Expression of connexin 30 and connexin 32 in hippocampus of rat during epileptogenesis in a kindling model of epilepsy

Objective Understanding the molecular and cellular mechanisms underlying epileptogenesis yields new in- sights into potential therapies that may ultimately prevent epilepsy. Gap junctions （GJs） create direct intercellular conduits between adjacent cells and are formed by hexameric protein subunits called connexins （Cxs）. Changes in the expression of Cxs affect GJ communication and thereby could modulate the dissemination of electrical discharges. The hippocampus is one of the main regions involved in epileptogenesis and has a wide network of GJs between different cell types where Cx30 is expressed in astrocytes and Cx32 exists in neurons and oligodendrocytes. In the present study, we evaluated the changes of Cx30 and Cx32 expression in rat hippocampus during kindling epileptogenesis. Methods Rats were stereotaxically implanted with stimulating and recording electrodes in the basolateral amygdala, which was electrically stimulated once daily at afterdischarge threshold. Expression of Cx30 and Cx32, at both the mRNA and protein levels, was measured in the hippocampus at the beginning, in the middle （after acquisition of focal seizures）, and at the end （after establishment of generalized seizures） of the kindling process, by real-time PCR and Western blot. Results Cx30 mRNA expression was upregulated at the beginning of kindling and after acquisition of focal seizures. Then it was downregulated when the animals acquired generalized seizures. Overexpression of Cx30 mRNA at the start of kindling was consistent with the respective initial protein increase. Thereafter, no change was found in protein abundance during kindling. Regarding Cx32, mRNA expression decreased after acquisition of generalized seizures and no other significant change was detected in mRNA and protein abundance during kindling. Conclusion We speculate that Cx32 GJ communication in the hip- pocampus does not contribute to kindling epileptogenesis. Granulomatous lobular mastitis secondary to Mycobacterium fortuitum

Granulomatous lobular mastitis is a rare inflammatory disease of the breast of unknown etiology. Most present as breast masses in women of child-bearing age. A 29-year-old female presented with a swollen, firm and tender right breast, initially misdiagnosed as mastitis. Core needle biopsy revealed findings consistent with granulomatous lobular mastitis, and cultures were all negative for an infectious etiology. She was started on steroid therapy to which she initially responded well. A few weeks later she deteriorated and was found to have multiple breast abscesses. She underwent operative drainage and cultures grew Mycobacterium fortuitum. Granulomatous lobular mastitis is a rare inflammatory disease of the breast. The definitive diagnose entails a biopsy. Other causes of chronic or granulomatous mastitis should be ruled out, including atypical or rare bacteria such as Mycobacterium fortuitum. This is the first reported case of granulomatous mastitis secondary to Mycobacterium fortuitum. With pathologic confirmation of granulomatous mastitis, an infectious etiology must be ruled out. Atypical bacteria such as Mycobacterium fortuitum may not readily grow on cultures, as with our case. Medical management is appropriate, with surgical excision reserved for refractory cases or for drainage of abscesses.

Mental health among Iranian combat veterans with ankle-foot neuromusculoskeletal injuries

Background:Veterans with purely physical disorders,such as ankle-foot neuromusculoskeletal disorders,are often neglected in psychological assessments because mental health evaluations are usually focused on those with a psychological disturbance or with a high percentage of injury.The purpose of this study was to evaluate the psychological condition of veterans with ankle-foot neuromusculoskeletal disorders.Methods:A cross-sectional study was performed between 2014 and 2016 on veterans with war-related ankle-foot injuries living in two provinces of Iran.An information form for demographic data and injury-related factors was used.Additionally,the previously validated Persian version of the Symptom Checklist-90-Revision(SCL-90-R)questionnaire was used for data collection.Results:The respondents were 215 male veterans with a mean age of 51.7±7.5 years.The most common mental health problems were observed for the somatization(24.7%),obsessions-compulsions(14.4%),and anxiety(12.6%).Based on the Global Severity Index(GSI),48.6%of individuals had a possible psychiatric/psychological illness.According to the multivariate regression analysis,GSI scores were significantly higher among veterans who were older than 27 years at the time of injury(P=0.005),had an associated injury(P=0.002),and had a history of hospitalization within the past 12 months for reasons other than their injury(P=0.035).Conclusion:Approximately half of the combat veterans with ankle-foot neuromusculoskeletal disorders likely had psychological problems.The evaluation of the patterns and predicting factors of psychological conditions may inform strategic planning efforts and decision-making,which,in turn,may provide a better quality of life for veterans.Further studies that utilize longitudinal designs are needed to evaluate and compare the psychological status of different groups of veterans and other groups in the general population.

