Autologous bone marrow derived stem cell therapy in patients with type 2 diabetes mellitus-defining adequate administration methods

AIM To carry out randomized trial for evaluating effects of autologous bone marrow derived stem cell therapy(ABMSCT) through different *** Bone marrow aspirate was taken from the iliac crest of patients. Bone marrow mononuclear cells were separatedand purified using centrifugation. These cells were then infused in a total of 21 patients comprising three groups of 7 patients each. Cells were infused into the superior pancreaticoduodenal artery(Group Ⅰ), splenic artery(Group Ⅱ) and through the peripheral intravenous route(Group Ⅲ). Another group of 7 patients acted as controls and a sham procedure was carried out on them(Group Ⅳ). The cells were labelled with the PET tracer F18-FDG to see their homing and in vivo distribution. Data for clinical outcome was expressed as mean ± SE. All other data was expressed as mean ± SD. Baseline and post treatment data was compared at the end of six months, using paired t-test. Cases and controls data were analyzed using independent t-test. A probability(P) value of < 0.05 was regarded as statistically significant. Measures of clinical outcome were taken as the change or improvement in the following parameters:(1) C-peptide assay;(2) HOMA-IR and HOMA-B;(3) reduction in Insulin dose; subjects who showed reduction of insulin requirement of more than 50% from baseline requirement were regarded as responders; and(4) reduction in HbA 1c. RESULTS All the patients, after being advised for healthy lifestyle changes, were evaluated at periodical intervals and at the end of 6 mo. The changes in body weight, body mass index, waist circumference and percentage of body fat in all groups were not significantly different at the end of this period. Microarray gene expression measurements are reported, used and archived usually to high numerical precision. However, properties of mRNA molecules, such as their low stability and availability in small copy numbers, and the fact that measurements correspond to a population of cells, rather than a single cell, makes high precision meaningless. Recent work shows that reducing measurement precision leads to very little loss of information, right down to binary levels. In this paper we show how properties of binary spaces can be useful in making inferences from microarray data. In particular, we use the Tanimoto similarity metric for binary vectors, which has been used effectively in the Chemoinformatics literature for retrieving chemical compounds with certain functional properties. This measure, when incorporated in a kernel framework, helps recover any information lost by quantization. By implementing a spectral clustering framework, we further show that a second reason for high performance from the Tanimoto metric can be traced back to a hitherto unnoticed systematic variability in array data: Probe level uncertainties are systematically lower for arrays with large numbers of expressed genes. While we offer no molecular level explanation for this systematic variability, that it could be exploited in a suitable similarity metric is a useful observation in itself. We further show preliminary results that working with binary data considerably reduces variability in the results across choice of algorithms in the preprocessing stages of microarray analysis.

Adoption Practice of Climate Change Adaptation Options among Nepalese Rice Farmers: Role of Information and Communication Technologies (ICTs)

This study attempts to understand the role of ICTs on adoption of climate change adaption options among the Nepalese rice farmers, using data from 773 households from seven districts—3 from Terai region and 4 from hilly region. Individual Farmer’s Awareness Index was developed to categorize the respondent knowledge of climate change adaptation and Ordered Logit Model was used to examine the factors influencing their adaptation options in present of ICTs. The result revealed that 65% farmers perceived knowledge about temperature, rainfall and other relative information from various ICT devices that they pose. Farmers received such information mostly from Radio (71%), TV (69%) and mobile phone (62.5%) and argued these three devices are the most prominent, easy access and practical devices to receive such information. 86% farmers used such devices on the daily basis and 90% and more users opined that the information provided from such devices is in their own language and fully understandable. From ICT devices they pose, 71% of the farmers are receiving climate change information and 61% received agro-related information and the majority of them argued that such available information is very much informative and supportive of their resilience to climate change and use of available adaptation options. From the Farmers Awareness Index, this study found 19.8% farmers are high aware, 65.1% medium aware and 15.1% were less aware of the changing climate and its anomalies. Similarly, result from Ordered Logit Model shows that age (0.45***), gender (0.48**), market center (0.32*), bank access (0.54***), availability of subsidy (1.0***), agro-extension services (0.71**), access to TV (0.67***) and membership to a social network (3.20**) played a significant role in increasing farmers’ awareness of climate change which in turn lead to increased adoption of adaptation options available to the farmers. The adoption of the ICH E5 guidance by Japan, US, EU has led to more streamlined development and made important new drugs available more quickly in new regions. Central to this is the concept of a bridging study. A bridging study, while it can have multiple objectives and can take one of several designs, allows one to extrapolate to a new region, safety and efficacy data obtained in a foreign region. Clinical pharmacology principles play a key role in the E5 process and help determine the nature and extent of the bridging study. Thus, a medicinal agent is characterized as less or more sensitive to ethnic factors based on PK/PD characteristics. These include pharmacokinetic linearity or lack thereof, flat or steep dose/effect response curve, single or multiple pathways of metabolism, low potential for drug-drug and drug food interactions, and high or low bioavailability. In addition to PK/PD principles, several other aspects need to be considered. These include medical practice and conduct of clinical trials in different regions. If a medicinal agent is ethnically insensitive based on PK/PD characteristics, and medical practice and clinical conduct are similar between two regions, a bridging study may not be needed. Factors Associated with Mental Depression among Men Who Have Sex with Men in Southern India

Background: Mental health has been a largely neglected issue among Men Who Have Sex with Men (MSM) across the world. This study examines the prevalence and correlates of depression among MSM. Data and Methods: Data for this study are used from a cross-sectional Behavioral Tracking Survey—2012 conducted among 1176 MSM from Andhra Pradesh (undivided), a southern state of India. Depression of MSM was assessed using Patient Health Questionnaire-2 scale. Descriptive statistics, bivariate and multivariate logistic regression techniques were used for analysis. Results: More than one-third of MSMs (35%) in the survey reported to have depression. The likelihood of experiencing depression was 5 times higher among MSM who were mobile for sex work outside their place of residence (55% vs 17%, AOR: 5.2, 95% CI: 3.7 - 7.3) and had experienced physical or sexual violence (82% vs 33%, AOR: 6.0, 95% CI: 2.1 - 17.4) than their respective counterparts. Rates of depression were significantly higher among MSM who had experienced Sexually Transmitted Infections symptoms;knowledge of their HIV positivity;who didn’t use condoms during anal sex with any clients/partners;those who consumed alcohol and were in financial debt at the time of survey than others. Those who were associated with any community groups have significantly less chances of reporting depression. Discussion: The study certainly highlighted that the HIV prevention efforts with MSM in India require an integrated approach on addressing the mental health issues. To support this, programs and research-based evidence will be highly needed to ensure that mental health issues are properly addressed among MSM and other high risk groups.
