Effect of alternative antibiotics in treatment of cefotaxime resistant spontaneous bacterial peritonitis

AIM:To evaluate effective alternative antibiotics in treatment of cefotaxime-resistant spontaneous bacterial ***:One hundred cirrhotic patients with spontaneous bacterial peritonitis [ascitic fluid polymorphonuclear cell count(PMNLs) ≥ 250 cells/mm 3 at admission] were empirically treated with cefotaxime sodium 2 g/12 h and volume expansion by intravenous human *** patients were subjected to history taking,complete examination,laboratory tests(including a complete blood cell count,prothrombin time,biochemical tests of liver and kidney function,and fresh urine sediment),chest X-ray,a diagnostic abdominal paracentesis,and the sample subjected to total and differential cell count,chemical examination,aerobic and anaerobic *** were divided after 2 d by a second ascitic PMNL count into group Ⅰ;patients sensitive to cefotaxime(n = 81),group Ⅱ(n = 19);cases resistant to cefotaxime(less than 25% decrease in ascitic PMNL count).Patients of group Ⅱ were randomly assigned into meropenem(n = 11) or levofloxacin(n = 8) *** patients performed an end of treatment ascitic PMNL *** were considered improved when:PMNLs decreased to < 250 cells/mm 3,no growth in previously positive culture cases,and improved clinical manifestations with at least 5 d of antibiotic ***:Age,sex,and Child classes showed no significant difference between group Ⅰ and group Ⅱ.Fever and abdominal pain were the most frequent manifestations and were reported in 82.7% and 80.2% of patients in group Ⅰ and in 94.7% and 84.2% of patients in group Ⅱ,*** in group Ⅱ had a more severe ascitic inflammatory response than group Ⅰ and this was demonstrated by more ascitic lactate dehydrogenase(LDH) [median:540 IU/L(range:150-1200 IU/L) vs median:240 IU/L(range:180-500 IU/L),P = 0.000] and PMNL [median:15 000 cell/mm 3(range:957-23 822 cell/mm 3) vs 3400 cell/mm 3(range:695-26 400 cell/mm 3),P = 0.000] *** fluid culture was po

Comparison Study between Extracorporeal and Intracorporeal Anastomosis for Laparoscopic Right Hemicolectomy

Background: Nowadays, laparoscopic colectomy is considered a safe and effective surgical technique regarding short- and long-term outcomes, as well as specific oncologic outcomes. The anastomosis can be created intra- or extracorporeally. The goal of our study was to evaluate and compare short term outcomes of extra- and intra-corporeal anastomosis after laparoscopic right hemi colectomies. Aim of the Study: The goal of our study was to evaluate and compare short term outcomes of extra- and intra-corporeal anastomosis after laparoscopic right hemi colectomies. Methods: In the period from December 2014 to January 2019, all patients underwent laparoscopic right hemicolectomy for cancer colon who presented to surgical oncology department—south Egypt cancer institute and general surgery department—Assiut University was analyzed. Data like age, sex, body mass index (BMI), operative technique, operative times, blood loss, intra- and post-operative complications, pathology and hospital stay were reported and analyzed. Results: Twenty three (69.7%) patients underwent extracorporeal anastomosis while intracorporeal anastomosis was performed in ten (30.3%) patients. There was no significant difference in patient characteristics and demographic data in both groups (P > 0.05). There was no statistically significant difference in operative (operative time, blood loss or length of hospital stay) and postoperative (ileus, anastomotic leak, wound infection, incisional hernia, readmission, reoperation or deaths) details in the 2 groups except in length of the incision which was significantly shorter in the IA group (5.500 ± 1.269) vs. (6.565 ± 1.308) for EA (P = 0.015). Conclusion: No significant difference in short term outcomes of laparoscopic-assisted and total laparoscopic right colectomy. Intracorporeal anastomosis had shorter incision which may decrease wound-related complications.

Clinical Outcome of Early Enteral Feeding on Patients Post Esophagectomy

Objectives: Aim of our work is to study the effect of early enteral feeding through either nasoentral or feeding jejunostomy tube post esophagectomy on patients recovery and hospital stay. Background: Postoperative nutrition is a well known aspect of care in recent years and has been shown to decrease the incidence of complications and hospital stay. Enteral nutrition has been shown to be superior to parenteral nutrition as it is more physiological, safer, cheaper and early enteral nutrition has been clearly confirmed to reduce postoperative morbidity. Methods: This is randomized combined retrospective and prospective study that is conducted in surgical oncology department, South Egypt cancer institute, Assiut University;from October 2012 to October 2016. Patients in this study were divided into two groups: group 1 includes patients with feeding jejunostomy and group 2 is patients with nasoenteral tube. Results: 25 Patients included in this study (19 males & 6 females). All cases were primarily diagnosed as esophageal cancer, middle and lower 1/3 esophagus or proximal gastric carcinoma infiltrating cardia by clinical data associated with abdominal sonar and/or C.T scan and upper endoscopy with biopsy. There was no significant difference in catheter related complications (P value 0.238). There was no operative mortality. Conclusion: Early postoperative enteral nutrition was feasible and safe for patients undergoing esophagectomy. There is no significant difference between NE and FJ. Enteral nutrition either through nasoenteral or feeding jejunostomy is an effective method for postoperative nutritional support in this type of major surgery.

