Distribution of licensed acupuncturists and educational institutions in the United States at the start of 2023,during the late stage of the COVID-19 pandemic

This study was conducted to identify the number and density of active licensed acupuncturists(LAcs), as well as the number of accredited schools in acupuncture and Oriental medicine(AOM), as of January 1,2023, in the United States(U.S.). The number of active LAcs as of January 1, 2023 was 34,524, potentially33,364 after removing license duplication in multiple states, among which the largest three states were California(with 7317 LAcs [21.19% of the total]), new York(5024 [14.55%]) and Florida(2644 [7.66%]).The total number of LAcs decreased by 8.87% from 2018, and fell short of our projected number of LAcs in 2023 by 9037, or 20.75%. The overall LAc density in the U.S.Dmeasured as the number of LAcs per 100,000 populationDwas 10.36, less than in 2018. There were 56 active, accredited AOM schools which offered a total of 147 programs(121 at the level necessary for licensing [entry-level], 12 for an advanced practicing degree [advanced-level], and 14 for certifications). Broken down further, offerings included 50 master's degrees in acupuncture, 40 master's degrees in Oriental medicine, 31 entry-level doctorate degrees(10 in acupuncture and 21 in acupuncture plus Chinese herbal medicine), and 12advanced-level doctorate degrees in AOM. The certification programs included one in East-Asian Medical Bodywork and 13 in CHM. Among these schools in 2023, institutions in the West and East Coast states comprised 67.86%(decreased from 77.42% in 2018) of the national total. California, Florida and Illinois represented 39.29%. There were 48 jurisdictions with acupuncture practice laws in place.The data suggests that the acupuncture profession in the U.S. has been significantly impacted during the coronavirus disease 2019 pandemic.

BACKGROUnD Infective endocarditis(IE)is a life-threatening infection with an annual mortality of 40%.Embolic events reported in up to 80%of patients.Vegetations of>10 mm size are associated with increased embolic events and poor prognosis.There is a paucity of literature on the association of multiple vegetations with outcome.AIM To study the echocardiographic(ECHO)features and outcomes associated with the presence of multiple vegetations.METHODS In this retrospective,single-center,cohort study patients diagnosed with IE were recruited from June 2017 to June 2019.A total of 84 patients were diagnosed to have IE,of whom 67 with vegetation were identified.Baseline demographic,clinical,laboratory,and ECHO parameters were reviewed.Outcomes that were studied included recurrent admission,embolic phenomenon,and mortality.RESULTS Twenty-three(34%)patients were noted to have multiple vegetations,13(56.5%)were male and 10(43.5%)were female.The mean age of these patients was 50.Eight(35%)had a prior episode of IE.ECHO features of moderate to severe valvular regurgitation[odds ratio(OR)=4],presence of pacemaker lead(OR=4.8),impaired left ventricle(LV)relaxation(OR=4),and elevated pulmonary artery systolic pressure(PASP)(OR=2.2)are associated with higher odds of multiple vegetations.Of these moderate to severe valvular regurgitation(P=0.028),pacemaker lead(P=0.039)and impaired relaxation(P=0.028)were statistically significant.These patients were noted to have an increased association of recurrent admissions(OR=3.6),recurrent bacteremia(OR=2.4),embolic phenomenon(OR=2.5),intensive care unit stay(OR=2.8),hypotension(OR=2.1),surgical intervention(OR=2.8)and device removal(OR=4.8).Of this device removal(P=0.039)and recurrent admissions(P=0.017)were statistically significant.COnCLUSIOn This study highlights the associations of ECHO predictors and outcomes in patients with IE having multiple vegetations.ECHO features of moderate to severe regurgitation,presence of pacemaker lead,impaired LV relaxation,and elevated PASP and outcomes

