Study of the Factors Associated with Home Delivery in a Situation of Free Care in Senegal

Introduction: Home delivery is still a health problem in Kolda (Senegal). The aim of this work is to study the factors associated with the place of delivery in a situation of free care delivery. Method: A descriptive and analytical cross-sectional study was carried out. Sampling was in two-stage clusters. The study focused on women who gave birth in the last 12months. The data were collected during an individual interview at home. The collection focused on knowledge, attitudes and practices about delivery. Logistic regression was used to explore the determinants of childbirth at the level of health facility. Results: The average age was 25.4 ± 6.5 years. Among the interviewed women, education was reported in 35% of the women and 55% of women estimate that the closest health facility is within 5 km to their home. The time to get to the nearest health facility is less than 15 minutes for 39% of the interviewed women. The prevalence of home delivery was 43.5%. Home delivery was related to the remoteness of the health facility (2.43 [1.75 - 3.37]) but also to incomplete antenatal care (2.52 [1.59 - 4.00]). Support groups highlighted difficulties of access to health facilities because of remoteness. Interviews revealed a lack of involvement of husbands in seeking care for women. Cultural barriers are still there. Conclusions: Despite the gratuity of delivery, the remoteness of health structures and socio-cultural factors are still barriers to access to care for pregnant women. These aspects must be taken into account in health policies. Multi-lateral interventions should be implemented to provide solutions for this health problem.

Analysis of the Effect of Dietary Diversity on Anemia in Women of Reproductive Age in Senegal

Introduction: Anemia in women of reproductive age is a common health problem in Senegal. The objective of this research was to study the contribution of dietary diversity in the fight against anemia in women of reproductive age in two regions of Senegal using the propensity score method. Methodology: An observational, cross-sectional and analytical study of women of reproductive age and their households was conducted in the Kolda and Kedougou regions in 2015. This was a three-stage random sample survey. The data was collected during an individual interview. Conditional logistic regression was used to identify the risk factor for this anemia. Results: In this study, 1926 women of reproductive age were surveyed. The average age of women of reproductive age surveyed was 27.2 years with a standard deviation of 7.6 years. The median age was 26 years old. Married women were in the majority. Pregnant and lactating women were 43.8%. The percentage of women with adequate diets was 31.5%. The proportion of women with anemia was 51.8%, or 998 women of reproductive age. Adequate dietary diversity is the main factor associated with anemia in these women (OR = 0.75 (0.59 - 0.96)). Conclusion: The prevalence of anemia in women of reproductive age is still high in the southern regions of Senegal. To combat this phenomenon, it is therefore necessary to intensify interventions on the balance of diet, iron and folic acid supplementation in pregnant and lactating women and to strengthen the empowerment of women.

Midwives’ Professional Satisfaction and the Quality of Obstetric Care in Developing Country

Introduction: In Africa, poor working conditions affect the motivation of health care providers. The objective is to study midwives’ professional satisfaction on the technical quality of emergency obstetric care in Senegal. Material and Methods: This was a prospective study of 16 hospitals in Senegal. The job satisfaction of midwives was measured during a personal interview. The instrument used was validated in Senegal and Mali. Other data collected relate to professional and institutional characteristics. A treatment observation grid was used to measure the quality of obstetric care in labour wards. Mixed-model linear regression was used to estimate the effect of satisfaction on the quality of obstetric care. Results: A total of 65 midwives were interviewed and 325 patients observed. The average quality score was 8.7 ± 1.7. The satisfaction scores ranged from 42.2 ± 17.4 (salary) to 76.7 ± 12.1 (morale satisfaction). A positive and significant correlation was found between quality of care and management (cc = 0.56), remuneration (cc = 0.40), task (cc = 0.32), workload (cc = 0.24) and training (cc = 0.29). The linear mixed model shows that salary (β = 0.40), continuing education (β = 0.17) and management style (β = 0.42) improved the quality of care. Association between moral satisfaction and quality care was negative (β = ﹣0.53). Conclusion: The satisfaction of health professionals is a major determinant of the quality of obstetric care. Its inclusion in the fight against mortality has become imperative in developing.

Clinical Management and Long-Term Follow-Up of Patients with Peripartum Cardiomyopathy: A Prospective Registry Study

