Sometimes in free flap there is a venous congestion without an obstruction of the venous anastomosis or other organic causes of reduction venous drainage (haematoma, seroma compressing the pedicle). In these cases the authors suggest the application of nitroglycerine patch in the congested area of the flap few hours before the surgical exploration of the anastomosis. If there is a fast improvement of the clinical feature of the flap, the surgical exploration could be avoided. The authors underline that applying the nitroglycerin patch should not be regarded in any way as a therapy of a free flap venous thrombosis but only as an useful device, an option to be taken only when the surgeon is undecided whether to revisit the anastomosis or not.

In rural territories, the communities use energy sources based on fossil fuels to supply themselves with electricity, which may address two main problems: greenhouse gas emissions and high fuel prices. Hence, there is an opportunity to include renewable resources in the energy mix. This paper develops an optimization model to determine the optimal sizing, the total annual investment cost in renewable generation, and other operating costs of the components of a hybrid microgrid. By running a k-means clustering algorithm on a meteorological dataset of the community under study, the hourly representative values become input parameters in the proposed optimization model. The method for the optimal design of hybrid microgrid is analyzed in six operating scenarios considering:(1) 24-hour continuous power supply;(2) load shedding percentage;(3) diesel power generator(genset) curtailment;(4) the worst meteorological conditions;(5) the use of renewable energy sources including battery energy storage systems(BESSs);and(6) the use of genset. A mathematical programming language(AMPL) tool is used to find solutions of the proposed optimization model. Results show that the total costs of microgrid in the scenarios that cover 100% of the load demand(without considering the scenario with 100% renewables) increase by over 16% compared with the scenario with genset operation limitation. For the designs with power supply restrictions, the total cost of microgrid in the scenario with load shedding is reduced by over 27% compared with that without load shedding.

Diabetes is a disease with a high global burden.Current strategies have failed to limit the advancement and impact of the disease.Successful early diagnosis and treatment will require the development of new agents.In this sense,boroncontaining compounds have been reported as agents with the ability to reduce glycemia and lipidemia.They have also been used for labeling and measuring carbohydrates and other molecules linked to the initial stages of diabetes and its progression.In addition,certain boron compounds bind to molecules related to diabetes development and their biological activity in the regulation of elevated glycemia.Finally,it should be noted that some boron compounds appear to exert beneficial effects on diabetes complications such as accelerating wound healing while ameliorating pain in diabetic patients.

Undoubtedly, since its appearance, the interest and use of autologous fat transfer (AFT) as a breast reconstruction technique have been increasing, becoming one of the main surgical alternatives for aesthetic breast augmentation. This increase in its popularity has led to the development of new technologies to increase its efficacy and safety, however, it has inherently implied the inadequate use of this procedure, mainly when performed by unqualified medical personnel. We present the case of a patient with complications following a breast AFT for aesthetic purposes, performed by a general practitioner without a specialty in plastic and reconstructive surgery.

背景锁骨上皮肤已被广泛的研究，并在面部和颈部再造术中作为带蒂皮瓣使用。锁骨上皮肤作为游离皮瓣的使用不像带蒂皮瓣那么广泛。作者对锁骨上区实施了解剖研究，目的是评估获得供区瘢痕位于锁骨上折痕处的游离锁骨上皮瓣的可行性。本文描述了作者的解剖研究和初步的临床应用结果。方法对25例（巴黎笛卡尔大学15例新鲜尸体，利用彩色乳胶注射；因斯布鲁克医科大学10例甲醛溶液固定，不注射）尸体进行解剖研究，分析游离锁骨上穿支皮瓣的血管化和可行性。皮瓣用于巴勒莫大学整形外科的2例患者，1例面部修复术，1例癌症切除后口内修复术。结果在锁骨上区域平均发现4个来自颈横动脉的穿支。静脉穿支流人表浅静脉丛，而非颈横动脉的并行静脉。2例皮瓣均成功移植。结论锁骨上皮肤的血管化依赖来自颈横动脉的皮肤穿支，并流人表浅静脉丛。以这些血管为基础，游离的锁骨上皮瓣可安全获取，瘢痕隐藏在锁骨上折痕处。此皮瓣的初步临床应用取得满意效果，提示临床应用的可能性。

