BACKGROUND Transplant recipients commonly harbor multidrug-resistant organisms(MDROs),as a result of frequent hospital admissions and increased exposure to antimi-crobials and invasive *** To investigate the impact of patient demographic and clinical characteristics on MDRO acquisition,as well as the impact of MDRO acquisition on intensive care unit(ICU)and hospital length of stay,and on ICU mortality and 1-year mortality post heart *** This retrospective cohort study analyzed 98 consecutive heart transplant patients over a ten-year period(2013-2022)in a single transplantation *** was collected regarding MDROs commonly encountered in critical *** Among the 98 transplanted patients(70%male),about a third(32%)acquired or already harbored MDROs upon transplantation(MDRO group),while two thirds did not(MDRO-free group).The prevalent MDROs were Acinetobacter baumannii(14%),Pseudomonas aeruginosa(12%)and Klebsiella pneumoniae(11%).Compared to MDRO-free patients,the MDRO group was characterized by higher body mass index(P=0.002),higher rates of renal failure(P=0.017),primary graft dysfunction(10%vs 4.5%,P=0.001),surgical re-exploration(34%vs 14%,P=0.017),mechanical circulatory support(47%vs 26%P=0.037)and renal replacement therapy(28%vs 9%,P=0.014),as well as longer extracorporeal circulation time(median 210 vs 161 min,P=0.003).The median length of stay was longer in the MDRO group,namely ICU stay was 16 vs 9 d in the MDRO-free group(P=0.001),and hospital stay was 38 vs 28 d(P=0.006),while 1-year mortality was higher(28%vs 7.6%,log-rank-χ2:7.34).CONCLUSION Following heart transplantation,a predominance of Gram-negative MDROs was *** acquisition was associated with higher complication rates,prolonged ICU and total hospital stay,and higher post-transplantation mortality.

Right trisectionectomy for posterior liver tumors engaging the right and middle hepatic veins may lead to post-hepatectomy liver failure if the anticipated liver remnant is small. In such patients we developed a parenchymasparing one-step approach, that includes extrahepatic right portal vein ligation accompanied by en bloc resection only of segments 7, 8 and 4a and resection of the right and middle hepatic veins. The technique was applied in 3 patients with normal liver function, where according to the preoperative computed tomography the volume of segments 1, 2 and 3 ranged between 17% and 20% of the total liver volume. In all patients liver biochemistry improved rapidly postoperatively and a doubling of volume of segments 1, 2 and 3 was achieved by the third postoperative week, as extrahepatic right portal vein ligation ameliorated reperfusion injury of the remaining segments 5 and 6 and induced hypertrophy of segments 1, 2, 3 and 4b. There was no mortality or long-term *** are alive and free of disease 74, 50 and 17 months after the operation, respectively. We propose that the term ＂extended upper right sectionectomy＂ may be considered for the en bloc resection of segments 7, 8 and 4a, in future revisions ofthe Brisbane 2000 terminology of hepatic anatomy and resections.

Alternative access site choice after initial radial access site failure for coronary angiography and intervention

Background Transradial access for coronary catheterization is more technically challenging compared to the traditional transfemoral approach and radial access failure is quite common. The aim of this study is to describe the additional steps after initial radial access site failure in a high specialized forearm approach center. Methods A retrospective evaluation of all coronary catheterizations performed in our Department between January 2016 and December 2016 was performed, with focus on arterial access. Results One thousand three hundred forty six procedures were evaluated. The initial access site used was right radial [1173 procedures （87.1%）], left radial [120 procedures （8.9%）], fight ulnar [7 procedures （0.5%）], left ulnar [40 procedures （2.9%）] and femoral approach [6 procedures （0.4%）]. Radial artery can- nulation failure was observed in 37 procedures （2.9% of 1293 procedures with initial radial approach）. Failure of procedure completion after successful radial sheath insertion was observed in 46 procedures （3.6%）. The alternative access site after initial radial approach failure was contralateral radial [43 procedures （51.8%）], ipsilateral ulnar [22 procedures （26.5%）, contralateral ulnar [12 patients （14.5%）] and femoral approach [6 procedures （7.2%）]. Conclusion Forearm arteries can be used as alternative access site after initial radial approach failure in order to reduce the use of femoral approach during cardiac catheterization.

In this paper the author draws extensively upon her personal experience in witnessing storytelling in the oncological wing of the Laiko Hospital of Athens during her hospitalization in September-October 2004. More specifically, she concentrates on the presence of an inspired woman folk storyteller who, also a patient visited in the same period the hospital for personal treatment. Having the ability to tell a story skillfully, the specific storyteller ultimately tried with her narrations to ease the pain, both physical and spiritual, of patients that suffered from heavy or light forms of cancer, regardless of the fact that she herself was facing similar health problems. This essay sheds light on the persistence and function of tradition in our days and examines how illness and misfortune are ascribed and how the storyteller and listeners are connected. Her purpose is to find out what and how people learn and teach each other under special circumstances. This question has not been absent from the work of folklorists. The fieldwork was based on qualitative research, and the ethnographic method and collecting were used.

