Splenic injury caused by abdominal trauma during pregnancy is rare. Splenic injury associated with fetal death in utero following a fall from a tree top is even rarer. The authors report a case of splenic trauma associated with fetal death in utero following a fall from the top of a tree in a 19-year-old pregnant woman at 30 weeks amenorrhoea. She was referred from a 1st level health facility for trauma that had occurred the day before admission. She was in poor general condition (WHO performance status IV) and had a cardiovascular collapse. Ultrasound was used to diagnose haemoperitoneum and fetal death in utero. A CT scan was used to diagnose splenic lesions. Treatment consisted of splenectomy and caesarean section after resuscitation.

Background: Adolescents in developing countries have limited knowledge about the prevention of STIs and unwanted pregnancies. In several African countries, risky sexual behaviour persists, including early sexual debut, multiple sexual partners, economic and sexual exchange, and low condom and contraceptive use. The purpose of this study is to assess the sexual and reproductive health knowledge and behaviour of adolescents in rural Burkina Faso in order to improve their sexual and reproductive health. Method: This was a descriptive cross-sectional study with prospective data collection conducted from 07 March to 04 April 2022 in the area of the Demographic and Health Surveillance System of the Nouna Research Centre. A questionnaire was administered to a randomly selected sample of 1202 adolescents. Results: The mean age of the adolescents was 14.1 years and 56.6% were male. Only 58.2% of the adolescents had ever heard of HIV/AIDS. Of these, 95.7% did not know their HIV status. In addition, 15.8% had ever had sexual intercourse and 2.4% had been sexually active at an early age. Almost 45% had not used any contraceptive method the last time they had sex. Condoms were the most frequently used contraceptive method (47.4%). Among teenagers, 27.6% had been pregnant at least once, 7.3% were married and the average age at marriage was 18.45 years. Almost 7% (6.9%) of teenagers had more than one sexual partner. Conclusion: Adolescents have limited knowledge about reproductive health. Risky sexual behaviour persists. Awareness campaigns need to be intensified to improve their sexual and reproductive health.

Sexual Assault in the Department of Obstetrics and Gynaecology of the Bamako’s 5th Municipality Medical Center

Background: The authors report cases of sexual assault reported to the obstetrics and gynecology department of the Bamako’s 5th municipality medical center during a prospective study of eight months from 1st January to 31st August 31st, 2012. Results: Twenty-one cases of assault were reported out of a total of 13,482 consultations, representing a frequency of 0%, 15%. Most victims were represented by students and accounted 43% of the study population. In 76% of cases the victims knew their assailant. The type of sexual contact was genito-genital vaginal penetration in 67% of cases and the condom was used in only one case. The average time before consultation was 3 days. The preventive ARV treatment was administered in 24% of cases and emergency contraception in 43% of cases. Conclusion: Sexual assault is common in Bamako. The reality is probably underestimated because many cases are not reported. The victims are mostly children and adolescents. The risk of transmission of STIs and HIV is very high during the sexual assault. Education and awareness of the population are essential for early consultation. Training of health workers to care for victims of sexual assault is needed.

Tuberculosis of the Uterine Cervix: About a Case and Literature Review

The authors report a case of tuberculosis of the uterine cervix observed at the maternity unit of the Souro Sanon Teaching Hospital (CHUSS) of Bobo-Dioulasso. This is a rare localization of genital tuberculosis and can take the appearance of a cervical cancer. Biopsy with histological study of the lesion confirms the diagnosis. The frequency of this pathology can reach up to 8%. The extension to the endometrium and/or tubes in a young woman may compromise future fertility. Early and well conducted TB treatment allows the ad integrum restitution cervix.

