Chinese Medicine Pattern Differentiation and Its Implications for Clinical Practice

Chinese medicine practitioners apply the differentiation reasoning for decision-making. The wide scope of Chinese medicine intervention provides coverage of methods and techniques with applications to primary, secondary and tertiary levels of prevention. The rapid evolution of mathematical and computational techniques allowed the implementation of several models for pattern differentiation that were tested for several physiologic systems. Concurrently, it is argued that pattern differentiation might improve the efficacy of either traditional or conventional medical interventions. This article reviewed the influence of pattern differentiation into clinical practice organized by medical field： general pattern differentiation; genitourinary （recurrent cystitis）; cardiovascular （coronary heart disease; arterial hypertension; angina pectoris）; neurology （stroke）; surgery; metabolic （diabetes mellitus）; hepatic （cirrhosis）; gastrointestinal （chronic superficial gastritis）; orthopedic （low back pain; rheumatoid arthritis; cervical spondylosis; elbow arthritis）; oncology （gastric mucosal dysplasia; lung cancer）; gynecologic and obstetric manifestations （nausea and vomiting）. The reviewed studies presented achievements that have contributed to the integration of Chinese medicine and evidence-based medicine in the treatment of many mild and severe diseases. Target diseases considered as major public health problems were also investigated and the results are promising regarding the possibility to treat guided by pattern differentiation.

Determination of functional prognosis in hospitalized patients following an intensive care admission

AIM To investigate the factors associated with the functional progress of hospitalized patients following an intensive care *** Retrospective study including data from a cohort of 198 hospitalized patients following an intensive care admission and not requiring mechanical ventilation in a single tertiary referral hospital. A generalized linear model was used to identify the main effects of clinical and demographic variables on the outcomes of functionality(KATZ Index of Independence in Activities of Daily Living) and muscle strength(MRC Scale). The covariates identified as independent predictors were analysed using the receiver operating characteristic curves. The analysis differentiated the periods in the intensive care unit(ICU), in the Ward(WARD) and the total time of hospital stay(TOT).RESULTS Considering the functional outcome(ΔKATZ), the variables that significantly contributed to the model(P < 0.05) were the KATZ and MRC on admission, age, sepsis(no), and total length of stay(TLS). Regarding the muscle strength outcome model(ΔMRC), the predictors were MRC on admission, Simplified Acute Physiology Score III, previous stroke, TLS, and sex(female). The variable age(AUC = 0.664) discriminated the ΔKATZICU. The variables age(AUC = 0.712), KATZ in ICU(AUC = 0.590) and on ward admission(AUC = 0.746), and MRC on ward admission(AUC = 0.721) were discriminative for ΔKATZWARD. For ΔKATZTOT the variables KATZ on ICU admission(AUC = 0.621) and TLS(AUC = 0.617) were discriminative. For ΔMRCICU the variables SAPSIII(AUC = 0.661) and MRC on ICU admission(AUC = 0.653) were discriminative. MRC on ICU(AUC = 0.681) and ward admission(AUC = 0.553) were discriminative for ΔMRCWARD. TLS(AUC = 0.649) and MRC on ward admission(AUC = 0.696) discriminative for the Δ*** Specific functional, clinical and demographical variables at ICU admission are associated with the functional prognosis during the hospitalization period.

Different canopy characteristics of industrial eucalyptus may lead to differences in water evaporation and availability to *** study aims to understand rainfall partitioning in a young clonal plantation(age of 2-4.5 years)of three eucalyptus species by relating tree parameters:diameter at breast height,total height,and leaf area *** measured rainfall,throughfall,stemflow and litter interception,along with the tree *** eucalyptus trees had rainfall interception varying between 22 mm(for 178 mm of rainfall)and 42 mm(for 87 mm of te rainfall),throughfall between 106 mm(for 186 mm of rainfall)and 44 mm(for 74 mm of rainfall),and stemflow between 0.5 mm(for 92 mm of rainfall)and 1.4 mm(for 24 mm of rainfall).For the three species,rainfall interception varied between 12 and 48%,throughfall between 57 and 90%,and stemflow between 0.3 and 5.4%.The coefficient of determination between interception and rainfall was 0.76,indicating interception depends on other variables,possibly including antecedent rainfall,rainfall intensity,and *** decreased with a reduction in leaf area index caused by eucalyptus *** *** had 0.75 mm of throughfall per 1 mm of rainfall,whereas in *** and E saligna,these ratios were 0.71 and 0.68,*** in *** and *** had a higher positive relationship with the diameter at breast height of the trees,whereas in *** the highest relationship was with the *** results contribute to establishing management strategies,such as choosing suitable eucalyptus species to local climate,and to improve the synchronization of crop-demand versus soil-water-supply while maintaining streamflow to fulfill ecological and production needs.

