Assessment of Contributions of Community Tuberculosis Intervention to Tuberculosis Case Detection and Treatment Outcome in Two States in Nigeria

Background: Nigeria, one of the high burden Tuberculosis countries, has developed various intervention strategies to ensuring universal access to quality-assured tuberculosis diagnosis and treatment. However, low case detection and unsuccessful treatment outcome still remains a serious challenge in most states. Community Tuberculosis care was born against this background in 2010 as a complementing front to combat the challenges, bringing directly observed treatment short-course strategy beyond the clinic settings to the door steps of patients. This study evaluates the contributions of the intervention to Tuberculosis case detection and treatment outcomes in two states in Nigeria. Materials and Method: A retrospective study with multistage sampling technique was employed to review 23,241 presumptive Tuberculosis cases enrolled for management between 2014-2017. Chi-square (χ2) test was used for test of association between the independent variables and the main outcomes of the study, with statistical significance set at p-value of 5%. Results: The annual percentage increase for both states was 24.7%, 189.8% and 114.5% in the study group as against 5.2%, 44.6% and 65.6% in control group. Overall, 23.8% were bacteriologically positive (either AFB or Gene-Xpert MTB/Rif) and 1.4% were clinically evaluated to be positive, while 74.8% were bacteriologically negative. Of the total 5861 cases treated, successful treatment rate of the patients in the study group was 88.6% as compared to 76.1% in the control group. The relationship between the referral system from communities where the intervention program was implemented and the case detection/treatment outcome was statistically significant (OR 95% CI = 3.15, 2.95 - 3.35). Epidemiological Pattern of Lassa Fever Outbreak in Ondo State, Southwest Nigeria, 2014 to 2019

Introduction: Annual outbreak of Lassa Fever (LF) has been reported in Ondo State over the years. We analyzed LF surveillance data from January 2014 to December 2019 to describe the epidemiological pattern of the outbreak. Method: Lassa fever dataset from January 2014 to December 2019 was obtained from the State Ministry of Health. Variables analyzed include date of onset of symptom, age, gender, place (Local Government Area, LGA) and outcome of laboratory test. Data were summarized using frequencies, proportions, chart and maps. Results: From January 2014 and December 2019, 2141 suspected LF cases were reported. Of these, 551 cases were laboratory confirmed with 134 deaths recorded. The LF cases (suspected vs. confirmed) reported increased in 2016 (73 vs. 19), 2017 (207 vs. 76), 2018 (452 vs. 159) and 2019 (856 vs. 285) respectively. Most of the confirmed cases (89.5%) were ≥15 years while more than half (53.7%) were males. Prior to 2016, only two LGAs (Owo and Ose) recorded continued transmission of the disease. However, between 2016 and 2019, the disease had spread to 8 neighboring LGAs (P < 0.001) with the CFR declining from 67% in 2015 to 20% in 2019. Conclusion: We found an unusual increase in the suspected LF cases reported between January 2016 and December 2019 with a corresponding increase in the confirmed cases and high annual case fatality rates. Hence, we recommend intensified surveillance activities to enhance timely reporting of cases and laboratory confirmation to ensure early commencement of treatment to reduce the case fatality rate.

Background: The zygote of twins implants themselves separately and on different spots in the uterine endometrium. However, the growth of fetal membranes may be collective or singly. There is little data from sonographic view of assessment on the exact progression at different stages of gestation. More so, the realistic evidences from radiographic assessment of the compartmentalization of chorion membrane in the developing monochorionic and dichorionic twins are not sufficiently available, hence, the call to ascertain the exact progression of the chorion membrane through ultrasound scanning in gestational subjects. Aim: This present study examined the structural progression of embryonic growth pattern of chorionic membrane in monochorionic and dichorionic twins. Materials and Methods: The study utilized transabdominal ultrasound to periodically assess the progression of chorion membrane and advancement in compartmentalization of monochorionic and dichorionic twins as pregnancy proceeds. Results: The monochorionic membrane showed an enclosure that progresses to be more distinct and thickened around the two embryos with a unique T-shaped point of insertion in latter development. The partitioning of dichorionic membrane progresses to be less thickened and with a distinct lambda (λ) sign which is a wedge-shaped protrusion into the inter-twin space. Conclusion: Assessment showed that the growth pattern of chorion membrane varied with different parameters observed as early as in the first trimester. Hence during early gestational stage, a twin can be said to be monochorionic or dichorionic with key anatomical landmarks monitored. The growth progression could be used to project abnormality and on time treatment would be offered to improve perinatal outcome.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and Plasmodium species are the causative agents of coronavirus disease 2019 (COVID-19) and malaria respectively with similar clinical presentations. The objective of this study is to determine the burden of co-infection of SARS-CoV-2 and malaria in the general population. Five (5 mLs) of blood samples were collected for SARS-CoV-2 and malaria parasite test. The malaria test was performed using a commercially available one-step malaria antigen Plasmodium falciparum histidine-rich protein 2 (Pf HRP-II) rapid test kit. The results of the study showed that the participants that were coinfected with SARS-CoV-2 IgG and malaria were 13 (2.5%) in Lagos, 1114 (39.1%) in Delta and 49 (2.3%) in Sokoto States. The prevalence of coinfection of SARS-CoV-2 and malaria in urban areas in Lagos, Delta and Sokoto States were 7 (2.2%), 1373 (48.1%), and 5 (0.2%) respectively. In rural areas, the prevalence of coinfection of SARS-CoV-2 and malaria in Lagos, Delta and Sokoto States were 6 (0.3%), 365 (12.8%), and 44 (2.1%) respectively in this study. This suggests that participants in the urban areas were more prone to co-infections than the rural areas in Lagos and Delta states, while it was otherwise in Sokoto State. In conclusion, the co-infection of SARS-CoV-2 and malaria was very high in Delta State compared to the other States. It is important for clinics to screen for both diseases when patients present with symptoms of malaria. This is because the infections have similar symptoms and the public is quick to assume malaria infection without diagnosing for COVID-19 and vice versa.

