background and purpose Recent reports from our laboratory demonstrated the post-ischaemic expression profile of various matrix metalloproteinases(MMPs)in rats and the detrimental role of MMP-12 in post-stroke brain damage.We hypothesise that the post-stroke dysregulation of MMPs is similar across species and that genetic deletion of MMP-12 would not affect the post-stroke expression of other MMPs.We tested our hypothesis by determining the pre-ischaemic and post-ischaemic expression profile of MMPs in wild-type and MMP-12 knockout mice.Methods Focal cerebral ischaemia was induced in wildtype and MMP-12 knockout mice by middle cerebral artery occlusion procedure by insertion of a monofilament suture.One hour after ischaemia,reperfusion was initiated by removing the monofilament.One day after reperfusion,ischaemic brain tissues from various groups of mice were collected,and total RNA was isolated and subjected to cDNA synthesis followed by PCR analysis.results Although the post-stroke expression profile of MMPs in the ischaemic brain of mice is different from rats,there is a clear species similarity in the expression of MMP-12,which was found to be predominantly upregulated in both species.Further,the post-stroke induction or inhibition of various MMPs in MMP-12 knockout mice is different from their respective expression profile in wild-type mice.Moreover,the brain mRNA expression profile of various MMPs in MMP-12 knockout mice under normal conditions is also different to their expression in wild-type mice.Conclusions In the ischaemic brain,MMP-12 upregulates several fold higher than any other MMP.Mice derived with the genetic deletion of MMP-12 are constitutive and have altered MMP expression profile both under normal and ischaemic conditions.

Safety training is the exercise normally conducted for all the current and future employees of a company to identify and recognize occupational hazards and diseases as well as determine the appropriate controlling methods.Moreover,virtual reality(VR)is a technology developed to virtually simulate the surrounding envi-ronment to ensure immersive experience and interaction through artificial three-dimensional(3D)platforms.VR devices have been developed to be more compact,easy to use,and affordable to enable people to enjoy immersive virtual experiences and provide interactive and realistic content.This has made technology one of the most popular forms of media for different kinds of training,such as safety-related ones.Therefore,this study aimed to review the use of VR in safety training through the systematic literature review(SLR)method.The process focused on developing 4 primary questions(PQs)classified into 11 systematic research questions(SRQs)for discussion points concerning current developments in VR technology applications.These were further combined with the preferred reporting items for systematic reviews and meta-analyses(PRISMA)flow diagrams in selecting the relevant literature.The questions were also used to investigate the scenarios,methods,objectives,and outcomes of previous studies.The results showed the need for further studies on the application of VR technology in safety training in other fields such as firefighting,chemical industry,maritime,etc.Furthermore,several scenarios such as construction design,disaster response,rescue procedures,and others need to be included.This study also provides information on the gaps for future study,including the exploration of a broader range of industries and VR scenarios.

Endoscopic ultrasound-guided portal pressure gradient measurement in managing portal hypertension

Portal hypertension(PH)is still a challenging clinical condition due to its silent manifestations in the early stage and needs to be measured accurately for early detection.Hepatic vein pressure gradient measurement has been considered as the gold standard measurement for PH;however,it needs special skill,experience,and high expertise.Recently,there has been an innovative development in using endoscopic ultrasound(EUS)for the diagnosis and management of liver diseases,including portal pressure measurement,which is commonly known as EUS-guided portal pressure gradient(EUS-PPG)mea-surement.EUS-PPG measurement can be performed concomitantly with EUS evaluation for deep esophageal varices,EUS-guided liver biopsy,and EUS-guided cyanoacrylate injection.However,there are still major issues,such as different etiologies of liver disease,procedural training,expertise,availability,and cost-effectiveness in several situations with regard to the standard management.

