Knowledge and awareness of glaucoma in Mexican patients with and without glaucoma diagnosis in an Ophthalmology Referral Center

AIM: To assess and compare knowledge and awareness of glaucoma in subjects with and without glaucoma diagnosis attending an Ophthalmology Referral Center.METHODS: This cross-sectional study was conducted at Asociación Para Evitar la Ceguera in Mexico City, using a questionnaire formulated by a group of experts following the Delphi panel rules, and pre-tested in a pilot study.The questionnaire was applied and compared between:glaucoma patients, relatives of glaucoma patients and patients without glaucoma. Socio-demographic data was collected to assess correlation with the level of knowledge using Logistic regression models, estimating the odds ratios(OR), 95% confidence intervals, and P<0.05.RESULTS: Three hundred and ninety-four subjects were enrolled;with a median age of 61y. One hundred and thirty-four(34%) were patients with glaucoma, 152(38.6%)patients without glaucoma, and 108(27.4%) relatives of patients with glaucoma. Two hundred and ninety-one(73.9%) participants were aware of the term “glaucoma”.Regarding knowledge 46.7% had moderate knowledge,37.8% had poor knowledge, and 15.5% good knowledge.Overall, relatives of glaucoma patients had the highest scores, and patients without glaucoma got the lowest scores. A positive correlation was found between better knowledge and frequent ophthalmological examinations OR 2.24(P=0.02), higher education level OR 4.17(P=0.00) and having a family member with glaucoma OR 3.28(P=0.00).CONCLUSION: Awareness and knowledge of glaucoma in subjects attending an Ophthalmology Referral Center is predominantly moderate or poor. This has important implications regarding attitudes that can result in lack of follow up in ophthalmological care.

Atlantic salmon(Salmo salar)fed a carbohydrate-rich diet exhibit suboptimal growth performance,along with other metabolic disturbances.It is well known that gut microbes play a pivotal role in influencing metabolism of the host,and these microbes can be modified by the diet.The main goal of the present study was to determine the effect of feeding graded levels of digestible carbohydrates to Atlantic salmon on the distal intestine digesta microbiota at 3 sampling times(i.e.,weeks 4,8 and 12),during a 12-week trial.A low carbohydrate-to-high protein diet(LC/HP,0%wheat starch),a medium carbohydrate-tomedium protein diet(MC/MP,15%wheat starch)or a high carbohydrate-to-low protein diet(HC/LP,30%wheat starch)was fed to triplicate fish tanks(27 to 28 fish per tank).We performed an in-depth characterization of the distal intestine digesta microbiota.Further,growth parameters,liver histology and the expression of genes involved in hepatic neolipogenesis in fish were measured.Fish fed a HC/LP diet showed greater hepatosomatic and viscerosomatic indexes(P=0.026 and P=0.018,respectively),lower final weight(P=0.005),weight gain(P=0.003),feed efficiency(P=0.033)and growth rate(P=0.003)compared with fish fed the LC/HP diet.Further,feeding salmon a high digestible carbohydrate diet caused greater lipid vacuolization,steatosis index(P=0.007)and expression of fatty acid synthase(fas)and delta-6 fatty acyl desaturase(d6fad)(P=0.001 and P=0.001,respectively)in the liver compared with fish fed the LC/HP diet.Although,the major impact of feeding a carbohydrate-rich diet to Atlantic salmon in beta diversity of distal intestine digesta microbiota was observed at week 4(HC/LP vs MC/MP and HC/LP vs LC/HP;P=0.007 and P=0.008,respectively)and week 8(HC/LP vs MC/MP;P=0.04),no differences between experimental groups were detected after 12 weeks of feeding.Finally,at the end of the trial,there was a negative correlation between lactic acid bacteria(LAB)members,including Leuconostoc and Lactobacillus,with hepatic steatosis level,the hepatosomat

Objective:To investigate the vasorelaxant effect of organic extracts from Apium graveolens(A.graveolens)which is a part of a group of plants subjected to pharmacological and phytochemical study with the purpose of offering it as an ideal source for obtaining lead compounds for designing new therapeutic agents with potential vasorelaxant and antihypertensive effects.Methods:An ex vivo method was employed to assess the vasorelaxant activity.This consisted of using rat aortic rings with and without endothelium precontracted with norepinephrine.Results:All extracts caused concentration-dependent relaxation in precontracted aortic rings with and without endothelium;the most active extracts were Dichloromethane and Ethyl Acetate extracts from A.graveolens.These results suggested that secondary metabolites responsible for the vasorelaxant activity belong to a group of compounds of medium polarity.Also,our evidence showed that effect induced by dichloromethane and ethyl acetate extracts from A.graveolens is mediated probably by calcium antagonism.Conclusions:A.graveolens represents an ideal source for obtaining lead compounds for designing new therapeutic agents with potential vasorelaxant and antihypertensive effects.

