The Risk of Milrinone Use in Severe Neurological Insult and Stunned Myocardium

Acute neurological injuries cause catecholamine-induced cardiac damage and can result in or ex-acerbate preexisting cardiac failure. Milrinone (primacor) has been effective in the treatment of cardiac failure as well as cerebral vasospasm. We would like to present the outcomes of milrinone use in patients with major neurological injuries and preexisting compromised myocardial function. Four patients, age < 60 y/old, three women (two postpartum) and one man, will be presented. Two were diagnosed with a subarachnoid hemorrhage (SAH) coupled with severe vasospasm;one had status epilepticus (SE);and one had cerebral ischemia (CVA) due to carotid artery occlusion. All patients had cardiomyopathy with EF < 35%. All patients were on at least one press or prior to being put on milrinone. Three out of the four patients required an increase in norepinephrine or additional pressors after milrinone was added. All of the patients developed tachycardia, three severe and one mild. In two patients milrinone had to be discontinued due to the persistent hypotension. In our experience, use of milrinone drip in patients with preexisting cardiomyopathy led to hypotension and tachycardia, and brought about the need to increase or add new pressors, which in turn exacerbated catecholamine-induced myocardial injury.

Background and Purpose: To evaluate the role of preadmission conditions, dehydration specifically, in the development of subdural hematoma in the elderly. Methods: Retrospective chart review. Results: The most prevalent pre-admission medical condition was hypertension, followed by atrial fibrillation, CAD, hyperlipidemia, diabetes, and cancer. Out of 95 patients, 69 (73%) had features of dehydration, 27% were on diuretics and 28% were on antiplatelet agents and anticoagulation. Conclusions: Elderly population is prone to dehydration. Overzealous use of diuretics especially in combination with anticoagulation or antiplatelet therapy can contribute to the development of subdural hematomas, both spontaneous and traumatic.

Support Vector Machine(SVM)has become one of the traditional machine learning algorithms the most used in prediction and classification ***,its behavior strongly depends on some parameters,making tuning these parameters a sensitive step to maintain a good *** the other hand,and as any other classifier,the performance of SVM is also affected by the input set of features used to build the learning model,which makes the selection of relevant features an important task not only to preserve a good classification accuracy but also to reduce the dimensionality of *** this paper,the MRFO+SVM algorithm is introduced by investigating the recent manta ray foraging optimizer to fine-tune the SVM parameters and identify the optimal feature subset *** proposed approach is validated and compared with four SVM-based algorithms over eight benchmarking ***,it is applied to a disease Covid-19 *** experimental results show the high ability of the proposed algorithm to find the appropriate SVM’s parameters,and its acceptable performance to deal with feature selection problem.

The dynamic analysis of financial systems is a developing field that combines mathematics and economics to understand and explain fluctuations in financial *** paper introduces a new three-dimensional(3D)fractional financial map and we dissect its nonlinear dynamics system under commensurate and incommensurate *** such,we evaluate when the equilibrium points are stable or unstable at various fractional *** use many numerical methods,phase plots in 2D and 3D projections,bifurcation diagrams and the maximum Lyapunov *** techniques reveal that financial maps exhibit chaotic attractor *** study is grounded on the Caputo-like discrete operator,which is specifically influenced by the variance of the commensurate and incommensurate ***,we confirm the presence and measure the complexity of chaos in financial maps by the 0-1 test and the approximate entropy ***,we offer nonlinear-type controllers to stabilize the fractional financial *** numerical results of this study are obtained using MATLAB.

