Management of Non-Traumatic Urological Emergencies at Souro Sanou University Teaching Hospital of Bobo-Dioulasso (Burkina Faso)

Background: Generally, urological emergencies are assumed not to be very common, however, recent reports showed that they constitute an important aspect of the day-to-day urological practice. If not well and promptly managed, they may lead to serious morbidity or mortality. Objectives: To study the pattern of presentation, diagnosis and outcome of management of non-traumatic urological emergencies seen at the Emergency Department of Souro Sanou University Hospital in Bobo-Dioulasso. Patients and Methods: This was a retrospective and descriptive study over four years. It included patients of all ages and both sexes, admitted for non-traumatic urological emergencies in the surgical emergency department of Souro Sanou University Hospital. It took place from January 1, 2017 to December 31, 2020. Results: A total of 584 patients were reviewed in our study. Non-traumatic urological emergencies account for 6.3% of all surgical emergencies seen during the study period. The male-to-female ratio was 9.2 to 1. The mean age of the patients was 51.9 ± 23.9 years. Forty-five per cent of the patients presented within 48 hours of symptoms. The vast majority of the patients presented with difficulties with passing urine (41.6%), followed by cases of hematuria (18.4%). On admission, 154 patients (26.4%) presented with severe conditions such as anemia as seen in 40.9% of the cases and deterioration in the general health condition as seen in 34.4% of the patients. Leukocytosis was noted in 18.7% of the patients and anemia in 17.9%. Urine culture was positive in 15.4% of the patients and Escherichia coli was the most common pathogen found (40.6%). Ultrasound was the most requested examination (81.2%), followed by a computerized tomography (CT) scan (22%). The most frequent diagnoses were urine retention (42.9%), hematuria (16.9%) and renal colic (10.1%). Congenital radial club hand (CRCH) is a terminal deficiency characterized by the radial deviation of the hand, marked shortening of the forearm, and generalized underdevelopment of the extremity with an incidence rate of 1 per 100,000 live births. Both sexes are nearly or equally affected. Challenges that the disease poses to the patient are multifaceted, ranging from medical, social, cosmetic and functional issues which also are the multidimensional treatment modalities. We report a case of a 2-month-old baby with the severe case of bilateral CRCH. The patient had the centralization of the left ulna with Kwire stabilization. About 160d of correction was achieved. He did well and was discharged after two weeks at the orthopedic clinic for follow-up. Congenital radial club hand though uncommon may be amenable to surgery. Centralization is still the treatment of choice in severe cases and early intervention will offer a better outcome.

Profile of Amputations in National Orthopaedic Hospital, Dala, Kano

Introduction: Amputation is the removal of a body part through a bone, while disarticulation is a removal of a part through a joint. This common procedure has different causes depending on the geographical locations and cultural practices. These ranges from dead body part to malignancy. Our study focused on the profile of the amputees including the biodata, causes and the financial consequences of the procedure. Material and Methods: The case notes of all patients who had amputation from 1998-2012 (15 years) were collected and the biodata, intra-operative diagnosis, parts amputated and duration of hospital stay were retrospectively analysed. Results: One hundred and ninety six patients had various types of amputations for the duration of our study. 159 (81.12%) were males while 37 (18.88%) were females. 68 (34.69%) patients had below knee amputation. 54 (27.55%) had above knee amputation. 27 (13.78%) had above elbow amputation. 18 (9.18%) patients had below elbow amputation, while 30 (15.30%) patients had other forms of amputations. The highest number of amputees (72 patients) was within the age range of 21 - 30 yr and the least number (5 patients) was within the age range of 51 - 60 yr. The highest number of amputation (71 patients) was due to complications from by traditional bone setters intervention. Conclusion: In our study, we found that most cases were males with a mean age of 35 years and majority (34.69%) had below knee amputation, and that most cases suffered vascular occlusion leading to gangrene.