To better understand the effects of direct sowing under mulch-based cropping system (DMC) in Burkina Faso’s cotton production systems, randomized blocks of Fisher experimental design were implemented at Farako-Ba research station from 2010 to 2019. The study was conducted on lixisoil to evaluate DMC effects on biomass production, crops yields and soil chemical properties in a maize and cotton rotation system associated with cover crop. Conventional tillage and direct seeding without cover crop were compared to DMC under B. ruziziensis (GERM. & EVRARD), DMC under B. ruziziensis + M. cochinchinensis mulch and DMC under C. juncea (L.) mulch used in association with maize. Biomass production, crop yields and soil chemistry were evaluated. Results showed that over 10 years, no-till with or without a cover crop provided cotton seed and maize yields that were statistically equivalent to the tillage commonly practiced by farmers. Cover crop has allowed increasing the biomass production compared to Conventional Tillage and Direct Seeding. Maize yield has not varied significantly with the cover crop. After 10 years of maize and cotton rotation, the improvement raised from +27% to +38% for organic matter and from +15% to +29% for nitrogen with DMC including legumes such as M. cochinchinensis and C. juncea compared to Conventional Tillage on 0 - 5 cm depth. No significant differences were found on soil pH like P2O5 and K2O content. Although DMC with C. juncea used as cover crop did not provide the best biomass production, it contributes to increase nitrogen and organic matter and presents better mineral balances in 10 years of rotation. The 5 - 10 cm and 10 - 20 cm were little influenced by DMC systems.

Despite the complications related to the caesarean section, some couples choose it by desirability. This study aimed to understand the reasons for maternal requests for a planned caesarean section in Burkina Faso, in the absence of obstetric indications. A qualitative descriptive study was conducted in March 2019, consisting of twelve semi-structured interviews with women requesting caesarean section at Tingandogo Teaching Hospital in Burkina Faso. The operative reports were also examined. Thematic analysis based on Braun and Clarke approach was performed with QDA Miner software. Twelve patients were involved in the study. The mean age was 33 years. The majority had a higher level of education. Half were employees. The mean stay duration was 72 hours, with an estimated cost of CFA 300.000 (457,347 EUR). The choice of a planned caesarean was motivated by fear of birth (tokophobia), the positive feeling associated to the surgical technology, the doctor-patient relationship and the confidence in the quality of healthcare services such as reception and hygiene. Maternal reasons for requesting a planned caesarean section in Burkina Faso are multifactorial. Information, education and communication and shared decision-making strategies concerning the different modes of delivery and complications at the end of pregnancy, may contribute to reducing this problem.

Phosphorus (P) is an essential macronutrient for the growth and development of cocoa tree (Theobroma cacao Linn). Most of the soils used for cocoa farming in C?te d’Ivoire are low in phosphorus. But cocoa farmers generally have a widespread reluctance to invest in chemicals fertilizers due to high costs and environmental associated risk. Phosphate Solubilizing Bacteria (PSB) are kwon to play an important role in supply of phosphorous to plants in a sustainable manner in P deficient soils. The aim of this research was to screen the endophytic bacteria of cocoa nurseries able to solubilize tri-calcium phosphate. Seedlings of two varieties of cocoa (P7 and NA32) and seedlings of an all-comer, were grown on eighteen (18) samples soils collected in six producing regions of C?te d’Ivoire. A total of 218 endophytic bacteria were isolated and tested on the Pikovskaya’s agar medium, containing Ca3 (PO4) 2. The colonies with clear zone around the microbial growth were suspected as phosphate solubilizing bacteria. Out of 218 bacteria, 90 (41.28%) showed a clear zone around colonies after 7 days of incubation. The Phosphate Solubilization Index (PSI) ranged from 20% to 200%. Bacterial isolated from the soils of Duékoué locality showed the highest mean index of 137.67%. Five PSB (CEBSP5, CEBSP6, CEBSP7, CEBSP8, and CEBSP9) from Duékoué soils and two PSB (CEBSP12 and CEBSP13) from Soubré soils have a PSI ranged from 150% to 200%. Further study in greenhouse and in field condition will confirm the use of these PSB as biofertilizer to increase the available P content in soils, reduce environmental pollution and promote sustainable agriculture.