Impact of endoscopic ultrasound-guided radiofrequency ablation in managing pancreatic malignancy

Pancreatic malignancy is still the most lethal gastrointestinal malignancy.It has a very poor prognosis with low survival rate.Surgery is still the main treatment option for pancreatic malignancy.Most patients already have locally advanced and even late stage disease due to non-specific abdominal symptoms.Even though some cases are still suitable for surgical treatment,due to its aggressiveness adjuvant chemotherapy is becoming the standard treatment for controlling the disease.Radiofrequency ablation(RFA)is a thermal therapy that has been used as one of the standard treatments for liver malignancy.It can also be performed intraoperatively.There are several reports on percutaneous RFA treatment for pancreatic malignancy using transabdominal ultrasound and guided by computed tomography scan.However,due to its anatomical location and the risk of high radiation exposure,these methods seem to be very limited.Endoscopic ultrasound(EUS)has been widely used for pancreatic abnormality evaluation due to its ability to detect more accurately,especially small pancreatic lesions,compared to other imaging modalities.By the EUS approach,it is easier to achieve good visualization of tumor ablation and necrosis as the echoendoscope position is closer to the tumor area.Based on studies and a recent meta-analysis,EUS-guided RFA is a promising treatment approach for most pancreatic malignancy cases,but most studies only collected data from a small sample size.Larger studies are needed before clinical recommendations can be made.

Machine learning modeling for proton exchange membrane fuel cell performance

Proton exchange membrane fuel cell (PEMFC) is considered essential for climate change mitigation, and a fast and accurate model is necessary for its control and operation in practical applications. In this study, various machine learning methods are used to develop data-based models for PEMFC performance attributes and internal states. Techniques such as Artificial Neural Network (ANN) and Support Vector Machine Regressor (SVR) are used to predict the cell voltage, membrane resistance, and membrane hydration level for various operating conditions. Varying input features such as cell current, temperature, reactant pressures, and humidity are introduced to evaluate the accuracy of the model, especially under extreme conditions. Two different sets of data are considered in this study, which are acquired from, a physics-based semiempirical model and a 1-D reduced-dimension Computational Fluid Dynamics model, respectively. The aspect of data preprocessing and hyperparameter tuning procedures are investigated that are extensively used to calibrate the artificial neural network layers and support vector regressor to predict the fuel cell attributes. ANN clearly shows an advantage in comparison with SVR, especially on a multivariable output regression. However, the SVR is advantageous to model simple regressions as it greatly reduces the level of computation without sacrificing accuracy. Data-based models for PEMFC are successfully developed on both the data sets by adapting advanced modeling techniques and calibration procedures such as ANN incorporating the dropout technique, resulting in an R2 ≥ 0.99 for all the predicted variables, demonstrating the ability to build accurate data-based models solely on data from validated physics-based models, reducing the dependency on extensive experimentation.

Portal Hypertension in Nonalcoholic Fatty Liver Disease:From Pathogenesis to Clinical Practice

Portal hypertension in nonalcoholic fatty liver disease(NAFLD)mostly occur in cirrhotic stage.However,several experimental and clinical studies showed evidence of portal hypertension in NAFLD without significant or advance fibrosis.This early development of portal hypertension in NAFLD is associated with liver sinusoidal contraction by hepatocellular lipid accumulation and ballooning,which is also accompanied by capillarization and dysfunction of liver sinusoidal endothelial cells.Both of these impaired mechanical and molecular components can cause an increase in intrahepatic vascular resistance which lead to the increase of portal pressure in the absence of significant liver fibrosis.Extrahepatic factors such as insulin resistance and gut dysbiosis may also contribute to liver sinusoidal endothelial dysfunction and early portal hypertension in NAFLD.The clinical impact of early portal hypertension in NAFLD is still unclear.However,clinical tools for diagnosis and monitoring of portal hypertension in NAFLD are being investigated to predict high-risk patients and to guide therapy.

Validation of LittleEARS questionnaire in Hindi language

Background:Subjective measures of auditory development are equally important as objective measures to obtain a realistic image of the hearing status in infants and toddlers.Objectives:The objectives of the current study were to translate and validate the LittleEARS questionnaire into the Hindi language,to calculate its psychometric properties and establish a regression curve of the scores obtained as a function of age,to calculate the inter-test and test retest reliability of the same.The secondary objectives were to compare the scores obtained by the normal hearing children and those with hearing impairment and to plot a regression curve of total scores obtained by the hearing-impaired children as a function of the duration of auditory training attended since their first fit of the device.Materials and methods:The procedures involved conventional translation,reverse translation,and content validation before administering the questionnaire.The translated version was administered to parents of 59 children with normal hearing and 41 children with hearing impairment.Results:The finalized version had good reliability and efficient internal consistency with a Cronbach alpha value of 0.96.The mean scores obtained by the normal hearing children showed a progressive pattern as a function of age.Conclusion:The LittleEARS questionnaire has been successfully translated and validated into the Hindi language with excellent validity and reliability and can be used for screening and early identification of hearing impairment and in evaluating the outcome of audiological treatment approaches.

