Background: Coma is a highly frequent situation associated with several diseases. Objective: That is to assess the prognostic factors of comas at the intensive care unit of the university teaching hospital of Parakou. Patients and Methods: It is a cross-sectional study with descriptive and analytical purpose based on prospective gathering of data. It has been performed from March 1 to July 31, 2014. The population study consists of 100 adult patients admitted to the intensive care unit for coma. The vital and functional prognosis data are studied. Results: The admission frequency of comatose patients to the intensive care unit is 44.4%. Patients’ mean age is 27 ± 13.9 years with extremes of 16 and 88 years. The average length of stay in the intensive care unit is 5.2 ± 5.6 days with extremes of 2 hours 30 minutes and 30 days. Mortality rate is 30%. Poor prognostic factors are: average Glasgow score of 6.8 ± 2.9 (p = 0.01), average outpatient simplified severity index (OSSI) of 9.1 ± 3.5 (p = 0.02), pupillary abnormality (p = 0.00), motor weakness (p = 0.00), average glycemia of 1.5 ± 0.82 g/L (p = 0.02), average systolic blood pressure of 130.9 ± 44.5 mm Hg (p = 0.03), average diastolic blood pressure of 80.8 ± 22.3 mm Hg (p = 0.03), average respiratory rate of 30.9 ± 10.6 cycles/min (p = 0.03) and average temperature of 37.8°C ± 1.2°C (p = 0.03). Conclusion: Coma is a pathological entity with high frequency in clinical medicine. Its positive diagnosis is easy, but etiological research is hard. Hospital mortality is high and influenced by several factors. Taking into account, those factors in care strategies can improve their prognosis.

Prevalence and Factors Associated with Overweight and Obesity among Public Secondary School Teachers in Parakou, Benin, in 2021

Introduction: Overweight and obesity represent a public health problem in Africa due to the epidemiological transition. The objectives of this work were to determine the prevalence of overweight and obesity and to identify associated factors among public secondary school teachers in Parakou, Benin in 2021. Methods: We conducted a descriptive and analytical cross-sectional study. Teachers working in public secondary schools in Parakou during the 2020-2021 academic year, present at work and who gave their written informed consent, were included. A two-stage random sampling was carried out. Data were collected during an individual interview using a questionnaire followed by the measurement of anthropometric parameters and blood pressure. Overweight and obesity were defined by a body mass index ≥ 25 kg/m2. Multivariable logistic regression was performed to identify associated factors. Results: A sample of 325 teachers, including 88.6% of men, was recruited with an average age of 36.2 ± 6.8 years. The prevalence of overweight and obesity was 43.7% (95% CI [38.1%-44.8%]). It increased significantly with income (p Conclusion: The results show a high prevalence of overweight and obesity. Actions are necessary to prevent overweight and obesity among secondary school teachers in Parakou, in Benin.

Practice of Barrier Measures against COVID-19 Among Diabetic People Followed at Departmental University Hospital Center Ouémé-Plateau, Benin, in 2021

Introduction: COVID-19 is a pandemic disease caused by the coronavirus SARS-COV2 appeared in China in 2019. The aim of the study was to evaluate the practice of barrier measures against COVID-19 and to identify associated factors among diabetics followed at Departmental University Hospital Center Ouémé-Plateau, Benin, in 2021. Methods: This was a cross-sectional study conducted from June to August 2021, including diabetics aged 18 and over, hospitalized or received for medical visits in the department of medicine at the target hospital. Data were collected during an interview with each participant thanks to a questionnaire. “Good practice” was defined as regular practice of at least two of three barrier measures: hand washing, physical distancing and mask-wearing. Results: A total of 110 diabetics were included with a female predominance (55.5%) and a mean age of 57 ± 11.3 years. Among them, 89.1% washed their hands, 41.8% respected physical distancing and 12.7% wore a mask. The proportion of good practice was 47.3%. Only 5 practiced the three barrier measures. Only age group was associated with good practice. Conclusion: The study showed a low proportion of good practice of barrier measures against COVID-19 among diabetic people surveyed. It is important to continue the action for COVID-19 prevention among diabetic people in the targeted hospital and in other hospitals in Benin.