Contribution of Laser Photovaporisation in the Management of Lower Urinary Tract Symptoms Related to Prostatic Tumours: Preliminary Study of 29 Cases at the Saint Camille Hospital of Ouagadougou (Burkina Faso)

The treatment of prostatic tumour-related lower urinary tract symptoms (LUTS) uses a varied therapeutic arsenal. In our country, the treatment is dominated by open surgery. Laser treatment is becoming an efficient alternative to trans-urethral prostatic resection or prostatic adenomectomy. We report our experience with the results of Thulium laser photovaporization in the treatment of prostatic tumour-related LUTS. Materials and Methods: A prospective study was conducted between November 2018 and August 2019 at the Saint Camille hospital of Ouagadougou. The inclusion criteria were the presence of LUTS related to a prostatic tumour with an operative indication. Results: 29 patients were treated with Thulium laser prostatic photovaporization. The average age of patients was 72.58 ± 25.34 years (54 - 84 years). The average vaporization duration was 71.24 ± 21.65 minutes (15 - 210 minutes) for an average prostatic volume of 80.97 ml. The average duration of hospitalization for patients was 1.93 days (1 - 5 days). The average bladder sounding duration was 1.68 days (1 - 4 days). A capsular perforation, a sphincter lesion and urinary retention after removal of the catheter were the complications. Conclusion: Laser photovaporization is a newly used method at the Saint Camille Hospital. The benefit is the reduction of the risk of bleeding, the short duration of hospital stay and bladder sounding. Photovaporization deserves to be popularized in our daily practices.

The treatment of urinary stones uses a varied therapeutic arsenal. Nowadays the mini-invasive techniques are the most used. We report our first experience on the results of flexible and semi-rigid ureteroscopy in the treatment of the upper urinary tract stones. Materials and Methods: A 20-month prospective study on flexible laser ureteroscopy was conducted at the Saint Camille Hospital of Ouagadougou. The inclusion criteria were for patients who had given informed consent and the presence of an unilateral obstructive upper urinary tract stone with an indication of surgical management. Results: 54 patients were treated with Holmium laser photo vaporization between January 2018 and August 2019. The average age of patients was 37.74 ± 17 years (11 - 83 years). The men were predominant at 55.56%, or a sex-ratio of 1.25. The average size of stones of 17.55 mm ± 4.16 mm (11 mm to 25 mm) with pyelic, ureteral and calyceal localization in respectively 42.6%, 33.3% and 24.1% of cases. The average duration of the interventions was 77.92 ± 43.57 minutes (11 to 180 minutes). We used drainage in 90.91% of the cases. The average duration of hospitalization was 1.2 ± 0.73 days with extremes ranging from one day to 6 days. The vaporization without residual fragment which is a success was 78.46%. Conclusion: Laser ureteroscopy is a newly used method at the Saint Camille hospital. The achievement of good results and its low morbidity encourage us to promote its extension and its use in sub-Saharan African hospitals.

Objective: To describe the clinical aspects and the management of external genitalia gangrene in the Urology department of the Bobo Dioulasso CHUSS. Patients and methods: This was a retrospective and descriptive study over a period of 6 years from January 2011 to December 2016, which covered 54 patients hospitalized in the urology department of the University Hospital Souro Sanou for external genitalia gangrene. Results: The mean age of our patients was 53.8 years. The large ulcero-necrosis of scrotum was the main reason for reference. Our patients consulted on average 8.23 days after the onset of symptomatology. A risk factor was found in 44.2% of cases and a cause was identified in 65.3% of patients. A urogenital cause was present in 32.7%. Probabilistic antibiotic therapy and medical reanimation were systematic at admission. Debridement was performed on average 18 hours after admission and 10 patients had more than one debridement. We noted a mortality rate of 5.5%. Conclusion: Fournier’s gangrene is a serious infectious disease whose diagnosis is often delayed in our context. Early surgical debridement remains the essential therapeutic gesture to reduce mortality.
