Adult Kidney Cancer in mauritania: Clinical and Therapeutic Aspects

Introduction: since its creation, our urology department has taken care of tumor pathologies in particular kidney cancer in adults;our goal is to study the clinical and therapeutic epidemiological profiles of adult kidney cancer in mauritania. materials and methods: We carried out a retrospective study over a period of six years, from January 1, 2012 to December 31, 2017, including all cases of adult kidney cancer registered in the urology-andrology departments of the Cheikh Zayed Hospital and Pathological Anatomy of the National Hospital of Nouakchott (mauritania). Results: we collected 50 cases of kidney cancer. The average annual incidence was 8.3 cases. The average age of the patients was 52.98 years with extremes of 18 and 84 years. There was a female predominance (52%) or 29 women for 21 men. Lumbar pain was the most frequent clinical expression, more than half of the patients had symptoms over a period of at least 12 months before the first consultation. The left kidney was the most frequently affected. The right localization was demonstrated in 23 patients, the extension assessment was made with thoraco-abdomino-pelvic CT in 40 patients. Twelve patients had a tumor localized in the kidney. The extension assessment had made it possible to objectify the existence of metastases in 17 of our patients (37%). The preferred locations of these metastases were pulmonary and hepatic. Surgical intervention was performed in 44 patients (88%), of whom 36 underwent radical nephrectomy (72%), and two patients underwent partial nephrectomy (4.5%). Surgical abstention was decided from the outset in 6 patients (13.6%). None of our patients had received treatment with anti-angiogenics. The histological type most observed in our patients was renal cell carcinoma, observed in 34 patients, or 77.72%. At the time of the study, more than a third of the patients had died. Complications of Circumcision: Epidemiological, Anatomo-Clinical and Therapeutic Aspects

Circumcision refers, in its most widespread form, to the total or partial removal of the foreskin, leaving the glans exposed. It is a ritual practice imposed by mosaic law for Israelites, a tradition for muslims, a rite of passage to adulthood for many people of Africa and the East. Circumcision is also a widely used therapeutic surgical procedure for a number of balanopreputial affections. The aim of this study was to list cases of circumcision complications received in a urological hospital (CHR Saint Louis) and to analyze their epidemiological, anatomo-clinical and therapeutic aspects. Patients and methods: This was a retrospective, descriptive study of circumcision complications, carried out in the urology department of the Saint Louis regional hospital in Senegal over a 5-year period (January 1, 2013 to December 31, 2017). Results: Thirty-three (33) complications of circumcision were collated. The mean age of patients was 7.63 years [9 months - 16 years]. Duration of time for Consultation varied according to the type of complication, with an average of 1.6 years [1 day - 10 years]. Penile granuloma accounted for 36.3% of the complications (n = 12), local infection for 30.3% (n = 10) and urethro-cutaneous fistula for 18.2% (n = 6). For six patients with urethro-cutaneous fistula, we performed urethro-cutaneous splitting in 5 patients, while the same complication necessitated urethroplasty in another patient. We noted five cases of verge plasty for verge granuloma, one glan reimplantation and one glan plasty for the two patients with glans amputation. Conclusion: Circumcision is a medical procedure widely practised throughout the world for medical, cultural and aesthetic reasons. In Africa, the lack of health coverage and the shortage of doctors explain why this surgical procedure is so common. In Senegal, circumcision is still carried out in the majority of cases by paramedical staff, which explains the high rate of complications.

Contribution of Cardiac mRI in the Diagnosis of Acute myocarditis

Objective: To describe the mRI abnormalities observed in acute myocarditis. materials and methods: Retrospective cross-sectional study with a descriptive aim, carried out at the North Franche-Comte Hospital, over a period of 12 months, from January 2021 to December 2021. It covered all patients who received an mRI of heart disease and were diagnosed with myocarditis. The diagnosis of myocarditis was retained in all patients on the basis of two arguments: a T2 PSIR hyper signal and a late enhancement at 15 min in T1 PSIR with gadolinium. Results: myocarditis was diagnosed in 20 patients out of a total of 214 who performed cardiac mRI, i.e. 10.30% of cases. The average age was 33.7 ± 14.3 with extremes of 17 and 69 years. We observed a male predominance with 11 men (55%) for 9 women (45%) or a sex ratio of 1.2. Clinical suspicion of myocarditis and acute coronary syndrome were the main indications for mRI. The lesion sites were subepicardial (95%) and/or intramural (30%), respecting the subendocardium, interesting for the majority, segments 12 (anterolateral) in 50% and/or 11 (inferolateral) in 43% of cases. Global hypokinesia was observed in 30% of patients associated with a decrease in LVEF. There was no cardiac volume abnormality or valvular abnormality. Conclusion: Cardiac mRI is nowadays the most efficient non-invasive imaging in the diagnosis of acute myocarditis. The diagnosis of myocarditis was made on 2 pathognomonic signs, namely a T2 STIR hyper signal and late enhancement at 15 min in T1 PSIR after injection of gadolinium. The morphology and lesion locations were in agreement with those described in previous studies. Global hypokinesia and pericardial effusion were observed in some patients. On the other hand, there was neither valvular anomaly, nor cardiac volume anomaly.

