Vitamin D: Level of Vitamin D3 in AFB Positive PTB Patients in Initial Diagnostic Phase

Background: A relationship among vitamin D and pulmonary tuberculosis. Objective: To conclude the occurrence of Vitamin D insufficiency in subjects suffering from pulmonary TB. Design: This was a cross sectional study. Setting: Current study was performed at department of medicine, Peoples Medical College Hospital Nawabshah starting from January 2017 to December 2017. Samples: 385 AFB positive subjects of pulmonary tuberculosis after achieving the criteria of selection were recruited. Material and Methods: After a brief discussion, the patients were characterized for variable evaluation such as age, gender, sputum AFB and duration of disease and presence of low vitamin D levels. Sputum AFB criterion was used for Data collection. Fasting blood samples were collected for analysis of vitamin D levels in all subjects of AFB positive pulmonary tuberculosis. Results: In 385 diagnosed patients with smear positive PTB, 273 (70.9%) males and 112 (29.1%) were female. Vitamin D levels were within normal or sufficient range in 134 (34.8%), below normal range in 251 (65.2%) cases (insufficient in 09.56% (24/251) and deficient in 90.43% (227/251) cases). Conclusion: Decreased levels of vitamin D were common in subjects with AFB smear Positive PTB cases.

Effect of harmless acute pancreatitis score, red cell distribution width and neutrophil/lymphocyte ratio on the mortality of patients with nontraumatic acute pancreatitis at the emergency department

BACKGROUND: Harmless acute pancreatitis score(HAPS), neutrophile/lymphocyte ratio and red blood cell distribution width(RDW) are used to determine the early prognosis of patients diagnosed with nontraumatic acute pancreatitis in the emergency department(ED).METHODS: Patients diagnosed with acute pancreatitis(K 85.9) in the ED according to the ICD10 coding during one year were included in the study. Patients with chronic pancreatitis and those who had missing data in their files were excluded from the study. Patients who did not have computed tomography(CT) in the ED were not included in the ***: Ultimately, 322 patients were included in the study. The median age of the patients was 53.1(IQR=36–64). Of the patients, 68.1%(n=226) had etiological causes of the biliary tract. The mortality rate of these patients within the first 48 hours was 4.3%(n=14). In the logistic regression analysis performed by using Balthazar classification, HAPS score, RDW, neutrophile/lymphocyte ratio, age, diabetes mellitus and systolic blood pressure, the only independent variable in determining mortality was assigned as Balthazar classification(OR: 15; 95% CI: 3.5 to 64.4).CONCLUSIONS: HAPS, neutrophile/lymphocyte ratio and RDW were not effective in determining the mortality of nontraumatic acute pancreatitis cases within the first 48 hours. The only independent variable for determining the mortality was Balthazar classifi cation.

Helicobacter pylori is the microbial agent most responsible for gastro-duodenal ulcer and chronic gastritis, which can develop into carcinoma of the stomach. This study was performed in Wad Medani Teaching Hospital, Sudan to detect Helicobacter pylori in stomach samples, and evaluate the performance of the tests used, which were histological stains and PCR. Gastric biopsies were obtained from 105 referred patients during endoscopy, and fixed specimens examined by haematoxylin-eosin and Warthin-Starry silver stains, while DNA was extracted for glmM gene amplification. Epigastric pain was the most common symptom at 78% (82/105) and chronic gastritis recorded with 71% (68/105) of endoscopy results. Warthin-Starry silver stain gave 31% (33/105) as positive for Helicobacter pylori followed by glmM gene 27% (28/105) and haematoxylin-eosin 24% (25/105). The study indicated good performance of histological staining and high specificity of glmM gene in detection of Helicobacter pylori from gastric biopsies.

Acute and Chronic Toxicity of Thioacteamide and Alterations in Blood Cell Indices in Rats

Background: Thioacetamide (TAA) has been used extensively in the development of suitable animal models of acute and chronic liver injury employing various doses, times and routes of its administration, particularly in drinking water due to its resemblance with human liver fibrosis and cirrhosis. The aim of this study was to investigate and compare hematological alteration during the acute and chronic liver inflammation. Methods: Acute Liver inflammation was induced in Wistar rats via intraperitoneal injection of thioacetamide and the animals were sacrificed 12 h after the TAA administration. Induction of chronic liver inflammation was performed by continuous administration of TAA in the drinking water (200 mg/L) during 18 weeks of experiment. After that all animals were sacrificed and Blood samples were collected for further analysis. Results: Single intra peritoneal injection of TAA (300 mg/kg B.W.) induced an acute condition with hematological changes including leukocytosis with marked neutrophilia (P = 0.0429), lymphopenia, thrombocytosis as well as increased hemoglobin concentration (P 0.05) and decline of erythrocytic count (P = 0.0009). Eighteen weeks of uninterrupted supply of TAA (200 mg/L) in drinking water lead to chronic inflammation and the hematological alterations were leucopenia (P = 0.0197) accompanied with neutropenia and thrombocytopenia. Increase in RBCs (P = 0.0073) and Hb contents was also observed with a decline of red cell indices. Conclusion: Taken together these findings we can conclude that the animals respond differently under acute and chronic inflammatory condition with TAA administration. Leukocytosis with marked neutrophilia, thrombocytosis as well as increased hemoglobin concentration and decline of erythrocytic count were observed in acute while leucopenia accompanied with neutropenia and thrombocytopenia and increase in RBCs, Hb and Hct was also observed with a decline of other red cell indices during chronic phase.

