In that orcharding in early to mid twentieth century southeastern Australia involved use of certain heavy metal and As compounds in regular pest control spray procedures, some interest attaches to the possibility that these landparcels are underlain by soils with above background Cu, Pb and As levels. Interpretation of Land cover changes allowed land parcels previously occupied by orchards to be identified in the 1950s through time series air photos. A comparison of soil analysis results referring to soil samples from control sites, and from land parcels formerly occupied by orchardists, shows that contamination (above background) levels of cations in the pesticides can be found in the top 6 cm of former orchard soils. It is clear that digital spatial data handling and culturally informed air photo interpretation has a place in soil contamination studies, land use planning (with particular reference to re development) and in administration of public health.

AIM:To characterize small bowel(SB)tumors detected by capsule endoscopy(CE),and identify missed ***:The study included 145 consecutive patients in whom 150 CEs were *** CE,the medical records of the study population were *** of double-or single-balloon enteroscopy performed after CE and the results of surgery in all patients operated on were *** patients were contacted through telephone interviews or postal *** addition,the national cancer registry and the polish clinical gastrointestinal stromal tumor(GIST)Registry were searched to identify missed ***:Indications for CE included overt and occult obscure gastrointestinal bleeding(n=81,53.7%),anemia(n=19,12.7%),malabsorption(n=18,12%),abnormal CB follow through(n=9,6%),abdominal pain(n=7,5%),celiac disease(n=5,3%),neuroendocrine tumor(n=3,2%),Crohn’s disease(n=2,<2%),Peutz-Jeghers syndrome(n=2,<2%),other polyposes(n=2,<2%),and diarrhea(n=2,<2%).The capsule reached the colon in 115(76.6%)*** 150 investigations,CE identified 15SB tumors(10%),14 of which were operated on or treated *** included metastatic melanoma(n=1),adenocarcinoma(n=2),and GIST(n=3).Benign neoplasms included dysplastic Peutz-Jeghers polyps(n=4).Non-neoplastic masses included venous malformation(n=1),inflammatory tumors(n=2),and a mass of unknown histology(n=1).During the follow-up period,three additional SB tumors were found(2 GISTs and one mesenteric tumor of undefined nature).The National Cancer Registry and Polish Clinical GIST Registry revealed no additional SB neoplasms in the post-examination period(follow-up:range 4.2-102.5 mo,median 39 mo).The sensitivity of CE for tumor detection was 83.3%,and the negative predictive value was 97.6%.The specificity and positive predictive value were both 100%.CONCLUSION:Neoplasms may be missed by CE,especially in the proximal *** overt obscure gastrointestinal bleeding,complementary endoscopic and/or rad

Aim:Silybin(silibinin)is major biologically active flavonolignan extracted frommilk thistle(Sylibum marianum).Its biological activities include hepato-protection,anticancer properties,and antioxidant-and membrane-stabilizing ***-though membranes are postulated to be one of the cellular targets for silybin,littleis known about its interaction with phospholipid ***:In the presentwork,the interactions of silybin with phosphatidylcholine bilayers were studiedin detail using fluorescence spectroscopy,microcalorimetry and electron spinresonance ***:The results showed that silybin interacted withthe surface of lipid *** affected the generalized polarization of the fluores-cent probe Prodan,while not influencing the more deeply located *** the main phospholipid phase transition temperature as judged bymicrocalorimetry,and caused the immobilization of spin probe Tempo-palmitatelocated on the surface of *** mobility of spin probes 5-and 16-doxylstearic acid was not affected by ***-induced quenching of 1,6-diphe-nyl-1,3,5-hexatriene fluorescence indicated that some flavonoid molecules parti-tioned into the hydrophobic region of membranes,which did not change signifi-cantly the biophysical properties of the deeper membrane ***:Such a behavior of silybin in membranes is in accordance with its postulatedbiological functions and neglectable side effects of therapies using silybin.

BACKGROUND Elective total joint arthroplasty(TJA)procedures have been postponed as part of the coronavirus disease 2019(COVID-19)response to avert healthcare system *** hip arthroplasty(THA)and total knee arthroplasty(TKA)procedures comprise the highest volume of elective procedures performed at health care facilities *** To determine the demand for TJA despite the pandemic and the impact of surgery postponement on physical and mental *** We conducted a prospective cross-sectional telephonic interview-based study on patients awaiting THA and TKA at an academic institution in South *** questionnaire consisted of four *** first section recorded baseline demographic data and medical co-morbidities,the length of time spent awaiting TJA,and the patients’desire to undergo elective surgery despite the COVID-19 *** 2 and Section 3 assessed the patients’current physical and mental health,respectively,as a consequence of deferred surgical *** last section established the patients’perception of the healthcare system’s response to the COVID-19 pandemic and necessity to postpone elective *** received counseling and education on the current state of surgery during the COVID-19 pandemic and associated ***,patients were once again asked about their desire to undergo TJA during the COVID-19 *** We included 185 patients(65.95%female;mean age:50.28 years)awaiting TJA for a mean of 26.42±30.1 ***,88.65%of patients wanted TJA despite the COVID-19 *** awaiting TJA for 1-3 years were 3.3-fold more likely to want surgery than those waiting<1 year(P<0.000).Patients with comorbidities were 8.4-fold less likely to want TJA than those with no comorbidities(P=0.013).After receiving education,the patients wanting TJA decreased to 54.05%.Patients who changed their opinion after education had less insight on the increased morbidity(P=0.046)and mortality(P=0.001)asso

目的：评估B型利钠肽(BNP)预测心力衰竭患者预后的作用。设计：系统性综述，评估BNP在心力衰竭或无症状患者预后中的作用。数据来源：检索1994年1月至2004年3月Medline和Embase以及所检索到论文的参考文献。研究的选择和数据的提取：我们收集了所有评估BNP与心力衰竭患者和无症状患者死亡风险、心源性死亡、猝死、心血管事件关系的研究，包括最初的评价和治疗反应的评价。采用多元模型(包括BNP和左室射血分数都作为预测值)，来比较各变量的预后意义。由2位独立的研究者对研究和数据进行遴选。数据综合：采用BNP评估相对死亡风险或心血管事件的研究中，关于心力衰竭患者的有19篇，关于无症状患者有5篇。对于心力衰竭患者，BNP每上升100 pg/ml，相对死亡风险升高35%。35个多元模型使用了BNP这一预后指标。在其中的9个模型中，BNP是惟一达到显著性差异的指标——也就是说，其他变量没有包含BNP包含的预后信息。即便考虑到变量的范围，BNP似乎仍然是个有力的指标。结论：虽然有关预后的系统性综述存在固有的缺陷，包括可能的发表偏倚，但是本研究结果显示，无论对于无症状患者还是处于各阶段的心力衰竭患者，BNP都是一个有力的预后指标。