Assessment of Remotely Sensed Digital Elevation Models (DEMs) Compared with DGPS Elevation Data and Its Influence on Topographic Attributes

Accuracy and quality of DEM are of a great interest for a wide range of applications. In this study, quality of ASTER GDEM and SRTM DEMs were assessed in comparison with DGPS measurements. Impact of DEM resolution upon the accuracy of terrain representation and topographic attributes was also discussed. The study deduced that vertical error has a strong effect on error propagation and this highly obvious in higher elevations as the absolute standard error (SE) ranges between is ±0 - 2.5 and ±0 - 2.4 m for ASTER GDEM and SRTM respectively. This is reflected on slope and aspect as the vertical errors increase and uncertainty is relatively high in flat and low areas. Error propagation in low lands influenced drainage extraction and resulted in isolated and truncated water courses.

Primary Debulking Surgery for Stage III Epithelial Ovarian Cancer Has a Better Outcome Than Neoadjuvant Chemotherapy Followed by Interval Debulking Surgery

Background:?Ovarian cancer (OC) is the most lethal gynecologic malignancy. About 70% of ovarian cancer patients have advanced disease and often not totally resectable. Previous studies of neoadjuvant chemotherapy (NACT) followed by interval debulking surgery (IDS) versus primary debulking surgery (PDS) give inconsistent results. The aim of this retrospective study is to evaluate the outcome of the neoadjuvant chemotherapy followed by IDS versus PDS followed by adjuvant chemotherapy for the International Federation of Gynecology and Obstetrics (FIGO) stage III epithelial ovarian cancer.?Methods:?This study was performed on eligible patients with ovarian cancer admitted in Surgical Oncology and Medical Oncology departments at South Egypt Cancer Institute-Assiut University in the period from January 2010 to December 2015. Patients were divided into two groups, the first group included those who have NACT and underwent IDS plus adjuvant chemotherapy, and the second group included those who underwent PDS followed by adjuvant chemotherapy.?Results:?This study included 380 cases. One hundred and fifty-four patients (40.53%) had IDS. The remaining two hundred and twenty-six patients (59.47%) underwent PDS. In this study, treatment modality was significant for both disease-free survival (DFS) and overall survival (OS). DFS was significantly reduced after IDS when compared to PDS (median DFS: 33.00 months vs. 45.00 months, respectively;p??0.001). Also, OS was significantly reduced after IDS when compared to PDS. (Median OS: 43 months vs. 46 months, respectively;p = 0.047). Moreover, this drop of the survivals mainly occurred in specific subgroups such as the elderly patients, patients with bad performance status, suboptimal cytoreduction, as well as high-grade tumors. Microspherules-Bearing White Sandstone: Implication of Cosmic Impact Event near Jurassic-Cretaceous Boundary in West Central Sinai, Egypt

The presence of glass microspherules enclosing relict grains, shattered quartz and silicon carbide in white sandstone beds near the Jurassic-Cretaceous boundary in west central Sinai indicates a cosmic impact event. Characterization of the impact microspherules and proposing a reasonable scenario for their origin are the aims of this work. Field observations, optical, binocular, scanning electron and high-resolution transmitted electron microscopy investigations and chemical analyses were carried out. The study revealed that glass microspherules have high Al2O3 and FeO contents and low CaO and MgO contents. The high content of Al2O3 indicates that the source of microtektite-like microspherules is attributed to the melting of a clay-rich sandstone and carbonaceous matter, while the high content of FeO indicates admixing with projectile matter. The reaction between silica and carbon was carried out under conditions of high temperature (T > 1000°C) and carbon (C/Si > 1) which resulted in the production of silicon carbide with microdiamond intergrowth. Consequently, this intergrowth is in accordance with the impact origin via rapid condensation and growth within a vapor phase. In spite of the fact that no source crater has been recognized to date in the study area, the authors propose at least a single cosmic impact event scenario for the recorded glass microspherules in west central Sinai. The impact excavated the Paleozoic siliciclastic sedimentary rocks and then the glass microspherules showered the area of study. The deposition of microtektite-like glass particles within the white sandstone beds of the Malha Formation took place in the fluvial plain terrestrial environment. This setting precluded severe post-depositional reworking, yielding preservation of the glass particles in a primary layer. Optimum Timing and Complication of Completion Thyroidectomy for Differentiated Thyroid Cancer

