutropenic complication (p = 0.0001). There was also statistically significant association between pretreatment first cycle ANC (p = 0.001), pretreatment platelets count (p = 0.054), first cycle platelets count nadir (p = 0.041), number of involved lymph nodes (p = 0.031) and the occurrence of varies neutropenic complications. However, in multivariate analysis, ANC nadir was only the independent risk factor for occurrence of neutropenic. Conclusions: The first cycle absolute neutrophil counts nadir provides useful information on the risk of future neutropenia, febrile neutropenia and dose delay or reduction with estimated sensitivity and specificity, which were 64.5% and 77.6%, respectively. Effect of Titanium Dioxide Nano Particles Incorporation on Mechanical and Physical Properties on Two Different Types of Acrylic Resin Denture Base

The aim of this study is to clarify the effect of different concentrations of titanium dioxide nanoparticles (Nps) on the properties of two types of heat polymerized acrylic resin. The tested parameters were flexural strength, impact strength, and microhardness. The two types of acrylic resin used in this study were conventional unmodified (Implacryl, Vertex) and high impact heat polymerized acrylic resin (Vertex-Dental, Netherlands). Both types of acrylic resin were modified by using 1% and 5% TiO2 Nps powder. Specimen’s dimensions were prepared according to the American Dental Association Specification No. 12. Three types of specimens were prepared: 1) flexural strength specimens 50 mm × 10 (±0.2) mm × 3 (±0.2) mm, 2) impact strength test specimens 60 mm × 6.0 mm × 4.0 mm, 3) microhardnesss specimens 25 mm × 10 mm × 3 (±0.2) mm. For each test 6 groups were prepared (each group containing 5 samples). Thirty specimens were prepared in each of the three tests, amounting to a total number of 90 specimens. Mechanical properties such as flexural strength (FS), impact strength and microhardness of the above mentioned specimens were determined using universal testing machine, Izod pendulum impact testing machine and Vickers microhardness tester, respectively. ISO Specification No. 1567 was followed in microhardness test. The data was collected and statistically analyzed. Flexural strength considerably decreased by increasing TiO2 concentration in both types of acrylic resin. Impact strength of the conventional acrylic resin modified by 1% of additives significantly increased. The microhardness is significantly increased by addition of 5% of TiO2 Nps. The Incorporation of TiO2 nanoparticles into acrylic resins can adversely affect its flexural strength. Meanwhile, the impact strength can be modified by small percentage of additives (abt. 1%). A Retrospective Evaluation of Chemotherapy Regimens in Unselected Patients with Metastatic Non-Small Cell Lung Cancer

Background: Randomized clinical trials have demonstrated the benefits of chemotherapy in carefully selected non-small cell lung cancer (NSCLC) patients. How generalizable these results are to other NSCLC patients is unresolved. Methods: The outcomes of patients treated with standard chemotherapy regimens (paclitaxel / carboplatin;gemcitabine / carboplatin;pemetrexed / carboplatin;paclitaxel / carboplatin / bevacizumab) off study as first line therapy between 2002 and 2012 at our institution were compared to the reported results of trials supporting the FDA approval of these drugs and/or regimens. Results: In our population, 38.1% of the patients had hypertension, 11.9% of the patients were diabetic, 23.7% had chronic obstructive pulmonary disease (COPD), 11.9% had coronary artery disease (CAD) and 2.1% had renal or liver disease. Notably, the presence of a single or multiple comorbidities was associated with low overall survival compared to matched patients with no comorbidities (p = 0.007). Conclusion: The presence of single or multiple comorbidities is associated with inferior overall survival compared to matched groups without such pre-existing conditions.

Production of Citric Acid from Corncobs with Its Biological Evaluation

Corncobs could serve as a substrate for citric acid production using solid state fermentation technique. The culture optimization concerning substrate concentration, culture duration, pH, temperature and substrate hydrolysis was carried out for maximum productivity of citric acid. Under the optimized conditions, 48.4 g of citric acid was produced from 1 kg dry corncobs. Biological evaluation was carried out for citric acid such as melanin synthesis inhibitory, anti-allergy, anti-bacterial, and hyaluronic acid production activities. The results showed that citric acid has potent melanin inhibitory activity, good inhibition for β-hexosaminidase release and potent stimulatory effect for the production of hyaluronic acid. These activities (melanin synthesis inhibitory, anti-allergy and hyaluronic acid productive activities) of citric acid have been reported for the first time.
