Objective. - To eva.ua.e complica.ions of sa.rospinous liga.ent fixa.ion. Design. - Monocentric retrospective study. Pa.ients a.d methods. - Between Ja.ua.y 1991 a.d September 2002, 277 women (mea. a.e 64.9 yea.s, ra.ge 37 to 92 yea.s) underwent a.sa.rospinous liga.ent fixa.ion; 91% ha. a.menopa.sa. sta.us, a.d 15.5% used hormone repla.ement thera.y. 33.2% of the pa.ients ha. prior hysterectomy, 28.9% ha. a.history of surgery for prola.se, a.d 18.8% ha. a.socia.ed symptoms of stress urina.y incontinence. In a.l ca.es, sa.rospinous liga.ent fixa.ion wa. performed under visua. control using conventiona. stitch. Sa.rospin ous liga.ent fixa.ion wa. combined with the following procedures: a.terior va.ina. repa.r (N =137), a.ditiona. incontinence surgery (N =31), va.ina. hysterectomy (N =137), leva.or myorra.hy (N =203). Ma.n outcome mea.ures. - Intra.pera.ive complica.ions, postopera.ive complica.ions, long term pa.nful symptoms. Results. - Intra.pera.ive complica.ions were represented by 1 ca.e of va.cula. wound a.d four recta. injuries. Ma.n postopera.ive complica.ions were va.ina. ha.ma.oma. (N =6) a.d a.scesses (N =2). Long term symptoms were perinea. pa.n, scia.ic neura.gia. a.d dyspa.eunia. Discussion a.d conclusion. - There wa. no surgica. morta.ity, a.d we noted low ra.es of ma.or complica.ions. Sa.rospinous liga.ent fixa.ion a.sumes high priority in our thera.eutic regimen.

To eva.ua.e the influence of oxytocin on peropera.ive blood loss during myomectomy. From October 1998 to Ma. 2002, 94 pa.ients requiring surgica. myomectomy by la.a.otomy or by the va.ina. a.proa.h were enrolled in a.ra.domized double blind study. Pa.ients were ra.domized to two groups. In the first group (47 pa.ients) oxytocin wa. a.ministered during myomectomy a.d in the second group (47 pa.ients) a.pla.ebo wa. used. The ma.n outcome mea.ures were peropera.ive blood loss a.d ra.es of blood tra.sfusion a.d a.totra.sfusion. Peropera.ive blood loss wa. no different between the oxytocin group a.d the pla.ebo group (508 ± 558 ml versus 451 ± 336 ml; P= 0.55). Ra.es of a.totra.sfusion a.d blood tra.sfusion were a.so simila. in both groups. a.ministra.ion of oxytocin during myomectomy did not reduce peropera.ive blood loss in our study. The benefits of using oxytocin to prevent hemorrha.e during myomectomy seem to be limited.

Objective: This study wa. underta.en to compa.e morbidity for women undergoing la.a.oscopy- a.sisted va.ina. hysterectomy with bila.era. oophorectomy (La.HO) a.d va.ina. hysterectomy with bila.era. oophorectomy without la.a.oscopic a.sista.ce (VHO). Study design: Between a.ril 1, 2002, a.d Februa.y 1, 2004, a.prospective ra.domized study a. Ma.seille University Hospita. (La.Conception) included 48 pa.ients who underwent a.hysterectomy with prophyla.tic bila.era. oophorectomy for benign uterine conditions. These pa.ients were a.loca.ed to 2 groups (La.HO vs VHO). The study va.ia.les were dura.ion of surgery a.d of hospita.iza.ion a.d surgica. a.d postopera.ive complica.ions. Results: There wa. no significa.t difference in the dura.ion of surgery between the La.HO a.d VHO groups (100.2 ± 27.9 vs 83.9 ± 34.6, P = .08). The ra.e of complica.ions wa. significa.tly higher in the La.HO group (13/24 [54.1% ] vs 6/24 [25% ], P = .039). Conclusion: The overa.l complica.ion ra.e wa. higher with La.HO tha. VHO. It thus a.pea.s tha. la.a.oscopic a.sista.ce is not useful in performing va.ina. hysterectomies with prophyla.tic bila.era. oophorectomies in pa.ients without other rela.ed disorders (endometriosis, a.hesions, a.nexa. a.oma.ies).

Ra.tors prima.ily use soa.ing-gliding flight which exploits therma.s a.d ridge lifts over la.d to reduce energetic costs. However during migra.ion, these birds often ha.e to cross wa.er surfa.es where therma. currents a.e wea.; during these times, birds ma.nly use fla.ping (powered) flight which increa.es energy consumption a.d morta.ity risk. a. a.result, some species ha.e evolved stra.egies to reduce the a.ount of time spent over wa.er by ta.ing extensive detours over la.d. In this pa.er, we conducted a.meta.a.a.ysis of wa.er-crossing tendencies in a.ro-Pa.ea.ctic migra.ing ra.tors in rela.ion to their morphology, their flight performa.ce, a.d their phylogenetic rela.ionships. In pa.ticula., we considered the a.pect ra.io (ca.cula.ed a. the wing spa. squa.ed divided by wing a.ea., the energetic cost of powered flight, a.d the ma.imum wa.er crossing length regula.ly performed by a.ult birds. Our results suggest tha. energy consumption during powered flight predomina.ely a.fects the a.ility of ra.tors to fly over wa.er surfa.es.

Most of the individua.s migra.ing a.ross the Stra.t a.e hea.ing for the wintering qua.ters in a.rica. while fewer spend the winter in ***: In a.5-yea. study(2011-2015), between 26 a.gust a.d 30 September, we determined a.e a.d sex of a.tumn migra.ing ha.riers through this flywa.. In 2014 we determined, by ma.ine ra.a. a.d optica. ra.ge finder, the flight a.titude of migra.ing ***: a.tota. of 10,261 Western Ma.sh Ha.riers were counted during the whole study, with a. a.era.e of 2052 per a.tumn sea.on. a.ults outnumbered juveniles a.d ma.es outnumbered fema.es. Ha.riers flew a. lower a.titudes during the morning while la.ge flocks flew lower tha. single birds or sma.l ***: Our observa.ions a.e consistent with previous surveys a.d confirm tha. a.ult ma.es ha.e a.tendency to migra.e over a.long dista.ce, while substa.tia. numbers of a.ult fema.es a.d juveniles do not hea. for the wintering qua.ters in a.rica.
