Magnetic anchor guidance for endoscopic submucosal dissection and other endoscopic procedures

Endoscopic submucosal dissection(ESD) is a wellestablished, minimally invasive treatment for superficial neoplasms of the gastrointestinal tract. The universal adoption of ESD has been limited by its slow learning curve, long procedure times, and high risk of complications. One technical challenge is the lack of a second hand that can provide traction, as in conventional surgery. Reliable tissue retraction that exposes the submucosal plane of dissection would allow for safer and more efficient dissection. Magnetic anchor guided endoscopic submucosal dissection(MAGESD) has potential benefits compared to other current traction methods. MAG-ESD offers dynamic tissue retraction independent of the endoscope mimicking a surgeon's "second hand". Two types of magnets can be used: electromagnets and permanent magnets. In this article we review the MAG-ESD technology, published work and studies of magnets in ESD. We also review the use of magnetic anchor guidance systems in natural orifice transluminal endoscopic surgery and the idea of magnetic non-contact retraction using surface ferromagentization. We discuss the current limitations, the future potential of MAG-ESD and the developments needed for adoption of this technology.

Clinical Significance of Liver Function Abnormality in Patients with COVID-19:A Single-center Experience from Western India

Background and Aims:The impact of coronavirus disease-2019(COVID-19)on liver function remains to be fully *** study was designed to investigate such and determine the clinical significance in determining mortality ***:A retrospective study was conducted in patients with COVID-19 from March 2020 to July *** details were retrieved from electronic medical records to obtain clinical characteristics,medical history,laboratory tests,therapeutic intervention,and outcome ***:A total of 184 patients with COVID-19 were included(median age:45.5 years),comprised of 62.5%*** total,22(12.0%)patients had severe infection and 162(88.0%)had mild to moderate ***,95(51.6%)showed abnormal liver function test(LFT)and 17(9.2%)showed normal LFT at *** median age,hospital stay,and LFT were significantly higher in severe ***-severe infection(p<0.001).Out of 12 deaths,the majority were due to severe infection(n=11).Deaths were also due to acute respiratory distress syndrome(n=5),cardiac reasons(n=3),and sepsis with multiorgan failure(n=3).The median age,hospital stay and number of intensive care unit admissions were higher in patients having abnormal LFT compared to normal *** of elevated aspartate aminotransferase(42.8%and 40.4%),alanine transaminase(43.7%and 41.6%),and hypoalbuminemia(71.4%and72.7%)at admission and discharge were more common in severe *** mean survival was significantly lower in severe infection compared to those with non-severe disease(17.2 vs.52.3 days;p<0.001).Conclusions:Incidence of abnormal liver function was higher in patients with severe COVID-19 and was associated with prolonged hospital stay;mortality was associated with severity of *** ruling out the risk of liver injury,it is crucial to vigilantly monitor the liver function parameters in patients with COVID-19 admitted to hospital.

Turn-on fluorescence probe for selective detection of Hg(Ⅱ)by calixpyrrole hydrazide reduced silver nanoparticle: Application to real water sample

A simple and quick method for the synthesis of water dispersible stable silver nanoparticles has been developed. Calix[4]pyrrole octahydrazide （CPOH）, has been successfully used as a reducing as well as stabilizing agent for the synthesis of silver nanoparticles. CPOH-AgNps have been duly characterized by SPR, PSA, TEM and EDX-ray. The ability of CPOH-AgNps as selective and sensitive sensor for various ions （Pb（Ⅱ）, Cd（II）, Mn（Ⅱ）, Fe（Ⅲ）, Ni（Ⅱ）, Zn（Ⅱ）, Hg（Ⅱ）, Co（Ⅱ）, Cu（Ⅱ）） by colorimetry and spectrofluorimetry has been explored. CPOH-AgNps were found to be selective only for Hg（Ⅱ） ions. Nanomolar concentration of Hg（Ⅱ） ions can also be determined by spectrofluorimetry by increase in fluorescence intensity. Linear range of detection of Hg（Ⅱ） ions in water was found to be from 1nmol/L to mol/L. The method has been successfully applied for determination of Hg（Ⅱ） ions in ground water and industrial effluent waste water samples.

Gastroschisis and omphalocele are the two most common congenital defects of the anterior abdominal *** etiologies remain unclear,but while gastroschisis is associ-ated with young maternal age,maternal smoking,maternal alcohol and illicit drug use during pregnancy omphalocele is frequently associated with genetic disorders and other congenital anomalies[1].The incidence of gastroschisis in the United States(US)has increased with an estimated inci-dence of 4.6 per 10,000 live births(LB)between 2010 and 2014[2].A recent study capturing information on 20%of infants admitted to neonatal intensive care unit(NICU)in the US showed a decrease in gastroschisis hospitalizations while that of omphalocele remained stable[3].However,it lacks generalizability[3].Our aim was to provide updated trends of gastroschisis and omphalocele hospitalizations in the US between 2010 and 2018 using population-based and nationally representative databases.

