Prognostic Value of N-Terminal Pro-Brain Natriuretic Peptide in Acute Pulmonary Embolism

Patients with pulmonary embolism (PE) have a high risk of death and it is important to recognize factors associated with high mortality. N-Terminal pro-Brain Natriuretic Peptide (NT-pro BNP) has recently emerged as a promising biomarker for risk assessment in acute pulmonary embolism (PE). The aim of this study is to detect the in hospital prognostic value of NT-pro BNP in patients with acute (PE). Methods: This study included 64 patients diagnosed as (PE) with the mean age of 59.1 ± 16.5 years, 40 patients of them (62.5%) were male. All patients were subjected to 12 leads ECG. X-ray chest, laboratory tests including d-dimer, troponin I, NT-pro BNP, doppler ultrasound for the venous system of both lower limbs, Echocardiograhy and 64 multislices CT pulmonary angiography. Results: According to the admission level of NT-pro BNP our patients were divided into two groups: group I included 22 patients with normal NT-pro BNP (less than 300 pg/ml), and group II included 42 patients with elevated NT-pro BNP (more than or equal 300 pg/ml). Patients in group II were found to have a significantly higher incidence of heart failure (28.6% Vs 4.6%, p = 0.025), impaired kidney function (serum creatinine was 1.7 ± 0.6 Vs 1.1 ± 0.2, p = 0.018), tachypnea (85.7% Vs 54.5%, p = 0.006) and cardiogenic shock (26.2% Vs 0%, p = 0.014) but a significantly lower incidence of chest pain (21.4% Vs 45.5%, p = 0.04) and lower left ventricular ejection fraction (51.3% ± 16.9% Vs 67.3% ± 12.8%, p = 0.043) compared to group I. There were a significantly higher treatment with thrombolytic therapy (35.7% Vs 9.1%, p=0.021) and positive inotropics (35.71% Vs 4.55%, p = 0.006) in group II compared to group I. Also group II had a higher need for mechanical ventilation (26.12% Vs 4.55%, p = 0.04) and a longer in hospital stay (19.5 ± 10.3 Vs 5.3 ± 4.5, p = 0.001) than group I. AIM: To study the chemical constituents and their bioactivity of Polygonum amplexicaule. METHOdS: The isolation of compounds was achieved by chromatographic techniques and structure of the isolates was established by UV, IR, HRESI-MS and NMR including 1d and 2d experiments. RESULTS: Bioassay-guided fractionation of an ethanolic extract of Polygonum amplexicaule led to the isolation of a hitherto unidentified compound, 5, 6-dihydropyranobenzopyrone (1) along with nine previously known com-pounds (2-10). Compounds 2-10 were identified as amplexicine (2), catechin (3), rutin (4), quercetin-3-O-β-d-galactopyranoside (5), chlorogenic acid (6), galloyl glucose (7), caffeic acid (8), gallic acid (9) and scopletin (10). CONCLUSION: Compound 1 is new. Compounds 1-10 exhibited considerable antioxidant activity in a 1, 1-diphenyl-2-picrylhydrazyl (dPPH) radical-scavenging assay.

直肠癌(Colorectal Cancer,CRC)也被称为碗癌,是世界范围内导致死亡的主要原因之一,早期诊断已成为成功治疗的关键。随着卷积神经网络(CNN)的新进展,可以将不同的CRC图像分成不同的类别,确定该CRC图像的组织特性,指导医生进行病理诊断。由于医生手动分类耗时且容易误判,使用深度学习对CRC图形进行自动分类变得尤为重要。本论文将使用神经网络对CRC图像进行学习训练,实现高精度高准确的分类,并开发所需的软件工具使得本论文工作具有实际医学应用价值。本文使用德国肿瘤疾病中心(NCT)数据集。本文将迁移学习和微调技术与ResNet-50模型相结合对CRC组织病理学图像进行学习分类。实验结果表明,在所有的CNN网络体系结构中,基于NCT数据集,ResNet＿50模型能够高准确地对CRC组织病理学图像进行高精度分类,其准确率为97.7%。基于本文设计的模型与技术开发了直肠癌病理图像分类软件(AI pathologist)对CRC图像自动分类。在AI pathologist软件中输入CRC图像,工具对其进行自动分类并给出分类的概率。对于不确定的类别,工具对该图像进行标注,医生可以手动抽取CRC图像,进行手动分类。该工具能够对CRC图像进行高效和准确的分类,提高了CRC病理诊断效率,对CRC的临床诊断具有巨大应用价值。