Cytokines have been in focus of scientific in for more than decade now. Analyzing their expression has enabled a better understanding of pathogenesis of various diseases. Moreover, they are now for beyond their stage when they were of interest for pathophysiological research. In the present study, we used an assay to anti-inflammatory cytokines. One hundred and thirty sera and urine from egg positive individuals and 42 samples of healthy individuals in Eldeum locality—an endemic area of S. haematobum and 28 healthy individuals from non-endemic area were tested. Concentration of IL5 and IL10 is reverse. IL5 is low in serum but high in urine before treatment and high in serum and low in urine after treatment. IL10 is high in serum, low in urine before treatment and low in serum, high in urine after treatment.

Effect of exchange blood transfusion on oxygen saturation of neonates with severe neonatal jaundice by pulse oximetry

Objective:To determine if there was any difference in SpO2 readings during exchange blood transfusion(EBT).Methods:A prospective cross-sectional study of neonates with severe neonatal jaundice requiring EBT was *** saturation was recorded before,immediately and 15 minutes after EBT by using a pulse ***:This study included 30 neonates with 20 males and 10 *** age ranged from 1 to 12 days with a mean of(5.4±2.9)***-EBT SpO2 ranged from 90%to 98%with a mean value of(94.3±2.2)%;SpO2 in the end of EBT ranged from 85%to 99%with a mean value of(94.1±3.2)%;SpO2 at 15 minutes after EBT ranged from 77%to 99%with a mean value of(94.8±4.1)%.There was no significant difference between SpO2 values at onset of EBT and either immediately or 15 minutes after EBT(P=0.770 and 0.422,respectively).SpO2 showed no significant difference between neonates who were infused with blood of different storage times(0.999)and 15 minutes after EBT(P=0.887).Besides,SpO2 values were compariable in neonates with hematocrit<45%or≥45%at the onset of EBT(P=0.284),immediately(P=0.118)and 15 minutes after EBT(P=0.868).Conclusions:EBT does not affect SpO2 in neonates.

Assessment of Sub-Endometrial Junction Zone by 3-Dimensional Transvaginal Ultrasound in Unexplained Recurrent Implantation Failure and Its Effect on ICSI Outcomes

BACKGROUND: Sub-endometrial junction zone (JZ) plays an important role in most of reproductive functions. OBJECTIVE: To find out the effect of the sub-endometrial JZ thickness assessment by 3-D trans-vaginal ultrasound (TVUS) on intracytoplasmic sperm injection (ICSI) outcomes in patients with unexplained recurrent implantation failure (RIF). SETTING: ART Unit of Obstetrics and Gynecology Department, Faculty of Medicine, South Valley University, Qena, Egypt. DURATION: From April 2016 to October 2018. STUDY DESIGN: A prospective observational study. METHDS: Fifty couples with history of unexplained RIF in previous ICSI cycles and prepared for another ICSI cycle (group I) and fifty couples with unexplained infertility prepared for ICSI for the first time (group II) had been included in this study. At time of ovum pick up, 3-DTVUS was done for all cases in both groups for assessment of sub endometrial junction zone thickness and correlated with ICSI outcome. RESULTS: There were statistically significant differences between group I and group II in JZ thickness in the 3 uterine regions (fundus, anterior and posterior walls) with p 0.001, but there were mildly statistically significant differences between both groups in chemical and clinical pregnancy rates with p < 0.01. CONCLUSIONS: The thickness of JZ in patients with history of unexplained RIF was higher than those with unexplained infertility scheduled for ICSI. The JZ thickness was inversely correlated with increased embryo implantation rates in ICSI procedures, the thinner the JZ thickness was associated with higher pregnancy rates (both chemical and clinical pregnancy rates).

Ultrasonographic Characteristics of Diabetes Impacts in Kidneys’ Morphology

A total sample size consisting of 150 Diabetic male patients has been investigated by ultrasound system General Electric using gray-scale B-mode imaging with curvilinear transducer 3 MHz to assess the impact of diabetes in kidney morphology and it is distributed in Sudan. The collected data were the patient age, height, weight, kidneys size, ultrasound findings of involved kidneys, and duration of diabetes and residence region. The analyzed data show that the diabetes has been as endemic disease in central Sudan (Khartoum & Jazeera) representing 55% and in the west of Sudan representing 38%. The BMI of diabetic patients has been significantly (R2 = 0.6) decreasing following aging. The kidney size increases significantly as R2 = 0.75 and 0.6 for left and right kidney respectively. Their correlation is fitted in the following equations: y = 3.95x + 27.26 and y = 2.41x + 35.12 for the left and right kidney respectively. The impact of duration was a reduction in size significantly as R2 = 0.61 and 0.55 with a correlation fitted in the following equations: y = –2.22x + 139.9 and y = –1.51x + 96.59 for the left and right kidney respectively. The mean kidney length was (14.5 cm) and the renal cortex in the range of 2 - 2.3 cm, the kidneys size were so enlarged as 92.4 ± 11.7 and 121 ± 17.1 for the right and left kidney respectively while in late case of Diabetes, the kidney is more echogenic, atrophied size with loss of corticomedullary differentiation.