Surgical Treatment of Recent Traumatic Epiphyseal Detachments of the Distal Femur in the Orthopedic and Traumatological Surgery Department of the CHU Bocar Sidy Sall de Kati: About 20 Cases

Introduction: To our knowledge, no scientific study has been conducted at the university hospital of Kati on recent traumatic epiphyseal detachment fractures of the distal femur. However, every day, children present in consultation for knee trauma. Conjugal plate fractures are by definition single to children. Any break in any solution of continuity of this plate is called an epiphyseal fracture or detachment. Objectives: To evaluate the results of surgical treatment of traumatic epiphyseal in detachments of the distal femur recent in children and adolescents. Patients and method: This was a 13-month prospectively collected longitudinal descriptive study from September 1, 2016 to September 30, 2017. We identified and reviewed 20 medical records of recent traumatic epiphyseal detachments of the distal femur. The parameters studied were sociodemographic, lesion, surgical technique and evolutionary. Results: During the study period, the most affected group age was 12 to 17 years (85%) with an average of 14.65 ± 1.61 years (6 and 20 years). The victims were male (95%). The most frequent etiology was road traffic accident (95%), with a mean time to care of 26.42 ± 10.21 hours and Type II was frequently encountered (85%) with 35% of immediate complications. Cross-bracketing was the most commonly used surgical technique (85%). The result was good (65%). Conclusion: This study allowed us to observe a significant hospital incidence of epiphyseal detachment fractures of the distal femur. Cross pinning was the most commonly used surgical technique with fewer complications (35%) and a satisfactory result in 90% of cases.

Double Arthrodesis, Postero-medial Release and Posterior Tibial Transfer in One Step in Paralytic Inveterate Equine Varus Foot

Introduction: Varus equine foot deformity is common in developing countries. The management of these deformities is surgical in adults. Several surgical techniques have been described with more or less satisfactory results. To our knowledge, no study has been performed on the simultaneous association of double arthrodesis, posteromedial release, and posterior tibial transfer in a single operation in inveterate paralytic varus equines feet. The purpose of this work was to evaluate the results obtained. Patients and method: This was a retrospective descriptive study from January 01, 2018 to December 31, 2021. It concerned inveterate paralytic varus equines feet operated on by the simultaneous association in a single operative time of double arthrodesis of the foot, posteromedial release of the back foot and transfer of the posterior tibial muscle to the lateral cuneiform. We identified seven patients with a mean age of 22.1 years with extremes of 11 years and 36 years. There were three males and four females. The cause of the deformity was neurological in all cases. All patients had painful walking discomfort and shoeing difficulties. The average time to management was 13.3 years with extremes of 4 and 25 years. The chronology of the interventional steps was posteromedial release, arthrodesis, and transfer of the posterior tibial muscle to the lateral cuneiform. The average postoperative follow-up was 21.7 months with extremes of 6 and 48 months. The parameters studied were the duration of the procedure, complications related to the procedure, muscle strength at the last recoil, consolidation of the arthrodesis, residual pain, patient activity, gait perimeter, stepping, ankle mobility, residual deformity, footwear, protrusion of the transferred tendon, and the possibility of walking on the heel. Final results were graded according to the Angus and Cowell criteria. Results: No intraoperative complications were noted. An early superficial infection of the surgical site

Contribution of Imaging in the Diagnosis and management of Type 1 Autoimmune Pancreatitis Associated with main Bile Duct Cholangitis: A Case Report

Introduction: Autoimmune pancreatitis type 1 (AIP) is a rare condition characterized by inflammatory and fibrosing involvement of the pancreas, associated with elevated serum IgG4 and potential involvement of other organs. It often mimics a malignant tumor with which the differential diagnosis is often difficult. Observation: We report a case of AIP type 1, in a 70-year-old patient, with a heavy history of arterial hypertension, insulin-induced type II diabetes, overweight, gastric ulcer, ischemic heart disease with triple bypass, COPD, revealed by intense abdominal pain and acute evolution. The diagnosis was evoked on imaging, in front of an aspect of “sausage pancreas”, with delayed contrast enhancement of the pancreas, loss of lobulations, and the presence of a fibrous peripancreatic shell. The biological and histological assessment confirmed the diagnosis and avoided inappropriate surgical treatment. Conclusion: AIP type 1 is a rare pathology whose diagnosis is difficult. Only a perfectly interpreted imagery can evoke it. The biological assessment, in particular, the elevation of the serum level of IgG4 and the histology make it possible to confirm the diagnosis. The challenge in its management is to avoid unnecessary surgery and/or inappropriate treatments. Imaging, in this context, plays a preponderant role, in particular, thanks to the dynamic injection sequences of gadolinium chelates on mRI which suggest the fibro-inflammatory nature of pancreatic lesions.

