Microalbuminuria and Kidney Disease Risk in HIV Patients Taking Combined Antiretroviral Therapy

Objectives: This study proposes to evaluate risk factors for kidney disease in HIV patients treated chronically and correlate with microalbuminuria measurements. Methods: Review charts and analyses of microalbuminuria in subgroup of HIV patients treated at Ceara/Brazil. Results: 149 patients, 69.1% male, mean 38.5 years old, infection mean 86.8 months. Mean Creatinine Clearance 110.2%, Creatinine 0.97, Urea 27.76 mg/dl, CD4+ 600.37 cels/mm3 and detectable viral load 530.59 copies with 61.7% undetectable. Mean Dosages of microalbuminuria/24h 147, 46 ± 820, 45 (N = 48) and microalbuminuria (mg/dl) 32.05 ± 85.25 (N = 43). Kidney Diseases Classification analyses evidenced 6.4% patients in stages ≥3 and 6.2% presented altered Microalbuminuria/24h. Patients using Tenofovir (TDF) 27.27% had Stage 2 and protease inhibitors (PI) had 4.1% in Stage 3. Proteinuria was observed in 5% stage ≥3. Association PI/TDF had 4.1% in Stage 3. No statistical difference between CD4 > or 3 and microalbuminuria/24h > 300 mg (p = 0.69);detectable/undetectable viral load and microalbuminuria/24h (p = 0.63) or stage ≥3 (p = 0.17);relation to Diabetes or arterial hypertension and microalbuminuria 24 h (p = 0.5 and p = 0.21);relation stage ≥3 and microalbuminuria/24h (p = 0.33);relation HIV diagnoses >/ or or < 50 years and microalbuminuria/24h (p = 0.55) or microalbuminuria mg/d (p = 0.32). Relating comorbidities risk (Diabetes Mellitus plus Systemic Arterial Hypertension) to Kidney Diseases, it was found that 55.5% patients in Stage 3 or above with comorbidities compared with 15% with comorbidities in lower stages (P = 0.005). A Rare Presentation of Erdheim-Chester Disease Overlaped with Langerhans Histiocytosis

Erdheim-Chester Disease (ECD) is a rare condition and has various differential diagnoses with other forms of histiocytosis, classified as one of non-Langerhans histiocytosis. The diagnosis of this condition remains challenging because its presentation includes non-specific systemic manifestations that can affect different organs caused by deposition of lipids and fibrosis. Most common include bone pain followed by progressive weakness and different lung manifestations. This case is about a rare presentation of ECD with Langerhans Histiocytosis as overlap syndrome, with findings of both diseases in a middle aged woman that presented dyspnea as the first symptom. The patient was treated initially as heart failure and remained without any improvement, being admitted to investigate. After a stricted follow-up, bone and lung involvement were noticed and a skin biopsy unveiled xanthomatized macrophages accompanied by Touton giant cells. This condition remains an important clinical entity and should provide new insights for clinicians dealing with respiratory diseases.

Homocysteine and Cardiovascular Risk Factors in Overweight or Obese Children and Adolescents

Introduction: Among the extrinsic factors, homocysteine (Hy) stands out, which is an intermediate amino acid of the intracellular metabolism of methionine involved in the process of cellular oxidation, which promotes the installation of atheromatous plaques and, therefore, is considered as an emerging cardiovascular risk factor. Objective: To evaluate the plasma homocysteine levels (Hy) in overweight or obese children and adolescents and their relation with cardiovascular risk factors. Methods: A cross-sectional study was conducted from July 2011 to May 2012 with overweight or obesity children and adolescents aged 2 to 18 years followed at the Center for Childhood Obesity (IOC), Campina Grande-PB. A structured form was used to record demographic, socioeconomic and clinics variables and the patients underwent laboratory tests to define their lipid and glucose profiles and measurement of plasma Hy levels. Results: The study evaluated a total of 165 children and adolescents with mean age of 12.5 (±2.5) years;the majority were female (57.0%). Regarding the lipid profile, there was more individuals with low HDL cholesterol (88.5%). Plasma Hy levels were high in 24.2% of the sample. The mean Hy levels ranged from 4.3 to 18.9 μmol/L, being higher in males, obese adolescents and also in patients with high insulin levels and resistance. Conclusions: The results shown in this study emphasize the importance of detecting and controlling the plasma Hy levels as an independent cardiovascular risk factor, and the need for further studies to evaluate the clinical and biological factors related to alterations in its metabolism.

