Introduction: Joint manifestations are common in systemic lupus, affecting multiple joints or just one. These manifestations are present in 95% of patients. Pseudo-septic arthritis is a subset of systemic lupus that mimics septic arthritis caused by the deposition of lipid droplets in the joint. We present the case of a patient hospitalized for mono-arthritis, which revealed systemic lupus. Observation: The patient is a 19-year-old woman, single, without children, and with no previous medical history, who was hospitalized for fever and inflammatory polyarthralgia. The clinical examination revealed an inflammatory swelling of the right knee with patellar ballottement, yellow citrine synovial fluid, an inflammatory tumor on the left arm, which was round, mobile, and fluctuating with purulent content, edema of the lower limbs, and facial puffiness, along with a systemic inflammatory response syndrome. Paraclinical examination showed hyperleukocytosis with neutrophil predominance, microcytic anemia, thrombocytopenia, antistreptolysin O (ASLO) 3, suggesting inflammatory arthritis. Bacteriological tests did not isolate any pathogens. An arthrotomy with synovial biopsy was performed, and the histopathological examination supported non-specific synovitis. A joint ultrasound showed signs of arthritis with a septic appearance. During the third hospitalization for persistent fever, she presented with facial puffiness, anemia syndrome, systemic inflammatory response syndrome, and a malar rash across the bridge of the nose. Antinuclear antibodies returned positive with a titer of 1280 and a speckled fluorescence pattern, and anti-native DNA antibodies were positive at 60.1 (normal Conclusion: Pseudo-septic arthritis is a feature of lupus that can mimic septic arthritis. Monoarticular involvement is rare but possible. The absence of pathogens and the inflammatory nature of the synovial fluid should prompt consideration of a lupus-related etiology.

Topology Adaptation for Robust Ad Hoc Cyberphysical Networks under Puncture-Style Attacks

Many cyber physical networks will involve ad hoc deployments utilizing peer-to-peer communications. Examples include transportation systems where a group of moving cars communicate in order to avoid collisions, teams of robotic agents that work together in support of disaster recovery, and sensor networks deployed for health-care monitoring, monitoring the operation of a factory plant or to coordinate and actuate mechanisms for energy conservation in a building. These networks may face a variety of threats that puncture their connectivity and, should their performance degrade, the result could be catastrophic. Consider, for example, a vehicular ad hoc network where communication assists collision avoidance. In such a case, degradation could lead to vehicle accidents. Therefore, in order to overcome network performance degradations and the puncture of a network （such as blackhole or jamming） which is under attack, we propose an algorithm called the Fiedler Value Power Adjustment Topology Adaption （FVPATA）. FVPATA aims to dynamically adapt an ad hoc network＇s topology, even if the attacker varies its location and in the case of an interference-style attack by increasing the interference power. The algorithm utilizes the formulation from the graph theory which works with the Fiedler value to guide each node in wireless ad hoc network utilizing power adjustments to enhance the network＇s overall robustness. The advantage of the proposed mechanism is that it is a light-weight approach which is totally distributed, based on topology updates inherent in the Optimized Link State Routing （OLSR） protocol and, hence, it is unnecessary to introduce additional messages. Additionally, an algorithm was developed to resolve problems involving asymmetric links that arise in ad hoc networks by eliminating unnecessary energy consumption of Fiedler nodes. Distributed Beamforming Based Wireless Power Transfer:Analysis and Realization

This paper presents a new Wireless Power Transfer(WPT)approach by aligning the phases of a group of spatially distributed Radio Frequency(RF)transmitters(TX)at the target receiver(RX)*** approach can transfer energy over tens of meters and even to targets blocked by *** to popular beamforming based WPTs,our approach leads to a drastically different energy density distribution:the energy density at the target receiver is much higher than the energy density at other *** to this unique energy distribution pattern,our approach offers a safer WPT solution,which can be potentially scaled up to ship a higher level of energy over longer ***,we model the energy density distribution and prove that our proposed system can create a high energy peak exactly at the target *** we conduct detailed simulation studies to investigate how the actual energy distribution is impacted by various important system parameters,including number/topology of transmitters,transmitter antenna directionality,the distance between receiver and transmitters,and environmental ***,we build an actual prototype with 17 N210 and 4 B210 Universal Software Radio Peripheral(USRP)nodes,through which we validate the salient features and performance promises of the proposed system.

The 3-billion fossil question:How to automate classification of microfossils

Microfossil classification is an important discipline in subsurface exploration,for both oil&gas and Carbon Capture and Storage(CCS).The abundance and distribution of species found in sedimentary rocks provide valuable information about the age and depositional ***,the analysis is difficult and consuming,time-as it is based on manual work by human *** to automate this process face two key challenges:(1)the input data are very large-our dataset is projected to grow to 3 billion microfossils,and(2)there are not enough labeled data to use the standard procedure of training a deep learning *** propose an efficient pipeline for processing and grouping fossils by genus,or even species,from microscope slides using self-supervised *** we show how to efficiently extract crops from whole slide images by adapting previously trained object detection ***,we provide a comparison of a range of self-supervised learning methods to classify and identify microfossils from very few *** obtain excellent results with both convolutional neural networks and vision transformers fine-tuned by *** approach is fast and computationally light,providing a handy tool for geologists working with microfossils.

Rare case of tibial hemimelia,preaxial polydactyly,and club foot

A seven-month old female presented with left tibial hemimelia(or congenital tibial aplasia; Weber type VIIb, Jones et al type 1a), seven-toed preaxial polydactyly, and severe club foot(congenital talipes equinovarus). Definitive amputation surgery disarticulated the lower limb at the knee. This case report describes the anatomical findings of a systematic post-amputation examination of the lower limb's superficial dissection, X-ray s, and computed to mography (CT) scans. From the X-rays and CT scans, we found curved and overlapping preaxial supernumerary toes, hypoplastic first metatarsal, lack of middle and distal phalanges in one supernumerary toe, three tarsal bones, hypoplastic middle phalanx and no distal phalanx for fourth toe, andno middle or distal phalanges for fifth toe. The fibula articulated with the anteromedial calcaneus and the tibia was completely absent. We identified numerous muscles and nerves in the superficial dissection that are described in the results section of the case report. Due to the rarity of this combination of anatomical findings, descriptions of such cases are very infrequent in the literature.
