AIm To determine risk factors, causative organisms and antimicrobial resistance of bacterial infections following living-donor liver transplantation(LDLT) in cirrhotic *** This prospective study included 45 patients with hepatitis C virus-related end-stage liver disease who underwent LDLT at Ain Shams Center for Organ Transplant, Cairo, Egypt from January 2014 to November 2015. Patients were followed-up for the first 3 mo after LDLT for detection of bacterial infections. All patients were examined for the possible risk factors suggestive of acquiring infection pre-, intra-and post-operatively. Positive cultures based on clinical suspicion and patterns of antimicrobial resistance were identified. RESULTS Thirty-three patients(73.3%) suffered from bacterial infections; 21 of them had a single infection episode, and 12 had repeated infection episodes. Bile was the most common site for both single and repeated episodes of infection(28.6% and 27.8%, respectively). The most common isolated organisms were gramnegative bacteria. Acinetobacter baumannii was the most common organism isolated from both single and repeated infection episodes(19% and 33.3%, respectively), followed by Escherichia coli for repeated infections(11.1%), and Pseudomonas aeruginosa for single infections(19%). Levofloxacin showed high sensitivity against repeated infection episodes(P = 0.03). Klebsiella, Acinetobacter and Pseudomonas were multi-drug resistant(mDR). Pre-transplant hepatocellular carcinoma(HCC) and duration of drain insertion(in days) were independent risk factors for the occurrence of repeated infection episodes(P = 0.024).CONCLUSION mDR gram-negative bacterial infections are common post-LDLT. Pre-transplant HCC and duration of drain insertion were independent risk factors for the occurrence of repeated infection episodes.

Since the first report on laparoscopic distal pancreatec tomy(LDP) appeared in the 1990 s, the procedure ha been performed increasingly frequently to treat both benign and malignant lesions of the pancreas. man earlier publications have shown LDP to be a good alter native to open distal pancreatectomy for benign lesions although this has never been studied in a prospective randomized manner. The evidence for the use of LDP to treat adenocarcinoma of the pancreas is not as we established. The purpose of this review is to evaluat the current evidence for LDP in cases of pancreati adenocarcinoma. We conducted a review of English language publications reporting LDP results between1990 and 2013. All studies reporting results in patient with histologically proven pancreatic adenocarcinom were included. Thirty-nine publications were found and included in the results for a total of 309 cases of pan creatic adenocarcinoma(potential double publication were not eliminated). most LDP procedures are per formed in selected cases and generally involve smalle tumors than open distal pancreatectomy(ODP) proce dures. Some of the papers report unselected cases andinclude procedures on larger tumors. The number of lymph nodes harvested using LDP is comparable to the number obtained with ODP, as is the frequency of R0 resections. Current data suggest that similar short term oncological results can be obtained using LDP as those obtained using ODP.

AIm To investigate the clinical utility of serum annexin A2(ANXA2) as a diagnostic marker for early hepatocellular carcinoma(HCC).mETHODS This study was performed in HCC Clinic of Ain Shams University Hospitals, Cairo, Egypt and included: Group 1: Fifty patients with early stage HCC(Barcelona Clinic Liver Cancer stage A); Group 2: Twenty five patients with chronic liver disease; and Control Group: Fifteen healthy, age-and sex-matched subjects who were seronegative for viral hepatitis markers. The followinglaboratory investigations were done: Viral hepatitis markers [hepatitis B surface antigen and hepatitis C virus(HCV) antibodies], HCV RNA in HCV antibody-positive patients, serum alpha fetoprotein(AFP), and serum ANXA2 *** In this study, 88% of HCC patients(n = 44) were HCVpositive, while HBV infection represented only 8% of all HCC patients(n = 4); and two patients were negative for both viral markers. A highly significant difference was found between patients with HCC and chronic liver disease as well as controls with regard to serum ANXA2 levels(130, IQR 15-240; 15, IQR 15-17; and 17, IQR 15-30 ng/m L, respectively). The area under the curve of ANXA2 was 0.865; the cut-off value was established to be 18 ng/mL with a diagnostic sensitivity of 74% and a specificity of 88%, while the sensitivity and specificity of AFP at the cut-off value of 200 ng/dL were 20% and 100%, *** Serum ANXA2 may serve as a biomarker for the early detection of HCC.

Empowering Rice Farmers in Nueva Ecija, Philippines: A Strategic Approach to Boosting Income through Special Purpose Rice Production

Low income is the main problem of millions of farmers who are cultivating 4.81 million hectares in the Philippines. This problem is attributed to challenges specifically low yield, low quality of produce, and significant post-harvest losses, particularly in a resource-scarce setting. This study aimed to help increase the farmers’ income in selected towns of Nueva Ecija through the promotion and use of special purpose rice production technology. Twenty (20) farmer-cooperators in three towns of Nueva Ecija were selected to demonstrate the production technology covering a total area of 4.75 hectares during the dry season, and 3.3 hectares during the wet season, both in 2023. Extension modalities such as training, techno-demo establishment, information material and technical assistance provision, and market linkage were undertaken to enhance their knowledge, attitude, and skills on special rice production and enterprise. The study utilized the descriptive quantitative method wherein the data were gathered through surveys, specifically personal interviews. Results showed that 70% of the farmer-cooperators were male and 60% were living below the poverty threshold. The farmers were 53 years old on average and engaged in farming as their primary source of income. The average yield of special purpose rice particularly the CLS-2 variety was higher (6.46 mT/ha and 6.22 mT/ha during dry and wet seasons, respectively) than that of inbred or non-special rice. The income per hectare from special purpose rice production per hectare was Php 75,420.00, which is higher than that of regular rice at Php 29,389.00, giving a higher benefit of Php 46,031.00. The technology adopters and area planted increased (from 6 to 9 and from 1.4 hectares to 2.75 hectares, respectively) in Licab, Nueva Ecija. The opposite was true in Sto. Domingo and Guimba, Nueva Ecija where farmers were discouraged by the marketing issues that arose.

