Vancomycin flushing syndrome in orthopaedic practice:A case report

BACKGROUND Vancomycin flushing syndrome(VFS),also known as red man syndrome,is an allergic reaction to *** typically presents as a rash on the face,neck,and upper torso after intravenous administration of *** is blamed on rapid intravenous infusion of vancomycin during management and rarely happens after local use.A review of the literature showed that in the last 23 years,4 such cases have been ***,we add another case of VFS developed after slow local absorption of vancomycin in cement *** SUMMARY A 44-year-old male with a known case of hypertension,no history of allergies to medications,and a history of chronic osteomyelitis of the right tibia with discharging sinus over the anterolateral aspect of the *** pus culture grew Staphylococcus aureus,which was sensitive to clindamycin and *** patient underwent irrigation and debridement with the placement of vancomycin cement beads made from 4 g of vancomycin powder and 40 g of polymethyl *** hours postoperatively,the patient developed a pruritic,erythematous,macular rash predominantly on his face,neck,chest,and lower extremities and to a lesser extent his upper extremities.A diagnosis of VFS was made and was successfully treated with cetirizine(10 mg,oral)and methylprednisolone sodium succinate(125 mg,intravenous).The patient continued to have itching with a facial rash for 12 h with gradual improvement.A decision was made to not remove the beads as the patient continued to ***,the rash disappeared after 96 h with no further *** VFS can occur not only after rapid intravenous injection of vancomycin but also with local release,as in our *** orthopaedic surgeons routinely use vancomycin with polymethyl methacrylate in chronic osteomyelitis and revision arthroplasty,they should be aware of such a complication occurring.

Emerging insights into the function of very long chain fatty acids at cerebellar synapses

Very long chain-saturated and-polyunsaturated fatty acids(VLC-SFA and VLC-PUFA, respectively) are a functionally important class of fatty acids containing 28 carbons or more in their acyl chain. They are synthesized by the elongation of very long fatty acids-4(ELOVL4) enzyme, expressed mainly in the brain, retina, skin, testes, and meibomian gland, where these fatty acids are found(Agbaga et al., 2008). Further, these organs exhibit tissuespecific VLC-PUFA and VLC-SFA biosynthesis and incorporation into complex lipids for specific functions. in the brain, skin, and Meibomian glands, the ELOVL4 mainly makes VLC-SFA, which are incorporated into complex sphingolipids. in the retina, the ELOVL4 makes VLC-PUFA that are incorporated into phosphatidylcholine, that are critical for visual function, while in testes and sperm, the VLC-PUFA are incorporated into sphingolipids that are critical for fertility(Yeboah et al., 2021).

iDSSCNN-XgBoost:improved Dual-Stream Shallow Convolutional Neural Network Based on Extreme Gradient Boosting Algorithm for Micro Expression Recognition

Micro-expressions(ME)recognition is a complex task that requires advanced techniques to extract informative features fromfacial *** deep neural networks(DNNs)with convolutional structures have been ***,unlike DNNs,shallow convolutional neural networks often outperform deeper models in mitigating overfitting,particularly with small ***,many of these methods rely on a single feature for recognition,resulting in an insufficient ability to extract highly effective *** address this limitation,in this paper,an improved Dual-stream Shallow Convolutional Neural Network based on an Extreme Gradient Boosting Algorithm(iDSSCNN-XgBoost)is introduced for ME *** proposed method utilizes a dual-stream architecture where motion vectors(temporal features)are extracted using Optical Flow TV-L1 and amplify subtle changes(spatial features)via EulerianVideoMagnification(EVM).These features are processed by iDSSCNN,with an attention mechanism applied to refine the extracted effective *** outputs are then fused,concatenated,and classified using the XgBoost *** comprehensive approach significantly improves recognition accuracy by leveraging the strengths of both temporal and spatial information,supported by the robust classification power of *** proposed method is evaluated on three publicly available ME databases named Chinese Academy of Sciences Micro-expression Database(CASMEii),Spontaneous Micro-Expression Database(SMiCHS),and Spontaneous Actions and Micro-Movements(SAMM).Experimental results indicate that the proposed model can achieve outstanding results compared to recent *** accuracy results are 79.01%,69.22%,and 68.99%on CASMEii,SMiC-HS,and SAMM,and the F1-score are 75.47%,68.91%,and 63.84%,*** proposed method has the advantage of operational efficiency and less computational time.

