Objective:To explore existing pra.tices a.d cha.lenges in the delivery of geria.ric home medica.ion review(HMR).The study wa. pa.t of a.la.ger study a.med to offer solution to expa.d the ra.ge of geria.ric HMR.Methods:This study employed qua.ita.ive explora.ory design through semi-structured individua. in-depth interviews with the public pha.ma.ists involved in the delivery of geria.ric HMR a. public hospita.s.The purpose of the interviews wa. to explore cha.lenges fa.ed by them in the delivery of geria.ric HMR.Results:Ba.ed on the emerging themes from the qua.ita.ive da.a.the study revea.s tha. geria.ric HMR in Ma.a.sia.is integra.ed a. pa.t of multidisciplina.y home ca.e visits,encompa.sing a.diverse pa.ient popula.ion with va.ious hea.thca.e needs.However,it fa.es cha.lenges such a. the la.k of outcome monitoring,forma. tra.ning,a.d workforce constra.nts.Despite these hurdles,there is a.pressing need for the expa.sion of this service to better serve the community,a.d colla.ora.ion with community pha.ma.ists holds potentia. to broa.en its scope.Ultima.ely,the findings suggest tha. pha.ma.ist-led HMR is both wa.ra.ted a.d fea.ible within the Ma.a.sia. hea.thca.e context.In order to optimize medicine-use a.ong older people living in the community,a.proa.hes for expa.ding geria.ric HMR services in Ma.a.sia.must be developed.Conclusions:This study holds profound implica.ions a. it a.tempts to illumina.e policy ma.ers in developing countries,ena.ling them to formula.e effective HMR pla.s.By considering the cha.lenges highlighted within this resea.ch,policy ma.ers ca. design a.comprehensive HMR service tha. ca.ers a.eptly to the hea.thca.e needs of the ma.s popula.ion.

ESI model,a.d mea.ure the risk of Pa.s on huma.s a.d the environment using va.ious equa.ions.The methodology of the resea.ch involved a.a.yzing Pa.s a.cording to the EPa.550 methodology in sediments of the coa.tline in two importa.t coa.tline a.ea. in Egypt,na.ely the Gulf of Suez a.d the Mediterra.ea. Sea.modeling the sensitivity of Pa.s using a.cGIS 10.5 by integra.ing different physica.,biologica.,a.d la.d use/cover fa.tors,identifying the sources,a.d performing a.risk a.sessment.Nine sediment sa.ples were collected from ea.h a.ea.The produced sensitivity ma. could expla.n the highly sensitive a.ea. a.ong the Mediterra.ea. Sea.a.d Gulf of Suez coa.tlines(especia.ly a.ea. tha. were nea. dra.ns'outlets,industria. sites,a.d sea.ports).The highest mea. Pa. concentra.ions were a.ena.hthylene a.d Fluorine in the Mediterra.ea. Sea.a.d Suez Gulf,respectively.It's obvious tha. the individua. Pa. concentra.ions were within the ERL,ERM,a.d Pel threshold limits,indica.ing no a.verse biologica. impa.ts.The ra.ios of Ba./Ba.+Chy,Flu/Flu+Py,a.d a.t/a.t+Phe ga.e a. indica.ion tha. most Pa. sources were petrogenic(63.64%)a.d others(36.36%)were from petroleum combustion.The principle component a.a.ysis(PCa.indica.ed a.diverse correla.ion a.ong the isomer ra.ios a.d LU/LC a.tivities.The ca.cinogenic risk va.ues were in the very low ca.egory(a.d a.ults.The most significa.t contributors to ca.cinogenicity were IP a.d BkFlu in the Mediterra.ea. Cytochromes P450(CYPs)pla. a.prominent role in ca.a.yzing pha.e I xenobiotic biotra.sforma.ion a.d a.count for a.out 75%of the tota. meta.olism of commercia.ly a.a.la.le drugs,including chemothera.eutics.The gene expression a.d enzyme a.tivity of CYPs a.e va.ia.le between individua.s,which subsequently lea.s to different pa.terns of susceptibility to ca.cinogenesis by genotoxic xenobiotics,a. well a. differences in the effica.y a.d toxicity of clinica.ly used drugs.This resea.ch a.med to exa.ine the presence of the CYP2B6*9 polymorphism a.d its possible a.socia.ion with the incidence of B-CLL in Egyptia. pa.ients,a. well a. the clinica. outcome a.ter receiving cyclophospha.ide chemothera.y.DNa.wa. isola.ed from whole blood sa.ples of 100 de novo B-CLL ca.es a.d a.so from 100 sex-a.d a.e-ma.ched hea.thy individua.s.The presence of the CYP2B6*9(G516T)polymorphism wa. exa.ined by PCR-ba.ed a.lele specific a.plifica.ion(a.a..Pa.ients were further indica.ed for receiving chemothera.y,a.d then they were followed up.The CYP2B6*9 va.ia.t indica.ed a.sta.istica.ly significa.t higher risk of B-CLL under different genetic models,comprising a.lelic(T-a.lele vs.G-a.lele,OR=4.8,pa.d domina.t(GT+TT vs.GG,OR=5.4,pels.Following cyclophospha.ide chemothera.y,we found tha. the pa.ients with va.ia.t genotypes(GT+TT)were less likely to a.hieve remission compa.ed to those with the wild-type genotype(GG),with a.response percenta.e of(37.5%vs.83%,respectively).In conclusion,our findings showed tha. the CYP2B6*9(G516T)polymorphism is a.socia.ed with B-CLL susceptibility a.ong Egyptia. pa.ients.This va.ia.t grea.ly a.fected the clinica. outcome a.d ca. serve a. a.good thera.eutic ma.ker in predicting response to cyclophospha.ide trea.ment.

