Hepatobiliary manifestations in children with inflammatory bowel disease:A single-center experience in a low/middle income country

BACKGROUNDThere has been a worldwide increase in the reported incidence of inflammatorybowel disease (IBD) in children over the past 2-3 decades. The hepatobiliary (HB)manifestations of IBD have been well-studied in children in industrialized anddeveloped countries but are infrequently reported in low- and middle-incomecountries (LMIC) such as *** determine the prevalence of the HB manifestations in a cohort of Egyptianchildren with *** cross-sectional observational study was carried out over a period of 6 mo(between June 2013 to December 2013, at the Paediatric Hepatology andGastroenterology Units of Cairo University Children's Hospital, which is thelargest paediatric tertiary care centre in the *** study included 48 patients with confirmed IBD based upon clinical,laboratory, endoscopic and histopathological features, 29 (60.4%) were ***-four patients (50%) had ulcerative colitis (UC), 11 (22.9%) had Crohn'sdisease (CD) and 13 (27.1%) had unclassified-IBD (IBD-U), which was formerlyknown as indeterminate colitis. The mean age of the patients at the time ofpresentation was 8.14 (± sD 4.02) years and the mean age at the time of studyenrolment was 10.16 (± sD 4.19) years. All patients were screened for HBmanifestations by physical examination, liver function tests, imaging and liver biopsy when indicated. HB disorders were confirmed in 13 patients (27.1%).Transaminases were elevated in 3 patients (6.3%). Two patients (4.2%) hadelevated biliary enzymes (one was diagnosed as primary sclerosing cholangitis(PsC) and the other was diagnosed with PsC/autoimmune hepatitis overlapsyndrome and the third patient had hepatitis C virus infection. Introduction: Mucinous tubular and spindle cell carcinomas (MTsCC's) are recently described rare type of renal cell carcinoma (RCC). They may have morphological similarities to papillary RCC (papRCC) and in our case to carcinoma collecting ducts of Bellini. Case report: We report a rare case of renal tumor, mucinous tubular and spindle cell carcinoma in a 56-year-old woman. The tumor, located in the left kidney, was well circumscribed. MTsCC's are characterized by small, elongated tubules lined by cuboidal cells and/or cords of spindled cells separated by pale mucinous stroma. Discussion: MTsCC is a rare type of renal cell carcinoma. Pathologists should be aware of the histological spectrum of MTsCCs to ensure an accurate diagnosis. Careful attention to the presence of a spindle cell population may be helpful in the differential diagnosis in tumors with predominant compact tubular growth.

synovial sarcoma is a rare mesenchymal tumor. It occurs predominantly in the extremities especially in the region of the knee. The localization in palm hand is rare. We report a case of a 23 year-old man who present a monophasic synovial sarcoma of the left hand which presented as a painless cystic nodule. A surgical excision was performed. The result was good without recurrence. The synovial sarcoma is a rare malignant neoplasm that should be considered in the differential diagnosis of spindle neoplasms of hand.

The tumor desmoplastic small round cell in women is rare. Ovarian its location is exceptional. We report a patient of 20 years, the originalNorth Africawho consulted for an increase in abdominal volume with impaired general condition. Pelvic ultrasonography and computed tomography were in favor of multiple peritoneal masses with ascites. After surgical exploration, the histological and immunohistochemical data for diagnosis small round cell desmoplastic tumor of ovarian the patient was a candidate for poly chemotherapy but she was died a month later. Diagnosis relies on histological and immunohistochemical data. The demonstration of a nonrandom translocation t (11;22) (p13;q12) is specific for the disease. The management is multidisciplinary and combining surgery, a poly aggressive chemotherapy and radiotherapy. The prognosis remains poor.