Current Vegetation Pattern along Glacial Landscape in Central (Garhwal) Himalaya, India

Current vegetation patterns, biodiversity and adaptation of plants were studied during 1998-2001 in glacial landscape of Chaurabari situated above Kedarnath （30° 44＇ N- 79° 07＇ E; 3,000- 6,000 m） in Central Himalaya. Landscape was identified into different zones on account of the vegetation status, glacial features, geomorphology and altitudes. Cold environment with heavy snowfall, frost hailstorm and dense frost characterizes the study area Predominance of the soda rich feldspars indicates soda enrichment; orthoclase, microcline weathering and alternation would have contributed potash to the soil. The increasing severity of the environment as we ascend from timberline to snowline leads to progressive decline in the abundance and diversity of the plant species. The diversity of the higher plants decrease, while the diversity of microflora increase from alpine zone to snowline zone. Highly opulent and diverse flora with beautiful, delicate herbs occupy the alpine zone, but some specialized groups of the plants, particularly high energetic and cold resistant species reside in glacial environment. Asteraceae, Rananculaceae, Primulaceae, Rosaceae, Apiaceae and Ericaceae are the pioneer angiospermic families, while Anaphalis triplinervis, A. royleana, Androsacce sarmentosa, Cotoneaster rotundifolius,Lonicera myrtillus, Cassiope fastigiata, Gaultheria trichophylla and Erigeron multiradiatus are the pioneer species, which have invaded in glacial environment. Through its nature, alpine glacial ecotone can be seen easily due to environmental and edaphic differences.

Sleep apnea and fatty liver disease:The growing link and management issues

Obstructive sleep apnea(OSA) is associated with metabolic, cardiovascular and neuropsychological disorders, with substantial morbidity and economic costs. OSA has been estimated to affect 4%-11% of the population, depending on age. Obesity is a significant risk factor for OSA. Non alcoholic fatty liver disease(NAFLD) has emerged as an integral component of the metabolic syndrome, with insulin resistance as the central pathogenic feature. Estimates based on imaging and autopsy studies suggest that about 20%-30% of adults in the United States and other Western countries have NAFLD. Evidence now suggests that NAFLD is independently correlated to insulin resistance regardless of adiposity. Some authors have suggested that OSA may be another contributor to NAFLD development. In complex diseases, several or many different genes interact with environmental factors in determining disease presence or its phenotype. Individual genes only have a small effect on disease risk and can therefore be very difficult to identify. The genetic and hormonal determinants of OSA and NAFLD have received little attention. A wide variety of intermediate phenotypes and genes are involved in OSA and NAFLD which makes this syndrome genetically complex. Various adipokines, the most important of whichare leptin, adiponectin, tumor necrosis factor-alpha, resistin and interleukin-6, have a key role in NAFLD and OSA. Some studies have suggested that oxidative stress may also contribute to the development of NAFLD and OSA. Lifestyle intervention, insulin sensitizer drugs and bariatric surgery aim to improve metabolic syndrome, OSA and NAFLD but need further investigation.

Characterization of Negative Exponential Distribution through Expectation

For characterization of negative exponential distribution one needs any arbitrary non-constant function only in place of approaches such as identical distributions, absolute continuity, constant regression of order statistics, continuity and linear regression of order statistics, non-degeneracy etc. available in the literature. Path breaking different approach for characterization of negative exponential distribution through expectation of non-constant function of random variable is obtained. An example is given for illustrative purpose.

Cardiac Radiosurgery for the Treatment of Atrial Fibrillation

An estimated 4 million Americans currently have atrial fibrillation (AF), the most common form of cardiac arrhythmia. The annual growth rate of AF is approximately 9.2% and advanced statistical analyses project that the prevalence of AF will be 12.1 million in the United States by 2030. The current therapeutic options to treat AF put certain patient groups at a high risk for a serious bleeding event, stroke or thromboembolism, myocardial infarction, and death. The most effective treatment for AF—catheter ablation—has been widely reported to require repeat operations and only leaves 59.4% of patients AF-free after five years. A majority of the reported complications and increased risks for catheter ablation occur because the procedure is invasive. Thus, there is a need for a novel, non-invasive therapeutic option to treat AF that can potentially expand the treatable patient population, increase patient safety, and improve clinical outcomes. CyberHeart Inc. has been exploring the efficacy and potential of cardiac radiosurgery as an option to treat heart conditions such as AF. With its proprietary CardioPlanTM software, the CyberHeart System utilizes the Cyberknife stereotactic radiosurgery platform to non-invasively ablate the heart. Early studies have displayed that the CyberHeart System is highly accurate at delivering radiation to cardiac targets and effective at minimizing radiation exposure to healthy tissue that is in close proximity to the targeted anatomic region. The focus of this review is to present the current standard of treatment for AF, identify the specific patient population that is at a high risk for the current treatment modalities, and address the potential for the CyberHeart Stereotactic Radiosurgery System to noninvasively ablate cardiac tissue and return sinus rhythm to patients with AF.

Variation of Trapping Strength with Size and Number of Particles in a Single Trap

Optical tweezers use the radiation pressure to trap and manipulate the microscopic particles. Using various algorithms multiple traps are being formed which can trap a number of particles simultaneously. In contrast to multiple traps, many particles can be trapped at a single trap position. It is known that when two or more particles are trapped in a single trap they align themselves in axial direction and it appears as if only one particle is trapped. We present a study of the dependence of the optical trapping force on the number of particles in a single trap using equipartition method;the study was carried out for particles of different sizes. The trapping force was first found to increase then decrease with number of particles in trap for all particle sizes. We feel that our studies will be useful in applications of optical tweezers involving trapping of multiple particles in a single trap.
