Internal carotid artery (ICA) aneurysms are an unusual but serious cause of epistaxis. This epistaxis is massive and sometimes uncontrollable threatening the vital prognosis of patients. We report the case of a 16-year-old adolescent received in emergency with severe bilateral epistaxis, asthenia and grade-3 left exophthalmos. In his history, the subject had been the victim of an assault six months before consultation. He had received blows on the cephalic extremity with light but repeated epistaxis. The treatment consisted to blood products transfusion and local compression by sterile gazes. An ICA aneurysm in sphenoid sinus has been confirmed in a craniofacial CT scan coupled to vascular opacification. Although the ICA has a variable course in contact with the sphenoid sinus, massive epistaxis would be the consequence of a pronounced dehiscence of the ICA in the sphenoid sinus, particularly in a traumatic context. In front of this type of epistaxis in our context, general practitioners must be able to suspect a ruptured ICA aneurysm in the presence of exophthalmos and a notion of old or recent cranio-encephalic injury. additionally, due to the high morbidity and mortality of this condition, a monitoring algorithm is necessary for patients with head trauma to facilitate early detection.

Introduction: Myasthenia gravis is a chronic autoimmune neuromuscular disease, presents with weakness and fatigability of striated skeletal muscles. It is a rare disease in Cameroon. We report an uncommon case of myasthenia gravis in a patient with feeding difficulties, notion of oronasal reflux and swallowing disorders as first complaints. Observation: We report the case of a 29-year-old woman consulted at our department of Otolaryngology and Cervico-Facial Surgery for dysphagia and swallowing disorders. She also presented with facial diplegia, oculomotor paralysis, nasal voice, and dysarthria which has been evolving for several years now. The clinical examination revealed multiple cranial nerve palsies. The complementary workup showed a decrement of more than 50% in the electroneuromyography and the presence of anti-acetylcholine receptor autoantibodies in the blood workup. A diagnosis of myasthenia confirms clinical presumption. We initially observed a worsening of neuromuscular disorders despite the pyridostigmine treatment and subsequently a clear improvement of the clinical features concerning swallowing and speech disorders, oculomotricity and facial diplegia under a treatment combining prednisone and azathioprine. Conclusion: Myasthenia gravis is a rare and potentially fatal autoimmune neuromuscular disease. We thus highlight the atypical clinical presentation and therapeutic itinerary of our patient and the importance to think about this clinical diagnosis in front of any multiple cranial nerve paralysis in otolaryngology consultation.

Fatal bleeding in patients with tracheostomy is an uncommon incident, in the order of 0.1% to 1% of cases. It is secondary in 70% of cases to the innominate artery fistula in the tracheal lumen. The fistula is generally created after the necrosis of tracheal rings. The incriminated factors are the type of cannula used, the site of tracheal opening below the 3rd tracheal ring, tracheal infections and the proximity of the innominate artery to the tracheal axis. The outcome of this incident is fatal in most cases. We report the case of a 59-year-old patient with a tracheostomy on the 11th day of his admission to intensive care for severe head trauma secondary to a road accident. On the 22nd day of his admission, the occurrence of a cataclysmic and fatal haemorrhage through the tracheostomy tube evoked an arterio-tracheal fistula of the innominate artery.

Facial and Periorbital Emphysemas Following a Violent Sneezing: An Atypical Clinical Situation

Introduction: Facial emphysema is the presence of air in the subcutaneous tissues of the facial region. They can be clinically recognized by the crackling sensation felt when the affected area is palpated. Observation: The authors describe left orbito-facial emphysema that occurred after a violent sneezing episode in a 36-year-old patient. He had significant edema of the left facial and ipsilateral periorbital region associated with major emphysema and complete closure of the left eye. Nasal cavities endoscopy revealed inflammation of the distal orifice of the nasolacrimal duct. The clinical ophthalmologic examination performed in emergency showed left chemosis, slight ocular hypertonia of mechanical origin, and a slight decrease in visual acuity. Pupillary reflexes and retinography were normal. A craniofacial computed tomography (CT) revealed a significant left orbital emphysema, a fracture of the left medial orbital wall (ethmoidal lamina papyracea) with intraconal fat incarceration without entrapment of the medial rectus and significant air infiltration of all the left hemifacial soft tissues. A broad-spectrum antibiotic and anti-inflammatory treatment were instituted, as well as practical advice to prevent a recurrence. We observed progressive resorption of the edema with a return to the normal of the soft tissues and the palpebral cleft in 15 days. Conclusion: These atypical cases can be serious. It is essential to exclude signs of visual deficit and ocular compression. Multidisciplinary management is important.

Stereospecific recognition of chiral molecules plays a crucial role in biological systems. The μ-opioid receptor（MOR） exhibits binding affinity towards（-）-morphine, a well-established gold standard in pain management, while it shows minimal binding affinity for the（+）-morphine enantiomer, resulting in a lack of analgesic activity. Understanding how MOR stereoselectively recognizes morphine enantiomers has remained a puzzle in neuroscience and pharmacology for over half-a-century due to the lack of direct observation techniques. To unravel this mystery, we constructed the binding and unbinding processes of morphine enantiomers with MOR via molecular dynamics simulations to investigate the thermodynamics and kinetics governing MOR's stereoselective recognition of morphine enantiomers. Our findings reveal that the binding of（-）-morphine stabilizes MOR in its activated state, exhibiting a deep energy well and a prolonged residence time. In contrast,（+）-morphine fails to sustain the activation state of ***, the results suggest that specific residues, namely D1142.50and D1473.32, are deprotonated in the active state of MOR bound to（-）-morphine. This work highlights that the selectivity in molecular recognition goes beyond binding affinities, extending into the realm of residence time.

