Nigeria is an Afrotropical region with considerable ecological heterogeneity and levels of biotic *** its vertebrate fauna,reptiles have broad distributions,thus,they constitute a compelling system for assessing the impact of ecological variation and geographic isolation on species *** red-headed rock agama,Agama agama,lives in a wide range of habitats and,thus,it may show genetic structuring and ***,we tested the hypothesis that ecology affects its genetic structure and population *** inference phylogenetic analysis of a mitochondrial DNA(mtDNA)gene recovered four well-supported matrilines with strong evidence of genetic structuring consistent with eco-geographic *** differences among populations based on the mtDNA also correlated with geographic distance. The ecologicalniche model for the matrilines had a good fit and robust performance. Population divergence alongthe environmental axes was associated with climatic conditions, and temperature ranked highestamong all environmental variables for forest specialists, while precipitation ranked highest for theforest/derived savanna, and savanna specialists. Our results cannot reject the hypothesis that nicheconservatism promotes geographic isolation of the western populations of Nigerian A. ***, ecological gradients and geographic isolation impact the genetic structure and population divergenceof the lizards. This species might be facing threats due to recent habitat fragmentation,especially in western Nigeria. Conservation actions appear necessary.

This methodological paper explains the process of occupational therapy management of displaced persons due to disaster influence. It is yet to be part of the occupational therapy practice domain even in recent decades with the advancement in occupational therapy research evidence and development. It is based on the current knowledge and developments in occupational therapy client centered practice. This methodological and conceptual based study describes and illustrates the occupational therapy practice model of rehabilitating displaced persons due to disaster influence. The present paper explores the occupational therapy role in rehabilitating displaced persons through People’s Environmental Occupation, Ecology of Human Performance (EHP), and Occupational Therapy Practice Framework. These are integrated as a single conceptual model to describe how and process of occupational therapy practice area during the rehabilitation process of a displaced person(s). The paper finally presented a sequenced framework to guide occupational therapists practice in this new emerging practice area of rehabilitation. This study was developed based on three different theories to support the occupational therapy treatment process of a displaced person due to disaster of any nature. Its concept based without any statistical analysis.

Epistaxis is a common complaint which is rarely life-threatening, as most cases are self-limited and as such unreported. However, it may be a significant cause for concern if it becomes recurrent. Although a number of medications including topical antihistamines and non-steroidal anti-inflammatory drugs are known aetiological factors for epistaxis due to their antiplatelet effects, beta blockers are not being widely reported as a possible cause. This report presents the case of a 34-year-old G2P0+1, who was diagnosed with hyperthyroidism in pregnancy and subsequently reported epistaxis twice at different occasion and different hospital settings when propranolol was introduced for her treatment, with resolution of epistaxis after withdrawal of propranolol. The report aims to highlight and sensitize physicians to the possible risk of bleeding in patients placed on beta blockers especially propranolol for a wide range of medical condition due to its thrombocytopathic effect.

This study considers a theoretical analysis of natural convection flow of heat generating/absorbing fluid near an infinite vertical plate with ramped temperature. It is assumed that the bounding plate has a ramped temperature profile. Exact solutions of energy and momentum equations are obtained using the Laplacetransform techniques. Solutions are obtained for different values of the Prandtl number (Pr) and the heat sink parameter (S). Results of the ramped and isothermal temperature and velocity as well as Nusselt number and skin friction have been compared and presented with the help of graphs.

Xanthene and Xanthenediones are structural components of several bioactive and semi-synthetic molecules. This work described an expeditious synthesis of novel and hitherto unreported bis-trifluoromethyl xanthene dione derivatives. The reaction of two equivalents of 5-trifluoromethyl cyclohexane-1,3- dione with substituted aromatic aldehydes in the presence of ethanol containing 1 - 2 drops of HCl was facile under microwave irradiation. Short reaction time (25 min), good to excellent yields (80% - 95%), good atom economy, and simple workup are the major advantages of the above procedure. Moreover, the fluorinated products represent synthetically useful stable intermediates that could find applications in pharmaceutical, agricultural and material industries.

Impact of Relative Dose Intensity (RDI) on Survival in Non-Metastatic Breast Cancer: Nigerian Experience

Background: This study was initiated to determine practices patterns in adjuvant chemotherapy for non-metastatic breast cancer and to examine the relationship between received dose intensity (RDI) and survival in patients with breast cancer Nigeria. Methods: Our study was a retrospective analysis of patients with breast cancer recruited from 2012 and 2015. A total of 204 patients were initially entered into the study, 102 were lost to follow-up leaving 102 patients who were suitable for the survival analysis. Survival time was calculated from 106 days, the scheduled end of chemotherapy. Results: The total average RDI for patients was 74%. Over the 204 patients that were reviewed, 144 (70.6%) had some reduction of RDI. This subgroup had an average RDI of 63%. On average, 79% of the intended dose of chemotherapy was given. The time to completion of chemotherapy was 1.33 times that specified by the protocol. Dose delays an overall reduction was mainly attributed to intolerability and financial constraints. Survival by RDI showed a significant decrease in survival rate for patients with RDI of >49% (Hazard Ratio = 3.473, 95% CI 1.21 - 9.91, P = 0.020);RDI of 50% - 59% (Hazard Ratio = 3.916, 95% CI 1.01 - 15.18, P = 0.048);RDI of 60% - 69% (Hazard Ratio = 4.462, 95% CI 1.65 - 12.03, P = 0.003) compared with patients who received an RDI of 100%. Although associated with poorer prognosis, there were no significant changes in the survival rate for patients with RDI of 70% - 79% (Hazard Ratio = 1.667, 95% CI 0.56 - 4.96, P = 0.359);RDI of 80% - 89% (Hazard Ratio = 1.620, 95% CI 0.47 - 5.53, P = 0.441);RDI 90% - 99% (Hazard Ratio = 1.590, 95% CI 0.53 - 4.73, P = 0.405) compared with patients who received an RDI of 100%. Conclusion: This study provides evidence that decreased RDI of <70% in non-metastatic breast cancer patients is strongly associated with decreased overall survival.