Prolonged Obstructed Labor Is an Uncommon Presentation of a Giant Bladder Calculus: A Case Report and Literature Review

Prolonged obstructed labor is a critical intrapartum emergency. This intricacy is, however, unusual as a primary presentation of a giant bladder calculus. We report a case of a 25-year-old Para 6 + 1, who presented with a history of labor pains of 72 hours duration. She had background lower urinary tract symptoms with supra-pubic swelling and hematuria of 2 years duration. She had a single antenatal visit at 32 weeks however;no obstetric ultrasound scan was done before she went into labor. Examination revealed a highly placed fetal head with increase fetal heart rate. There was a hard mass bulging under the anterior vaginal wall. The diagnosis was a prolonged obstructed labor secondary to bladder mass with fetal distress was made. She had a caesarean section (CS) with the delivery of fresh stillborn. The bi-manually palpable bladder mass persisted post-CS. Her abdominopelvic ultrasound scan and abdominal X-ray post-CS revealed giant vesical calculus with obstructive uropathy. Urine microscopy culture and sensitivity yielded E. coli sensitive to Ciprofloxacin;other laboratory investigations were normal. She was treated for UTI and 2 weeks post-CS, she had opened cystolithotomy. The stone weighed 536 g. Her postoperative recovery was uneventful.

Metastatic Cervical and Supraclavicular Lymphadenopathy from Prostate Mimicking Lymphoma: A Case Series

Prostate cancer is the second most common cancer in men worldwide and the commonest cause of mortality in men. It is the commonest diagnosed cancer in African men. The earliest and common sites of metastasis are the axial skeleton and regional lymph nodes. Initial presentation of the metastatic disease with cervical or supraclavicular lymphadenopathy is rarely reported. We report 3 cases of metastatic cancer of the prostate presenting initially to physicians with huge cervical and supraclavicular lymph nodes enlargement which on biopsy revealed metastatic adenocarcinoma. On further evaluation, there were non-bothersome storage symptoms, elevated prostate specific antigen and abnormal digital rectal examination. Transrectal ultrasound (TRUS)-guided biopsy of hypoechoic nodules revealed high-grade adenocarcinoma of the prostate on histopathological examination. The cervical and supraclavicular lymphadenopathy resolved after commencement of androgen deprivation therapy. We advocate for prostate cancer screening in African men above 50 years of age presenting with cervical or supraclavicular lymphadenopthy to primary care physicians even in the absence of lower urinary tract symptoms.

Bacterial Blood Isolates in Children: Conventional vs. Bactec Automated Blood Culture System in a Tertiary Health Centre in Gombe, North East Nigeria

Background/Aim: Blood culture is critical in the diagnosis and treatment of blood stream infections (BSIs) especially in children. BSIs are among the most common cause of morbidity/mortality and blood culture has remained the gold standard for diagnosis. We sought to compare Blood Culture Isolates (BCI) from conventional and Bactec automated blood culture system (ABCS) among paediatric patients at the Federal Teaching Hospital Gombe (FTHG) Nigeria. Methods: BCI in children (0 - 18 years) by conventional method from 2008-2012 and Bactec Automated culture system from 2015-2020 were retrieved from the clinical microbiology laboratory register. Information analyzed included, age, sex, month, and year and blood culture isolates. Results: There were 5276 (56.9% males, 43.1% females) and 1169 (54% males, 46% females) Blood Culture Isolates by CM and ABCS respectively. Overall positive culture isolates were 9.7% (515/5276) in CM and 45.9% (536/1169) in ABCS (p = 0.01). Positivity rate in newborn was 13.3% (282/2114) by CM and 40.9% (219/263) by ABCS p = 0.01;under-5 was 10.5% (448/4253) vs. 37% (359/873) (p = 0.01);Gram positive 32.6% (172) vs. 65% (759) (p = 0.01;Gram negative 55% (2910) vs. 34% (397) (p = 0.01). Staph aureus 22% (114/515) by CM vs. 61.9% (332/536)) by ABCS (p = 0.01);Klebsiella 24.9% (128/515) by CM vs. 7.5% (40/536) p = 0.01) in ABCS, E. coli 8.9% (46/515) vs. 2.1% (11/536) p = 0.01;Proteus vs. 1.1% (6/515) by ABCS, Pseudomonas 3.3% (17/515) vs. 5.6% (30/536) p = 0.05, Alkaligenes 1% (5/515) vs. 8.2% (44/536) p = 0.01 and Citrobacter 1% (5/515) vs. 8.4% (45/536) p = 0.01. Conclusion: Blood culture yield was five times higher with Bactec compared with Conventional method.

