Impact of time to surgery from injury on postoperative infection and deep vein thrombosis in periprosthetic knee fractures

Purpose:Periprosthetic fracture (PPF) is a serious complication that occurs in 0.3%-2.5% of all total knee arthroplasties used to treat end-stage arthritis. To our knowledge, there are no studies in the literature that evaluate the association between time to surgery after PPF and early postoperative infections or deep vein thrombosis (DVT). This study tests our hypothesis that delayed time to surgery increases rates of postoperative infection and DVT after PPF ***:Our study cohort included patients undergoing PPF surgery in the American College of Surgeons National Surgical Quality Improvement Program database (2006-2015). The patients were dichotomized based on time to surgery: group 1 with time ≤2 days and group 2 with time >2 days. A 2-by-2 contingency table and Fisher's exact test were used to evaluate the association between complications and time to surgery groups, and multivariate logistic regression was used to adjust for demographics and known risk ***:A total of 263 patients (80% females) with a mean age of 73.9 ± 12.0 years were identified receiving PPF surgery, among which 216 patients were in group 1 and 47 patients in group 2. Complications in group 1 included 3 (1.4%) superficial infections (SI), 1 (0.5%) organ space infection (OSI), 1 (0.5%) wound dehiscence (WD), and 4 (1.9%) deep vein thrombosis (DVT);while complications in group 2 included 1 (2.1%) SI, 1 (2.1%) OSI, 1 (2.1%) DVT, and no WD. No significant difference was detected in postoperative complications between the two groups. However, patients in group 2 were more likely (p = 0.0013) to receive blood transfusions (57.5%) than those in group 1 (32.4%).Conclusion:Our study indicates patients with delayed time to surgery have higher chance to receive blood transfusions, but no significant difference in postoperative complications (SI, OSI, WD, or DVT) between the two groups.

塔吊是高层建筑施工现场的主要设备之一。在恶劣的工作条件,交付时间的压力和影响塔吊正常操作的其他因素下,各种荷载在高度上提升和运输。作为高层施工的核心设备,塔吊在施工过程带来了巨大的优势,但同时塔吊影响施工现场的安全具备过大的风险。因此,此论文的第一阶段研究影响塔吊正常操作的安全因素并且分析塔吊安全因素对发生事故的影响率;此研究的第二阶段旨在采用定量方法来确定这些因素的相对权重。本论文的第一阶段研究旨在确定影响塔吊在高层建筑施工现存正常操作的安全因素。此外,此研究对各因素发生事故的影响程度进行了评估,并把得到的结果以图形形式呈现了。在进行研究的第一阶段所使用的研究方法基于全面文献综述,调查卷以及跟来自三个不同公司的专家团队的访谈,这些专家在进行访谈时候从事安全总监,安全经理,已经项目经理,并且对塔吊有一定的了解。本论文的第二阶段旨在采用定量方法评估以及权重这些安全因素。首先,需要使用一种可以把从专家获取的知识转为数字的方法。因此,通过访谈,文献综述以及调查卷,把层次分析法确定为适当的多属性决策工具。确定方法之后,要求专家对已确定的因素进行评估。专家评估完之后,使用了层次分析法对每个因素进行了定量分析,并提出了结果。结果实现,在影响塔吊的主要因素当中,操作员的熟练程度排名为第一,并且此结果表明操作员数量程度的主要性。对第二阶段所采用的研究方法为全面的文献综述为了确定适当的定量方法,专家访谈为了能够得出相应的结果。此外,这些因素可用于进一步研究,以减轻由于塔吊在高层建筑工地上操作而带来的风险。