Background and purpose Depression is common after stroke and is often treated with antidepressant medications(AD).ADs have also been hypothesised to improve stroke recovery,although recent randomised trials were neutral.We investigated the patterns of in-hospital AD initiation after ischaemic stroke and association with clinical and readmission outcomes.Methods All Medicare fee-for-service beneficiaries aged 65 or older hospitalised for ischaemic stroke in participating Get With The Guidelines-Stroke hospitals between April and December 2014 were eligible for this analysis.Outcome measures included days alive and not in a healthcare institution(home time),all-cause mortality and readmission within 1-year postdischarge.Propensity score(PS)-adjusted logistic regression models were used to evaluate the associations between AD use and each outcome measure.We also compared outcomes in patients prescribed selective serotonin reuptake inhibitors(SSRIs)AD versus those prescribed non-SSRI ADs.Results Of 21805 AD naive patients included in this analysis,1835(8.4%)were started on an AD at discharge.Patients started on an AD had higher rates of depression and prior ischaemic stroke,presented with higher admission National Institutes of Health Stroke Scale score and were less likely to be discharged home.Similarly,patients started on an SSRI had lower rates of discharge to home.Adjusting for stroke severity,patients started on an AD had worse all-cause mortality,all-cause readmission,major adverse cardiac events,readmission for depression and decreased home-time.However,AD use was also associated with an increased risk for the sepsis,a falsification endpoint,suggesting the presence of residual confounding.Conclusions Patients with ischaemic stroke initiated on AD therapy are at increased risk of poor clinical outcomes and readmission even after PS adjustment,suggesting that poststroke depression requiring medication is a poor prognostic sign.Further research is needed to explore the reasons why depression is associated with w

We apply a two pronged approach to analyze the presence of Environmental Kuznets Curve, EKC, in three types of emissions in the People’s Republic of China from 1994 to 2014: a structural breakpoint analysis and a model based test. We apply it to PM2.5, industrial waste and domestic water pollution time series, and find evidence of EKC in all three types of emissions that require appropriate policy.

Objective： Vascular calcification is the consequence of the complex interaction between genetic, environmental, and vascular factors, which ultimately lead to the deposition of calcium in the tunica intima （atherosclerotic calcification） or tunica media （M6nckenberg＇s sclerosis）. Vascular calcification is also closely related to other pathologies, such as diabetes mellitus, dyslipidemia, and chronic kidney disease. It has been concluded that the degree of vascular calcification may vary from person to person, even if the associated pathologies and environmental factors are the same. Therefore, this suggests an important genetic contribution to the development of vascular calcification. This review aimed to find the most recent evidence about vascular calcification pathophysiology regarding the genetic aspects and molecular pathways. Data Sources： We conducted an exhaustive search in Scopus, EBSCO, and PubMed with the keywords ＂genetics and vascular calcification＂, ＂molecular pathways, genetic and vascular calcification＂ and included the main articles from January 1995 up to August 2016. We focused on the most recent evidence about vascular calcification pathophysiology regarding the genetic aspects and molecular pathways. Study Selection： The most valuable published original and review articles related to our objective were selected. Results： Vascular calcification is a multifactorial disease; thus, its pathophysiology cannot be explained by a single specific factor, rather than by the result of the association of several genetic variants, molecular pathway interactions, and environmental factors that promote its development. Conclusion： Although several molecular aspects of this mechanism have been elucidated, there is still a need for a better understanding of the factors that predispose to this disease.

AIM：To assess the proportion of refractive errors in the Mexican population that visited primary care optometry clinics in fourteen states of Mexico. METHODS：Refractive data from 676 856 patients aged 6 to 90 y were collected from optometry clinics in fourteen states of Mexico between 2014 and 2015. The refractive errors were classified by the spherical equivalent（SE）, as follows：sphere＋？ cylinder. Myopia（SE〉-0.50 D）, hyperopia（SE〉＋0.50 D）, emmetropia（-0.50≤SE≤＋0.50）, and astigmatism alone（cylinder≥-0.25 D）. A negative cylinder was selected as a notation.RESULTS：The proportion（95% confidence interval） among all of the subjects was hyperopia 21.0%（20.9-21.0）, emmetropia 40.7%（40.5-40.8）, myopia 24.8%（24.7-24.9） and astigmatism alone 13.5%（13.4-13.5）. Myopia was the most common refractive error and frequency seemed to increase among the young population（10 to 29 years old）, however, hyperopia increased among the aging population（40 to 79 years old）, and astigmatism alone showed a decreasing trend with age（6 to 90y; from 19.7% to 10.8%）. There was a relationship between age and all refractive errors（approximately 60%, aged 50 and older）. The proportion of any clinically important refractive error was higher in males（61.2%） than in females（58.3%; P〈0.0001）. From fourteen states that collected information, the proportion of refractive error showed variability in different geographical areas of Mexico.CONCLUSION：Myopia is the most common refractive error in the population studied. This study provides the first data on refractive error in Mexico. Further programs and studies must be developed to address the refractive errors needs of the Mexican population.
