Backgr.und: Infor.ation on the long-ter. per.usion status of patients after.successful sur.er. for.giant r.tinal tear.(Gr.) macula-off r.egmatogenous r.tinal detachment (r.D) is limited. Pur.ose: To examine long-ter. str.ctur.l, functional, and per.usion outcomes in nor.al contr.l eyes and eyes tr.ated for.differ.nt degr.es of Gr.-associated extensions of r.D. Methods: One emmetr.pic nor.al eye (contr.l), one healthy highly myopic eye (contr.l myopic), and thr.e eyes sur.ically tr.ated for.Gr. (sur.ical), wer. included in the study for.a long-ter. compar.son of study outcomes. The sur.ical eyes wer. classified based on the degr.e of Gr.-associated r.D extension as follows: one eye with Gr.-associated r.D extension ˚one eye with Gr.-associated r.D extension between 180˚- 270˚and one eye with Gr.-associated r.D extension > 270˚. Str.ctur.l, functional, and per.usion outcomes wer. compar.d with those of the contr.l eyes. r.sults: All thr.e eyes wer. phakic and the condition was monocular. The mean age of the patients was 48.67 ± 8.50 year. (r.nge, 39 - 55 year.). All thr.e eyes had Gr. macula-off r.D. The mean pr.oper.tive time for.Gr. sur.er. was 1.2 weeks. The mean pr.- and postoper.tive best cor.ected visual acuities (BCVA) wer. 1.87 logMAr.and 0.46 logMAr. r.spectively. The mean postoper.tive follow-up per.od was 19.67 ± 5.69 months. Pr.lifer.tive vitr.or.tinopathy r.sulted in multiple sur.er.es in one eye (31.5%). Long-ter. postoper.tive optical coher.nce tomogr.phy (OCT) showed abnor.al r.tinal thickness, ellipsoid zone disr.ption, and exter.al limiting membr.ne line discontinuities in one eye. OCT angiogr.phy yielded abnor.al per.usion indices in the sur.ically tr.ated eyes. Conclusions: Our.data showed multiple str.ctur.l alter.tions in spectr.l-domain OCT biomar.er.. One eye that developed secondar. epir.tinal membr.ne (Er.) pr.lifer.tion showed a significantly impr.ved BCVA after.pr.lifer.tion and inter.al limiting membr.ne wer. r.moved. P
