The Role of Nursing Care in Drug Compliance and Quality of Life in Patients after Coronary Artery “Bypass” Surgery

Cardiovascular disease (CVD) is a broad term that includes a whole range of diseases that affect the heart and blood vessels. Cardiovascular diseases (and not only) or as they are known by the abbreviation (SKV), are as numerous as they are complex, attacking the heart and blood vessels, causing many deaths globally, not to mention, are among the main causes. Arteriosclerosis is diagnosed, mainly on the basis of family and medical history, in which case a physical examination and specific tests are performed, which include: blood tests, Doppler ultrasound, electrocardiogram, chest X-ray, echocardiography, angiography and stress test, etc. The study was conducted at the University Clinical Center of Kosovo (UCCK). The paper is based on data obtained from the research protocol of patients of this clinic, in the period 2016-2018. This study included 100 patients with heart disease and their treatment, with special emphasis on coronary artery bypass grafting. Our goal was to emphasize the care, as well as the quality of life of patients, after coronary artery bypass surgery. This process has always been supervised by the head nurse of the clinic, in order to fully preserve the privacy of the patients included in this research. From the results of this research, we found that males dominate in the number of patients included in the study, age over 50 dominates, and we also found that most patients have relatives who suffer from heart disease. We also see that 18% of patients continue to smoke after surgery, while 70% of them stated that their health after surgery is much better than last year, and 84% of them stated that they use therapy after surgery. Data from another study were performed by the hospital CHF, Self-Care (Self-Care of Heart Failure Index). Evaluation of photosynthesis, physiological, and biochemical responses of chickpea (Cicer arietinum L. cv. Pirouz) under water deficit stress and use of vermicompost fertilizer

One goal in the face of drought stress conditions is to increase growth and yield through the reduction of negative effects of stress. Vermicompost can play an effective role in plant growth and development and in reducing harmful effects of various environmental stresses on plants due to its porous structure, high water storage capacity, having hormone-like substances, plant growth regulators, and high levels of macro and micro nutrients. This study considered the physiological, biochemical, and photosynthetic responses of the chickpea to different combinations of vermicompost and water stress in a greenhouse environment. Two factors were involved, addition of vermicompost to soil at four ratios： control （100 wt% （weight percentage） sol0; 10 wt% vermicompost＋90% soil; 20 wt% vermicompost＋80 wt% soil; 30 wt% vermicompost＋70 wt% soil weight percentage, and treatment of water stress at three levels including 75, 50, and 25% of field capacity. The results showed that vermicompost had a significant effect on all traits under stress and non-stress conditions. Application of vermicompost in soil, especially at the levels of 20 and 30 wt% significantly increased all studied traits under non-stress conditions. Under moderate stress conditions, vermicompost at 30 wt% treatment resulted in a significant increase in the photosynthetic pigments, CO2 assimilation rate, internal leaf CO2 concentration, transpiration, the maximal quantum yield of photosystem II （PSII） photochemistry （FJF）, concentrations of Ca and K in root and leaf tissues, proline and soluble protein contents in root tissues. Peroxidase （POX） and catalase （CAT） enzyme activities decreased significantly with increasing proportions of vermicompost, but the activity of superoxide dismutase was not significantly different. Primary Extra-Gastrointestinal Stromal Tumors (E-GIST) of the Liver, about Two Cases and Review of the Literature Oncology Hospital Experience at Chu Hassan II in FES

Introduction: Primary extra-gastrointestinal stromal tumors (E-GIST) of the liver are rare. The clinical presentation can range from asymptomatic to bleeding or manifestations of mass effect. Cancer surgery followed by adjuvant treatment with imatinib is the standard of care. However, in special circumstances, a cytoreductive approach may represent a therapeutic option. We describe here the case of two patients who presented with a painful and protruding epigastric mass, the first case was a localized E-GIST candidate for neoadjuvant treatment with imatinib with complete resection surgery thereafter with very good evolution, and the second is that of an E-GIST of the liver at an advanced stage with an evolution very quick to death. Conclusion: Primary liver E-GIST is a rare condition. Treatment is based on systemic therapy and total resection surgery. However, cytoreductive surgery will be required when complete resection is not possible.