Per-and polyfluoroalkyl substances(PFAS)are persistent organic pollutants of concern be-cause of their ubiquitous presence in surface and ground water;analytical methods that can be used for rapid comprehensive exposure assessment and fingerprinting of PFAS are needed.Following the fires at the Intercontinental Terminals Company(ITC)in Deer Park,TX in 2019,large quantities of PFAS-containing firefighting foams were deployed.The release of these substances into the Houston Ship Channel/Galveston Bay(HSC/GB)prompted con-cerns over the extent and level of PFAS contamination.A targeted liquid chromatography-tandem mass spectrometry(LC-MS/MS)-based study of temporal and spatial patterns of PFAS associated with this incident revealed presence of 7 species;their levels gradually de-creased over a 6-month period.Because the targeted LC-MS/MS analysis was focused on about 30 PFAS molecules,it may have missed other PFAS compounds present in firefighting foams.Therefore,we utilized untargeted LC-ion mobility spectrometry-mass spectrome-try(LC-IMS-MS)-based analytical approach for a more comprehensive characterization of PFAS in these water samples.We analyzed 31 samples from 9 sites in the HSC/GB that were collected over 5 months after the incident.Our data showed that additional 19 PFAS were detected in surface water of HSC/GB,most of them decreased gradually after the incident.PFAS features detected by LC-MS/MS correlated well in abundance with LC-IMS-MS data;however,LC-IMS-MS identified a number of additional PFAS,many known to be compo-nents of firefighting foams.These findings therefore illustrate that untargeted LC-IMS-MS improved our understanding of PFAS presence in complex environmental samples.

Inflammatory bowel disease(IBD),including Crohn’s disease(CD)and ulcerative colitis(UC),is a chronic inflammatory disease of the digestive tract with increasing prevalence globally.Although venous thromboembolism(VTE)is a major complication in IBD patients,it is often underappreciated with limited tools for risk stratification.AIM To estimate the proportion of VTE among IBD patients and assess genetic risk factors(monogenic and polygenic)for VTE.METHODS Incident VTE was followed for 8465 IBD patients in the UK Biobank(UKB).The associations of VTE with F5 factor V leiden(FVL)mutation,F2 G20210A prothrombin gene mutation(PGM),and polygenic score(PGS003332)were tested using Cox hazards regression analysis,adjusting for age at IBD diagnosis,gender,and genetic background(top 10 principal components).The performance of genetic risk factors for discriminating VTE diagnosis was estimated using the area under the receiver operating characteristic curve(AUC).RESULTS The overall proportion of incident VTE was 4.70%in IBD patients and was similar for CD(4.46%),UC(4.49%),and unclassified(6.42%),and comparable to that of cancer patients(4.66%)who are well-known at increased risk for VTE.Mutation carriers of F5/F2 had a significantly increased risk for VTE compared to non-mutation carriers,hazard ratio(HR)was 1.94,95%confidence interval(CI):1.42-2.65.In contrast,patients with the top PGS decile had a considerably higher risk for VTE compared to those with intermediate scores(middle 8 deciles),HR was 2.06(95%CI:1.57-2.71).The AUC for differentiating VTE diagnosis was 0.64(95%CI:0.61-0.67),0.68(95%CI:0.66-0.71),and 0.69(95%CI:0.66-0.71),respectively,for F5/F2 mutation carriers,PGS,and combined.CONCLUSION Similar to cancer patients,VTE complications are common in IBD patients.PGS provides more informative risk information than F5/F2 mutations(FVL and PGM)for personalized thromboprophylaxis.

The gut microbiome plays a key role in the health-disease balance in the human body.Although its composition is unique for each person and tends to remain stable throughout lifetime,it has been shown that certain bacterial patterns may be determining factors in the onset of certain chronic metabolic diseases,such as type 2 diabetes mellitus(T2DM),obesity,metabolic-associated fatty liver disease(MAFLD),and metabolic syndrome.The gut-liver axis embodies the close relationship between the gut and the liver;disturbance of the normal gut microbiota,also known as dysbiosis,may lead to a cascade of mechanisms that modify the epithelial properties and facilitate bacterial translocation.Regulation of gut microbiota is fundamental to maintaining gut integrity,as well as the bile acids composition.In the present review,we summarize the current knowledge regarding the microbiota,bile acids composition and their association with MAFLD,obesity,T2DM and metabolic syndrome.