缺硼(B)以及B毒是大多数园艺产品降低作物生产力和产量损失的主要因素。柑橘经常生长于B缺乏和过量的地域,但有关B缺乏和过量对柑橘生长、根形态、抗氧化酶活性以及柑橘叶片气体交换变化的影响的报道很少。B缺乏和过量对柑橘植物的机制了解尚不明了,对科学家来说仍然是一个亟待解决的问题。目前的工作目的是研究缺B和过量B胁迫下植物生长、根形态、叶气交换和抗氧化酶的影响。并通过电子显微镜(TEM)观察缺B处理对植物细胞(叶绿素、类胡萝卜素)的影响,以及缺B对不同形态B及对K、Cu、Zn、Fe等离子吸收情况的影响。主要进行水培实验,设置5个B水平,分别为缺B (B0),足够的B (B20, B10, B40)和过量B (B100)。主要结果如下:1、缺B和过量B影响了柑橘植物的根系以及地上部的生长。同时也显示根系颜色变化、根尖增大、侧根增大、根叉减少、根长减少、根系减少以及表面积明显减少等可见症状。缺B通过抑制根系营养液吸收而引起根系生长,而过量B可刺激新根的产生,但抑制其进一步发育。在B缺乏时,主要的可见症状出现在根部,而B过量则为老叶叶片边缘呈烧焦状。2、此外,抗氧化酶过氧化物酶(POD),过氧化氢酶(CAT)和超氧化物歧化酶(SOD)和抗坏血酸过氧化物酶(APX)在缺B条件下降低,并且在B过量时有也有一定程度的降低,但是在B20处理时显著升高。B缺乏时MDA浓度降低,且随着B浓度升高MDA含量显着升高。此外,缺B和高B浓度处理均使得叶片气体交换,光合速率,气孔导度,蒸腾速率和细胞间CO2明显降低,且以上所有特征参数在B20时处达到最高值。3、当B浓度增加时,游离态B和半束缚态B增加。此外,B处理影响K和Zn的吸收,相比于正常B处理,缺B和过量B显著降低K和Zn的吸。Fe和Cu的吸收不受B浓度的影响。在缺B和过量B处理植物中,Fe和Cu吸收增加。4、TEM研究表明,与其他处理相比,缺B处理细胞壁中层薄弱、叶绿体数量严重影响,且显著降低叶绿素和类胡萝卜素含量,而B20处理细胞胞间薄层和叶绿体数量正常,叶绿素和类胡萝卜素显着增加。在B过量处理中,细胞胞间薄层加厚,叶绿素和类胡萝卜素含量也显著降低。

Low Ca^2＋ Content and Ca^2＋/Na^＋ Ratio in Leaf Tissues Determine Salinity Tolerance in Spanish Type Groundnut （Arachis hypogeal L.）

The study was performed with seven groundnut varieties/genotypes and F1s derived from crossing in all possible combinations without reciprocal among the mentioned varieties/genotypes. The objective was to assess whether low Ca2＋ content and Ca2＋/Na＋ ratio of leaf tissue or stem tissue determine salinity tolerance in terms of economic yield （kernel yield） in groundnut. It revealed that the varieties, “Binachinabadam-6”, “Binachinabadam-5” and the F1 G2 × G3 were most tolerant based on kernel yield under 8 dS/m and 10 dS/m salinity stresses. These two tolerant varieties and the F1 also showed lower Ca2＋ and Ca2＋/Na＋ ratios in leaf tissue, which indicated lower Ca2＋ and Ca2＋/Na＋ ratio of leaf tissue determined salinity tolerance in terms of kernel yield in Spanish type groundnut. These findings could be applied in future plant breeding applications for screening salt tolerant Spanish type groundnut genotypes.

Synthesis, spectral characterization and biological activities of Organotin(IV) complexes with <i>ortho</i>-vanillin-2-hydrazinopyridine (VHP)