Background and Objective: HIV infection is a major global Public Health threat worldwide, particularly in Sub-Saharan Africa of which Benin. The level of knowledge determines the attitudes and behaviors of the populations towards this infection. The study objective was to assess knowledge, attitudes and practices related to HIV infection among motorbike taxi drivers (MTD) in Parakou in 2021. Methods: This was a descriptive cross-sectional study targeting MTD in Parakou in 2021. Participants were selected by cluster sampling. Pretested Digitized questionnaire using KoboCollect@ applicationserved as a data collection tool. Knowledge, attitudes and practices variable were treated on a score scale. A knowledge score was considered to reflect a good knowledge of HIV if at least two-thirds of the knowledge statements had been correctly answered provided the subject recognized the sexual route as one of modes of HIV transmission, identified at least one preventive measure and meant the incurability of the disease. Quantitative and qualitative variables were appropriately described using the EPI Info 7.***.3.3 software. The participant was classified at positive attitude/practice for HIV prevention, when it has a score of at least 80% and suggests a good preventive measure face a risk of exposure to HIV. Results: A total of 374 subjects were recruited into the study. The mean age was 31.51 ± 7.76 years. Most participants (86.06%) had good knowledge of condom use as an HIV prevention method. The sources of information mentioned were mainly the media (77.07%), relatives or friends (63.38%), and field-workers from non-governmental organizations (37.26%). Routine HIV testing was 50.53%. Among participants, 76.10% reported at least two different sexual partners. Condom use was 59.18 % during the casual sexual intercourse. Accessibility to Safe Drinking Water and Diarrheal Diseases: A Quasi-Experiment on a Case of Water Well Drilling in the Village of Kassouala, Municipality of Tchaourou, Benin, January 2018-July 2019

Background: In 2017, 900 million people in the world did not have sustainable access to safe drinking water (SDW). In addition, between 2016 and 2020, the global population with safely managed drinking water at home increased from 70 percent to 74 percent. Drinking water insecurity is the daily situation of people in developing countries. The lack of SDW supply is at the root of many diseases, including diarrheal diseases. Kassouala is a village in the municipality of Tchaourou without access to SDW, but having benefited from the drilling of a well in September 2018. The objective of this study was to study the effect of access to safe drinking water on the frequency of diarrheal diseases in Kassouala between January 2018 and July 2019. Methods: We conducted a quasi-experimental study in Kassouala using the natural experiments of the village of Bérétou as a control group for estimating the effect of a causal nature. There were double temporal (January 2018-July 2019) and geographical (Kassouala-Bérétou) comparisons based on data collected from health care registers. A population-based comparability survey of the two villages was conducted among 170 households in each village (experimental village, control village). A two-stage cluster sampling procedure was used to select the survey participants. Data were collected from heads of household by semi-structured questionnaire. We used Pearson or Mantel-Haenszel chi-square tests, as appropriate, and the “difference-in-difference” method to assess the effect. Results: In Kassouala, the proportion of households with access to safe drinking water had increased to 78.88%, whereas it was nil in 2018 before the well drilling, and the frequency of diarrheal diseases decreased significantly from January 2018 before drilling to July 2019 after drilling (57.11% to 44.64%;p Conclusion: Access to safe drinking water in Kassouala has a causal effect on the reduction of diarrheal diseases. Introduction: Gynaecological and breast cancers (GBC) are the most frequent and most serious cancers of women. They are frequent in Benin with an average of 108.5 cases per year. Objective: To identify factors associated with women’s knowledge, attitudes and practices regarding GBC. Methods: This was a cross-sectional, prospective, descriptive and analytical study. Women of childbearing age residing in the commune of Glazoué in Benin for at least six months were included. The WHO cluster sampling technique was used. The Bruno De Finetti postulate and the Likert scale were used to study women’s knowledge, attitudes and practices. Data were collected and analysed using Epi Data 3.1 and Epi info 7.***.2.6 software. Results: A total of 540 women were registered. The mean age was 27.1 ± 8.84 years with extremes of 15 and 49 years. They were married (61.1%), had secondary education (40.2%) and lived in urban areas (61.9%). They had a low level of knowledge (75.4%), an adapted attitude (93.0%) and poor practice (88.9%). Occupation (p = 0.000), level of education (p = 0.007) and place of residence (p = 0.001) significantly influenced knowledge of GBC. The attitude was adequate (93%) and the practice was poor (88.9%). Inappropriate attitude was significantly associated with the level of education (p = 0.006). Conclusion: Women in the commune of Glazoué in Benin have a low level of knowledge of GBC with an adapted attitude and poor practice. Several factors are associated, hence the need to take them into account to facilitate GBC screening.

This study was conducted on the Ngaoundaba Crater Lake sediments to infer provenance,weathering conditions,organic matter accumulation,and trace metal *** Lake sediments were collected using a manual core sampler at 5 to 8 m water *** sediment cores from the littoral and center of the lake were analyzed for grain size distribution,water content(WC),organic matter content,mineralogy,and major and trace element *** Ngaoundaba sediments were classified as silt and sandy *** show high content in organic matter,which is more to the littoral than to the center of the lake,varying from 14.6%to 24%and21.2%to 40.8%,*** grain surface features identified by Scanning Electron Microscopy(SEM)show both chemical and mechanical microtextures with subrounded to angular shape suggesting both proximal and distal *** lake sediments are composed of quartz,kaolinite,and hematite,with low amounts of feldspars,rutile,calcite,illite,and *** indices such as the chemical index of alteration(CIA),chemical index of weathering(CIW),and index of compositional variability(ICV)indicate moderate to intense chemical weathering in the source area and immature to mature *** geochemical composition indicates that the sediments were derived from felsic to intermediate igneous rocks,such as granitoids,and mafic alkali lavas like *** environmental risk assessment of trace metals obtained by enrichment factor(EF)and geo-accumulation index(I-geo)shows low contamination of the lake sediments.

