Yolk Sac Tumor of the Ovary in 18 Egyptian Cases: Does It Really Differ?

Background: Ovarian Yolk sac tumor (OYST) is a rare entity of malignant ovarian germ cell tumors (MOGCT). Abdominal pain, a rapidly growing distending mass or irregular vaginal bleeding is the main presentation. Serum AFP is elevated in nearly all cases. The standard management is fertility preserving surgery with adjuvant chemotherapy. Aim of Work: To report and analyze retrospectively recorded cases that were either treated at National Cancer Institute/Egypt or referred there for advice about therapy. Materials and Methods: This is a retrospective single-institutional analysis of 18 cases of OYST treated at National Cancer Institute-Cairo University from January 2011 till December 2015. The clinical and pathological characteristics, treatment, and outcomes of these patients were analyzed. Results: Data from eighteen patients were obtained. The median age was 18 years (range: 15 - 22). Abdominal pain was the most common presentation (89%). The mean tumor size was 21cm (range: 8 - 30 cm). Eleven of our cases (61%) were stage I, seven cases and (39%) were stage IV at presentation. Fifteen cases (83%) underwent fertility preserving procedure & the standard surgical staging. Panhysterectomy & formal staging procedure was done only in two cases (11%). One case (6%) underwent bilateral salpingo-oophorectomy. 2 cases (11.1%) only underwent lymph node biopsy. 11 patient (61.1%) showed pure type YST while mixed type was present in the remaining 7 cases (38.8%): Dysgerminoma (one case, 5.6%), Dysgerminoma + immature teratoma (one case, 5.6%), Immature teratoma (2 cases, 11.1%) and Teratoma (3 cases, 16.7%). AFP was extremely elevated in all cases at presentation (median 4191 ng/mL;ranging: 725 ng/mL - 402,908 ng/mL). It showed decreased level after surgery (median 145 ng/ mL;ranging: 2 ng/mL - 38,000 ng/mL) & normalized after chemotherapy except for progressive disease. The Impacts of Building Regulations on the Thermal Performance and Energy Consumption of Residential Buildings in Riyadh City-Saudi Arabia

Riyadh city is the fastest growing city in Saudi Arabia. The rapid urban growth that happened recently in Riyadh was not based on the traditional urban planning principles, which have been established and applied for the city development process. The imported building regulations have created a new urban structures and street patterns. The contemporary urban form in Riyadh city is based mainly on traffic and economic consideration with the neglect of environmental dimensions. This research aims to examine the impacts of building regulations on the thermal performance of residential buildings in Riyadh city, with the ultimate goal of establishing planning guidelines that consider the environmental conditions of the city. The methodology adopted for achieving the aim of this study consists of two phases. First, the literature related to building regulations development in Riyadh, as of 2018, was reviewed. Second, buildings energy simulation was conducted to examine the thermal performance of the typical current status of residential building blocks in Riyadh city, and then several changes to building regulations were made to investigate their impacts on the thermal performance of buildings. The results showed that the impacts of Riyadh building regulations on the thermal performance of residential buildings differ across the evaluated cases. The ratio of building height to street width, urban block street orientation, and building orientation are the main factors affecting thermal performance of buildings within urban block. The study also concludes that adjusting the ratio of building height to the distance between buildings could have a significant impact in reducing cooling loads. This study will help policy makers, planners and designers to investigate the shortcoming in the current building regulations.

Simulation of Dual-Band MWCNT Ink-Based Spiral Antenna: A Comparative Study

In this paper, we compare a dual-band, square spiral microstrip patch antenna constructed from Multi-Walled Carbon Nanotubes (MWCNT) ink for wearable application simulated by Computer Simulation Technology Microwave Studio (CST MWS) by our work simulated by Advanced Design System (ADS) electromagnetic simulator using the same material characterization. The reflection coefficient is –12 dB at 1.2276 GHz for MWCNT and –13 dB at 1.25 GHz for the copper simulated by CST MWS and reflection coefficient is –12.235 dB at 1.234 GHz for MWCNT and –18.36 dB at 1.243 GHz for the copper simulated by ADS and the reflection coefficient is –27dB at 2.47 GHz for MWCNT and –13 dB at 2.53 GHz for the copper simulated by CST MWS and the reflection coefficient is –26.08 dB at 2.48 GHz for MWCNT and –17.031 dB at 2.47 GHz for the copper simulated by ADS. We show the meandering of the surface current on the radiating in spiral patch. The antenna gain is found to be –12.5 dBi at 1.22 GHz for MWCNT and is found –12.05 dBi at 1.25 GHz at CST MWS and the antenna gain is found to be –11.85 dBi at 1.235 GHz for MWCNT and is found –12.25 dBi at 1.243 GHz at ADS and the antenna gain is found to be –4.25 dBi at 2.47 GHz for MWCNT and is found –4.01 dBi at 2.53 GHz at CST MWS and the antenna gain is found to be –4.23 dBi at 2.47 GHz for MWCNT and is found –4.88 dBi at 2.45 GHz at ADS. We show a close agreement in the results obtained by the two simulation software's CST MWS and ADS. The results are given for both MWCNT and Copper characterizations.

