Serious Myasthenia at the Department of Internal Medicine of Point G University Teaching Hospital: A Clinical Case

Introduction: Myasthenia is a rare and disabling autoimmune disease. Few studies were devoted to this pathology. We report a clinical case of myasthenia in the Department of Internal Medicine at the Point G University Teaching Hospital. Observation: This is a 41-year-old patient of Malian origin, a trader with a history of hemorrhoidectomy in 2011 and familial hypertension, was admitted on August 19, 2014, for muscle weakness, dysphonia, and dysphagia. The disease started 1 year before admission at the department with progressive muscle weakness of the upper limbs aggravated by repetitive movements and spreading to the head “drooping head” and inferior limbs, associated with general fatigue, dysphonia and selective dysphagia for solid foods. The diagnosis of myasthenia was maintained based on the clinical signs such as ptosis, bilateral diplopia, and weakness with lower limbs muscle strength rated at 3/5, and confirmed with confirmatory exams;a positive anti-acetylcholine receptor antibody (RIA), the post-synaptic neuromuscular conduction block at ENMG and the positive neostigmine pharmacological test. The patient received Neostigmine (prostagmine) 0.5 mg one ampoule in IM/day and Prednisone at a dosage of 1 mg/kg/day. The evolution was marked by a moderate improvement of the symptomatology afterward the patient was evacuated to Tunisia on family request where he received a course of immunoglobulin 2 g/kg in 2 days. The evolution in Tunisia was favorable. The patient returned in Mali and death occurred after 2 months in a context of respiratory distress. Conclusion: Myasthenia is a rare but serious disease requiring careful management and monitoring to reduce respiratory complications.

公共市场因其经济重要性而成为实施发展政策的重要支柱。然而,公共合同很容易受到腐败的影响。为了根除这一祸害,许多人主张在公共采购中遵守透明原则。本论文分析了几内亚共和国公共采购系统的透明度原则。然而,由于一系列与公共采购程序的复杂性和长度相关的因素,公共采购极易受到腐败的影响。一般而言,公共采购系统中的腐败表现在贪污腐败、权力兜售、利益冲突、徇私舞弊、多收费用、不履行合同、贿赂、非法采购受众等方面。这些腐败行为不仅对国民经济产生重大影响,而且还加剧了所有公民对领导人和公共机构的不信任。腐败是任何国家政治、经济和社会发展的重大障碍。根除这一祸害需要实施有效的政策。透明度作为公共采购的重要支柱之一,一方面被揭示为促进良好经济治理的工具。然而,它必须完全适用。此外,在本案文的背景下,这是一个将公共采购透明度作为打击腐败的工具的问题。应当指出,公共合同,鉴于其对公共行政的重要性,受法规的约束。因此,在整个文本中,我们试图证明公共采购的透明度在多大程度上有助于打击腐败,同时强调管理腐败的立法。为此,我们首先介绍了透明度的含义以及它如何通过立法应用于公共采购。然后,我们讨论了透明度作为公共采购中其他三个相互关联的重要支柱的衔接,即竞争、诚信和补救权。显然,这些措施的整体应用可以极大地促进根除腐败现象。最后,通过强调公民参与、民间社会在反腐败斗争中的贡献,我们简要介绍了透明度作为一种手段,使民众能够控制公共支出,特别是与公共合同相关的支出,从而使各种行为者承担责任.

Hemoglobinopathy and Systemic Lupus: A Rare Association

The authors report a case of systemic lupus associated with a composite heterozygosis SC with thalassemic component in a 19-year-old patient hospitalized for anemia and polyarthralgia who has a staturoponderal delay, macular erythematous lesions in butterfly wings on the face and ears, photosensitivity, puffy face, alopecia, pubic and axillary hair loss, scalp dermatophytosis, painful swelling of the interphalangeal joints, wrists and knees. The hemoglobin electrophoresis showed a compound heterozygote SC associated with β thalassemia. Antinuclear antibodies were positive with an anti-Sm positive antibody. Conclusion: The diagnosis of both diseases can be difficult when symptoms are concomitant and look alike.