Oral and peri-oral signs and symptoms of herbal dentifrices in patients in two oral medicine clinics in Lagos—A preliminary study

Background: Herbal dentifrices appear to have become an attractive alternative for some consumers and its use has gained appreciable acceptance in Nigeria. This could be partly due to the perception that herbal toothpastes like other herbal products are “natural”, devoid of chemicals and therefore superior to regular toothpastes. Aggressive advertisement of the products may also contribute to this new trend. Mucosal symptoms such as glossitis and intolerance to spicy foods seen in patients attending the oral diagnosis clinic are usually associated with anaemia and nutritional deficiency states. The disturbing trend of an increasing number of patients presenting with such oral mucosal symptoms associated with the use of herbal dentifrices prompted this study. This trend was observed in two isolated oral medicine clinics. Objective: To sensitize oral health, allied professionals and consumers on the possible adverse effects of regular use of some herbal dentifrices sold in Nigeria. Materials and Methods: Routine patients of two oral medicine clinics in Lagos, namely the Randle General Hospital and the Lagos University Teaching Hospital who complained of some adverse mucosal signs and symptoms following the regular use of two identified herbal toothpastes made up the study population. The study was conducted between April 2010-April 2011. A detailed history and examination was carried out on these patients and clinical photographs of oral signs observed was taken in consenting patients. Data analysis was done using the Epi-info 6 software. P values ≤ 0.05 was considered statistically significant and data was presented in table format. Results: A total of 45 patients, 9 (20%) males and 36 (80%) fe-males were seen. The age range of patients was 14 - 78 years;mean age 45.5 ± 14.9 years. To characterize extended-spectrum beta-lactamase (ESBL) and extra-intestinal pathogenic Escherichia coli (ExPEC) associated virulence genes in E. coli isolates from chickens and humans in Trinidad and Tobago. This cross sectional study was conducted over a three-month period. A total of 471 E. coli isolates;160 from humans treated at a regional tertiary hospital and 311 from chicken caecal samples from “pluck shops” in Trinidad & Tobago were identified using both conventional and molecular microbiological methods. Phenotypic confirmation of ESBL producing E. coli isolates from humans was by Microscan system (Siemens, USA) while the double disk diffusion method was used for the chicken isolates. Polymerase chain reaction (PCR) analysis was used to determine the ESBL and ExPEC-associated virulence genes in representative human isolates and all chicken isolates. From the 311 chicken E. coli isolates, 49.2% (153/311) produced ESBL, while 56.3% (90/160) from humans were ESBL positive. All human and chicken ESBL isolates were 100% susceptible to carbapenems and aminoglycosides antimicrobials. PCR detected 21.1% blaCTX-M, 13.3% blaTEM and 7.8% blaSHV genes among E coli isolates from humans compared to 0.6% blaCTX-M and 48.6% blaTEM genes in chickens. PCR analysis revealed diverse virulence profiles among the isolates. There was a high occurrence rate of ExPEC-asso- ciated virulence genes in E. coli isolates from both humans and chickens. However, the CTX-M-1 genes were most predominant in humans while TEM occurred in chic- ken isolates. The diverse ESBL and virulence associated gene profiles encountered in E. coli isolates from humans and chickens on the surface depicts no similarity or relationships despite occurrence in both cohort groups. Therefore E. coli strains from chickens and humans require further investigation to determine their clonal relatedness or transmission in the country.

Cadaveric Evaluation of Lumbar Pedicle Morphometry among Nigerians

Pedicle screw is employed in several cases of spine disorders such as fractures, pseudarthrosis, spondylolisthesis, degenerative changes among others. Its essence is to fix the vertebral body in position until fusion is complete. The success of this technique depends on factors like choice of size of screw for a particular pedicle size and shape. Thus, adequate knowledge of the morphometry of lumbar pedicle may avert intraoperative and postoperative complications associated with this technique, especially, neurological impairments. In this study, we determined the vertical and horizontal diameters, interpedicular distance and gender differences of 180 lumbar vertebral pedicles (140 male, 40 female pedicles) using digital vernier calipers. Results revealed a significant increase in pedicle height and width from L2 to L5. Interpedicular distance increased significantly down the vertebrae from L2 to L5. Mean vertical and horizontal diameters, interpedicular distance were not significantly different in both sexes. This study recommends a thorough check of individual’s spinal anatomy in pre-operative assessments.
