Prognostic Features of BCR-ABL Genetic Variations in Acute Lymphoblastic Leukemia

Background: Acute lymphoblastic leukemia (ALL) is a hematologic malignancy which results from accumulation of lymphoid progenitor cells in the bone marrow and/or extramedullary sites. Philadelphia chromosome (Ph1) positive ALL, a high-risk cytogenetic subset, accounts for 25% - 30% of adult ALL cases but occurs in less than 5% of children. We aimed with this study to detect BCR-ABL genes fusion, amplification and deletion in ALL patients, using extrasignal-fluorescence in situ hybridization (ES-FISH), and to assess their relation with other standard prognostic factors and therapeutic response. Patients and Methods: this study was carried out on 39 newly diagnosed ALL patients. All patients were subjected to: history, clinical examination and laboratory investigations, which included complete blood count (CBC), peripheral blood (PB), bone marrow (BM) examination, immunophenotyping and fluorescence in situ hybridization using extra-signal probe to detect BCR-ABL genes fusion. Results: this study showed statistical analysis of patients’ t(9;22) with other factors revealed, significant association (p 35 years, hepatosplenomegaly, absence of lymphadenopathy, tLC ≥ 50 × 109/L, absolute PB blasts ≥ 4.4 × 109/L, immunophenotyping and other aberrations. Conclusion: BCR/ABL fusion gene analysis by ES-FISH may serve as a prognostic marker in adulthood ALL. the age, tLC and t(9;22) represent the significant standard prognostic factors in relation to patients’ outcome.

Objectives: the aim of the study is to assess volumetric Corticospinal tract infiltration by Astro-cytoma using Diffusion tensor tractography. Material and Methods: Preoperative, anatomic (t1- and t2-weighted) and diffusion tensor MRI were performed in 9 patients with supratentorial gliomas (WHO grade II and III). the tumors were manually segmented from the t1- and t2-weighted MRI, and their volume calculated. A three-dimensional tractography was performed in each case. A second segmentation and volume measurement was performed on the tumor regions intersecting adjacent white matter fiber tracts. Results: We identified that white matter tracts were infiltrated by 6 Astrocytoma tumors. the median tumor volume (±standard deviation) in our patient population was 33 ± 26.82 ml. the median tumor volume (±standard deviation) infiltrating white matter fiber tracts was 4.15 ± 9.23 ml. the median fraction of tumor volume infiltrating white matter fiber tracts was 26.3% ± 10.1%. Conclusions: Diffusion tensor MR tractography is a reliable preoperative assessment tool since it detects extensive white matter infiltration by Astrocytoma irrespective of brain tumors volume. Recommendations: Prospective large population studies are required to clarify how infiltration relates to tumor location.

Background: Frozen section (FS) has a valuable role in the diagnosis of ovarian tumors. It is considered a pivotal point in guiding the surgical therapy, particularly in premenopausal women. In postmenopausal women, it may be required as well to avoid unnecessary surgical staging in benign ovarian tumors. Aim: this study aims to evaluate the accuracy of intraoperative frozen section in ovarian neoplasms in postmenopausal women. Materials and Methods: A retrospective analysis was done for intraoperative FS for suspected ovarian neoplasms. the study was conducted in Oncology Center, Mansoura University from March 2008 to December 2014. the frozen and paraffin section reports were compared, and overall accuracy, sensitivity, specificity, positive and negative predictive values were determined. Results: the study included 105 patients and the overall accuracy of FS in determining malignancy was 81.7%. the sensitivity for malignant tumors was 75.32% with specificity of 100%. For benign tumors, the sensitivity and specificity were 100% and 93%, respectively. Borderline tumors had the lowest sensitivity of 100% with specificity of 95.19%. Conclusion: the present study concurs that frozen section is an accurate test for diagnosis of benign and malignant tumors in postmenopausal women thus determining the extent of surgery done for them. On the other hand, accuracy rates for borderline tumors are low.

Aim: the aim of this study was to determine the effect of training on endothelium-dependent vasodilatation in patients with coronary artery disease (CAD) after revascularization and healthy young men. Background: Impaired endothelial function has been observed in patients with CAD and those with CAD risk factors. Studies have shown that exercise can enhance endothelial function. Methods: this experimental cross-sectional study was conducted on patients with CAD (3 months after CABG and PCI) and students of medical school in 2011. Endothelium dependent dilation of the brachial artery was determined by using high-resolution vascular ultrasonography through flow-mediated vasodilatation (FMD) after induction of ischemia, and the data were analyzed using SPSS, dependent t-test and ANCOVA. Findings: the findings showed that at baseline, FMD was reduced in revascularized patients, when compared with healthy young men, after 8 weeks, and exercise training significantly improved FMD in patients underwent training group [from 4.31 ± 1.45 (SD)% to 6.15 ± 0.773 (SD)%, p p ed unchanged, and even after aerobic training, it did not significantly modify the brachial artery diameter in these groups. Conclusion: Our study demonstrates that endothelial dysfunction persisting in CAD patients after revascularization and aerobic training can improve endothelial function in different vascular beds in CAD patients and healthy young men. this may contribute to the benefit of regular exercise in preventing and restricting cardiovascular disease.