Hepatocellular carcinoma(HCC)is a disease of public health concern in Nigeria,with chronic hepatitis B and C infections contributing most to the disease *** the increasing incidence of HCC,surveillance practices for early diagnosis and possible cure are not deeply rooted in the *** article aims to review the current status of HCC surveillance in Nigeria,stressing the encounters,breaches,and potential *** factors,such as limited tools for screening and diagnostics,insufficient infrastructure,and low cognizance among the doctors,and the general public affect the surveillance practices for HCC in ***,the lack of standardized guidelines and protocols for HCC surveillance further intensifies the suboptimal diagnosis and ***,there are opportunities for refining surveillance practices in the *** would be achieved through boosted public health sensitization campaigns,integrating HCC screening into routine clinical services,and leveraging technological developments for early detection and ***,collaboration between government agencies,healthcare providers,and international organizations can facilitate the development of comprehensive HCC surveillance programs person-alized to the Nigerian ***,HCC surveillance practice faces substantial *** addressing the drawbacks and leveraging prospects,Nigeria can improve HCC surveillance,with subsequent improved outcomes for individuals at risk of developing the disease.

Introduction: Automated blood culture systems for incubation and growth monitoring have become the standard in high-income countries (HICs), but are still relatively expensive and not universally available for implementation in most low- and middle-income countries (LMIC). We aimed to report blood culture isolates using Automated technique in children and adults admitted into the Federal Teaching Hospital Gombe from 2016 to 2020. Materials and Methods: Blood Culture Isolates in children (0 - 18 years) and adults (>19 yrs) by Bactec 9050 Automated culture system from 2016-2020 were retrieved from the medical and laboratory register. Information analyzed included, age, sex, month, and year and culture growth and reported antibiotic sensitivity. A Bactec Blood culture tests is $20 in this facility. In Nigeria, the minimum monthly wage is $70 (Official currency exchange rate is N423/US Dollar). Results: Of the 1713 blood cultures performed, children 0 - 18 years were 1322 (77.2%) and adult (19 years above) (22.8%). Overall positivity was 733 (42.2%) with males 385 (52.5%). Of the 1322 Blood cultures (BC) in children 615 (46.5%) were positive for isolates and adults 118 (30.2)%. Blood culture positivity decreased with increasing age with newborns 251 (34.5%) and adults > 65 years 18 (2.5%). Staphylococcus aureus constituted 61.3% of all isolates and was the leading isolates in all age groups;Alkaligenes (9.1%);Citrobacter 8.1%, Klebsiella 6.7%;Pseudomonas 6.1%;E. coli 2.7%;Enterococcus 2%;Proteus 1%. Of the Antimicrobial resistance priority isolates E. coli susceptibility ranged from 71% to Gentamycin and 100% to Cefixime;Klebsiella from 25% sensitivity to Amikacin to 78% each to chloramphenicol and ciprofloxacin;Salmonella was 100% sensitive to chloramphenicol, ciprofloxacin and cefuroxime. Klebsiella was 100% sensitive to Cefoxitin;Proteus sensitivity ranged from 35% to ampicillin and 100% to ciprofloxacin and cefuroxime. Staph aureus sensitivity was 35% to cefoxitin,

Five Centimeter Concrete Nail Aspiration in a 2-Year-Old Nigerian Child: A Call for More Parental/Care-Givers’ Education: A Case Report

Introduction: Foreign body (FB) aspiration is a common cause of respiratory emergency in early childhood. Parents and care-givers are usually unaware of initial time of FB aspiration, and the varied symptoms may be confusing to them. Therefore, education on ways of avoiding the inci-dence is paramount, and prompt hospital presentation will reduce morbidity and mortality. Case Report: A 5 cm concrete nail, which was aspirated by a 2-year-old male Nigerian child of the Hausa ethnic group, was successfully removed by rigid bronchoscopy. We emphasize the need for more parental/care-givers’ education on foreign body aspiration among children. Conclusion: Infants commonly play with objects by inserting them in various orifices of their body. This, therefore, predisposes them to aspiration, which will go unnoticed at the time of aspiration. There is a need for repeated parental/care-givers’ education on dangers of allowing children to play alone. Parents/care-givers should be conversant with varied presentations of FB aspiration in children, as this will reduce the morbidity and mortality to the minimum.