Introduction: Large cell neuroendocrine carcinoma (LCNEC) or non small cell neuroendocrine carcinoma the ovary is a rare entity and is frequently associated with ovarian surface epithelial tumors. However, its association with serous carcinoma is rarer, in our knowledge tree cases has been described in international literature and the first case in Moroccan literature. Case Report: A 54-year-old woman presented with a pelvic mass measuring 15 cm in diameter. she underwent an exploratory laparotomy with resection of the pelvic mass. Diffuse and nodular intra-abdominal disseminations were observed. Immunohistochemistry stain confirmed the diagnosis of large cell neuroendocrine carcinoma with serous carcinoma. The patient received three courses of carboplatin and paclitaxel and she's still alivewith a decline of 6 months. Its clinicopathologic association is discussed and the literature is reviewed. Conclusion: In summary, ovarian LCNEC is an aggressive tumor with a tendency to present at advanced stage and cause death within a mean of 17 months after diagnosis;however, some patients, particularly those with stage I disease and/or those who have received platinum-based therapy, may have a more favorable prognosis.

Objectives:To identify and test the antibiotic susceptibility of nosocomial coliform bacilli and investigate the presence of oqxA and oqxB genes among the multidrug-resistant(MDR)***:One hundred and twenty different healthcare-associated infection samples were *** bacilli were isolated,identified by conventional methods,and then antibiotic susceptibility tests were done using the VITEK2 system and disk diffusion *** operon was identified using a conventional PCR-based *** and oqxB genes were compared between MDR Klebsiella pneumonia(***)phenotypes and MDR Escherichia coli(***)***,oqxAB operons were compared between phenotypes of *** and *** ***:seventy coliform bacilli were isolated with the predominance of *** and ***,82.1%of *** strains and 53.3%of *** isolates were MDR *** more oqxB genes alone were found in MDR *** than that in MDR *** phenotypes(χ^(2)=10.160,P=0.003).OqxAB operon was significantly more in MDR phenotypes of *** than that in the susceptible phenotypes(Ps significantly less of this operon in susceptible *** isolates than that in susceptible *** isolates(Psitive isolates that were also resistant to fluoroquinolones,tetracycline,trimethoprim,and chloramphenicol,most probably suggested functional ***:MDR coliform bacilli are strongly implicated in healthcare-associated *** should be paid to the presence of oqxAB pump,as an important mechanism in the development of resistance against many antimicrobials because it contributes to co-resistance with other categories;therefore,this pump could be a good target for efflux pump inhibitors.

Under conditions differing from those subjected for central limit theorem, the spatial autocorrelation function of speckle pattern resulting from illuminated rough surface is investigated. Its dependence on different illuminating apertures and the average of the roughness heights is presented theoretically and experimentally. The experiments were carried out using a set of circular and square apertures having different sizes. The results indicate that, increasing the size of the illuminating aperture leads to a decrease in the width of the main lobe of the spatial autocorrelation function.

Objective:To observe the effect of temperature and extraction process on the estimation of antioxidant activity of various organic crude extracts from the leaves of Thymus vulgaris(***)species native to sultanate of ***:The dry powder samples of *** were extracted with methanol using two different extraction *** methanol crude extracts from the leaves of *** were defatted with water and extracted successively with different polarities of solvents with increasing polarities,e.g.,hexane,ethyl acetate,chloroform and ***:The yield of methanol crude extract by soxhlet extraction method is better than maceration *** yield of extraction was increasing with increasing *** antioxidant activity of different crude extracts from both extraction methods was measured by DPPH with *** soxhlet extraction method,the activity result found in butanol crude extracts was highest and the lowest in hexane crude extract as the following order of butanol>methanol>ethyl acetate extract>chloroform>hexane ***,by maceration method,the activity was highest in ethyl acetate and lowest in chloroform as the order of ethyl aectate>methanol extract>butanol>hexane>***:In conclusion,the maceration method is the best method for the evaluation of antioxidant activity.

Desmoplastic small round cell tumor (DsRCT) is a rare and highly aggressive variety of sarcoma arising typically from abdominal or pelvic peritoneum. Diagnosis and treatment approaches of this entity are complex and require a skilled, experienced, multidisciplinary team. Authors report their experience with a case of an intraabdominal DsRCT arising in a 37-year-old young man in order to discuss the clinico-pathological and radiological behavior of this tumor and underline diagnostic and therapeutic difficulties.