Five new organotin(IV) complexes of ortho-vanillin-2-hydrazinopyridine hydrazone with formula [RnSnCl4–n(VHP)] [R = Me2, n = 2 (2);R = Ph2, n = 2 (3);R= nBu2, n = 2 (4);R = nBu, n = 2 (5) and R = 1, n = 0 (6)] have been synthesized by direct reaction of ortho-vanillin-2-hydrazinopyridine hydrazone [(VHP), (1)], base and organotin(IV) chloride(s) in absolute methanol. The hydrazone ligand [(VHP), (1)] and its organotin(IV) complexes (2-6) have been characterized by UV-Visible, FT-IR and 1H NMR spectral studies. Spectroscopic data suggested that in the complexes (2-4), the ligand (1) acted as a neutral bidentate ligand and is coordinated to the tin(IV) atom via the azomethine nitrogen and pyridyl nitrogen atoms, whereas the ligand (1) acted as a uninegative tridentate ligand and coordinated to the tin(IV) atom through phenolic-O, azomethine-N and pyridyl-N atoms in complexes (5-6). The toxicity of the ligand (1) and its organotin (IV) complexes (2-6) were determined against Artemia salina. Organotin(IV) complexes showed moderate activity against Artemia salina. The ligand (1) and its organotin(IV) complexes (2-6) were also tested against four types of bacteria namely Bacillus cereus, Staphylococcus aureus, Escherichia coli and Enterobacter aerogenes. All organotin(IV) complexes and the free ligand (1) showed better antibacterial activities against bacteria. Among the organotin(IV) complexes (2-6), diphenyltin(IV) complex (3) showed higher activity against the four types of bacteria.

The case studies are about atypical bilateral, postoperative Cystoid Macular Edema (CME) in two patients who underwent bilateral uncomplicated pha- coemulsification surgery with a trifocal Intraocular Lens (IOL). In postoperative follow-up, both the cases presented with complaints of bilateral blurring of vision. Irvine-Gass syndrome has been diagnosed after evaluation of the retina by fundoscopy and OCT macula. Posterior subtenon steroid injection has been prescribed and given. Regular OCT examinations indicated substantial ede- ma improvement. Subsequent improvement in visual acuity was also noted in both cases. Cystoid macular edema can happen in any case irrespective of the quality of IOL. The visual outcome is satisfactorily responsive towards steroid medication.

最近,印尼和中国的外交关系在复牌后变得越来越近了。在1990年两国家发出了“两国外交关系的恢复公报”这驱使他们在许多方面有着良好的合作。签署联合协议后,印尼-中国在政治,经济,军事等方面建立了伙伴关系。两国关系自1980年以来有所改善。中国外长钱其琛会见了总统印尼苏哈托和国务部长Moerdiono于1989年,就两国外交关系的恢复问题进行了讨论。于1989年12月,两国举行了会谈关于两国恢复关系的技术问题,并签署了会议纪要。双方发表了和解协议于印尼对中国的债务义务并在两国外交关系的恢复公报。1990年7月,印度尼西亚外交部长阿里·阿拉塔斯应邀访问了中国,双方提出了印尼对华债务的协议并对两国恢复外交关系发出了公报。苏西洛·班邦·尤多约诺总统在位期间(SBY)即2004-2009和2009-2014两个时期,印尼和中国之间的关系日益蓬勃发展。2005年4月25日中国国家主席胡锦涛访问印度尼西亚并与印尼签署“战略伙伴关系”协议一事标志着两国外交关系达到顶峰。战略伙伴关系联合宣言发布后2005年4月在雅加达,2010-2015印度尼西亚——中华人民共和国正在加强的签署战略伙伴关系行动计划开始实施,中印尼战略成为未来的“全面战略伙伴关系”。

The effect of melatonin in functional heartburn: A randomized, placebo-controlled clinical trial

Background: Melatonin is a safe and effective treatment for patients with functional pain and gastrointestinal disorders. Objective: To evaluate the efficacy and safety of melatonin for 3 months in patients with functional heartburn. Methods: In the randomized, placebo-controlled pilot study, patients with functional heartburn were treated with omeprazole 20 mg before breakfast and randomized to receive either melatonin 6 mg (n = 20), nortripty line 25 mg (n = 20), or placebo (n = 20) at bedtime for 3 months. Gastroesophageal Reflux Disease-Health-Related Quality of Life (GERD-HRQOL) scores were assessed at baseline and at the end of the study. Results: Melatonin improved GERD-HRQOL scores compared with nortriptyline (P = 0.0015) and placebo (P < 0.0001). Fewer adverse events were reported by patients receiving melatonin compared with those receiving nortriptyline or placebo. Conclusions: Melatonin was a safe and effective treatment for functional heartburn.