Thedeployment of the internet of Things(ioT)with smart sensors has facilitated the emergence of fog computing as an important technology for delivering services to smart environments such as campuses,smart cities,and smart transportation *** computing tackles a range of challenges,including processing,storage,bandwidth,latency,and reliability,by locally distributing secure information through end *** of endpoints,fog nodes,and back-end cloud infrastructure,it provides advanced capabilities beyond traditional cloud *** smart environments,particularly within smart city transportation systems,the abundance of devices and nodes poses significant challenges related to power consumption and system *** address the challenges of latency,energy consumption,and fault tolerance in these environments,this paper proposes a latency-aware,faulttolerant framework for resource scheduling and data management,referred to as the FORD framework,for smart cities in fog *** framework is designed to meet the demands of time-sensitive applications,such as those in smart transportation *** FORD framework incorporates latency-aware resource scheduling to optimize task execution in smart city environments,leveraging resources from both fog and cloud *** simulation-based executions,tasks are allocated to the nearest available nodes with minimum *** the event of execution failure,a fault-tolerantmechanism is employed to ensure the successful completion of *** successful execution,data is efficiently stored in the cloud data center,ensuring data integrity and reliability within the smart city ecosystem.

Effects of Ni doping on the structural properties and collapse of magnetic ordering in NdFe_(1-x)Ni_xO_3(0.1 ≤x≤ 0.7) orthoferrites

Nd Fe_（1-x）Ni_xO_3（0.1 ≤ x ≤ 0.7） orthoferrites are synthesized by solid state reaction method, and the structural properties of these materials are investigated by employing x-ray diffraction（XRD）, scanning electron microscopy（SEM） and M o¨ssbauer spectroscopy. The orthorhombic structure is observed in all systems; however, with the increase in Ni doping,the increase in tolerance factor and the decrease in the cell volume are observed. Orthorhombic distortion decreases with Ni content increasing up to 50%, while above 50% Ni doping it increases. SEM examination indicates the increases in grain size and intermixing of grains with increase in Ni concentration. Comparison between bulk and theoretical densities shows that in each of all samples porosity is less than 2%. M？ssbauer spectroscopic investigations are performed to explain local structure, Fe oxidation states and collapse of the magnetic ordering. in these samples the Fe oxidation state remains＋3 and there is no considerable increase in hole states observed; however due to mismatch of the ionic radii between Fe^（3＋） and Ni^（3＋）, octahedral distortions, sagging and distribution of hyperfine parameters increase with increase in Ni concentration. The major factors behind the collapse of magnetic ordering in the Ni-doped systems are the weakening of the super-exchange interactions, decrease in the Neel temperature, increase in spin–spin relaxation frequency and high spin to low spin transition.

Carbon potentials of different biochars derived from municipal solid waste in a saline soil

There are numerous studies conducted on biochar for its carbon (C) sequestration potential;however,there are limited studies available on the behavior of salt-affected soils related to biochar ***,more studies are needed to elucidate the mechanisms through which biochar affects saline soil *** this study,biochars were produced from solid waste at pyrolysis temperatures of 300,500,and 700?C (BC300,BC500,and BC700,respectively)and applied to a saline soil to evaluate their impacts on soil carbon dioxide (CO_(2)) efflux,C sequestration,and soil quality.A soil incubation experiment lasting for 107 d was *** results showed that soil CO_(2) efflux rate,cumulative CO_(2) emission,active organic C (AOC),and organic matter (OM)significantly increased with BC300 application to a greater extent than those with BC500 and BC700 as compared to those in the no-biochar control (CK).However,soil C non-lability did not significantly increase in the treatments with biochars,except BC700,as compared to that in *** improving the soil quality by increasing the soil AOC and OM,BC300 showed positive impacts in terms of increasing CO_(2) emission from the saline soil,while BC500 and BC700 showed greater potentials of sequestering C in the saline soil by increasing the soil non-labile C *** recalcitrance index (R50) values of BC500 and BC700 were>0.8,indicating their high stability in the saline *** could be concluded that biochars pyrolyzed at high temperatures (?500?C)could be suitable in terms of C sequestration,while biochars pyrolyzed at low temperatures (?300?C) could be suitable for improving saline soil quality.

近几十年来，由于道路网络的扩张和城市化的蓬勃发展，道路建设对典型草原的影响引起了广泛的关注。然而，道路类型和距离对植物种群生长的综合影响仍然知之甚少。因此，本研究以陇东黄土高原微温微干典型草原为研究对象，探讨了道路建设（道路类型、距离）对菊科优势种茵陈蒿（Artemisia capillaris）和伴生种阿尔泰狗娃花（Aster altaicus）生长繁殖影响的驱动因素及其作用机制。结果表明，道路建设促进茵陈蒿的横向生长和无性繁殖，促进阿尔泰狗娃花的垂直生长和有性繁殖。阿尔泰狗娃花具有更强的更新潜力。普通公路茵陈蒿和阿尔泰狗娃花的地上生物量分别比其它两种道路平均高49%和27%，因此其对普通公路建设的响应更加敏感。此外，道路建设主要通过影响土壤进一步影响两个种群的地上生物量在群落中的占比。阿尔泰狗娃花的地上生物量及其在群落中的占比在距离20—35 m的范围存在明显阈值。因此需要注意将此范围内的植被保护和恢复措施与道路管理动态结合，影响植物生长繁殖，调控群落构建过程，以应对道路建设对种群和群落关系扰动的巨大挑战。