Acute Correction of Varus Knee by Biplanar Medial Opening-Wedge High Tibial Osteotomy and Fixation with TomoFix Plate

Background: Biplanar medial opening wedge high tibial osteotomy (MOWHTO) is a treatment method that allows adequate correction of symptomatic varus knee deformity. However, MOWHTO tends to decrease patellar height and increase the posterior tibial slope, which can affect the knee joint stability. Objective: The aim of this study was to describe the technique of biplanar MOWHTO and fixation by TomoFix plate, as well as to evaluate the accuracy of the planned correction and the postoperative tibial slope. Patients and Methods: This prospective cohort study was conducted on patients who presented with varus knee deformity and underwent biplanar MOWHTO and fixation by TomoFix plate during the period from March 2016 to March 2017. Assessment of patients included pre- and postoperative Knee and function scores, mechanical femorotibial angle (mFTA), posterior tibial slope angle (pTSA), range of motion (ROM) and radiological evaluation of the healing of the osteotomy site. Results: The recruited patients were 13 (8 men and 5 women), with average age 31.7 years old. The knee and functional scores improved from the preoperative mean of 45 and 41 to the postoperative mean of 85 and 72 points, respectively. The average knee flexion was 115 degrees, which at the final follow-up remained unchanged except for one case. The mean preoperative mFTA was 13.5°varus and decreased post-operatively to a mean of 3°valgus. The average postoperative follow-up period was 12 months (10 - 18 M). Conclusion: The biplanar MOWHTO allows preservation of posterior tibial slope while correcting the varus knee adequately.

Phytochemical and biological activities of Crataegus sinaica growing in Egypt

Objective:To evaluate the cardiac activity and hepatoprotection of Crataegus sinaica (***).Methods:All the isolated compounds were isolated by open-column liquid chromatography(CC) using sephadex LH-20 as stationary *** of the column was performed with EtOH or *** phytochemical investigation of the young stem of *** for the first time together with the leaves and flowers lead to the isolation and identification of quercetin,hyperoside,vitexin-2"-0-rhamnoside,epicatechin,procyanidin B2 and procyanidins ***:Rats treated with the low and high dose of *** leaves with flowers extract showed 15%and 17%reduction in the heart rate,and reduction in the STsegment by 107%and 57%;*** T-amplitude was decreased by 59%of the high dose *** the other hand,the young steins and leaves with flowers extracts of *** on primary culture of rat hepatocytes monolayer indicated a hepatoprotection for the total extract,ethyl acetate,butanol,and chlor of orm fractions at 100μg/mL,75μg/mL,50μg/mL,and 25μg/mL; ***:The results of these chemical and biological studies suggest the use of *** growing in Egypt as a preventive drug against cardiovascular and hepatic diseases. It has been well recognized that interactions between the gut microbiota and host-metabolism have a proven effect on *** gut lumen is known for harboring different bacterial *** by-products and structural components,which are derived through the gut microbiota,generate a signaling response to maintain *** microbiota is not only involved in metabolic disorders,but also participates in the regulation of reproductive hormonal *** phyla,which are localized in the gut,allow for the metabolization of steroid hormones through the stimulation of different *** hormones such as progesterone,estrogen and testosterone play a pivotal role in the successful completion of reproductive *** in this mechanism may lead to reproductive *** bacteria can affect the metabolism,and degrade steroid hor-mones and their relevant *** behavior of the bacteria can safely be implemented to eliminate steroidal compounds from a polluted *** this review,we summarize the meta-bolism of steroid hormones on the regulation of gut microbiota and vice-versa,and also examined the significant influence this process has on various events of reproductive ***,the evidence suggests that steroid hormones and gut microbiota exert a central role in the modification of host bacterial action and impact the reproductive efficiency of animals and humans.