In the south Eastern Desert of Egypt,two contrasting types of magmatism(mafic and felsic) are recorded in the Wadi Kalalat area,and form the Gabal El Motaghiarat and Gabal Batuga intrusions,*** two intrusions post-dates ophiolitic and arc associations represented by serpentinite and metagabbro-diorite,*** mafic intrusion has a basal ultramafic member represented by fresh peridotite,which is followed upward by olivine gabbro and anorthositic or *** mafic intrusion pertains to the Alaskan-type mafic-ultramafic intrusions in the Arabian-Nubian Shield(ANS)being of tholeiitic nature and emplaced in a typical arc *** the other hand,the Gabal Batuga intrusion comprises three varieties of fresh A-type granites of high K-calc alkaline nature,which is peraluminous and garnetbearing in parts.A narrow thermal aureole in the olivine gabbro of the mafic intrusion was developed due to the intrusion of the Batuga *** results in the development of a hornfelsic melagabbro variety in which the composition changed from tholeiitic to a calc-alkaline composition due to the addition of S_(i)O_(2),Al_(2)O_(3),alkalis,lithosphile elements(LILEs) such as Rb(70 ppm) and Y(28 ppm) from the felsic *** the thermal aureole,Rb amounts 2-8 ppm and Y lies in the range <*** is believed that the Gabal Batuga felsic intrusion started to emplace during the waning stage of an arc system,with transition from the pre-collisional(i.e.,arc setting) to post-collisional and within plate *** from which the Gabal Batuga granites were fractionated is high-K calc-alkaline giving rise to a typical post-collisional A-type granite(A_(2)-subtype) indicating an origin from an underplating crustal ***,it is stressed here that the younger granites in the ANS are not exclusively post-collisional and within-plate but most likely they started to develop before closure of the arc *** possible source(s) of mafic magmas

Neoproterozoic island arc assemblage of the Arabian–Nubian Shield(ANS)in the Eastern Desert(ED)of Egypt comprises juvenile suites of metavolcanics(MV),large amounts of meta-sedimentary rocks(MS),and voluminous metagabbros-diorites(MGD)and syn-tectonic intrusions of older granitoids(OG).We report here the updates of these four rock units in terms of classification,distribution,chemical characteristics,geodynamic evolution,metamorphism,and *** addition,we discuss these integrated data to elucidate a reasonable and reliable model for crustal evolution in the *** main features of these rock units indicate their relation to each other and the geodynamic environment dominated by early immature oceanic island arcs to primitive continental *** information of the island arc metavolcanic and plutonic rocks(gabbros,diorites,tonalites,and granodiorites)furnish evidence of the genetic *** include proximity and a coeval nature in the field;all protolith magmas are subalkaline in nature following calc-alkaline series with minor tholeiitic affinities;common geochemical signature of the arc rocks and subduction-related magmatism;their similar enrichment in LREEs;and similar major element compositions with mafic melts derived from metasomatized mantle *** volcano-sedimentary and the OG rocks underwent multiphase deformation events whereas the MGD complexes deformed *** on the magmatic,sedimentological,and metamorphic evolutions constrained by geochronological data as well as the progressive evolutionary trend from extensional to compressional regimes,a possible gradual decrease in the subducted slab dip angle is the most infl uential in any geodynamic model for arc assemblage in the ED of Egypt.

In this paper,new four fifth-order fractional nonlinear equations are derived and *** frac-tional terms are defined in the conformable sense and these equations are then solved using two effective methods,namely,the sub-equation and the generalized Kudryashov *** methods were tested on the proposed models and succeeded in finding new solutions with different values of parameters.A graphical representation of some results is provided and proves the efficiency and applicability of the proposed methods for providing solutions with known physical *** methods are good candi-dates for solving other similar problems in the future.

Modern technological advancements have made social media an essential component of daily *** media allow individuals to share thoughts,emotions,and *** analysis plays the function of evaluating whether the sentiment of the text is positive,negative,neutral,or any other personal emotion to understand the sentiment context of the *** analysis is essential in business and society because it impacts strategic *** analysis involves challenges due to lexical variation,an unlabeled dataset,and text distance *** execution time increases due to the sequential processing of the sequence ***,the calculation times for the Transformer models are reduced because of the parallel *** study uses a hybrid deep learning strategy to combine the strengths of the Transformer and Sequence models while ignoring their *** particular,the proposed model integrates the Decoding-enhanced with Bidirectional Encoder Representations from Transformers(BERT)attention(DeBERTa)and the Gated Recurrent Unit(GRU)for sentiment *** the Decoding-enhanced BERT technique,the words are mapped into a compact,semantic word embedding space,and the Gated Recurrent Unit model can capture the distance contextual semantics *** proposed hybrid model achieves F1-scores of 97%on the Twitter Large Language Model(LLM)dataset,which is much higher than the performance of new techniques.