Recovery of metallic oxide rich biochar from waste chicken feather

Over the years,the devastating impact of climate change has become pronounced due to inadequate regulation for waste disposal in a developing country like *** chicken feather-based metal oxide-rich(MOR)biochar was produced using a top-lit updraft reactor at a peak temperature of 417.2℃to give a yield of 28.19%.The results were analyzed using Brunauer-Emmett-Teller BET,Scanning Electron Microscopy(SEM),Fourier Transform Infrared Spectroscopy(FTIR),X-ray diffraction(XRD),Energy Dispersive Spectroscopy(EDS),and X-ray fluorescence spectroscopy(XRF).The result shows that the sample is mesoporous(pore diameter of 2.132 nm)with a BET surface area of 105.7 m^(2)/***(33.147 wt%)and SO_(3)(25.673 wt%)were the major oxides present in notable proportions on the material surface,which were evident in the SEM *** major elemental compositions of the raw sample were oxygen(36.917 wt%)and calcium(23.690 wt%),and the major minerals present were quartz,marialite,davyne,and *** present study elucidates the successful and efficient material recovery route for the preparation of adsorbents and precursors for many product synthesis processes.

Evaluation of Tuberculosis Treatment Outcome of TB/HIV Co-Infection: A Four-Year Retrospective Cohort Study in HIV-Prevalent Setting of North Central Nigeria

Background: Despite the availability of highly effective treatment for decades, Tuberculosis (TB) remains a major health problem in Nigeria due to the increasing association between HIV and TB observed over the past three decades when HIV was discovered. However, the proportion of TB and or TB/HIV co-infected patients who have successful TB treatment outcome is not well known. This study determined the treatment outcome of TB/HIV co-infected patients with HIV negative patients in two states in Nigeria. Materials and Methods: A retrospective study of secondary data from eight Directly Observed Treatment Short (DOTS) course and Anti- Retroviral Therapy (ART) service providers in Benue and Federal Capital Territory (FCT), Nigeria, was carried out. The period under review covers January, 2010 to December, 2013. Results: Out of the total 5266 TB cases reviewed, the HIV prevalence rate was 52%. They were predominantly (53.3%) male with mean age of 34.4 years (SD = 15.1 years). More than two-third (72.5%) of HIV-negative patients had successful treatment compared to 1718 (62.7%) HIV-positive patients. Of the 2334 HIV co-infected patients, 19.5% defaulted, 11.5% had died, 5.6% were transferred out and 0.7% failed treatment compared to HIV-negative patients amongst whom 18.3% defaulted, 3.6% died, 3.9% were transferred out and 1.6% failed treatment (p Conclusion: The favourable treatment outcome of HIV-negative patients is more than that of HIV-positive patients and the most probable predictable factor responsible is the CD4 count of patient;indicating that TB/HIV co-infection has remained a major public health problem in Benue state and FCT. Hence there is the need for sustained strengthening and expansion of the national TB/HIV programmes.