Background: Despite improved preoperative diagnostics, incidental postoperative detection of differentiated thyroid cancer in the final histology is still common. In most of these cases, completion thyroidectomy is recommended by national and international guidelines, although secondary surgery is associated with an increased operative risk. The optimal timing of completion thyroidectomy is still controversial. Patients and Methods: The patients admitted to surgical oncology department, SECI, with diagnosis of differentiated thyroid cancer;during the period from January 2008 to December 2015;were rewired for age, sex, type of 1st operation, histopathological result, type of 2nd operation and time interval between the 2 operation, complication of 2nd operation and morbidity. 118 patients underwent completion thyroidectomy;those patients were divided according to timing of completion operation into 3 groups: Group A is from one week to 3 months and include 64 patients;Group B is from 3 - 6 months and include 30 patients;Group C is more than 6 months and include 24 patients. Clinical complications and oncologic outcomes were analyzed. The mean follow-up was 80 ± 10 months. Result: we record 118 patients under completion thyroidectomy. Ages range from 79 to 13 years. Papillary thyroid cancer were 96 and follicular thyroid cancer were 22. The overall rates of transient and persistent postoperative hypocalcemia were 19.5% and 4.2%, respectively. The rates of persistent hypocalcemia were found in group A and B but not in group C. Transient or persistent vocal cord paresis was observed in 9 (7.6%) and 3 patients (2.5%). The incidence of persistent vocal cord paresis (VCP) was significantly higher in groups A and B than in group C. There was no significant difference regarding survival among the 3 groups;however recurrence is higher in group A. Conclusion: Considering perioperative morbidity and oncologic outcomes, completion thyroidectomy should be performed at least 3 to 6

New Look on Ureterocolic Diversion with Adjuvant Radiotherapy;SECI (South Egypt Cancer Institute) Experience

Background: Ureterosigmoidostomy was the first form of continent urinary diversion and had broad popularity during the first half of the 20th century. Primary enthusiasm was followed by disappointment when serious problems such as pyelonephritis, electrolyte imbalance, renal calculi and renal function deterioration become evident. In the 1950s these life threatening complications led to avoid this urinary diversion in favor of uretro-intestinal conduits. Objectives: Our aim was to evaluate our experience in ureterocolic shunt after radical cystectomy for bladder cancer. Methodology: This study was conducted in surgical oncology department, South Egypt cancer institute, Assiut university;in the period from January 2012 to January 2017 and including 115 patients (75 males & 40 females). Results: This is retrospective study including 115 patients (75 males & 40 females). Operative time was 130 minutes ranging between 2 to 2.5 hours. Average blood loss was 450 ml. After removal of rectal tube all patients were continent except 15 patients who were improved after 1 - 3 days. Within a month 12 patients had night incontinence and 5 patients had day time incontinence. After that day and night continence gradually improved in all patients except two females who had persistent night soiling and was in need for night time rectal tube. Conclusion: Ureterosigmoidostomy regains the interest of surgical oncology and urologist because of its simplicity and absence of appliance as many patients refusing cutaneous stoma and others are not suitable for orthotopic substitutes.

A new series of dimeric Cu（II） and Ni（II） complexes with some aroylhydrazones of a-pyridoin were synthesized and characterized using different physical techniques. Their chemical formulae were based on their microanalysis and IR data. The structures of the solid complexes were determined from the electronic, IR and ESR spectral studies as well as their magnetic susceptibility measurements. The ligands acted as bi-, tri- and tetra-dentate forming different dinuclear complexes with different structures. The assumed molecular structures based on the experimental results were also confirmed by the molecular mechanics calculations. The extraction ability of the hydrazones has been investigated by liquid-liquid extraction for Cu（II） and Ni（II）.

Programmed death-1/programmed death-L1 signaling pathway and its blockade in hepatitis C virus immunotherapy

Chronic hepatitis C virus(HCV) infection is a public health issue that often progresses to life-threatening complications, including liver cirrhosis, fibrosis, and hepatocellular carcinoma. Impaired immune responses to HCV are key features of chronic HCV infection. Therefore, intervention strategies usually involve enhancing the immune responses against HCV. Cytotoxic CD8+ T lymphocytes(CTLs) play a critical role in the control of HCV infection. However, their cytolytic function can be impaired by the expression of co-inhibitory molecules. Programmed death-1(PD-1) receptor and its ligand PD-L1 function in a T cell co-inhibitory pathway, which either blocks the function of CTLs or the differentiation of CD8+ T cells. During chronic HCV infection, the immune inhibitory receptor PD-1 is upregulated on dysfunctional HCV-specific CD8+ T cells. As such, blockade of the PD-1/PD-L1 pathway in these CD8+ T cells might restore their functional capabilities. Indeed, clinical trials using therapies to block this pathway have shown promise in the fostering of anti-HCV immunity. Understanding how chronic HCV infection induces upregulation of PD-1 on HCV specific T cells and how the PD-1/PD-L1 interaction develops HCV specific T cell dysfunction will accelerate the development of an efficacious prophylactic and therapeutic vaccination against chronic HCV infections, which will significantly improve HCV treatments and patient survival. In this review, we discuss the relationship between PD-1 expression and clinical responses and the potential use of PD-1 blockade for anti-HCV therapy.