Background: The incidence of intracranial metastases (ICMET) has been steadily rising, and its frequency with respect to primary brain tumours is relatively high. Objective: The objectives of this study were to elucidate the current epidemiology and describe the clinical, diagnostic and therapeutic features of ICMET in Yaounde. Method and findings: A descriptive cross-sectional study was done in the neurosurgery departments of the General and Central Hospitals of Yaounde during the period from January 2016 to December 2022. We included all medical booklets of patients admitted for a tumoral intracranial expansive process with our target population being patients with histological evidence of ICMET, and did a retrospective inclusion of data using a pre-established technical form aimed at collecting sociodemographic data, clinical data, paraclinical data, and the treatment procedures. Analysis was done using the SPSS statistical software. A total of 614 cases of intracranial tumors were included among whom 35 presented histological evidence of ICMET. This gives a frequency of 5.7%. The sex ratio was 0.94, the mean age was 55.68 +/- 14.4 years, extremes 28 and 86 years and the age range 50 - 59 was affected in 28.57% of cases. The clinical presentation included signs of raised intracranial pressure (headache, blurred vision, vomiting) in 26 cases (74.3%), motor deficit 48.6%, seizures 17.1%. The mode of onset was metachronous in 71.4% and synchronous in 28.6%. The imaging techniques were cerebral CT scan in 82.9%, cerebral MRI in 40%, TAP scan in 22.9%. The metastatic lesions were supratentorial in 94.3% and single in 62.9%. The primary cancers found were breast cancer (31.4%), lung cancer (25.7%), prostate cancer (17.1%), thyroid cancer (5.7%), colon cancer (2.9%), and melanoma (2.9%). The therapeutic modalities were total resection (68.6%), radiotherapy (37.1%). Conclusion: Intracranial metastases are relatively frequent. Introduction: Metastatic spinal tumors (MST) refer to secondary involvement of the vertebral column by hematogenously-disseminated metastatic cells. They could affect either the bony structures or the spinal cords. Mechanical instability and neurologic deficits resulting from spinal cord compression are the most common manifestations. Surgical intervention remains the most effective treatment for about 20% of patients who present with spinal cord compression. The prognosis is relatively poor. This work has as objectives to describe: the diagnostic tools, the different modalities of management and the prognostic elements of spine metastasis. Methodology: We conducted an ambispective cross-sectional descriptive study;with retrospective data collection from January 2015 to December 2021 and prospective collection from January to April 2022 in the “neurosurgery” unit of the Yaounde Central Hospital and the “Oncology and neurosurgery” units of Yaounde General Hospital. Result: We included 101 patients. The M/F sex ratio was 1.66. The average age of the participants was 56.44 years (±14.19 SD) with a median of 58 years. Metastatic spinal tumors were discovered in 61.39% of patients with a previously known primary tumor and 21.78% of patients had newly discovered tumors. The neurologic examination revealed a vertebral syndrome in 79.21% of cases, radicular syndrome in 60.40% and sub-lesional syndrome in 59.89%. Sensory disorders accounted for 39.60% and sphincter disorders accounted for 34.65%. According to the degree of severity, the lesions were classified as Frankel E (37.62%) followed by Frankel D (21.78%). Metastatic lesions were mostly found at the thoracic vertebrae (68.25%) and lumbar vertebrae (22.22%). The most represented primary tumors were: prostate tumors (41.58%) and breast tumors (23.76%);followed by malignant hemopathies (15.84%). 为推进长江经济带耕地保护和绿色低碳发展,利用当量因子法、环境成本法等测算耕地生态服务价值和负外部性价值,结合耕地生态足迹和生态承载力,将长江经济带11省(直辖市)划分为盈余区、平衡区与赤字区,对耕地生态外溢价值进行评估,并分析时空变化特征。结果显示:(1)2011-2021年长江经济带耕地生态价值总量呈上升趋势,由3670.37亿元上涨至4661.55亿元,耕地生态外溢价值缓慢上升,由1353.82亿元上涨至1970.77亿元。(2)长江经济带耕地资源整体处于盈余状态,耕地生态价值总体呈现出“东低西高”的态势,四川省耕地生态价值和盈余量最高,上海市耕地生态价值最低。(3)2011-2021年间,浙江省和上海市处于耕地生态赤字区,四川省处于耕地生态高盈余区,重庆市和贵州省处于耕地生态平衡状态,江西省、安徽省和江苏省耕地生态盈亏无显著变化,云南省和湖北省耕地生态盈余水平有所上升,湖南省耕地生态盈余水平有所下降。基于此,长江经济带各省(直辖市)应充分利用自身资源和优势,制定和实施适应性的耕地保护策略,推动耕地资源的可持续利用,为实现长江经济带的生态优先、绿色低碳发展目标做出贡献。

Globally,urbanization and a steady increase in population generate a huge amount of wastes,which leads to a series of economic,social,and environmental changes,mainly in developing countries.There is an utmost need for efficient management strategies for the beneficial utilization of these wastes into useful products.Among these strategies,composting is gaining attention due to its benefits of solid waste management,such as proper sterilization,and economical and effective bioconversion of lignocellulosic wastes to valuable products.Composting is an effective and sustainable approach for the management of various lignocellulosic wastes.This process comprises a series of effective waste treatment steps to ensure sustainable agriculture.Different composting methods have been explored for solid waste management.Furthermore,the influence of various factors relevant to composting has been elucidated.Microbes play a significant role in enhancing the degradation of lignocellulosic residues by secreting different hydrolytic enzymes.Compost has been utilized for increasing soil properties and improving plant growth.

Fog computing is an emergent and powerful computing paradigm to serve latency-sensitive applications by executing internet of things(IoT)appli-cations in the proximity of the network.Fog computing offers computational and storage services between cloud and terminal devices.However,an efficient resource allocation to execute the IoT applications in a fog environment is still challenging due to limited resource availability and low delay requirement of services.A large number of heterogeneous shareable resources makes fog computing a complex environment.In the sight of these issues,this paper has proposed an efficient levy flight firefly-based resource allocation technique.The levy flight algorithm is a metaheuristic algorithm.It offers high efficiency and success rate because of its longer step length and fast convergence rate.Thus,it treats global optimization problems more efficiently and naturally.A system framework for fog computing is presented,followed by the proposed resource allocation scheme in the fog computing environment.Experimental evaluation and comparison with the firefly algorithm(FA),particle swarm optimization(PSO),genetic algorithm(GA)and hybrid algorithm using GA and PSO(GAPSO)have been conducted to validate the effectiveness and efficiency of the proposed algorithm.Simulation results show that the proposed algorithm performs efficient resource allocation and improves the quality of service(QoS).The proposed algorithm reduces average waiting time,average execution time,average turnaround time,processing cost and energy consumption and increases resource utilization and task success rate compared to FA,GAPSO,PSO and GA.
