Evaluation of In-VitroAnti-Tuberculosis Activity of Tetrapleura tetrapteraCrude and Fractions on Multidrug Resistant Mycobacterium tuberculosis

The management, control and elimination of tuberculosis (TB) have been difficult with the advent of HIV and cases of multidrug resistant (MDR-TB) tuberculosis. The cases of multidrug resistance to rifampicin and isoniazid pose greater challenges on first line and second line drugs to eliminate TB. The study is aimed at establishing anti-tuberculosis activity of Tetrapleura tetraptera against Mycobacterium tuberculosis and MDR-TB and the phytochemical present. The leaves of Tetrapleura tetraptera were collected, weighed, dried and pulverized to powder. The pulverized leaves of Tetrapleura tetraptera were subjected to 70% methanol extraction and screened for phytochemical. The crude extract was further purified into fractions using silica gel and thin layer chromatography techniques. M. tuberculosis and MDR-TB were obtained from positive acid fast bacilli sputa of TB patients and confirmed using GeneXpert to differentiate genotypic drug susceptible M. tuberculosis and MDR-TB. The sputa were digested using sodium hydroxide-cysteine technique and cultured in Middlebrook 7H9. The crude extract and fractions were screened for anti-tuberculosis activity using tetrazolium microtitre plate assay. The results showed that Tetrapleura tetraptera crude had activities against M. tuberculosis at 7.4 ± 0 mg/ml and 27.5 ± 0 mg/ml for MDR-TB. One of the fractions inhibited the growth of M. tuberculosis at 0.24 ± 0 mg/ml and MDR-TB at 0.89 ± 0 mg/ml. The phytochemical screened includes tannins, alkaloids, saponins, flavonoids, phenols and resins. T. tetraptra possesses anti-tuberculosis potential at low concentration on MDR-TB and can be a lead compound in drug development for the treatment of tuberculosis and multidrug resistant tuberculosis.

Bacterial Uropathogen among Benign Prostatic Hyperplasia Patients at a Tertiary Hospital in Nigeria

As the lumen of the prostatic urethra becomes compromised by fibroadenomatous growth in the periurethral region of the prostate of men with Benign prostatic hyperplasia (BPH), urine outflow is obstructed progressively resulting in incomplete bladder empting causing stasis and may predispose patients to infection. Mid stream urine samples were collected from 94 BPH patients. Macroscopy, microscopy, culture and antibiotic susceptibility test were carried out on isolated uropathogens. Isolated bacteria were characterized using biochemical tests. Isolated bacterial pathogens include Proteus mirabilis, Escherichia coli, Pseudomonas aeruginosa, Klebsiella oxytoca, Morgarella morgani, K. pneumonia and Enterococcus faecalis in order of frequency. The isolates show highest resistance of 87.1% to Ciprofloxacin and least resistance of 12.9% to Imipenem. The isolates were found to be multi-drug resistant and the 12.9% resistance to Imipenem suggests presence of Carbapenemase producing bacteria among the isolates.

Molecular Characterization of Extended Spectrum β-Lactamase Genes in Clinical E. coliIsolates

Extended Spectrum Beta-Lactamases (ESBLs) encoding genes (TEM, SHV and OXA) were amplified from multidrug resistance E. coli. The multidrug resistance E. coli isolates from different clinical sources were documented to be plasmid encoded and resistance against β-lactam and cephalosporin. Conventional laboratory analysis showed that seventy percent (70%) of the selected multidrug resistant clinical isolates were ESBLs positive, showing a ≥5 mm increase in zone diameter for either antibiotics compared to its zone when tested alone. The antibiotic susceptibility result showed that 100% of the isolates were resistant to amoxicillin-clavulanic acid, amoxicillin, cefuroxime and ampicillin-sulbactam while 90% of the isolates were resistant to ceftazidine and tetracycline, 80% to ofloxacin, 70% to ceftriazon, nalidixic acid, cefalexin, 60% to ciprofloxacin, 50% to nitrofurantoin, 40% to chloramphenicol and 20% to gentamicine. The multiplex PCR with primers TEM (931bp), SHV (868), OXA-2 (478), aac(3)-IIa (900) and rmtA (634), which are genes responsible for extended spectrum β-lactamase and aminoglycoside resistance in E. coli shows that: isolate W15 comprises of three (3) resistant gene, which corresponds with TEM resolving as a 931 base pair, SHV 868 base pair, and a 478 bp indicating OXA-2 that is faint probably indicating a low concentration of the gene. Isolate B2 comprises single resistant gene, which is interpreted as OXA-2 with 478 base pair while isolate URO2, U64 and S45 comprises of two resistance genes which resolve as 868 and 478 base pair indicating SHV and OXA-2 respectively. However, isolates S57, U58 and B7 showed no gene amplification despite the various degree of resistance in MIC and antibiotic susceptibility profile test obtained with conventional detection analysis. Prevalence of hepatitis B virus infection and associated risk factors among adult females infected with Human Immunodeficiency Virus in Ogun State,Nigeria