BACKGROUND The development of fully functional small diameter vascular grafts requires both a properly defined vessel conduit and tissue-specific cellular *** stromal cells(MSCs) derived from the Wharton's Jelly(WJ) tissue can be used as a source for obtaining vascular smooth muscle cells(VSMCs),while the human umbilical arteries(h UAs) can serve as a scaffold for blood vessel *** To develop VSMCs from WJ-MSCs utilizing umbilical cord blood platelet *** WJ-MSCs were isolated and expanded until passage(P) 4. WJ-MSCs were properly defined according to the criteria of the International Society for Cell and Gene Therapy. Then, these cells were differentiated into VSMCs with the use of platelet lysate from umbilical cord blood in combination with ascorbic acid,followed by evaluation at the gene and protein levels. Specifically, gene expression profile analysis of VSMCs for ACTA2, MYH11, TGLN, MYOCD, SOX9,NANOG homeobox, OCT4 and GAPDH, was performed. In addition,immunofluorescence against ACTA2 and MYH11 in combination with DAPI staining was also performed in VSMCs. HUAs were decellularized and served as scaffolds for possible repopulation by VSMCs. Histological and biochemical analyses were performed in repopulated h *** WJ-MSCs exhibited fibroblastic morphology, successfully differentiating into"osteocytes", "adipocytes" and "chondrocytes", and were characterized by positive expression(> 90%) of CD90, CD73 and CD105. In addition, WJ-MSCs were successfully differentiated into VSMCs with the proposed differentiation protocol. VSMCs successfully expressed ACTA2, MYH11, MYOCD, TGLN and SOX9. Immunofluorescence results indicated the expression of ACTA2 and MYH11 in VSMCs. In order to determine the functionality of VSMCs, h UAs were isolated and decellularized. Following the evolution of currently enforced Performance Based Design standards of reinforced concrete (RC) structures for durability, the designer, rather than complying with given prescriptive limits, may instead specify a cementitious mix design that is proven to exhibit a code prescribed resistance level (class) to a given exposure environment. Such compliance will lead to the protection of the steel reinforcement from corrosion and the cementitious mortar from degradation, during the design lifespan of the structure, under aggressive environmental exposure conditions such as, marine or deicing salts and carbonation. In this context, the enhancement of the physical and durability properties of common cement-based mortars under chloride exposure are experimentally investigated herein. In particular, the experimental program reported herein aims to evaluate the influence of incorporating multi-walled carbon nanotubes on the physical and mechanical properties of reinforced mortars against chloride ions. Furthermore, the anticorrosion protection of cementitious composites prepared with nanomaterials at 0.2% w/w is further investigated, by comparing all test results against reference specimens prepared without any additive. Electrochemical (Half-cell potential, corrosion current) and mass loss of reinforcement steel measurements were performed, while the porosity, capillary absorption and flexural strength were measured to evaluate the mechanical and durability characteristics of the mortars, following a period of exposure of eleven months;SEM images coupled with EDX analysis were further recorded and used for microstructure observation. The test results indicate that the inclusion of the nanomaterials in the mix improved the durability of the mortar specimens, while the nano-modified composites exhibited higher chloride penetration resistance and flexural strength than the corresponding values of the reference mortars. To evaluate the effect of local surgical adhesive glue (albumin/glutaraldehyde-Bioglue) on the healing of colonic anastomoses in rats. METHODSForty Albino-Wistar male rats were randomly divided into two groups, with two subgroups of ten animals each. In the control group, an end-to-end colonic anastomosis was performed after segmental resection. In the Bioglue group, the anastomosis was protected with extraluminar application of adhesive glue containing albumin and glutaraldehyde. Half of the rats were sacrificed on the fourth and the rest on the eighth postoperative day. Anastomoses were resected and macroscopically examined. Bursting pressures were calculated and histological features were graded. Other parameters of healing, such as hydroxyproline and collagenase concentrations, were evaluated. The experimental data were summarized and computed from the results of a one-way ANOVA. Fisher’s exact test was applied to compare percentages. RESULTSBursting pressures, adhesion formation, inflammatory cell infiltration, and collagen deposition were significantly higher on the fourth postoperative day in the albumin/glutaraldehyde group than in the control group. Furthermore, albumin/glutaraldehyde significantly increased adhesion formation, inflammatory cell infiltration, neoangiogenesis, and collagen deposition on the eighth postoperative day. There was no difference in fibroblast activity or hydroxyproline and collagenase concentrations. CONCLUSIONAlbumin/glutaraldehyde, when applied on colonic anastomoses, promotes their healing in rats. Therefore, the application of protective local agents in colonic anastomoses leads to better outcomes.

Although reinforced concrete structures are able to withstand towards a variety of adverse environmental conditions, reinforcement corrosion could lead to concrete structure deterioration. The present study examines four different ways of using corrosion inhibitors against pitting corrosion. In particular, it was investigated the chloride penetration resistance of reinforced cement mortars using corrosion inhibitor applied in three different ways. The corrosion behavior of the specimens was evaluated by electrochemical methods such as Linear Polarization Resistance and Halfcell Potential Resistance. In addition, the mass loss of steel rebars against time of partially immersion in sodium chloride (NaCl) solution was carried out in the lab. The experimental results showed that the corrosion systems examined in the study provide anticorrosion protection on steel rebars against chlorides comparing with the reference group.
