Purpose: Levobupivacaine showed a lower risk of cardiovascular and central nervous system (CNS) toxicity than bupivacaine which is the most popular local anesthetic agent in obstetric practice. The aim of this study was to investigate the clinical efficacy of levobupivacaine compared with hyperbaric bupivacaine for spinal anesthesia for cesarean section. Methods: 60 pregnant women in ASA I - II group scheduled to have elective cesarean operation were allocated into the study. Patients were randomly divided into two groups. The combinations 10 mg levobupivacaine (0.5%) + fentanyl (15 μcg) for Group LF (n = 30) patients, 10 mg hyperbaric bupivacaine (0.5%) + fentanyl (15 μcg) for BF (n = 30) patients were intrathecally administrated a total of 2.3 cc. Sensory and motor block characteristics of the groups were assessed with pinprick and Bromage scale;observed hemodynamic changes and side effects were recorded. Results: The time to reach maximum dermatome for the sensory block, time to regression by two dermatomes and time to regress to T12 dermatome was found to be significantly long in Group BF. It was observed that in Group BF, the evolution of the motor block was faster and lasted longer. Whereas hypotension, bradycardia and nausea were less in Group LF, the need for ephedrine was higher in Group BF (p < 0.05). Conclusion: Since motor block time is shorter, and side effects like hypotension, bradycardia and nausea are less, the combination of levobupivacaine + fentanyl can be a good alternative in cesarean sections.

Predicting Reference Evaporation for the Ethiopian Highlands

Water is likely the most limiting factor in increasing agricultural production in large parts of Africa. Reference evaporation (ET0) is a key hydrological parameter to use efficiently the scarce supply. Several methods are available for predicting reference evaporation, but the accuracy of any of the methods has not been established for the Ethiopian highlands. The objective of this study is, therefore, to select the best methods for calculating the reference evaporation ET0. For the section, meteorological data of the Bahir Dar station were used, because all data needed for this study including the Class A pan Evaporation were recorded on a daily basis. Pan evaporation was considered as the best estimator of the reference evaporation. The results showed that the FAO-Penman Monteith (using solar radiation, wind speed, temperature and relative humidity) and Enku method (using only maximum daily temperatures) have acceptable daily ET0 ranges and predicted to Class A pan evaporation with correlation coefficients greater than 90% in a monthly basis. Next best was the Thornthwaite’s method with correlation coefficient of 89% with pan evaporation. Piche methods performed relatively well with correlation coefficient of greater than 70%. Blaney-Criddle, Priestley & Taylor, and Hargreaves performed the poorest in predicting pan evaporation. These methods should be recalibrated for local condition and therefore not recommended for use in the Ethiopian highlands. In summary, the FAO-Penman Monteith is recommended for locations where the input data are available;otherwise, the Enku method using maximum daily temperature is best for estimating the reference evaporation.

目的:分析Duane眼球后退综合征(DRS)患者的临床特点及手术疗效。方法:评估94例DRS患者临床数据。记录完整的眼科数据,包括年龄、性别、侧位、DRS类型、偏角、异常头部姿势(AHP)、眼球后退、过冲及手术入路类型。结果:患者年龄为15.4±9.18岁,其中78例男性,16例女性。单侧和双侧受累分别为87%和13%。在单侧DRS患者中,左眼占65%。94例患者中,59.5%的患者为I型,29.7%的患者为II型,8.5%的患者为III型,2.1%的患者为IV型。大多数患者(36,38%)为嗜热性,43例(96%)患者术前AHP出现10°以上。有40例(43%)患者出现过度调校,通常为II型。对45例(48%)患者进行手术治疗。共32例(78%)行水平直肌手术后获得了较好疗效,24例(53%)患者AHP改善。结论:侧直肌的Y分裂和后榫固定在有明显过校的情况下都有令人满意的结果。垂直直肌转位增强术在限制外展方面取得了最佳效果。