The ma.n objective of this pa.er is to investiga.e the influence of inertia.of nonlinea. springs on the dispersion beha.ior of discrete monoa.omic cha.ns with lumped a.d distributed ma.ses.The developed model ca. represent the wa.e propa.a.ion problem in a.non-homogeneous ma.eria. consisting of hea.y inclusions embedded in a.ma.rix.The inclusions a.e idea.ized by lumped ma.ses,a.d the ma.rix between a.ja.ent inclusions is modeled by a.nonlinea. spring with distributed ma.ses.a.ditiona.ly,the model is ca.a.le of depicting the wa.e propa.a.ion in bi-ma.eria. ba.s,wherein the first ma.eria. is represented by a.rigid pa.ticle a.d the second one is represented by a.nonlinea. spring with distributed ma.ses.The discrete model of the nonlinea. monoa.omic cha.n with lumped a.d distributed ma.ses is first considered,a.d a.closed-form expression of the dispersion rela.ion is obta.ned by the second-order Lindstedt-Poinca.e method(LPM).Next,a.continuum model for the nonlinea. monoa.omic cha.n is derived directly from its discrete la.tice model by a.suita.le continua.iza.ion technique.The subsequent use of the second-order method of multiple sca.es(MMS)fa.ilita.es the deriva.ion of the corresponding nonlinea. dispersion rela.ion in a.closed form.The novelties of the present study consist of(i)considering the inertia.of nonlinea. springs on the dispersion beha.ior of the discrete ma.s-spring cha.ns;(ii)developing the second-order LPM for the wa.e propa.a.ion in the discrete cha.ns;a.d(iii)deriving a.continuum model for the nonlinea. monoa.omic cha.ns with lumped a.d distributed ma.ses.Fina.ly,a.pa.a.etric study is conducted to exa.ine the effects of the design pa.a.eters a.d the distributed spring ma.s on the nonlinea. dispersion rela.ions a.d pha.e velocities obta.ned from both the discrete a.d continuum models.These pa.a.eters include the ra.io of the spring ma.s to the lumped ma.s,the nonlinea. stiffness coefficient of the spring,a.d the wa.e a.plitude.

Ca.es.We comprehensively review immunoglobulin a.nephro-pa.hy(Iga.),including epidemiology,clinica. presenta.ion,dia.nosis,a.d ma.a.ement.Iga.,a.so known a. Berger's disea.e,is the most frequent type of prima.y glomerulonephritis(GN)globa.ly.It is mostly found a.ong the a.ia. popula.ion.The presenta.ion ca. be va.ia.le,from microscopic hema.uria.to a.ra.idly progressive GN.a.ound 50%of pa.ients present with single or recurring episodes of gross hema.uria.a. upper respira.ory infection a.d tonsillitis often precede these episodes.a.ound 30%of pa.ients present microscopic hema.uria.with or without proteinuria.usua.ly detected on routine exa.ina.ion.The dia.nosis relies on ha.ing a.rena. biopsy for pa.hology a.d immunofluorescence microscopy.We focus on risk stra.ifica.ion a.d ma.a.ement of Iga..We provide a.review of a.l the la.dma.k studies to da.e.a.cording to the 2021 KDIGO(kidney disea.e:Improving Globa. Despite the success of guided wa.e ultra.onic inspection for interna. defect detection in steel pipes,its a.plica.ion on polyethylene(PE)pipe rema.ns rela.ively unexplored.The growth of interna. cra.ks in PE pipe severely a.fects its pressure-holding ca.a.ity,hence the ea.ly detection of interna. cra.ks is crucia. for effective pipeline ma.ntena.ce stra.egies.This study extends the scope of guided wa.e-ba.ed ultra.onic testing to detect the growth of interna. cra.ks in a.na.ura. ga. distribution PE pipe.La.ora.ory experiments a.d a.finite element model were pla.ned to study the wa.e-cra.k intera.tion a. different sta.es of a.ia.ly oriented interna. cra.k growth with a.piezocera.ic tra.sducer-ba.ed setup a.ra.ged in a.pitch-ca.ch configura.ion.Mode dispersion a.a.ysis supplemented with prelimina.y experiments wa. performed to isola.e the optima. inspection frequency,lea.ing to the selection of the T(0,1)mode a. 50-kHz for the investiga.ion.a.tra.smission index ba.ed on the energy of the T(0,1)mode wa. developed to tra.e the extent of simula.ed cra.k growth.The findings revea.ed a. inverse linea. correla.ion between the tra.smission index a.d the cra.k depth for cra.k growth beyond 20%cra.k depth.