Engaging with the More-Than-Human and Decolonial Turns in the Land of Shinto Cosmologies: “Negative” Comparative Education in Practice

Purpose:In this explorative,self-reflective article,I attempt to extend the methodological discussion of a“negative”approach to comparative education that I have recently articulated elsewhere.Here,I demonstrate how I attempted to put in practice negative comparative education by drawing on my experience at the Shanghai workshop,Beyond the Western Horizon in Educational Research,organized by Iveta Silova,Jeremy Rappleye,and Yun You at the East China Normal University.Design/Approach/Methods:I conceive my participation in the Shanghai workshop as a disruptive moment wherein my previous forms of knowing and being were challenged.More specifically,I discuss how my attempt to situate the ecofeminist and decolonial literature,the two of the three main bodies of literature introduced through the workshop,within the context of Japanese education triggered me to reconsider the role of Shinto in Japanese education and question my own firm identification with the liberal–left politics within Japan.Findings:I reflect upon how the initial sense of discomfort and the remaining sense of ambivalence took me on a journey of unlearning and relearning about the limits of my knowing,my own political subjectivities,and the place of Shinto cosmologies in Japanese education.Originality/Value:I conclude the discussion with a few provisional thoughts that point us to an approach to comparative education that makes our research“a matter of concern”across multiple linguistically bounded,national and regional scholarly communities.

本文对联合国马里多层面综合稳定团(MINUSMA)进行了批判性分析,特别是通过分析其部署于2013年7月至2017年底的目标,进行的任务,挑战和成就。近年来,马里正面临一场深刻的危机及由此产生的严重政治、安保、社会经济、人道主义和人权后果。2012年3月22日,在马里北部被武装团体打败的部队中一些心怀不满的士兵发起兵变,导致军事政变。安保局势在2013年1月初严重恶化。伊斯兰捍卫者组织和西非圣战统一运动人员在伊斯兰马格里布基地组织支持下,向南推进。安全理事会第2110号决议决定设立联合国马里多层面综合稳定团(马里)。根据该决议,该稳定团将支持政治进程,并开展若干与安保相关的稳定任务,将重点放在主要人口中心和通信线路、保护平民、监测人权、创造提供人道主义援助和让流离失所者返回的条件、扩大国家权力和为自由、包容、和平的选举做准备。自2012年1月马里北部冲突爆发以及2012年3月22日政变引发的体制危机以来,马里联合国在区域,非洲大陆甚至是国际层面进行不断的努力,寻求持久的解决办法。马里稳定团有责任依照联合国安理会自2013年4月以来通过的各项决议的规定,稳定马里北部,维持武装团体的驻扎和解除武装以及恢复该国国家主权。本文发现马里稳定团在执行其任务时面临许多挑战。马里北部和中部越来越不安全,叛乱已在拖入普遍的民族冲突。签协议时没有考虑到冲突的真正原因,于是实施阿尔及尔协议仍然是一个大问题。阿尔及尔协议不是带来和平,而是促成人口分裂。本文对马里独立以来的前图阿里格叛乱事件进行了简要介绍,它强调了他们解决的原因和方式。在阐述了联合国部队在其任务中所涉及的困难之后,本文评估其参与维持马里和平的影响。本文最后对马里局势作出结论,并对未来的维和行动提出一些有用的建议。

Gastric adenocarcinoma of fundic gland type with signet-ring cell carcinoma component: A case report and review of the literature

A depressed lesion was found at a gastric angle of 76-yearold Japanese woman by esophagogastroduodenoscopy. Four years prior, she was diagnosed with a Helicobacter pylori infection but no eradication was performed. The pathological diagnosis of biopsy specimens was signet-ring cell carcinoma. Endoscopic submucosal dissection(ESD) was performed. Histopathological examination of the ESD specimen revealed proliferation of well-differentiated tubular adenocarcinoma mimicking fundic gland cells at the deep layer of the lamina propria mucosae. These tumor cells expressed focally pepsinogen-Ⅰ, diffusely MUC6, and scattered H^+/K^+ ATPase according to immunohistochemistry. Therefore, we diagnosed this tumor as gastric adenocarcinoma of fundic gland type(GA-FG). Adjacent to the GA-FG, proliferation of signet-ring cell carcinoma which diffusely expressed MUC 2 and MUC 5AC was observed. Intestinal metaplasia was focally observed in the surrounding mucosa of the signet-ring cell carcinoma. To the best of our knowledge, this is the first case report of GA-FG with a signet-ring cell carcinoma component. The origin of signet-ring cell carcinoma, i.e., whether it accidentally arose from a non-neoplastic mucosa and coexisted with the GA-FG or dedifferentiated from the GA-FG is unclear at present. We expect the accumulation of similar cases and further analysis to clarify this issue.