An AlCoCuCrFeNiTi high-entropy alloy(HEA) was prepared by mechanical alloying and sintering to study the effect of Ti addition to the widely studied AlCoCuCrFeNi system. The structural and microstructural characteristics were investigated by X-ray diffraction(XRD), scanning electron microscopy(SEm), and transmission electron microscopy(TEm). The formation of four micrometric phases was detected: a Cu-rich phase with a face-centered cubic(fcc) structure, a body-centered cubic(bcc) solid solution with Cu-rich plate-like precipitates(fcc), an ordered bcc phase, and a tetragonal structure. The XRD patterns corroborate the presence of a mixture of bcc-, fcc-, and tetragonal-structured phases. The Vickers hardness of the alloy under study was more than twice that of the AlCoCuCrFeNi alloy. Nanoindentation tests were performed to evaluate the mechanical response of the individual phases to elucidate the relationship between chemical composition, crystal structure, and mechanical performance of the multiphase microstructure of the AlCoCuCrFeNiTi HEA.

Introduction: Highly active antiretroviral tritherapies have improved the quality of life of people living with HIV (PVVIH) and extended their life expectancy. However, the nervous system is faced with a neurotropic virus that evolves chronically, sometimes creating neurological disorders directly or indirectly linked to stigmatization or therapeutic effects. Objective: to determine the prevalence of depression associated with suicidal risk (SR) and identify associated factors. Patients and method: This was a prospective descriptive and analytical study from January 1 to November 30, 2022, including all HIV-positive patients receiving or not receiving antiretroviral therapy. The study took place in the infectious diseases department of Brazzaville University Hospital. This department has an inpatient capacity of 36 beds, and an outpatient capacity of 25 patients per day, three times a week. This is the largest center for PVVIH in Brazzaville. The Patient Heath Questionnaire 9 (PHQ-9) was used to assess the degree of depression and suicidal risk. Therapeutic adherence was assessed using the morisky questionnaire. Data were analyzed using SPSS version 23 software. Qualitative variables were expressed as numbers and percentages, and quantitative variables as means and standard deviations. Statistical tests were used according to their applicability criteria. For all tests, the significance threshold was set at 0.05. Results: A total of 150 patients were consulted, constituting the sample size. The prevalence of depression and suicidal risk was 32%. These patients had an average age of 42.83 ± 10.24 years, were female (n = 101;67.3%), Christian (n = 97;64%), single (n = 68;45.3%). Stigmatization (n = 75;50%) was the reason for attempted suicide. HIV was perceived as treatable (n = 103;68.7%), unlucky (n = 62;41.3%). Patients were WHO stage 1 (n = 105;70%). RS was present in 47 cases (31.3%). A chronosequence analysis of forest recovery on abandoned agricultural fields in Nicaragua

Species composition, diversity and population structure of woody species recovered on three abandoned agricultural fields after 4, 9 and 14 years were characterized. Seedlings, saplings/poles and mature trees were identified and counted in 70 plots of 100 m^2 in each abandoned site. A total of 13, 29 and 22 families represented by 17, 48 and 44 species were registered in 4-, 9- and 14-year-old stands, respectively. There was a shift in dominant species across successional stages. Lonchocarpus acuminatus had the highest importance value in the 4-year old stand, whereas, myrospermun frutescens, Guazurna ulmifolia and Cordia alliodora had the highest importance value in the 9-year-old-stand and Caesaeria corymbosa, muntingia calabura, Gliricidia sepium and Tabebuia rosea in the 14-year-old stand. The total stem density increased from 5011 to 9631 individuals per hectare as the age of abandonment increased from 4 to 14 years. The total basal area of individuals _〉 I cm d.b.h, also increased with the age of abandonment. Overall, small individuals （〈 10 cm dbh） contributed to more than half of the total basal area. Species diversity was the highest in the 9-year old stand followed by 14- and 4-year-old stands. We concluded that floristic composition of secondary forests recovers rapidly to the mature forest level compared to structural attributes, which is consistent with the general successional trajectories of tropical dry forest.

苏拉威西地区特有鸟种苏拉山鸫(Geomalia heinrichi)的分类地位,是属于画眉科还是鸫科,长期以来一直备受争议。基于线粒体细胞色素 b 基因、细胞核肌红蛋白内含子 2 以及鸟氨酸脱羧酶内含子 6–7 基因,我们估计了鸫科 43 个分类群的系统发育关系。结果表明,苏拉山鸫是地鸫属的一个分支,并具有较高的支持率。我们建议将 Geomalia 并入地鸫属(Zoothera),并命名为 Zoothera heinrichi。