Introduction: Peripartum cardiomyopathy (PPCM) is a rare pathology in Western countries but is common in Africa. Its progression is highly variable, left ventricular function improves in almost one-third to one-half of patients. In sub-Saharan Africa, there are few prospective cohort studies. We aimed to describe the long-term evolutionary aspects of this pathology in a sub-Saharan African country, so we developed a PPCM registry;here, we present the first results after 2 years of follow-up. Methodology: This work was performed at the cardiology clinic of the Aristide Le Dantec Teaching Hospital of Dakar from January 01, 2017, to January 01, 2021, for a total duration of 4 years. This was an observational, longitudinal prospective study including patients admitted for peripartum cardiomyopathy. Results: During our study, 5372 patients were admitted to the cardiology clinic. Considering the inclusion criteria, 79 patients were consecutively recruited. The mean age was 30.5 ± 6.7 years, ranging from 18 to 42 years. Half of the patients came from rural areas (56.3%), and 78.2% of patients had a low socioeconomic status. Multiparity and twin pregnancies were noted in 72.8% and 20% of the patients, respectively. A total of 91% of patients had advanced NYHA stage 4 heart failure, and 3 patients had cardiogenic shock. Left ventricular dilatation was found in 52 patients, and severe left ventricular systolic dysfunction was found in 50 patients (90.9%). During hospitalization, 19 patients (34.5%) had complications. The evolution in the hospital was favourable in 45 patients (81.8%). The global mortality rate was 7.3% at 2 years. In multivariate analysis, fewer patients with a dilated left ventricle, a severe alteration of the LVEF and an advanced age progressed towards remission. Conclusion: The long-term evolution of PPCM is very variable. Despite a good rate of remission, progression to end-stage heart failure and death is not negligible in cases of advanced maternal age

Air Pollution Related to Traffic and Chronic Respiratory Diseases (Asthma and COPD) in Africa

Introduction: Chronic respiratory diseases (CRD) are obvious effects of air pollution and the third reason of death in developing countries. In Africa, air pollution from road traffic is one of the main causes of poor air quality. We set out to systematically review existing published researches on traffic related to air pollution and CRD, particularly asthma and Chronic Obstructive Pulmonary Disease (COPD) in Africa. Methods: A literature search of PubMed, Scholar and LISSA databases, published journals, reference articles, published up to 31 December 2016, has been done by using a research strategy procedure. Texts were reviewed for inclusion. Studies were included if they met the following criteria: 1) the relationship between asthma or COPD with ambient air pollution related to road traffic was studied and 2) the population included people from Africa or lived in Africa. Articles written in English and French were included. Results: Fifty-five articles were selected in this review, of which twenty-seven were on air pollution and CRD in Africa. The proximity of the residence or workplace to the traffic is associated with an increased risk of asthma with a dose-response relationship. The estimated prevalence of COPD varies between 2.7% and 38.5%. Conclusion: There is little research on traffic related to air pollution and CRD in Africa. Strategies to reduce traffic related to air pollution in African cities have been proposed in order to have a healthier ambient air.

Assessment of Women’s Dietary Diversity in Southern Senegal

Introduction: Lack of dietary diversity is a nutritional problem among poor people in developing countries whose daily diet is usually based on a starch source with one or two additional components. Senegal has food insecure regions such as Kolda and Kedougou despite improved agricultural production. This study aims to investigate the determinants of dietary diversity in the Kolda and Kedougou regions. Methodology: It was a cross-sectional, descriptive and analytic household-level study in two southern regions of Senegal (Kolda and Kedougou) between October and December 2015. This was a three-stage random sample survey. Dietary diversity in women of reproductive age was measured using the Women’s Dietary Diversity Score (WDDS) recommended by Food and Agriculture Organization of the United Nations (FAO). The WDDS is a global qualitative indicator of nutrition, which counts the number of food groups consumed by a person in the 24 hours preceding the survey. The data was collected during an individual interview. Linear regression was used to identify the associated factors of dietary diversity. The free and informed consent of the individuals surveyed and the protection of anonymity were respected. Results: A total of 1926 women were surveyed. The mean of WDDS was 2.9 with a standard deviation of 1.4. The Kedougou department has a better WDDS with 3.3 (±1.2) followed by the Kolda department with a score of 3.2 (±1.3). The higher the household’s welfare score, the better the WDDS with a ßcoefficient of 0.11 [0.09 - 0.13]. Households that are in urban areas have a better score than households in rural areas (0.27 [0.13 - 0.41]). The improvement of knowledge in food diversification, in good culinary practice improves the dietary diversity score in women. Conclusion: The WDDS is low in the southern regions of Senegal. Study of the Determinants of the Use of Breast Cancer Screening in Women 35 to 65 Years in the Region of Thies (Senegal)