在图论中,图G和H的字典序积(或合成图)G ·H是一个图,使得:G·H的顶点集是笛卡儿积V(G)×V(H)G· H中任何两个顶点(u,v)和(x,y)相邻当且仅当u与x在G中是相邻的或u = x且v与y在H中是相邻的。图G的厚度t(G)是将G分解成不交的平面子图的并的最少平面子图数。确定任意图的厚度是NP困难的,因此很难获得一个图的精确的厚度值。在本文中,我们将研究两条不同路图的字典序积的厚度以及路图和完全图的字典序积的厚度。第一章主要介绍图的厚度的起源还有本文所需的一些基本知识。第二章主要通过引入平面图及研究平面性的一些已知的结果。第三章主要介绍两类特定的图:图运算将应用的路图和完全图。第四章主要介绍字典序积的基本结构,展示其他图乘积的一些特殊性。在第五章主要研究两条路图的字典序积的厚度以及路图和完全图的字典序积的厚度。

电信领域的自由化使得马里巴马科的两家运营商“Orange-Mali”和“SOTELMA-Malitel”之间形成形成一种竞争状态。运营商“Orange-Mali”在此种竞争动态下表现更加突出,其客户数量大幅度增加,也就是说目前部署的政治策略吸引了更多的客户。在此情况下,用户数量大量增加,有助于电信公司增强对消费者的了解,做出新的评估。顾客满意度是提升顾客忠诚度的一个很好的手段,特别是在商业领域。因此,这项工作的总体目标是对巴马科电信领域的客户满意度进行分析。为了实现这一目标,本研究主要集中在400个受访的消费者样本,据此收集在马里巴马科电信部门使用“Orange-Mali”移动网络的用户满意程度的信息。根据已提出的研究问题,引导研究的进展。研究人员采用不同的工具、技术、方法和设备,例如:问卷调查和观察,从而适当的开展研究是很重要的。同时,我们对获取到的信息进行数据分析以对获取的结果进行解读、形成结论。在我们的研究中,结果完全以受访者的反馈为根据,研究人员判定这些受访者的反馈是充分并且具有代表性的,足以让研究者根据结果进行概括。这些信息对于统计分析也是完全足够的。之后,通过问卷调查获得的数据,我们根据受访者信息进行处理;在A部分,我们根据受访者的人口统计信息展示和分析反馈数据;在B部分,我们根据问卷上客户满意度的相关问题来分析这些数据。大多数参与者是男性受访者;400名参与者中有255人是男性,145人是女性。受访者客户被要求用“1～5”的程度表示来回答第9到31个问题,“1”表示完全反对,“5”表示完全同意。大约60%的受访者认为“Orange-Mali”的工作人员知识渊博,随时可以为客户提供服务,并且可以对客户信息进行保密,他们可以满足客户的需要;受访者感觉他们可以信任“Orange-Mali”的工作人员并且认为他们受到了一线工作人员的特别关注。研究结果表明,“Orange-Mali”的客户对于整体客户体验感到满意。在建议方面,研究人员建议:为了更好地了解并提高客户的满意度和期望值,“Orange-Mali”必须最终建立客户忠诚度并吸引更多消费者。

Systemic RNAi of V-ATPase subunit B causes molting defect and developmental abnormalities in Periplaneta fuliginosa

The vacuolar (H+)-ATPases (V-ATPases) are ATP-driven proton pumps with multiple functions in many organisms. In this study, we performed structural and functional analysis of vha55 gene that encodes V-ATPase subunit B in the smokybrown cockroach Periplaneta fuliginosa (Blattodea). We observed a high homology score of the deduced amino acid sequences between 10 species in seven orders. RNAi of the vha55 gene in R fuliginosa caused nymphal/nymphal molting defects with incomplete shedding of old cuticles, growth inhibition, as well as bent and wrinkled cuticles of thoraxes and abdominal segments. Since growth inhibition caused by vha55 RNAi did not interfere in the commencement of cockroach molting, molting timing and body growth might be controlled by independent mechanism. Our study suggested V-ATPases might be a good candidate molecule for evolutionary and developmental studies of insect molting.