the chronic myeloproliferative neoplasms (CMPN) are a group of clonal hematopoietic stem cell disorders in which large numbers of red blood cells, white blood cells, or platelets grow and spread excess in the bone marrow and the pe- ripheral blood. Cytogenetic analysis of the t (9:22) and molecular detection of BCR/ABL is the main diagnostic criteria in Philadelphia positive CMPN (CML). the identification of non-receptor tyrosine kinase JAK2 mutations (exon 14 JAK2 V617F and exon 12) have significantly contributed to our understanding of the molecular mechanisms in the pathogenesis of Philadelphia negative CMPN such as polycythemia vera (PV), essential thrombocythemia (Et) and primary myelofibrosis (PMF) patients. According to the revised WHO classification, JAK2 mutation is considered as a major diagnostic and clonal marker in Philadelphia negative CMPN which will play a major role in designing personal- ized treatments for the disease. JAK2 V617F mutation frequency is unknown in Saudi Arabia. therefore, investigation of the JAK2 V617F mutation was carried out in DNA samples from 78 peripheral blood specimens corresponding to patients with polycythemia vera (PV) (n = 11), Chronic myeloid leukemia (CML) (n = 45), essential thrombocythemia (Et) (n = 10), idiopathic myelofibrosis (MF) (n = 12). We used polymerase chain reaction and direct DNA sequencing to detect the JAK2 mutation. Overall, the incidence of the JAK2 V617F mutation was 91% in PV, 40% in Et, and 25% in MF. this approach proved to be reliable and more sensitive in detecting the mutation. two essential findings arose from our study. First, this technique could be carried out with DNA samples, even partially degraded, from routinely processed BM or peripheral blood specimens. Objective the purposes of this study were to （1） assess the clinicoepidemiological characteristics of esopha-geal cancer patients, （2） analyze the prognostic factors determining treatment failure and survival, and （3） evaluate the results of various treatment modalities for locoregional and disseminated disease and their ef ect on disease-free survival and overal survival （OS）.Methods Clinicoepidemiological retrospective data from 81 esophageal cancer patients treated at the Na-tional Cancer Institute of Cairo between 2007 and 2011 were evaluated. Results the study showed that patients with esophageal cancer commonly present with local y advanced disease （87.7% had t-stage 3 and 12.3% had t-stage 4）. there was a significant correlation between surgery and survival; patients who received radical surgery and postoperative radiation had a better median survival than patients who received radical radiotherapy （20 months vs. 16 months, respectively; P = 0.04）. there was also a significant statistical correlation between radical concomitant chemoradiotherapy （NCRt） and pal iative treatment. Patients who received radical NCRt had a better median survival than patients who received pal-liative radiotherapy （16 months vs. 10 months, respectively; P = 0.001）. the median fol ow-up period for al patients was 7 months. the median OS of the whole group was 12 months. the OS after 1 and 2 years was 57.8% and 15%, *** High-dose NCRt is an acceptable alternative for patients unfit for surgery or with inoperable disease. High-dose radiation is more ef ective than low-dose radiation in terms of local control, time to relapse, and OS. Further study using a larger series of patients and introducing new treatment protocols is necessary for a final evaluation.

the Mediterranean Sea is home to an interesting biodiversity. the current cumulative and descriptive study aims to enumerate some relatively large marine mammals, reptiles, and fishes that have been spotted, caught, by-caught, or stranded on the coast of the Gaza Strip, Palestine, which extends about 42 km along the Mediterranean Sea. this 20-year study from 2003 to 2022 relied much on frequent field visits, meetings, and discussions with stakeholders, following local media sites and social media pages, and photography. the study recorded at least 26 giants or relatively large marine organisms on the Mediterranean coast of the Gaza Strip, Palestine. Certainly, the coming years may bring other marine organisms of relatively large sizes. Marine mammals included three species of cetaceans with the Fin Whale (Balaenoptera physalus) being the largest mammal and even the largest animal ever recorded in this study. Marine reptiles included three species of sea turtles, the largest of which is the Leatherback Sea turtle (Dermochelys coriacea), which is in fact the world’s largest sea turtle. the bony fishes were represented by ten species, the largest of which was the Ocean Sunfish (Mola mola), which is the largest bony fish in the world. the cartilaginous fishes included ten species with the largest specimens encountered were the Shortfin Shark (Isurus oxyrinchus), Bluntnose Sixgill Shark (Hexanchus griseus), Scalloped Hammerhead Shark (Sphyrna lewini), and Giant Devil Ray (Mobula mobular). Seabirds were not included in the study. the Alexandria Pompano (Alectis alexandrinus) and the Silver-Cheeked toadfish (Lagocephalus sceleratus), which are bony fishes, appear to be the relatively smallest marine organisms recorded here. In conclusion, the role of the various parties must be coordinated to ensure the sustainability of human activities and their compatibility with the task of conserving local marine biota, including the gigantic or relatively large ones.