We present numerical studies on the switching characteristics of a fiber Bragg grating (FBG) with modulation in the third order nonlinear index of refraction along it’s length. The FBG is operating in a continuous wave regime (CW). This study was done taking into account the possible asymmetry brought by the non harmonic modulation of the nonlinearity, leading to different reflection and transmission characteristics, that depend on the direction of propagation along the modulated nonlinear FBG. This phenomenon may be useful for applications like an optical isolator. It was found that for a set of values of the modulation parameter, the FBG can exhibit multistable states. The numerical studies were obtained starting from the coupled-mode equations solved from the coupled-mode theory and simulated using the fourth-order Runge-Kutta method.

Spatial Analysis of Affected Areas by Extreme Hydrological Events in Rio de Janeiro (The Host City for the 2016 Olympic Games)—Brazil

The most frequent types of disasters in Brazil are associated with extreme hydrological events. Adding to this situation, in Brazil, there are the extreme sociospatial discrepancies that are historically constructed. They leave marks in space, making specific areas, for political interest or not, more vulnerable to the different mentioned processes. To this extent, the public management should plan actions and, also, it may act in a more holistic way adding new trends, such as, the use of geotechnologies that are applied to environmental management in the realm of urban planning. From a case study in the city of Rio de Janeiro, this paper aims to contribute and draw attention to such issues. It may reach its target through pointing and spatially analyzing which areas are in the most critical situation related to the occurrence of extreme hydrological events. To achieve this goal, it was proposed a methodology to inventory the occurrence of inundation/flooding for a certain period, in this case 2001-2008. Besides, the methodology could systematize data and integrate them with other important issues for supporting the process of identification and analysis of the most critical areas. Historical aspects of occupation and the most vulnerable socio-environmental aspects were raised, too, in order to validate the highlighted areas. This integration was enabled concerning the support of geoprocessing techniques. The results of this integration subsidized the mapping and spatial analysis of the affected areas in the city and the criticality in relation to the extreme hydrological events. Finally, it was possible to observe that the identification of the most critical affected areas does not mean the exhaustion of the subject. The location of these areas is directly linked to the sample that was used as the basis for analyses. This study aimed to describe structure, functionality and development of pediatric heart transplant families, from the understanding of the experienced chronic condition. A descriptive, exploratory study using a qualitative approach was developed in a hospital situated in a city of Northeast Brazil, in 2012. The study was conducted with three families, using the Calgary Family Assessment Model (CFAM). In addition to the genogram, an analysis of the children’s/adolescents’ family members was conducted at the time of interview. Two families were binuclear families and one was mononuclear. From the evaluation, it was possible to determine the care and support network of children and adolescents with heart transplants, as well as some difficulties faced by the investigated families. Nurses are important in this family context since the visualization of the family genogram can generate a change in the nursing care to each family.

depending on the treatment and weight of the breasts or abdomen, they may exceed volumes considered giant and morbidly obese. In these cases, and when the patient’s BMI is high above 40 kg/m2, the weight of the breasts or abdomen produces what we consider suffocation when the patient is placed in horizontal position on surgical tables, decreasing his respiratory capacity and increasing the difficulty in treating respiratory or embolic risks. An 8-kg breast on the patient’s chest prevents normal breathing. An abdomen with a volume of 30 kg causes difficulties in all senses, making the physiological expansion of the lungs impossible and even preventing surgical assistance to patients. These patients are almost always customers who sleep in the sitting position to breathe better. The gigantic extirpation of the surgical parts facilitates a better respiratory expansion reducing by a large percentage the risk of death, what we call body relief. This relief does not free the patient from bariatric surgery for a possible weight loss, which is vital for the proper functioning of the organs and decreasing arterial hypertension and diabetes.
