Astigmatic correction with implantation of a light adjustable vs monofocal lens: a single site analysis of a randomized controlled trial

AIM: To evaluate the light adjustable lens(LAL) vs a standard monofocal lens in achieving target astigmatic refraction and improving postoperative uncorrected distance visual acuity(UDVA). METhODS: This randomized controlled clinical trial included 40 patients with pre-existing astigmatism and visually significant cataract. Twenty-eight patients received the LAL and 12 control patients received a monofocal intraocular lens(IOL) after cataract extraction at a single institution. The patients with the LAL underwent adjustment by ultraviolet(UV) light postoperatively plus subsequent lock-in procedures and all patients returned to clinic for follow up of study parameters at 6, 9, and 12 mo. Manifest refraction, distance visual acuity, and adverse events were recorded at each ***: The mean cylinder before adjustment in eyes with the LAL was-0.89±0.58 D(-2.00 to 0.00 D) and-0.34±0.34 D(-1.25 to 0.00 D) after lock-in(p=1.68 x10-8). The mean cylinder in patients with the monofocal lens was-1.00±0.32 D(-1.50 to-0.50 D) at 17-21 d postoperatively, which was statistically different from the LAL cylinder postlock-in(p=1.43 x10-6). UDVA in the LAL group was 20/20 or better in 79% of patients post lock-in with good stability over 12 mo compared with 33% of the control patients with UDVA of 20/20 or better. CONCLUSION: These results demonstrate that the LAL is more effective in achieving target refractions and improving postoperative UDVA in patients with pre-existing corneal astigmatism than a standard monofocal lens.

Discussions on the correspondence of dissipative particle dynamics and Langevin dynamics at small scales

We investigate the behavior of dissipative particle dynamics （DpD） within different scaling regimes by numerical simulations. The paper extends earlier analytical findings of Ripoll, M., Ernst, M. h., and Espafiol, p. （Large scale and mesoscopic hy- drodynamics for dissipative particle dynamics. Journal of Chemical physics, 115（15）, 7271-7281 （2001）） by evaluation of numerical data for the particle and collective scaling regimes and the four different subregimes. DpD simulations are performed for a range of dynamic overlapping parameters. Based on analyses of the current auto-correlation functions （CACFs）, we demonstrate that within the particle regime at scales smaller than its force cut-off radius, DpD follows Langevin dynamics. For the collective regime, we show that the small-scale behavior of DpD differs from Langevin dynamics. For the wavenumber-dependent effective shear viscosity, universal scaling regimes are observed in the microscopic and mesoscopic wavenumber ranges over the considered range of dynamic overlapping parameters.

近年来随着核工业的快速发展,以及福岛和切尔诺贝利核电站泄漏事故,引起公众对水体放射性污染的广泛关注。放射性锶是核燃料的主要裂变产物之一和核废物的主要组分,半衰期长,在水环境中有较强的迁移性,危害人类健康和环境,因此研究如何从水中分离去除Sr非常重要。采用水热法合成了一种含钠、锰、锡和硫的金属硫化物吸附剂(NMTS-2),通过改变锰的用量、水量、反应温度和反应时间等优化吸附剂的合成。此外,为进一步降低成本,用锡盐替代金属锡,且控制锡盐的用量最低,同时保持了较高的Sr去除率;与Na ZTS和Na TS的相比,NMTS-2的原料分别下降69%和76%。合成NMTS-2的优化原料配比为1.150 kg NaCO/kg、0.550 kg Mn SO/kg、0.411 kg Sn Cl/kg、0.696 kg S/kg、0.361 kg hO/kg,优化反应条件和参数为200°C和24小时。采用SEM、XpS、pXRD等多种手段表征NMTS-2的形貌特征、元素组成和结构稳定性,发现元素均匀分布在吸附剂表面,其在p h值不低于4时能够保持化学稳定性,低于365℃时保持热稳定性。为确定NMTS-2对水中Sr的吸附特性,完成了吸附动力学、等温线、影响因素和吸附选择性的研究,结果表明:NMTS-2吸附水中初始浓度为5.000mg/L的Sr,在5min后达到吸附平衡,符合拟二级动力学模型;298 K时最佳拟合吸附等温线符合Langmuir吸附等温模型,最大吸附量为47.04 mg/g,与实验值46.10 mg/g非常吻合;吸附剂投加量为0.5000 g/L、初始Sr浓度为5.000mg/L、NMTS-2在p h值为4-12时,对Sr的去除率为96.81%-99.96%,分配系数(K)为6.06×10-4.70×10 m L/g;水中共存离子会降低Sr的吸附去除率,影响顺序为Ca>Mg>>Na>K;NMTS-2对自来水和湖水的锶具有优异的吸附选择性;此外,NMTS-2吸附去除Sr的机理为离子交换和配位作用,去离子水中Sr的解吸速率可以忽略不计。综上所述,即使在强酸性或强碱性的放射性废水中,NMTS-2对放射性Sr的吸附也具有快速、有效和高选择性,可以为放射性含锶废水的应急处理提供支持。

Effect of airplane transport of donor livers on post-liver transplantation survival