Epidemiology of Lower Extremity Artery Disease in People Living with HIV Followed at the Departmental University Hospital Center Ouémé-Plateau in 2019

Introduction: Lower extremity artery disease (LEAD) is a serious cardiovascular disease. People living with the human immunodeficiency virus (HIV) are at risk. The aim of the study was to determine the prevalence of LEAD and identify the associated factors among people living with HIV who were followed at the departmental university hospital Ouémé-Plateau in Benin. Methods: This was a cross-sectional study. It included all HIV-infected people who were monitored at the department of medicine of the target hospital during the study period and met the inclusion criteria (followed for at least three months, aged at least 25 years, and having given their written consent to participate). Data were collected during an individual interview, followed by the measurement of parameters. The ankle brachial index ≤ 0.9 was used for the diagnosis of LEAD. Associated factors were searched through a multivariable logistic regression. Results: The prevalence of LEAD was 34.2% among 222 respondents having a mean age was 42.9 ± 10.8 years and a female predominance (77.5%). No significant association was found between the presence of LEAD and sociodemographic factors. The presence of LEAD was significantly associated with hypertension and antiretroviral therapy. Hypertensive patients had a higher risk of LEAD compared to non-hypertensive patients (OR = 1.98, 95% CI [1.04-3.83], p = 0.037). Those who were receiving second-line therapy also had a higher risk of LEAD compared to those on first-line therapy (OR = 2.95, 95% CI [1.14-7.60], p = 0.025). Conclusion: This study showed a high prevalence of LEAD especially among hypertensive patients and those who were receiving second-line antiretroviral therapy. LEAD diagnosis and management should be included in the routine care of people living with HIV in Benin.

Intracerebral Hemorrhage in Pointe a Pitre Hospital

Background: In caribbean space stroke incidence is 50% higher than in Europe. This study aims to describe characteristics of intracerebral hemorrhage in Guadeloupe. Methods: This was a retrospective study of hospitalized patients with a no-traumatic intracerebral hemorrhage during one year from 2013 to 2014. Survivors were contacted by telephone at the end of the study and;if accept, vital status and Rankin scale were assessed. Results: In one year, 1418 patients were hospitalized for an acute stroke. 112 had no-traumatic intracerebral hemorrhage (7.9%). The sex ratio F/H was 0.72. Mean age of the population was at 63.0 ± 14.7 years. Hypertension and diabetes were the leading risk factors notice with respectively 62 and 25 patients. Intracerebral hemorrhage distribution was: deep 48.2%, lobar 30.4%, sub-tentorial 17.9%, meningeal 3.5%. Severity signs distribution were mass effect 79%, ventricular contamination 43% and cerebral herniation 37%. Mean during of in-hospital stay was 10.6 ± 10.3 days and in-hospital mortality was 22 patients (19.6%). A year and a half after emergency 5 additional death (4.5%) between 35 patients survivors was noticed with median Rankin scale 1.5. Conclusion: Intracerebral hemorrhage is less frequent in African American as compared to Caucasian;the epidemiological excess of strokes in this population is related to ischemic strokes.

Diabetes in Borgou Department in Benin: Prevalence and Associated Factors

An epidemic trend is observed in the evolution the epidemiology of diabetes mellitus worldwide. In a nationwide survey conducted in 2008, prevalence of diabetes was estimated to 2.6% in the hole Beninese population and to 4.6% in Borgou department representing the highest in the country. Aim: To determine the prevalence of diabetes mellitus in the adult population and investigate associated factors in Borgou department. Method: A survey was conducted from September 30th to November 20th 2011. In a random sampling using the STEP wise approach of surveillance of chronic diseases recommended by the World Health Organization, 4597 subjects over 18 years old (1935 men and 2662 women) were selected. After informed consent, every subject was submitted to the STEP questionnaire and anthropometric measures. Capillary blood was drawn in fast condition to determine glycaemia. Diabetes was defined by fasting glycaemia above 1.26 g/l. Chi square test was used for statistical analysis and difference was considered significant with p < 0.05. Results: Prevalence of diabetes was 12.4%. There was not a significant difference between men (8.5%) and women (9.4%), p = 0.33. Prevalence of previously known diabetes was 1.2% suggesting a large proportion of undiagnosed diabetes. Prevalence of diabetes increased significantly with age (p < 0.0001). There was no association with instruction level (p = 0.21). Other factors significantly associated with diabetes were place of residency (p < 0.0001), ethnicity (p = 0.002), marital status (p < 0.0001), obesity (p = 0.017) and high blood pressure (p < 0.0004). Conclusion: There is a rapid increase in diabetes prevalence in Borgou department in Benin confirming the epidemic trend of the disease. A large proportion of these diabetic subjects remained undiagnosed, so untreated and then, exposed to precocious chronic complications and precocious mortality.