Background:Hepatitis B virus(HBV)infection is prevalent in sub-Saharan Africa,including Nigeria,and is frequently observed in individuals co-infected with human immunodeficiency virus(HIV).Objective:This study aims to evaluate the prevalence of serological markers for hepatitis B virus and identify the associated risk factors among women with HIV undergoing highly active antiretroviral therapy(HAART)in Ogun State,***:Ethical approval was obtained from the Babcock University Health Research Ethics Committee(BUHREC)to recruit a total of 110 adult women infected with HIV,receiving treatment at the HIV clinics of Babcock University Teaching Hospital(BUTH)in Ilishan-Remo and General Hospital in Ijebu-Ode,both located in Ogun State,*** participants’HIV status were confirmed using three rapid diagnostic kits:Determine(Abbott Laboratories,Tokyo,Japan),Unigold HIV(Trinity Biotech Plc Bray,***,Ireland),and 1/2 Stat Pak(Abbott Laboratories,Tokyo,Japan)(Chembio Diagnostic Systems,New York,USA).Additionally,an HBV 5 in 1 Panel manufactured by Innovation Biotechnology Co.,Ltd in Beijing,China,was employed to detect HBV markers qualitatively in serum ***:Out of the 110 subjects that voluntarily participated in the study,4(3.6%)tested positive for HBsAg,2(1.8%)tested positive for HBsAb,81(73.6%)tested positive for HBeAg,3(2.7%)tested positive for HBeAb,and 65(59.1%)tested positive for *** was no significant correlation between the occurrence of HBsAg and the socio-demographic characteristics of the participants(P>0.05).Various risk factors were identified,including lack of knowledge about HBV,absence of HBV vaccination history,history of blood transfusion,organ transplant,and engaging in unprotected sex,among ***:The findings highlight the presence of HBV infection among HIV-positive women undergoing HAART in Ogun State,Nigeria,particularly within the age groups of 18–25 years and 26–30 *** results emphasize the n

Purpose: To evaluate the pattern of ocular and visual complications of head injury. Method: A prospective study of 225 head-injured patients managed at the University of Ilorin TeachingHospital, Ilorin,Nigeria. For the purpose of extracting vaisual complications, a Neurosurgeon and an Ophthalmologist examined each patient and appropriate investigations were carried out. Patients with ocular morbidity were analysed for age, sex, cause of injury, admission Glasgow coma score, and associated injuries in addition to findings at ophthalmic and neurosurgical evaluations. Results: Two major types of ocular and visual complications were observed in 57 (25.3%) of 225 head-injured patients studied. Soft-tissue injury to the globe and adnexae included periorbital ecchymosis, subconjuctival haemorrhage, lid laceration, or rarely globe rupture in 29 patients. Neuro-ophthalmic ocular cranial nerve palsies occurred in 28 patients, while orbital fracture was encountered in two patients. Ocular injuries were multiple in 60%of cases. The patients, comprising 37 male and 20 female subjects, were aged 9 months to 57 years (mean=28 years). Traffic accident was the leading cause of head injuries (84.2%), while fall from height (7%), assault (7%), and gunshot (1.8%) were miscellaneous causes. Conclusion: Injury to the globe and adnexae and ocular cranial nerve palsies constitute the most common oculovisual complications following head injury in our centre.