Ethnobotanical Investigation of Insecticidal Plants and Evaluation of the Insecticidal Activity of Capsicum chinense on Sitophilus zeamaïs of maize and Aphis craccivora of Groundnuts: Case of Boffa, BokéRegion (Republic of Guinea)

The present study, which aims to contribute to the valorization of Capsicum chinense, focuses on ethnobotanical investigations and evaluation of insecticidal activity on Sitophilus zeamaïs and Aphis craccivora, as well as the germination power of infected maize and peanut grains treated and not treated with Capsicum chinense leaf powder. To do this, the moisture content of corn and peanuts was determined by the techniques: drying in the sun and in the oven. The insecticidal effect of the powder was tested on maize and peanut kernels infected by the AGRAR (2013) method, followed by a test of the germination potency of these treated grains. The average moisture values of corn and groundnut kernels are 14.51% and 12.25% respectively. The results show, a higher insecticidal efficacy of Capsicum chinense leaf powder on Sitophilus zeamaïs and Aphis craccivora with doses (2, 4 and 8 g). The average germination rates of maize and peanut kernels infected and treated with this powder are: (16.5%;63%;99.5%) and (21.25%;96.5%;99.75%), respectively. This study could be a bio-insecticide alternative for the peasant community in the conservation of cereal and legume products.

Introduction: External hemorrhoidal thrombosis is a clinical is the most common postpartum proctologic accident. The aim of this work was to study hemorrhoidal thrombosis in recent mothers, in order to help improve its management. Patients and methods: This was a descriptive and analytical cross-sectional study that was carried out at Saint Camille Hospital in Ouagadougou, Burkina Faso. Data collection was prospective and took place over a period of three months from September 1 to November 30, 2019. The study population consisted of recent births. The interview with the patients and their physical examination enabled the collection of data. Results: During the study period, 384 women were examined and 56 (14.6%) of whom presented with external hemorrhoidal thrombosis. The average age of patients who presented with external hemorrhoidal thrombosis was 29 years ± 5.2. The external hemorrhoidal thrombosis appeared in the form of a painful swelling at the level of the anal margin. In multivariate analysis, dyschezia and personal history of hemorrhoidal disease were risk factors for external hemorrhoidal thrombosis in the postpartum period (OR = 37.4 [6.8 - 205.7];OR = 23.9 [3.3 - 174.2]. Consumption of fruit and foods rich in dietary fiber were protective factors against the occurrence of this risk (OR = 0.02 [0.002 - 0.3]). Conclusion: External hemorrhoidal thrombosis remains a complication to be taken into account during the monitoring of postpartum women. It is important to encourage pregnant women to consume foods and fruits rich in fiber seem to reduce the risk of its occurrence.

Fortuitous Discovery of a multifocal Enchondromatosis in a Case at the “Luxembourg” Hospital Center in Bamako

The aim of this study was to clarify the interest of standard radiography in the management of Ollier’s disease. Observation: Enchondromas are benign lesions that may present on imaging with nonspecific features in children, and they are relatively large lesions, with frequent endosteal erosion and rare matrix mineralization. We report a case of predominantly right-sided multifocal enchondromatosis in a 5-year-old girl with no known medical history, diagnosed fortuitously on standard radiography during a trauma assessment and confirmed by histology. No sign of pain was noted after a 6-month follow-up. But the radiographic control noted signs of diffuse osteoarthritic remodeling in the surgical areas and early fusion of the growth cartilages. Conclusion: Ollier’s disease is rare, you have to know how to think about it in the face of fortuitous discoveries, especially at an early age. Enchondromas are benign lesions that may present on imaging with nonspecific features in children. In all cases, standard radiography is essential in the diagnosis and follow-up of Ollier’s disease.

Objectives: To describe the practice of ultrasound-guided TAP in the management of postoperative pain after gyneco-obstetric surgery. methods: This was a descriptive prospective study carried out at the Gynecology-Obstetrics department of the Ignace Deen National Hospital over a period of three (03) months from February 01, 2020 to April 31, 2020. Results: In total, we collected 95 patients. These patients had a mean age of 30 ± 9.5 years. The ASA I class was the most represented with 76% of the cases and the cesarean was the most performed intervention. Regarding the assessment of the pain score by the simple verbal scale (SVE) postoperatively at rest, the mean SLE scores at H6 were 0.17 ± 0.38;at H12 of 1.15 ± 0.62;at H24 of 0.84 ± 0.51;at H36 0.45 ± 0.52 and at H48 0.09 ± 0.29. On mobilization, the mean pain scores were 0.77 ± 0.51 at H6, at H12 1.89 ± 0.61;at H24 of 1.53 ± 0.56;at H36 of 1 ± 0.29 and at H48 of 0.82 ± 0.44. The majority of our patients (66.3%) had a mobilization time of less than 24 hours. The mean length of stay was 3.1 ± 1.3 days and most patients (82%) were satisfied with the management of their pain by ultrasound-guided TAP block. Conclusion: Ultrasound-guided TAP is an effective technique for the management of postoperative pain in gyneco-obstetrics surgery. Its integration in a context of multimodal analgesia could improve the management of postoperative pain in gynecological obstetrics.