Incidental Diagnosis of a Brown Tumor Mimicking Bone and Lung Metastasis during a Parathyroid Scintigraphy

We report herein a case of a 40-year-old male patient with chronic renal failure presenting a severe hyperparathyroidism with an elevation of parathormone level evaluated in nuclear medicine department for MIBI-Technetium-99m parathyroid scintigraphy. The parathyroid scintigraphy revealed the appearance of a preferential fixation of the MIBI-99mTc opposite the lower left pole of the thyroid and opposite the upper part of the right hemi thorax. A subsequent single-photon emission computed tomography-computed tomography focused on the cervico-thoracic region was performed and showed an ectopic parathyroid adenoma associated with an incidental brown tumor mimicking bone and lung metastases. Our case report confirms the usefulness of additional hybrid SPECT-CT imaging in the management of hyperparathyroidism.

Colonic schistosomiasis:A case report

Efficacy and Safety of Travoprost and Timolol Fixed Combination Compared to Travoprost in Patients with Primary Open Angle Glaucoma and Ocular Hypertension

Purpose: To compare intraocular pressure (IOP)-lowering efficacy and safety of travoprost 0.004% and travoprost 0.004% and beta-blocker 0.5% fixed combination ophthalmic solution in patients with open-angle glaucoma and ocular hypertension. Methods: In this prospective, multicentre clinical trial, 62 patients received travoprost 0.004% (n = 31) or travoprost 0.004% and beta-blocker 0.5% fixed combination (n = 31). Efficacy and safety were compared across treatment groups over 2 years. IOP reduction and adverse events were examined at 3, 6, 12 and 24 months for each group. Results: Mean IOP at the first visit in the travoprost 0.004% group was 26.4 (SD ± 2.1), and travoprost 0.004%/timolol 0.5% group was 26.3 (SD ± 2.1). Mean IOP after 24 months in the travoprost 0.004% group was 20.5 (SD ± 1.5) and travoprost 0.004%/timolol 0.5% group was 18.5 (SD ± 1.5). There were statistically significant differences in IOP in both eyes after third visit (after 1 year) and fourth visit (after 2 years). Conclusion: After 2 year of treatment, travoprost 0.004%/timolol 0.5% produced clinically relevant IOP reductions in patients with open-angle glaucoma or ocular hypertension that were greater than those produced by travoprost 0.004% alone.

Compound Heterozygous C282Y/H63D Mutation in Hemochromatosis: A Case Report

Hereditary hemochromatosis is a condition characterized by iron overload, which is both treatable and preventable. It’s mainly related to hepcidin deficiency related to mutations in genes involved in hepcidin regulation. Iron overload increases the risk of disease such as liver cirrhosis, heart disease and diabetes. Two HFE genotypes have been commonly described in cases of iron overload, C282Y homozygosity and C282Y/H63D compound heterozygoty. The diagnosis of this rare disease now can be explored by biological and imaging tools. We report a case of compound heterozygous C282Y/H63D discovered by family screening for elevated serum ferritin.

Pre-Sacral Chordomas, about Two Cases and Review of the Literature

The World Health Organization (WHO) defines chordoma as a malignant tumor of intermediate or low grade, developing at the expense of embryonic remnants of the notochord. These tumors represent 8.4% of all malignant bone tumors. We report 2 cases collected in the medical oncology department at the CHU Hassan II in Fes. In whom the clinical examination revealed a mass and/or signs of loco-regional compression. The tumor was located in the sacrum in both patients. The diagnosis was based on the data of the anatomopathological examination showing the association of a lobulated architecture with classic physaliphore cells and a mucoid intercellular substance. Immunohistochemistry was performed for both patients. The principle of therapeutic management was based on radiotherapy and targeted therapy since surgery was difficult given the local tumor extension.