Recent advances in big and streaming data systems have enabled real-time analysis of data generated by Internet of Things(IoT)systems and sensors in various *** this context,many applications require integrating data from several heterogeneous sources,either stream or static *** such as Apache Spark are able to integrate and process large datasets from different ***,these frameworks are hard to use when the data sources are heterogeneous and *** address this issue,we propose a system based on mediation techniques for integrating stream and static data *** integration process of our system consists of three main steps:configuration,query expression and query *** the configuration step,an administrator designs a mediated schema and defines mapping between the mediated schema and local data *** the query expression step,users express queries using customized SQL grammar on the mediated ***,our system rewrites the query into an optimized Spark application and submits the application to a Spark *** results are continuously returned to *** experiments show that our optimizations can improve query execution time by up to one order of magnitude,making complex streaming and spatial data analysis more accessible.

Hierarchically porous carbons(HPCs)with multimodal pores have attracted considerable attention due to their unique physical and chemical properties and various application potentials in heterogeneous catalysis,environmental treatment,and energy storage and ***,we report a general and simple zinc salts-assisted method for the synthesis of HPCs with varied porosity and chemical func-tionalities by the direct carbonization of diverse biomass and *** the carbonization,zinc salts are thermally decomposed into nanoparticles that serve as in-situ templates to introduce nanopores in *** prepared HPCs exhibit high specific surface areas(up to 2432 m2 g-1),large pore volumes(up to 4.30 cm^(3)g^(-1)),and broad pore size ***,the zinc salts can be recovered and recycled,supporting the sustainable production of HPCs on large *** prepared HPCs-supported catalysts with atomically dispersed metal sites exhibit promising electrocatalytic performance for the oxygen reduction reaction.

As an economically critical pelagic migratory species,yellowfin tuna(Thunnus albacores,YFT)is very sensible to physical and environmental conditions,such as sea surface temperature(SST),ocean heat content(OHC),and the mixed layer depth(MLD).We investigated the impact of SST,OHC,and MLD on fluctuations of YFT catch in the western/eastern Indian Ocean using the long time series of 63-year environmental and YFT *** found that the impact of SST on YFT was heavily overestimated in the past,and MLD plays a more critical role in the YFT catch *** the MLD deepens(>34.8 m),SST was more influential in predicting the catches of YFT than OHC in the western Indian Ocean,and OHC was more critical to YFT than SST in the eastern Indian ***,when the MLD shallows(<34.8 m),MLD was more vital to predict the catch per unit effort(CPUE)of YFT than SST/OHC in the *** 2000,there was an asynchronous pattern of YFT CPUE induced by higher frequency variations and ocean hiatus of SST/OHC signals in the western and eastern Indian Oceans *** impact of the subsurface hiatus may induce the decrease of YFT in the eastern Indian *** above findings clarified a non-stationary relationship between the environmental factors and catches of YFT and provided new insights into variations in YFT abundance.

Impacts of central-Pacific El Niño and physical drivers on eastern Pacific bigeye tuna

Bigeye tuna Thunnus obesus is an important migratory species that forages deeply,and El Niño events highly influence its distribution in the eastern Pacific *** sea surface temperature is widely recognized as the main factor affecting bigeye tuna(BET)distribution during El Niño events,the roles of different types of El Niño and subsurface oceanic signals,such as ocean heat content and mixed layer depth,remain *** conducted A spatial-temporal analysis to investigate the relationship among BET distribution,El Niño events,and the underlying oceanic signals to address this knowledge *** used monthly purse seine fisheries data of BET in the eastern tropical Pacific Ocean(ETPO)from 1994 to 2012 and extracted the central-Pacific El Niño(CPEN)indices based on Niño 3 and Niño ***,we employed Explainable Artificial Intelligence(XAI)models to identify the main patterns and feature importance of the six environmental variables and used information flow analysis to determine the causality between the selected factors and BET ***,we analyzed Argo datasets to calculate the vertical,horizontal,and zonal mean temperature differences during CPEN and normal years to clarify the oceanic thermodynamic structure differences between the two types of *** findings reveal that BET distribution during the CPEN years is mainly driven by advection feedback of subsurface warmer thermal signals and vertically warmer habitats in the CPEN domain area,especially in high-yield fishing *** high frequency of CPEN events will likely lead to the westward shift of fisheries centers.

The adaptation of insects to environments relies on a sophisticated set of behaviors controlled by molecular and physiological *** the past several decades,accumulating studies have unveiled the roles of non-coding RNAs(nc RNAs)in regulating insect *** RNAs assume particularly pivotal roles in the behavioral plasticity of insects by rapidly responding to environmental *** RNAs also contribute to the maintenance of homeostasis of insects by fine-tuning the expression of target ***,a comprehensive review of nc RNAs'roles in regulating insect behaviors has yet to be ***,we present the recent progress in our understanding of how nc RNAs regulate various insect behaviors,including flight and movement,social behavior,reproduction,learning and memory,and *** refine the intricate mechanisms by which nc RNAs modulate the function of neural,motor,reproductive,and other physiological systems,as well as gene expression in insects like fruit flies,social insects,locusts,and ***,we discuss potential avenues for future studies in nc RNA-mediated insect behaviors.