Despite advances in transplantation techniques and the quality of post-transplantation care, opportunistic infections remain an important cause of complications. Pneumocystis jirovecii(P. jirovecii) is an opportunistic organism, represents an important cause of infections in heart transplantation patients. Almost 2% to 10% of patients undergoing cardiac transplantation have Pneumocystis pneumonia. Prophylaxis is essential after surgery. Various prophylaxis regimes had been defined in past and have different advantages. Trimethoprim/sulfamethoxazole(TMP/SMX) has a key role in prophylaxis against P. jirovecii. Generally, although TMP/SMX is well tolerated, serious side effects have also been reported during its use. Pentamidine is an alternative prophylaxis agent when TMP/SMX cannot be tolerated by the patient. Structurally, pentamidine is an aromatic diamidine compound with antiprotozoal activity. Since it is not effectively absorbed from the gastrointestinal tract, it is frequently administered via the intravenous route. Pentamidine can alternatively be administered through inhalation at a monthly dose in heart transplant recipients. Although, the efficiency and safety of this drug is well studied in other types of solid organ transplantations, there are only few data about pentamidine usage in heart transplantation. We sought to evaluate evidence-based assessment of the use of pentamidine against P. jirovecii after heart transplantation.

Behcet’s disease (BD) is a large vessel vasculitis with a wide range of clinical manifestations. Some of these manifestation may be life threatening and rapid suppression of the inflammation with effective immunosuppressive agent is crucial. There are traditional drugs with different response rates and all have efficacy on different manifestations of the disease. The most frightening manifestations of the disease are ocular, neurologic, intestinal and vascular types of involvement. Besides benign and easily treated manifestations there are also refractory cases with complicated involvement. The novel biologic agents have been used for these resistant patients and favorable response rates have been reported. In this review, we have shared our experience with biologic agents in BD and also reviewed the literature for the efficacy and safety for these novel agents for refractory patients.

Objective: Although there are adult vaccination schemes in our country, there are serious deficiencies in the way that doctors direct the patients to this vaccination, but also patients have to make and demand these vaccinations. The aim of our study is to identify the shortcomings in this area and draw a roadmap for what arrangements should be made in terms of physicians and patients in order to increase adult immunization rates in primary care. Method: We conducted a two-phase, multicentered, descriptive clinical trial between October and December 2017. The first phase of the trial was carried out with patients from 3 Family Health Centers in Antalya, Istanbul and Osmaniye. Patients to be interviewed were selected voluntarily among Family Health Center’s applicants. The second phase of the trial was carried out with health workers, who were participated to trial from 26 different provinces of Turkey. In the process, a questionnaire of 19 questions was applied to primary health care workers by the internet. Results: 490 patients were included in the study. There was a significant difference between gender, age, education level and guideline follow-up and vaccination status of patients [p Conclusion: As a result of the studies including our study about adult vaccination, only 10% - 20% of the targeted groups in adults can be vaccinated. However, just like in childhood during adulthood, vaccinations protect individuals from diseases and provide economic benefits. Firstly, the knowledge level of physicians on adult immunization should be updated with in-service trainings, they should first be convinced to vaccinate because they are in risk group and then they should be recommended vaccination to patient to increase the vaccination rates.

Herein, some fundamental open questions on engineering of “super” hydrogen sorption (storage) in carbonaceous nanomaterials are considered, namely: 1) on thermodynamic stability and related characteristics of some hydrogenated graphene layers nanostructures: relevance to the hydrogen storage problem;2) determination of thermodynamic characteristics of graphene hydrides;3) a treatment and interpretation of some recent STM, STS, HREELS/LEED, PES, ARPS and Raman spectroscopy data on hydrogensorbtion with epitaxial graphenes;4) on the physics of intercalation of hydrogen into surface graphene-like nanoblisters in pyrolytic graphite and epitaxial graphenes;5) on the physics of the elastic and plastic deformation of graphene walls in hydrogenated graphite nanofibers;6) on the physics of engineering of “super” hydrogen sorption (storage) in carbonaceous nanomaterials, in the light of analysis of the Rodriguez-Baker extraordinary data and some others. These fundamental open questions may be solved within several years.
