Aggressive surgical approach in patients with adrenal-only metastases from hepatocellular carcinoma enables higher survival rates than standard systemic therapy

Background:Although guidelines recommend systemic therapy even in patients with limited extrahepatic metastases from hepatocellular carcinoma(HCC),a few recent studies suggested a potential benefit for resection of extrahepatic ***,the benefit of adrenal resection(AR)for adrenal-only metastases(AOM)from HCC was not proved *** is the first study to compare long-term outcomes of AR to those of sorafenib in patients with AOM from ***:The patients with adrenal metastases(AM)from HCC were identified from the electronic records of the institution between January 2002 and December *** who presented AM and other sites of extrahepatic disease were ***,the patients with AOM who received other therapies than AR or sorafenib were ***:A total of 34 patients with AM from HCC were *** of these,22 patients had AOM,6 receiving other treatment than AR or ***,8 patients with AOM underwent AR(AR group),while 8 patients were treated with sorafenib(SOR group).The baseline characteristics of the two groups were not significantly different in terms of age,sex,number and size of the primary tumor,timing of AM diagnosis,Child-Pugh and eCOG *** a median follow-up of 15.5 months,in the AR group,the 1-,3-,and 5-year overall survival rates(85.7%,42.9%,and 0%,respectively)were significantly higher than those achieved in the SOR group(62.5%,0%and 0%at 1-,3-and 5-year,respectively)(P=0.009).The median progression-free survival after AR(14 months)was significantly longer than that after sorafenib therapy(6 months,P=0.002).Conclusions:In patients with AOM from HCC,AR was associated with significantly higher overall and progression-free survival rates than systemic therapy with *** results could represent a starting-point for future phase II/III clinical trials.

High levels of homocysteine downregulate apolipoprotein e expression via nuclear factor kappa B

AIM: To investigate the effect of high homocysteine(Hcy) levels on apolipoprotein e(apoe) expression and the signaling pathways involved in this gene ***: Reverse transcriptase polymerase chain reaction(RT-PCR) and Western blot were used to assess apo e expression in cells treated with various concentrations(50-500 μmol/L) of Hcy. Calcium phosphatetransient transfections were performed in HeK-293 and RAW 264.7 cells to evaluate the effect of Hcy on apoe regulatory elements [promoter and distal multienhancer 2(Me2)]. To this aim, plasmids containing the proximal apoe promoter [(-500/+73)apoe construct] alone or in the presence of Me2 [Me2/(-500/+73)apoe construct] to drive the expression of the reporter luciferase gene were used. Co-transfection experiments were carried out to investigate the downstream effectors of Hcymediated regulation of apoe promoter by using specific inhibitors or a dominant negative form of IKβ. In other co-transfections, the luciferase reporter was under the control of synthetic promoters containing multiple specific binding sites for nuclear factor kappa B(NF-κB), activator protein-1(AP-1) or nuclear factor of activated T cells(NFAT). Chromatin immunoprecipitation(ChI P)assay was accomplished to detect the binding of NF-κB p65 subunit to the apoe promoter in HeK-293 treated with 500 μmol/L Hcy. As control, cells were incubated with similar concentration of cysteine. NF-κB p65 proteins bound to DNA were immunoprecipitated with anti-p65 antibodies and DNA was identified by PCR using primers amplifying the region-100/+4 of the apoe gene. ReSULTS: RT-PCR revealed that high levels of Hcy(250-750 μmol/L) induced a 2-3 fold decrease in apoe m RNA levels in HeK-293 cells, while apo e gene expression was not significantly affected by treatment with lower concentrations of Hcy(100 μmol/L). 目的摆位误差是影响乳腺癌保留乳房术后调强放疗的主要因素.合适的体位固定技术能有效地降低摆位误差。本研究利用锥形束CT( cone-beam computed tomography, CBCT)探讨体膜联合乳腺托架与体膜联合体板2种固定技术在乳腺癌保留乳房术后调强放疗前的摆位误差,从而得出较优的体位固定方式。方法选取2017-11-01-2018-02-25新疆医科大学附属肿瘤医院收治的34例接受保留乳房术后调强放疗的乳腺癌患者,将其平均分为A和e 2组,A组使用体膜联合乳腺托架,e组使用体膜联合体板固定装置,在CBCT引导下对IMRT治疗前摆位进行验证,患者首次及以后每周1次均行CBCT扫描。将重建好的CBCT图像与计划CT图像行自动匹配,得到患者在左右(X)、前后(Y)和头脚(Z)3个方向上的摆位误差,并进行比较。结果 2组34例患者共获得196组CBCT验证图像,A组摆位误差的绝对值中位数X(左右)为1. 0 mm, Y(上下)为1. 0 mm,Z(前后)为2. 0 mm,B组摆位误差的绝对值中位数X(左右)为2. 0 mm,Y(上下)为 1. 5 mm,Z(前后)为 3. 0 mm;2 组比较 X 方向 Z= 2. 081, P = 0. 037;Y 方向 Z= 2. 537,P = 0. 011;Z 方向Z= 2. 791,P = O. 005;差异均有统计学意义;A组X方向的位移分布 5 mm 为 0;Z 方向 V 3 mm 为 81. 6%,3 5 mm 为 15.3%,>5 mm为3. 1%。e组X方向的位移分布V3 mm为82. 7 %, 3 5 mm为9. 2%,>5 mm为& 2%;Y方向V3 mm为8& 8%,3 5 mm 为 7. 1 %,>5 mm 为 4. 1%;Z 方向<3 mm 为 55. 1%,3 5 mm 为 34. 7%,〉5 mm 为 10. 2%, 2 组位移分布比较X方向(P = 0. 043),Y方向(P = 0.003),Z方向(P<0. 001),差异均有统计学意义。结论乳腺癌保留乳房术后调强放疗中采用2种不同固定技术,应用体膜联合乳腺托架固定技术在3个方向上均优于体膜联合体板。

The aim of this study was to demonstrate the efficacy of the minimally invasive technique for recurrent pectus excavatum. Fifty patients with recurrent pectus excavatum underwent a secondary repair using the minimally invasive technique. Data were reviewed for preoperative symptomatology, surgical data, and postoperative results. Prior repairs included 27 open Ravitch procedures, 23 minimally invasive (Nuss) procedures, and 2 Leonard procedures. The prior Leonard patients were also prior Ravitches and are therefore counted only once in the analyses. The median age was 16.0 years (range, 3-25 years). The median computed tomography index was 5.3 (range, 2.9-20). Presenting symptoms included shortness of breath (80%), chest pain (70%), asthma or asthma symptoms (26%), and frequent upper respiratory tract infections (14%). Both computed tomography scan and physical exam confirmed cardiac compression and cardiac displacement. Cardiology evaluations confirmed cardiac compression (62%), cardiac displacement (72%), mitral valve prolapse (22%), murmurs (24%), and other cardiac abnormalities (30%). Preoperative pulmonary function tests demonstrated values below 80%normal in more than 50%of patients. Pectus repair was done using a single pectus bar (66%), 2 bars (32%), or 3 bars (2%). Stabilizers were used in 88%of the patients. Median length of surgical time did not significantly differ from that of primary surgeries. Complications were slightly higher than those in primary repairs and included pneumothorax requiring chest tube (14%), hemothorax (8%)-, pleural effusion requiring drainage (8%), pericarditis (4%), pneumonia (4%), and wound infection (2%). There were no deaths or cardiac perforations. Initial postoperative results were excellent in 70%, good in 28%, and fair in 2%. Late complications of bar shift requiring revision occurred in 8%.