Comparative Effects of Cocoa Pod Husk and Oil Palm Bunch Ash on Nutrient Uptake, Growth and Dry Matter Yield of Cocoa (The-obroma cacao) in Ibadan, Southwest Nigeria

The high premium placed on the organically produced cocoa in the international market coupled with its health and environmental benefits necessitated recent focus on research into the use of agricultural wastes as source of nutrients in cocoa (T. cacao) production. The study was carried out at the Cocoa Research Institute of Nigeria (CRIN), Ibadan (Lat. 7°25'N Long. 3°25'E), South-western, Nigeria during the 2010/2011 seedling production season to compare the effects of NPK (20:10:10) fertilizer, Cocoa Pod Husk and Oil Palm Bunch Ash (CPHA and OPA) on the seedling growth and dry matter yield of cocoa (T. cacao). Equivalent amount of different rates of two nutrients sources CPHA and OPA were applied one month after planting at 0, 1, 2, 3, 4 and 5 t/ha respectively, recommended rate of 10 kg·N of the conventional NPK (20:10:10). Fertilizer was used as reference fertilizer. Results indicated that all the fertilizer materials positively and significantly (p > 0.05) increased the growth parameters considered. The influence of the ash materials irrespective of sources on the nutrient uptake, dry matter yield of cocoa seedlings and shoot to root ratio were either higher than or had a comparable results with the reference fertilizer (NPK 20:10:10). CPHA and OPA applied at a rate of 4 t/ha significantly (p > 0.05) increased the plant height and root length of cocoa seedlings respectively relative to control and in—organic fertilizer and could therefore be recommended for the prospective and existing Nigerian cocoa farmers for raising their seedlings in the nursery before transplanting to the field.

Objectives: Rapid and accurate identification of persons infected with SARS-CoV-2 which causes COVID-19 is key to managing the pandemic. The urgent need to scale up access to COVID-19 testing in Nigeria has led to the government’s introduction of the use of COVID-19 Ag rapid diagnostic test (RDT) across various settings in the country. However, field performance evaluation of the rapid SARS-CoV-2 antigen detection test is required to be conducted periodically and compared with the gold standard real-time reverse transcription-polymerase chain reaction (RT-PCR) test for diagnosis of COVID-19 cases. Design: A prospective COVID-19 screening and un-blinded verification of the performance of the STANDARD Q COVID-19 Ag test kit. Setting: The rapid SARS-CoV-2 antigen detection test, StandardTM Q COVID-19 Ag kit was compared with the RT-PCR test for detection of SARS-CoV-2 in nasopharyngeal samples for COVID-19 screening from persons and personnel attending a national youth camp orientation exercise during the second wave of the COVID-19 outbreak (January to March 2021) in Ondo state, southwest Nigeria. Participants: Three hundred fifty-one persons and personnel were screened for COVID-19 infection. Results: Of 351 respondents screened, 68 (19.4%) were positive, and 264 (75.2%) were negative for both COVID-19 Ag RDT and RT-PCR assay. The rapid SARS-CoV-2 antigen detection test’s sensitivity and specificity were 78.16% (95% CI = 68.02% - 86.31%) and 100.0% (95% CI = 98.61% - 100.0%), respectively and the diagnostic accuracy was 94.59% (95% CI: 92 - 97). Introduction： GDM （gestational diabetes mellitus） is a common complication of pregnancy, and prevalence has become alarming. Insulin remains the cornerstone in management of diabetes mellitus but oral agents have been increasingly viewed as potential alternatives to insulin because it＇s economically low in cost, easier to administer and affordable to monitor compare to insulin. Aim： This study aimed to determine the effects of Gliclazide on the heart cytoarchitectonic and some biochemical indices of pregnant STZ （streptozotocin）-induced diabetic rats compare with insulin. Methods： Twenty （20） pregnant Sprague Dawley rats weighing between 130 g and 150 g were used for the experiments. The rats were divided into four （4） groups： Group 1 （Non-diabetic）, Group 2 （Diabetic）, Group 3 （Diabetic ＋ Gliclazide） and Group 4 （Diabetic ＋ Insulin）. The glucose levels and body weights were monitoring daily. The experimental rats were sacrificed on the 19th on gestational period, blood samples and organs were collected for biochemical and histo-morphological examinations. Results： Gliclazide group rats showed significant increase in body weight compared with diabetic group Q9 〈 0.05）. The blood glucose level of rats in gliclazide group was significantly reduced compared with other groups. There was significant increase in reduced GSH （glutathione） of gliclazide and insulin groups compared with diabetic group. MDA （malondialdehyde） and CAT （catalase） levels activities were significantly increase in diabetic group compared with other groups. Hormonal profiles and hematological parameters are significantly increased in gliclazide, and insulin group compared with diabetic group. There were distortions in the structural organization of heart of diabetic group while gliclazide and insulin groups showed remarkable improvements of the degenerative changes of the myocardium.