Introduction: In Senegal, breast cancer is the second most common cancer of women after cervical cancer. It is a cancer accessible to clinical and radiological screening. The objective of this study was to study the determinants of the use of breast cancer screening among women aged 35 to 65 in the Thies region in 2015. Material and Methods: It was a descriptive and analytical cross-sectional study. It was conducted with a sample of women selected using a two-stage cluster survey. The study used a questionnaire to collect data on the individual characteristics of women, knowledge, attitudes and practice on breast cancer and quality of care in an individual interview at the homes of women after consent. The data was entered on the Epi Info 3.5.3 software and analyzed with the R 3.3.3 software. A description of qualitative variables across proportions and quantitative variables by mean and standard deviation was performed. Parametric or non-parametric tests were used to examine the association between the dependent variable (breast cancer screening) and explanatory variables (predisposing factors, resources, health needs) according to their applicability conditions and at a risk α equal 5%. Finally, all variables with p less than 0.25 were used to construct an explanatory model by simple logistic regression. Results: The study involved 960 women aged 35 to 65 years. The average age was 43.8 years with a standard deviation of 8.3 years. The rate of breast cancer screening was 25.7%. Educated women (OR adjusted = 1.89 [1.25 - 2.85]), women who resided in urban settings (adjusted OR = 1.79 [1.05 - 3.04]), those who experienced at least one risk factor for breast cancer (OR adjusted = 2.71 [1.81 - 4.06]) were more likely to use screening breast cancer. In contrast, women with low socioeconomic status (adjusted OR = 0.66 [0.45 - 0.97]) were less likely to be screened. Introduction: Many health problems in pregnant women result in part from inadequate nutrition. Much progress has been made in improving the nutritional situation in Dakar, but it remains a concern to this day. Promoting dietary diversity is an effective way to combat any kind of nutrient deficiency. This study aims to investigate the determinants of dietary diversity in pregnant women in the Dakar region. Method: It was a cross-sectional, descriptive, analytical study with a three-stage survey. The calculated sample was 292 pregnant women. Socio-demographic, economic, pregnancy-related data, related to practices and knowledge on nutrition were collected using a questionnaire and an observation grid. The collection was conducted from November 01 to December 15, 2017 among pregnant women in the Dakar region. Data were entered using epi info software and analyses were done with R. Simple linear regression was used to find the explanatory factors of dietary diversification. The fit of the regression model was done by the Hosmer Lemeshow test. The odds ratios were measured with a 5% alpha risk. Results: A total of 300 pregnant women were surveyed in two different health districts. Prevalence and Associated Factors of Postpartum Depression: A Study Conducted in Health Centres in Dakar (Senegal)

Introduction: Postpartum depression is the most common psychiatric disorder affecting women in the perinatal period. According to the WHO, it affects 10% - 15% of women who have just given birth. In Africa, and particularly in Senegal, epidemiological studies focusing exclusively on postpartum depression are rare or non-existent. For this reason, we conducted this screening study on postpartum depression to assess the socio-demographic characteristics and associated factors. Methods: This was a prospective cross- sectional study with descriptive and analytical aims. It was carried out in the twelve Dakar referral health centres during the period August 2021 to June 2022. Results: At the end of our survey we had recruited one hundred and twenty-two (122) participants with a PPD prevalence rate of 25.41%. The most common age group was 24 - 29 years. 40% of the participants were of Fulani ethnicity, followed by Wolof. Married women made up 93% of the sample. The school enrolment rate was 89%. Most participants were not in paid employment. Domestic violence was present in 14% of couples. 28% of mothers reported experiencing a stressful life event during their pregnancy. 22% of the women had a chronic medical condition, of which 55% had high blood pressure. 1% of the women reported a personal psychiatric history and 5% a family psychiatric history. Most women had between two and four healthy living children. The pregnancy was unwanted in 17% of cases and 12% of participants had a complication. 6% were dissatisfied with the quality of their antenatal care. The caesarean section rate was 18%. In terms of associated factors, eight factors were identified in a bivariate analysis: single marital status, lack of support from partner, domestic violence, conflict with in-laws, and the occurrence of a stressful life event during pregnancy, the presence of a personal medical history, lack of satisfaction with pregnancy follow-up, and mode of delivery by caesarean section. Traffic Air Pollution and Respiratory Health: A Cross-Sectional Study among Bus Drivers in Dakar (Senegal)

Introduction: Road traffic exposes bus drivers to the detrimental effects of air pollutants on respiratory health. This study determined the frequency of chronic respiratory illnesses and its related factors among bus drivers in Dakar, Senegal. Methods: This study had a cross-sectional design conducted in a total of 178 bus drivers in Dakar, Senegal. A questionnaire was used to inquire about socio-economic characteristics, occupational factors and respiratory symptoms of bus drivers. Lung function tests were used to determine the presence of asthma and Chronic Obstructive Pulmonary Disease (COPD). The relationship between our variables of interest and respiratory diseases was determined by logistic regression analysis. Results: The results of the study show that 57.9% of bus drivers had a chronic cough, and 65.7% had recurrent cold. Lung function tests showed that 38.8% of bus drivers had asthma and 30.3% COPD. Multivariate analysis found that recurrent cold increased the risk of asthma (OR = 6.3, 95% CI: 1.12 - 35.79) and COPD (OR = 7.7, 95% CI: 1.14 - 52.8). The respiratory health status of bus drivers depended on the work area (OR = 3.2, 95% CI: 1.13 - 9.31). Conclusion: The respiratory symptoms and illnesses observed among bus drivers are associated with their exposure to air pollutants from road traffic.