Entropy model to assess sediment resuspension probability and trap efficiency of small dams

In the Brazilian drylands, there are tens of thousands of small dams. Despite their paramount importance to the rural population, they are rarely monitored. Water demand increases with time while, simultaneously, siltation reduces reservoir water capacity and availability. Reservoir siltation models are, therefore, mandatory to manage the numerous ungauged small dams in these *** objective of the current study is to improve sediment trap efficiency(TE) modeling by including resuspension as a key probabilistic process. The Shannon entropy was used to estimate the sediment resuspension probability, which was merged with the Camp model and generated the RETSED entropy model. To validate the RETSED model, an experimental check dam(ECD, 300 m3), located in the Gilbu es desertification site, Brazil, was monitored hourly during one hydrological year(July 2018-July 2019). Measurements show that the annual volumetric decline of the check dam was 12%; and that the average trap efficiency equaled 86%. Only 9.5% of the hourly sediment concentration outflow exceeded the average plus one standard deviation, showing that the reservoir is well mixed; a fact which highlights the relevance of the resuspension process. Three empirical models failed to mimic the experimental results: Churchill(TE = 99%), Brune(TE = 75%), and Maryland(TE = 94%). According to the RETSED entropy model, the resuspension probability during the experiment was 10%and TE = 81%, a value only 6% below the measured one. The Camp model simulated TE = 89%, only 3%higher than the measured value, but confirmed the Camp model’s tendency to overestimate TE due to a disregard of resuspension. The deterministic model showed low sensitivity concerning the hydrodynamic effects, whereas the entropy formulation proved to be more consistent with physical behavior: the resuspension probability clearly increased and trap efficiency decreased with rising reservoir discharge.

Induced pluripotent stem cells(iPSC)technology has propelled the field of stem cells biology,providing new cells to explore the molecular mechanisms of pluripotency,cancer biology and aging.A major advantage of human iPSC,compared to the pluripotent embryonic stem cells,is that they can be generated from virtually any embryonic or adult somatic cell type without destruction of human *** addition,iPSC can be generated from somatic cells harvested from normal individuals or patients,and used as a cellular tool to unravel mechanisms of human development and to model diseases in a manner not possible *** these fundamental aspects of human biology and physiology that are revealed using iPSC or iPSC-derived cells,these cells hold an immense potential for cell-based therapies,and for the discovery of new or personalized pharmacological treatments for many ***,we review some of the current challenges and concerns about iPSC *** introduce the potential held by iPSC for research and development of novel health-related *** briefly present the efforts made by the scientific and clinical communities to create the necessary guidelines and regulations to achieve the highest quality standards in the procedures for iPSC generation,characterization and long-term ***,we present some of the audacious and pioneer clinical trials in progress with iPSC-derived cells.

Quantitative Study on Nonmetallic Inclusion Particles in Steels by Automatic Image Analysis With Extreme Values Method

The presence of nonmetallic inclusion particles which appear during steelmaking process is harmful to the properties of steels, which is mainly as a function of some aspects such as size, volume fraction, shape, and distribution of these particles. The automatic image analysis technique is one of the most important tools for the quantitative determination of these parameters. The classical Student approach and the Extreme Values Method （EVM） were used for the inclusion size and shape determination and the evaluation of distance between the inclusion particles. The results thus obtained indicated that there were significant differences in the characteristics of the inclusion particles in the analyzed products. Both methods achieved results with some differences, indicating that EVM could be used as a faster and more reliable statistical methodology.

