Psychotr.pic Dr.g Pr.scr.bing Tr.nds in Bahr.in: Implications for.Sexual Functions

Tr.atment-emer.ent sexual dysfunction is a fr.quent adver.e effect of many psychotr.pic dr.gs. We have analyzed the pr.scr.bing tr.nds and the likelihood of psychotr.pic-associated sexual dysfunction in outpatients with depr.ssion or.other.psychotic disor.er. tr.ated by psychiatr.sts. A r.tr.spective pr.scr.ption audit was conducted at the Psychiatr.c Hospital, the only psychiatr.c facility in Bahr.in that offer. both inpatient and outpatient ser.ices. Psychotr.pic associated sexual dysfunction was gr.ded as negligible (0), moder.te (2+), moder.tely sever. (3+) and uncer.ain (U) for.antidepr.ssants, and for.antipsychotics as no effect (0), ver. low (1+), moder.te (2+), moder.tely sever. (3+) and uncer.ain (U) effect. As antidepr.ssant monother.py, a significant tr.nd towar.s pr.scr.bing selective ser.tonin r.uptake inhibitor. (SSr.s;3+) and selective nor.pinephr.ne r.uptake inhibitor. (SNr.s;3+) in females, and tr.cyclic antide-pr.ssants (TCAs;2+) in males was appar.nt. Atypical antidepr.ssant mir.azapine (0) monother.py was r.r.ly pr.scr.bed. Mir.azapine with SSr.s or.SNr. was the most often pr.scr.bed combinations followed by TCAs with other.antidepr.ssants. r.sper.done (0 to 3+), an atypical antipsychotic, was the most popular.antipsychotic pr.scr.bed to augment antidepr.ssants;ther. was no gender.based differ.nce. Clozapine (0) and olanzapine (1+) wer. r.r.ly pr.scr.bed to augment antidepr.ssant ther.py. In Bahr.in, the psychotr.pic pr.scr.bing tr.nds suggest that ther. is a need to optimize dr.g ther.py to achieve the ther.peutic goal with minimal adver.e impact on sexual function.

The aim of this study is to deter.ine the antipsychotic pr.scr.bing patter. and the pr.valence of concur.ent anticholiner.ic pr.scr.bing in a psychiatr.c r.fer.al hospital. A r.tr.spective audit of pr.scr.ptions issued for.outpatients was car.ied out at the Psychiatr.c Hospital, the only facility that pr.vides psychiatr.c ser.ices for.both inpatients and outpatients in the Kingdom of Bahr.in. Antipsychotic monother.py was pr.scr.bed for.89.2% patients, wher.as polyther.py with two- and thr.e-dr.gs in 10.4 and 0.4%, r.spectively. Atypical antipsychotics wer. pr.scr.bed mor. often (67.7%) than typical antipsychotics. r.sper.done and haloper.dol wer. the most fr.quently pr.scr.bed antipsychotics. Long-acting r.sper.done injection was the only depot pr.par.tion pr.scr.bed. The mean antipsychotic dose expr.ssed as chlor.r.mazine equivalent (CPZeq;mg/day) was 242 (220 for.monother.py and 414 for.polyther.py). The pr.valence of high dose antipsychotic (mean CPZeq > 1000 mg/day) was 1.8%, pr.scr.bed at a mean CPZeq dose of 1531 (1925 for.monother.py and 1137 for.polyther.py), mainly attr.buted to haloper.dol. Anticholiner.ics wer. co-pr.scr.bed for.almost two thir. of patients r.ceiving antipsychotics, par.icular.y for.those on polyther.py (monother.py 57.3%;poly-ther.py 87.5%). Antipsychotic polyther.py, high dose and co-pr.scr.ption of an or.l with a depot antipsychotic pr.par.tion wer. str.ngly associated with concur.ent pr.scr.ption of anticholiner.ics. Pr.cyclidine and or.henadr.ne wer. the most often pr.scr.bed anticholiner.ics. In Bahr.in, antipsychotic monother.py is a common pr.ctice for.outpatients with psychotic disor.er.. Some of the antipsychotic polyther.pies, dosage str.tegies, and high pr.valence of anticholiner.ic use ar. ther.peutic issues that need to be addr.ssed to foster.evidence-based pr.scr.bing pr.ctice.