Gypsum-bonded particleboard manufactured from agricultural based material

Gypsum-bonded particleboard(GBPB) panels were made from various mixtures of particles of bagasse(Saccharum officinarum L.) and wheat straw(Triticum aestivum L.), bonded with different ratios of particle/gypsum. This study examined the feasibility of bagasse and wheat straw particles in the production of GBPB. One-layer experimental GBPBs with a density of 1.05 or 1.20 g·cm 3were manufactured at different ratios of bagasse/wheat straw, i.e., 100%/0%, 93.75%/6.25%, 87.5%/12.5%, 75%/25%, 50%/50%, 25%/75% and 0%/100% using two particle/ gypsum composite ratios, i.e., 1/2.75 and 1/3.25 by weight. Thickness swelling(TS), water absorption(WA), modulus of rupture(MOR), modulus of elasticity(MOE) and internal bond strength(IB) properties of the boards were evaluated and a statistical analysis was performed in order to examine the possible feasibility of these agricultural residues for use in commercial GBPB manufacturing. We determined that WA of panels decreases as the amount of straw increases to 100% and the LR/G(wood/gypsum) ratio decreases to 1/3.25, whereas the TS of panels decreases as the proportion of straw decreases to 0% and the LR/G ratio increases to 1/2.75. The experimental results also show that the MOR and MOE of panels containing 0%, 6.25% and 12.5% wheat straw with a LR/G ratio of 1/2.75 were higher than those of panels made from 25%–100% wheat straw with a LR/G ratio of 1/2.75, as well as those from all other percentages of straw with a LR/G ratio of 1/3.25. On the other hand, the IB of panels containing more than 12.5% straw with LR/G ratios of 1/2.75 and 1/3.25 were lower than those of panels made from 0–12.5% straw also with both LR/G ratios. Panels consisting of 0%, 6.25% and 12.5% wheat straw with LR/G ratios of 1/2.75 and 1/3.25 met the minimum EN standard requirements of mechanical properties for general purposes. All of the panels containing 0–100% wheat straw with a LR/ G ratio of 1/2.75 or 1/3.25 met the required level of TS for 24-h immersion.

Preoperative clinical and paraclinical predictors of choledocholithiasis

BACKGROUND:Choledocholithiasis is the most common cause of obstructive jaundice and cholangitis and occurs in about 10%of patients with symptomatic gallstone.The aim of this study was to find preoperative and non-invasive tests for predicting common bile duct stones(CBDs). METHODS:Findings of clinical examination,history laboratory tests and ultrasound(US)in 60 patients with CBDs were compared retrospectively,with 60 patients with gallstones.The data were collected from medical charts The sensitivity,specificity and positive predictive value (PPV)were determined. RESULTS:The comparison between the two groups showed significant differences in anorexia,itching,dark urine subicterus(slightly elevated serum bilirubin without clinical evidence of jaundice)and jaundice appearance increased bilirubin level,aspartate aminotransferase(AST) and alanine aminotranferase(ALT),CBD diameter>6 mm and stone observation under US(P<0.05).The highest specificity(100%)and PPV(100%)were found in dark urine and pale colored stool,history of cholangitis,and icterus.Among paraclinical tests,alkaline phosphatase (ALP)had the highest specificity(72.1%)and PPV(12.5%) Under US,stone observation in CBD was the most important factor with a sensitivity of 94.5%,a specificity of 100%and a PPV of 100%. CONCLUSIONS:The most important factors for predicting CBDs are history of cholangitis and pancreatitis,presence of icterus,dark urine,pale colored stool,elevation of ALP and sonographic evidence of chledocholithiasis.

Retroperitoneal High Grade Undifferentiated Pleomorphic Sarcoma as a Second Metachronous Neoplasm Following Intracranial Meningioma

Introduction: Meningiomas and retroperitoneal sarcomas are two distinct rare tumors, with an incidence of 2 and 0.4 cases per 100,000 respectively. Case Report: We report the case of a 39 year old woman with a history of intracranial meningioma who was found two months later to have a large retroperitoneal sarcoma on CT scan after presenting to the emergency department complaining of abdominal pain. Pathologic evaluation was consistent with a high grade undifferentiated sarcoma. Discussion: It is highly unusual for a 39 year old otherwise healthy patient to present with two rare, seemingly unrelated tumors. It is our suspicion that there is a genetic component involved, however to date no association has been described in the literature between intracranial meningiomas and extracranial sarcomas. Conclusion: Besides being the first report of a retroperitoneal sarcoma occurring as a second metachronous neoplasm following an intracranial meningioma, we suspect that there is a genetic component involved. Further studies may indicate a genetic mutational link.

Agronomic Behavior of a New Cereal （Primary 6x Tritipyrum： AABBEbEb） in Comparison with Modern Triticale and Iranian Bread Wheat Cultivars

The novel salt-tolerant cereal, primary Tritipyrum, is the third artificially synthesized crop plant. The agronomic traits and grain yields of 13 primary 6x Tritipyrum lines were evaluated and compared with five modern 6x triticale lines and nine bread wheat cultivars at Bahonar University, Kerman, Iran in a completely randomized block design with three replications. The primary Tritipyrum lines increased the grain yield per plant by 2.8% and 14.1% compared to triticale promising lines and Iranian bread wheat cultivars, respectively. Although the primary Tritipyrum lines were late maturing, they were shorter than the triticale lines and wheat cultivars and superior in many characters such as penultimate leaf area, flag leaf length, penultimate leaf length and time to milky ripeness. Genotypes were divided into four groups for agronomic and morphological traits and four groups for grain yield by cluster analyses. All clusters showed significant differences (α = 5%) for all traits and the first cluster, which comprised primary Tritipyrum lines, had a higher mean grain yield than the other clusters. The results of this first large-scale trial indicated the high adaptation of primary Tritipyrum lines to conditions in Kerman province in South-east of Iran.

Clear Cell Papillary Renal Cell Carcinoma: A Rare Subtype

Introduction: Renal cell carcinoma (RCC) is generally divided into clear cell, papillary, and chromophobe subtypes. Clear cell papillary RCC is a recently described subtype. Case report: We report the case of a 42 year old female who was found on computed tomography scan of the abdomen to have a cyst which appeared to involve the right kidney. Pathology of the cyst wall revealed features consistent with a RCC, clear cell papillary type. Discussion: Clear cell papillary RCC are a recently described entity with less than 100 cases reported in the literature. Most cases in the literature have been associated with end-stage renal disease, however more case are being reported in patients without renal disease. Conclusion: Clear cell papillary RCC is a rare distinct entity in the spectrum of renal cell carcinoma. Based on the literature they appear to be clinically indolent tumors;however, more research is required to further characterize and prognosticate these rare tumors.