Diagnosis of pancreatic tumors by endoscopic ultrasound-guided fine-needle aspiration

AIM： To evaluate the diagnostic accuracy of endoscopic ultrasound-guided fine-needle aspiration （EUS-FNA） for pancreatic solid tumors larger or smaller than 3 cm, and cystic lesions.METHODS： From January/1997 to December/2006, 611 patients with pancreatic tumors were subjected to EUSFNA. The final diagnosis was obtained either by surgery （356 cases） or after a mean clinical follow-up of 11.8 mo in the remaining patients.RESULTS： There were 405 solid tumors, 189 cystic lesions and 17 mixed. Pancreatic specimens for cytological assessment were successfully obtained by EUS-FNA in 595 （97.4%） cases. There were 352 （57.6%） malignancies and 259 （42.4%） benign tumors. Among the malignancies, pancreatic adenocarcinomas accounted for 67% of the lesions. Overall, the sensitivity, specificity, positive and negative predictive values, and accuracy of EUS-FNA were, respectively, 78.4%, 99.2%, 99.3%, 77.2% and 87.2%. Specifically for solid tumors, the same parameters for neoplasms larger and smaller than 3 cm were, respectively, 78.8% ys 82.4%, 100% ys 98.4%, 100% vs 99%, 54.8% vs 74.1% and 83.1% vs 87.8%. For cystic lesions, the values were, respectively, 72.2%, 99.3%, 97.5%, 91% and 92.2%.CONCLUSION： EUS-FNA can be used to sample pancreatic tumors in most patients. Even though the negative predictive value is inadequate for large solid tumors, the results are rather good for small solid tumors, especially concerning the sensitivity, negative predictive value and diagnostic accuracy. Among all pancreatic lesions, EUS-FNA for cystic lesions can reveal the best negative predictive value and diagnostic accuracy, both higher than 90%.

Ischaemic heart disease(IHD)is a major cause of morbidity and mortality *** there have been major advances in this field,these patients are still a higher risk *** such,strategies to mitigate risk and tailor secondary prevention measures are of the utmost *** rehabilitation(CR),encompassing several domains including exercise training,cardiovascular risk factor optimization,nutritional and psychological assessments,as well as other ancillary interventions has shown to be one of the pillars in the contemporary management of patients with ***,CR is associated with several benefits in this population,ranging from functional capacity to improvements in *** this,there are still several issues concerning the optimal application of CR which are still not fully ascertained,such as lack of referral and completion,as well as questions related to programme design(particularly among patients with multiple comorbidities).In this review,we aim at presenting a pragmatic overview on the current role of CR in the management of individuals with IHD,while also discussing some of the caveats in the current data,as well as future concepts which could help improve the uptake and personalization of this pivotal time-tested intervention.

Concrete Compressive Strength Estimation by Means of Nondestructive Testing: A Case Study

Estimation of the mechanical properties of concrete in an existing structure is possible with the results of surface hardness and ultrasound tests (nondestructive tests (NDTs)). For both the use of correlation curves is necessary, it is established between NDT and the results of the compressive strength of test specimens or extracted from structure. The objective of this study is to produce correlations between the results of surface hardness and ultrasound NDTs and the compressive strength of the structural concrete in the bleachers of a soccer stadium in the city of Cianorte, which is located in the northwest part of the state of Paraná, Brazil. This concrete structure, which is approximately 26 years old, has some defects, such as corrosion, concrete segregation and cracks. Concrete spalling in one of the slabs has recently raised some concern. Another significant issue is the absence of records regarding concreting of the bleachers’ structure. Therefore, mapping the reinforcement was initially performed according to the results of a surface hardness test, as recommended by standard [1]. An ultrasound test was simultaneously performed according to standard [2] for the same points employed in the surface hardness test. The concrete specimens were extracted according to the recommendations of standard [1] to determine compressive strength, perform the NDT and construct the correlation curves for the results. A total of 26 concrete specimens were obtained from all structures of the bleachers. From the methodology and the results of the study, highly reliable equations were obtained from the correlation curves among the compressive strength of the concrete specimens and the values of the surface hardness index and the ultrasound wave propagation velocity.

We consider the problem K（x）Uxx = utt , 0 〈 x 〈 1, t 〉 0, with the boundary condition u（O,t） = g（t） E LZ（R） and ux（O,t） = 0, where K（x） is continuous and 0 〈α≤ K （x）〈＋∞. This is an ill-posed problem in the sense that, if the solution exists, it does not depend continuously on g. Considering the existence of a solution u（x, .） E H2（R） and using a wavelet Galerkin method with Meyer multiresolution analysis, we regularize the ill-posedness of the problem. Furthermore we prove the uniqueness of the solution for this problem.