In any closed environment considered,it can be seen that the acoustic parameters are inherently not constant over the entire area considered.In a closed environment,it is ideally preferred to maintain the acoustic parameters as constant so that there exists better quality of sound leading to better auditory perception with respect to the audience.Practically,some of the acoustic parameters like reverberation time and clarity do not strictly pertain to the pattern obtained theoretically.In this paper,simulations are carried out using I-SIMPA under different values of Sound Transmission Class(STC),source position,distribution and the chamber dimensions and provides an insight into the behaviour of these acoustic parameters and the appropriate values that have to be infused into the system to build the chamber.The acoustic environment is modelled keeping an actual closed room in mind and testing is done with respect to different values of surface absorption coefficients(practically indicated using Sound Transmission Class)and dimensions to determine ideal conditions.

Color Doppler ultrasound imaging in varicoceles: Is the difference in venous diameter encountered during Valsalva predictive of palpable varicocele grade?

Objective:The clinical grading system for varicoceles is subjective and dependent on clinician experience.Color Doppler ultrasound(US)has not been standardized in the diagnosis of varicoceles.We aimed to determine if US measurement of varicocele could be predictive of World Health Organization(WHO)varicocele grade.Methods:Men who presented for either scrotal pain or infertility to a tertiary men’s health clinic underwent physical examination,and varicoceles were graded following WHO criteria(0Zsubclinical,1,2,3).US was used to measure largest venous diameter in the pampiniform plexus bilaterally at rest and during Valsalva maneuver.Receiver operator characteristic curve analysis was used to determine if resting diameter,diameter during Valsalva,or change in diameter between at rest and during Valsalva provided the highest sensitivity and specificity for determining clinical grade.Threshold values for diameter were determined from these receiver operator characteristic curves.Results:A total of 102 men(50 with clinical varicocele and 52 with subclinical varicocele)were included.Diameter at rest was the best ultrasonographic discriminator between subclinical and clinical varicoceles(area under the curve[AUC]Z0.67)with a diameter threshold of 3.0 mm(sensitivity 79%,specificity 42%).Diameter during Valsalva had the greatest AUC for determining clinical Grades 1 versus 2(AUCZ0.57)with diameter threshold of 5.7 mm(sensitivity 71%,specificity 33%).For differentiating between Grades 2 and 3,diameter at rest had the greatest AUC of 0.65 with a threshold of 3.6 mm(sensitivity 71%,specificity 58%).Conclusion:Our results corroborate other studies that have shown a weak correlation between US and clinical grading.The use of diameter during Valsalva was less predictive than diameter at rest and was only clinically significant in differentiating between Grade 1 and 2 varicocele.A standardized method for determining clinically relevant varicoceles on US would allow for improved patient counseling and clinical decision-making.

Non-variceal upper gastrointestinal bleeding(NVUGIB)is one of the challenging situations in clinical practice.Despite that gastric ulcer and duodenal ulcer are still the main causes of acute NVUGIB,there are other causes of bleeding which might not always be detected through the standard endoscopic evaluation.Standard endoscopic management of UGIB consists of injection,thermal coagulation,hemoclips,and combination therapy.However,these methods are not always successful for rebleeding prevention.Endoscopic ultrasound(EUS)has been used recently for portal hypertension management,especially in managing acute variceal bleeding.EUS has been considered a better tool to visualize the bleeding vessel in gastroesophageal variceal bleeding.There have been studies looking at the role of EUS for managing NVUGIB;however,most of them are case reports.Therefore,it is important to review back to see the evolution and innovation of endoscopic treatment for NVUGIB and the role of EUS for possibility to replace the standard endoscopic haemostasis management in daily practice.