Short Communication: Effect of Timing of Introduction to Pasture Post Calving and Supplementation with <i>Saccharomyces cerevisiae</i>on Milk Fatty Acid Profiles in Early Lactation dairy Cows

There is increased public awareness of the effect of dietary fatty acid (FA) profile on human health. Therefore, when devising nutritional management strategies for dairy cows it is important to evaluate the effects of said strategies on the FA profile of the milk. This experiment investigates the effects of two early PP nutritional management strategies (NM);abrupt introduction to pasture (AP) or a total mixed ration for 21 d followed by a gradual introduction to pasture over 7 d (GP), with (Y) or without (C) live yeast (YS) on milk fatty acid (FA) profile. Forty multiparous dairy cows were assigned to one of four dietary treatments in a two (AP vs. GP) by two (Y vs. C) factorial, randomized block design. The experiment was conducted from d 1 to 70 PP. Pasture, TMR and concentrate samples were taken weekly to assess the chemical and FA composition. Milk yield was recorded daily and individual milk samples were collected weekly to determine milk FA composition. There was no interaction between NM strategy and YS supplementation on milk FA. Similarly, YS supplementation did not affect milk FA profile. However, GP had higher concentrations of C10 (P = 0.04), C12 (P = 0.01), C14 (P = 0.02) and medium chain FA (P = 0.02) vs. AP. Whereas AP had higher concentrations of the FA cis-9, C18:1 (P d unsaturated FA (P = 0.01) and lower concentrations of saturated FA (P = 0.01) vs. GP. These results suggest that abruptly introducing the early lactation dairy cow to a pasture based diets positively alters the FA composition of the milk produced when compared to the milk from a dairy cow gradually introduced to pasture.

Azoniaallene salts as versatile building blocks in the synthesis of antibacterial and antifungal heterocyclic compounds

Substituted 2-azoniaallene salts 1 are strong bifunctional electrophiles, undergo cyclization reactions furnish many series of heterocyclic compounds, where reacted with p-tolyl urea, phenyl thiourea and thiosemicarbazone derivatives to afford triazinium salts, and converted to corresponding free bases 3, 5, 7 under treatment with Na2CO3. While triazole derivatives 8 and 9 were obtained by the reaction 2-azoniaallene salts 1 with benzohydrazide and phenyl hydrazine, respectively. Benzoxazinium salts 10 and 11 were acquired when asymmetric 2-azoniaallene salt reacted in (1:1) ratio with p-cresol and 3-methyl-1-phenyl-5-pyrazolone, respectively. The reaction of 2-azoniaallene salt with malononitrile furnished the primidinium salt 12 which underwent neutralization with Na2CO3 followed by heterocyclization with hydrazine hydrate afforded the bicyclic compound 3-aminopyrazolo[3,4-d]pyrimidine 14, which is highly reactive for nucleophilic addition to phenyl isothiocyanate to furnish thiourea derivative 15. Moreover, 14 undergo condensation with aldehydes to give imine derivatives 16a,b. All free base compounds were screened for their antimicrobial activities.

Comparison of Ising Model and Potts Model on Non-Local directed Small-World Networks

Further to the investigation of the critical properties of the Potts model with q = 3 and 8 states in one dimension (1d) on directed small-world networks reported by Aquino and Lima, which presents, in fact, a second-order phase transition with a new set of critical exponents, in addition to what was reported in Sumour and Lima in studying Ising model on non-local directed small-world for several values of probability 0 P dels discussed previously, will be re-examined to study differences between their behavior on directed small-world networks for networks of different values of probability P = 0.1, 0.2, 0.3, 0.4 and 0.5 with different lattice sizes L = 10, 20, 30, 40, and 50 to compare between the important physical variables between Ising and Potts models on the directed small-world networks. We found in our paper that is a phase transitions in both Ising and Potts models depending essentially on the probability P.

The hepatoprotective activity of the ethanol extract of Astragalus kahiricus(Fabaceae) roots against ethanol-induced liver apoptosis was evaluated and it showed very promising hepatoprotective actions through different *** extract counteracted the ethanol-induced liver enzymes leakage and glutathione *** addition,it demonstrated anti-apoptotic effects against caspase-3 activation and dNA fragmentation that were confirmed by liver histopathological ***,the phytochemical study of this extract led to the isolation of four cycloartane-type triterpenes identified as astrasieversianin II(1),astramembrannin II(2),astrasieversianin XIV(3),and cycloastragenol(4).The structures of these isolates were established by HRESI-MS and 1d and 2d NMR *** antimicrobial,antimalarial,and cytotoxic activities of the isolates were further evaluated,but none of them showed any activity.
