Generation and detection of pulsed terahertz waves in gas： from elongated plasmas to microplasmas

The past two decades have seen an exponential growth of interest in one of the least explored region of the electromagnetic spectrum, the terahertz （THz） frequency band, ranging from to 0.1 to 10 THz. Once only the realm of astrophysicists studying the background radiation of the universe, THz waves have become little by little relevant in the most diverse fields, such as medical imaging, industrial inspection, remote sensing, fundamental science, and so on. Remarkably, THz wave radiation can be generated and detected by using ambient air as the source and the sensor. This is accomplished by creating plasma under the illumination of intense femtosecond laser fields. The integration of such a plasma source and sensor in THz time-domain techniques allows spectral measurements covering the whole THz gap （0.1 to 10 THz）, further increasing the impact of this scientific tool in the study of the four states of matter. In this review, the authors introduce a new paradigm for implementing THz plasma techniques. Specifically, we replaced the use of elongated plasmas, ranging from few mm to several cm, with sub-mm plasmas, which will be referred to as microplasmas, obtained by focusing ultrafast laser pulses with high numerical aperture optics （NA from 0.1 to 0.9）. The experimental study of the THz emission and detection from laser-induced plasmas of submillimeter size are presented. Regarding the microplasma source, one of the interesting phenomena is that the main direction of THz wave emission is almost orthogonal to the laser propagation direction, unlike that of elongated plasmas. Perhaps the most important achievement is the demonstra- tion that laser pulse energies lower than 1 μJ are sufficientto generate measurable THz pulses from ambient air, thus reducing the required laser energy requirement of two orders of magnitude compared to the state of art. Relationship of MTHFR gene polymorphisms with renal and cardiac disease

AIM: To investigate the effects of different methylenetetrahydrofolate reductase(MTHFR) 677C>T gene polymorphism and hyperhomocysteinemia for the development of renal failure and cardiovascular events, which are ***: We challenged the relationship, if any, of MTHFR 677C>T and MTHFR 1298A>C polymorphisms with renal and heart function. The present article is a reappraisal of these concepts, investigating within a larger population, and including a subgroup of dialysis patients, if the two most common MTHFR polymorphisms, C677 T and A1298 C, as homozygous, heterozygous or with a compound heterozygous state, show different association with chronic renal failure requiring hemodialysis. MTHFR polymorphism could be a favorable evolutionary factor, i.e., a protective factor for many ominous conditions, like cancer and renal failure. A similar finding was reported in fatty liver disease in which it is suggested that MTHFR polymorphisms could have maintained and maintain their persistence by an heterozygosis advantage mechanism. We studied a total of 630 Italian Caucasian subject aged 54.60 ± 16.35 years, addressing to the increased hazard of hemodialysis, if any, according to the studied MTHFR genetic polymorphisms. RESULTS: A favorable association with normal renal function of MTHFR polymorphisms, and notably of MTHFR C677 T is present independently of the negative effects of left ventricular hypertrophy, increased IntraRenal arterial Resistance and hyperparathyroidism. CONCLUSION: MTHFR gene polymorphisms could have a protective role on renal function as suggested by their lower frequency among our dialysis patients in end-stage renal failure; differently, the association with left ventricular hypertrophy and reduced left ventricular relaxation suggest some type of indirect, or concurrent mechanism.

AIM:To assess the prognostic value and risk classification improvement of metabolic staging(MS)with Initial2-[18F]-fluoro-2-desoxy-D-glucose positron emission tomography(FDG-PET)in initial staging of Hodgkin’s Lymphoma(HL)patients to predict 5 years overall survival(5y-OS)and event free survival(EFS).METHODS:A total of 275 patients were included in this retrospective study,155 patients were staged with conventional anatomical staging(AS),and 120 also submitted to MS(FDG-PET).Prognostic analysis compared 5y-OS and 5y-EFS of patients staged with AS and ***-adjusted models incorporated clinical risk factors,computed tomography and FDG-PET ***:During the follow up of 267 evaluated patients,220(122 AS and 98 MS)achieved complete remission after first-line therapy(median follow-up:70±29 mo),treatment failure occurred in 79 patients and 34 *** 5y-EFS for early vs advanced disease in AS patients was 79.3%and 66.7%,and 85.6%and53.6%in MS patients,respectively(P<0.01).The5y-OS for early and advanced disease with AS was91.3%and 81.5%,and 97.5%and 80.7%for patients staged with MS,*** proportional hazards analysis demonstrated that FDG-PET added signifcant prognostic information and improved risk prediction(P=0.02).CONCLUSION:Initial staging FDG-PET could be used as an accurate and independent predictor of OS and EFS in HL,with impact in 5y-EFS and OS.