CT-Guided Lumbar Facet Joint Infiltration: Accessibility, Accuracy and Functional Outcome

Background: Pain generated from lumbar facet joint affection is considered a common cause of low back pain. Image-guided facet joint infiltration is performed to reduce pain severity and to confirm its source. Aim: The objective of this study is to assess the accessibility, and accuracy and to evaluate the functional outcome of the CT-guided lumbar facet joint infiltration in management of low back pain. Subjects and Methods: This retrospective study included thirty four patients. All patients were diagnosed with low back pain due to lumbar facet syndrome. Adequate conservative therapy failed to improve the patient’s symptoms. Totally, 81 lumbar facet joints were treated by CT-guided intra-articular infiltration. Mean time of hospital stay was 6 - 8 hours. In the procedure technique, measures were applied to reduce the patients’ radiation exposure. The response to treatment was evaluated by the visual analogue scale (VAS) before procedure and at follow-up visits. Results: Among 34 adult patients included in this study, 26 were males and 8 were females. The mean age was 49.5 ± 8.5 years. Mean Duration of low back pain on admission was 8.2 ± 3.5 months. Bilateral CT-guided intra-articular infiltration was performed in 23 patients (67.5%). Assessing the response after facet joint infiltration, 82.4% of the patients showed immediate pain improvement after the procedure;85.3% of the patients reported pain relief after 1 month and 67.6% at 12 month follow up. There was a statistically significant relief of pain after the intervention at 12 month follow up (p Conclusion: Lumbar Facet joint infiltration guided with CT scanning seems to be a reliable and safe procedure for low back pain management. Beside immediate and long term pain relief achieved using this minimally invasive technique;CT guidance provides an accessible and accurate route for the needle with low radiation dose exposure.

Three-dimensional seismic attenuation tomography of Egypt

The main purpose of the paper is assessing the three-dimensional (3-D) seismic tomography beneath Egyptto reveal the laws of the tectonic activity, dynamic features of the crust and the upper mantle as well as the thermal structure. Thecoda wave attenuation (Q-1c ) was obtained using the single scattering theory for the central frequencies of interest laid between 1 and 24 Hz. A regionalization of the estimated Q-1c values was performed by means of a generalized inversion *** obtained spatial distribution of 3-D attenuation results revealshigh contrasts between East and West Egypt. A remarkable contrast in the attenuation levels was compared with the tectonic structures, geothermal gradient and heat flow features. The highest attenuations are concentrated in the east and north western offshore regionsat central frequency 1.5 Hzthat draw a good matching with the seismic andthermal features of Egypt. Smaller attenuation levels were detected with young sediments of the Nile Valley from South to the northern triangle of Nile Delta basin except seismic active areas. Low or normal attenuation was detected at western desert where there is a stable and simple shelf. We can conclude that the extended highest attenuation joins to the strong seismic sources and geothermal structures at lower frequency and the centralized high attenuation takes place at moderate seismic sources at a higher frequency. The 3D attenuation maps can draw not only tectonic and geothermal structures but also the general geologic structure map.

Virtues of Using a Double Lumen Tube for Anesthesia in Patients with Endobronchial Tumours Requiring Open Lung Resection or Bronchoplastic Procedures

Objective The benefits and risks of using double lumen tube (DLT) in open thoracotomy are not well studied and the relative contraindication for using it in cases of endobronchial tumours is not proven. In this study, we compared our experience with using DLT versus single lumen tube (SLT) for anesthesia in patients requiring an open thoracotomy for resection of endobronchial tumours. Methods A prospective observational study was performed in a single tertiary care university hospital in patients with endobronchial tumours anesthetized with single and double lumen tubes for open thoracotomy procedures over a period from 2010 till 2018. Results One hundred and six patients with endobronchial tumours were studied. There were 76 males. Median age was 32 years (14 - 62). In 96 cases, endobronchial pathology was a typical carcinoid. 58 patients were anesthetized using a DLT and 48 using a SLT. Four cases of near miss from potentially fatal intraoperative tumour migration occurred in the SLT group (p = 0.025). There was only one case of mild tumour bleeding from the DLT group and time of insertion was longer (16.2 vs. 4.5 min p Conclusions We conclude that it is safe to place a double lumen endo tracheal tube for patients with endobronchial tumours requiring open lobectomies or bronchoplastic procedures. An additional benefit of DLT use is increasing surgical satisfaction by reducing spillage and tidal volume loss during surgical anastomosis of the open airway.
