Doppler Ultrasound Abnormalities of the Lower Limbs in Patients with Diabetic Foot at the Yopougon University Hospital in Ivory Coast

Background: Diabetic foot is a frequent complication of diabetes, of multifactorial origin, associating nervous and/or circulatory disorders to which infection is often added. Its care is multidisciplinary and requires coordination between different actors. In this context, arterial doppler ultrasound is essential in the diagnosis of diabetic arteriopathy and participates in therapeutic decision-making. The aim of this study was to describe the abnormalities found on Doppler ultrasonography of patients hospitalized for the diabetic foot in the Ivory Coast. Methods: We conducted a retrospective descriptive study including 235 patients hospitalized for the diabetic foot in the Endocrinology department of the Yopougon University Hospital from February 2002 to December 2015. All records of patients who performed arterial Doppler ultrasound of the lower limbs were selected and the various abnormalities were described. Results: The sex ratio (M/F) was 1.1 (124 men to 111 women). The predominant age group was 61.2 years. Type 2 diabetes was found in 97.4% of cases. The average evolution of diabetes was 8.9 years with extremes of 1 and 32 years. Wet gangrene was the most common type of lesion found in 75.8% of cases. On ultrasonography, 98.7% of the patients had an arterial abnormality of the lower limbs. It was bilateral in 62.2% of cases, dominated by atheromatous overloads and mediacalcosis in 46.4% and 43.8% of cases, respectively. Arterial stenosis was lesions hemodynamically found in 76.3% of cases and arterial occlusions in 32.7% of cases. The amputation rate was 72.7%. Conclusion: This study shows the high frequency of arterial abnormalities in patients with the diabetic foot with a predominantly distal involvement. Arterial Doppler ultrasound of the lower limbs remains an essential tool in the care of diabetic foot. This easily accessible, non-invasive examination has a key role in therapeutic decision-making.

Integrating information on the glycemic index (GI) and the glycemic load (GL) of diet is limited in C?te d’Ivoire because of the lack of data. Thus, this study was undertaking for the local management and prevention of diabetes mellitus and its complications based on nutritional data (GI and GL values). The study included ten healthy subjects with 7 males, 3 females (28 ± 2 years on average age and 20.5 ± 1.7 on average BMI). Participants tested three different meals with equal carbohydrate load (50 g). Blood samples were obtained at 0, 15, 30, 45, 60, 90 and 120 min before and after consumption for glucose levels determination. GIs were determined using a standard method with glucose as reference food and data were used for GLs calculating. Data showed that GIs value of pounded yam with eggplant sauce and cassava paste with granulates palm nut sauce were high ranging to 94 to 86 respectively while rice with groundnut sauce, have a low GI (GI = 45). Nevertheless, the GLs of the all the test foods are high with the values of 47, 43 and 23 (g) for pounded yam with eggplant sauce, cassava paste with granulates palm nut sauce and rice with groundnut sauce respectively. According to GIs and GLs data, the three test foods must be consumed moderately in a diet. So, it is important to associate GL data to GI data of Ivorian traditional foods for the management and the prevention of diabetes mellitus in C?te d’Ivoire and in others countries sharing the same food tradition.

Modulation of Postprandial Glucose Bioavailability in a Cohort of Healthy and Pre-Diabetics Subjects during Ramadan Fasting in cote d'Ivoire

Objective: After the Ramadan fasting period, there is an increasing prevalence of diabetes in Cote d’Ivoire, in diabetes care centers. The objective of this study was to assess the consumption of sugary foods during this period and to determine the risks of diabetes in Muslim populations. Methods: Information was collected on the type and amount of sugary foods consumed. Methods for the preparation of these foods have been determined. Intake frequencies and postprandial glycemic responses were studied. The usual statistical methods were used. Results: Between 70% and 85% of the “fasters” consume plenty of these sugary foods during this period, with an average added sugar level varying between 10% and 35% and a daily consumption of between 250 cl and 0.5 l. The added sugar leads to hyperglycemia in both healthy subjects and pre diabetics. Conclusion: Regular consumption of these sugary foods results in significant hyperglycaemia throughout the month of fasting with a significantly higher blood glucose level in pre diabetic subjects. This makes pre diabetics people more at risk of diabetes among Muslim populations during Ramadan fasting.

Introduction: Early puberty is defined by development of secondary sexual characteristics before the age of 8 years in girls and 9 years in boys. In the West, one in five children out of 100,000 is concerned. Little information on sub-Saharan African patients exists concerning this pathology. Objective: To determine the etiology and clinical characteristics of early puberty in a cohort of Ivorian children. Methods: We conducted a cross-sectional study between 2015 and 2017 in children admitted to early puberty in the unique Endocrinology Service of the country. The epidemiology and clinical characteristics were assessed. Results: The eight patients involved were all females. Their mean age at diagnosis was 7 years and 3 months and the mean age of first symptoms begun at 5 years and 6 months. Seven patients had a central precocious puberty (CPP) including five cases of idiopathic CPP (ICPP) treated by (GnRH agonist) and two cases with secondary precocious puberty (SPP). One of them has a hypothalamic hamartoma and the other a sequelae of encephalopathy. The last case had a peripheral precocious puberty (PPP) causes by ovarian dystrophy. Conclusion: Our results confirm the predominance of idiopathic central precocious puberty particularly in girls.