AIM To evaluate the effect of long haul airplane transport of donor livers on post-transplant outcomes. METhODS A retrospective cohort study of patients who received a liver transplantation was performed in perth, Australia from 1992 to 2012. Donor and recipient characteristics information were extracted from Western Australian liver transplantation service database. patients were followed up for a mean of six years. patient and graft survival were evaluated and compared between patients who received a local donor liver and those who received an airplane transported donor liver. predictors of survival were determined by univariate and multivariate analysis using cox *** One hundred and ninety-three patients received alocal donor liver and 93 patients received an airplane transported donor liver. Airplane transported livers had a significantly lower alanine transaminase(mean: 45 U/L vs 84 U/L, p = 0.035), higher donor risk index(mean: 1.88 vs 1.42, p hemic time(CIT)(mean: 10.1 h vs 6.4 h, p here was a weak correlation between CIT and transport distance(r2 = 0.29, p p was six years and 93 patients had graft failure. Multivariate analysis found only airplane transport retained significance for graft loss(hR = 1.92, 95%CI: 1.16-3.17). One year graft survival was 0.88 for those with a local liver and was 0.71 for those with an airplane transported liver. One year graft loss was due to primary graft non-function or associated with preservation injury in 20.8% of recipients of an airplane transported liver compared with 4.6% in those with a local liver(p = 0.027). CONCLUSION Airplane transport of donor livers was independently associated with reduced graft survival following liver transplantation.

OBJECTIVES: The purpose of this study was to investigate the relationship of serum digoxin concentration(SDC) and outcomes in women with heart failure(hF). BACKGROUND: Controversy continues concerning the clinical utility of digoxin in women with hF. METhODS:Our analysis was retrospective with data from the Digitalis Investigation Group(DIG) trial. The principal study analysis reviewed 4,944 patients with hF due to systolic dysfunction who survived for at least 4 weeks(all 3,366 patients randomized to placebo and the 1,578 of 3,372 patients randomized to digoxin who had serum concentration measured 6 to 30 h[inclusive] after the last dose of study drug at 4 weeks). RESULTS: Continuous multivariable analysis demonstrated a significant linear relationship between SDC and mortality in women(p=0.008) and men(p=0.002, p=0.766 for gender interaction). Averaging hazard ratios(hRs) across serum concentrations from 0.5 to 0.9 ng/ml in women produced a hR for death of 0.8(95%confidence interval[CI] 0.62 to 1.13, p=0.245) and for death or hospital stay for worsening hF of 0.73(95%CI 0.58 to 0.93, p=0.011). In contrast, SDCs from 1.2 to 2.0 ng/ml were associated with a hR for death for women of 1.33(95%CI 1.001 to 1.76, p=0.049). CONCLUSIONS: Retrospective analysis of data from the DIG trial indicates a beneficial effect of digoxin on morbidity and no excess mortality in women at serum concentrations from 0.5 to 0.9 ng/ml, whereas serum concentrations ≥1.2 ng/ml seem harmful.

Ergot Alkaloid Effects on Bovine Sperm Motility <i>In Vitro</i>

Cattle in some parts of the world graze pastures that consist of tall fescue that may contain ergot alkaloid contamination. Those ergot alkaloids are associated with reduced reproductive rates in cattle. Our objective was to determine if ergot alkaloids [dihydroergotamine (DhET), ergonovine (EN), and ergotamine (ET)] directly affect bovine sperm characteristics. Spermatozoa were collected from mature Angus (n = 2) and Balancer (n = 4) bulls. Within bull, treatments were structured as a 3 × 5 factorial with three alkaloids (DhET, EN, and ET) and five concentrations of each alkaloid (0, 33, 66, 100, or 200 μM). Spermatozoa (25 × 106) were incubated (39?C) in 1 mL of modified sperm medium. Sperm motility characteristics were evaluated using CASA (hamiliton Thorne IVOS, Beverly, MA) at 0, 3, and 6 h after initial alkaloid exposure. Initial sperm motility was (69% ± 1.1%) and declined (p = 0.01) to (35% ± 2.6%) at 6 h. Sperm motility decreased (p p h characteristics. Our results demonstrate that ergot alkaloids (ET and DhET) can directly alter bovine sperm motility and morphology, which adds to our understanding of how ergot alkaloids may hinder cattle reproductive rates.

Objective:Unplanned 30-day readmissions result in increased costs and decreased patient *** objective of this study was to compare readmission rates before and after a multidisciplinary quality improvement initiative that focused on patient and staff education,use of targeted skilled nursing facilities,and appropriate use of patient observation ***:This was a quality improvement study of all unplanned admissions to the head and Neck Oncology service at a tertiary care facility during a 3-year period between October 2015 and September *** October 2016,the head and Neck Oncology service revised its discharge practices for patients undergoing extirpative and/or reconstructive *** changes included enhancing patient education,increasing the use of a skilled nursing facility with directed staff education and patient handoffs by advanced practice nurses,and appropriate utilization of 23-h observation status for returning *** readmission rate from the pre-intervention era(October 2015 through September 2016)was compared to the readmission rate from the post-intervention era(October 2016 through September 2018).Secondary outcomes were the rates of 23-h observation within 30 days of the discharge as well as emergency room visits within 30 days of ***:In this sample of 449 patients,161(35.9%)were observed before the change-in-practice(before October 2016),and 288(64.1%)were observed following the change-in-practice(after September 2016).On univariable analysis,the risk of readmission declined by approximately 41.4%from the pre-intervention era,though this conclusion was not statistically significant(p=0.06).On multivariable analysis,patients at moderate or high risk of death were 2.31 times more likely than those at minor risk of death to readmit within 30 days(p=0.03).Similarly,those with recurrent or persistent cancer were 3.33 times more likely than those undergoing initial curative surgical management of cancer t
