Corona Virus Disease-2019(COVID-19)continues to spread rapidly in the *** has dramatically affected daily lives,public health,and the world *** paper presents a segmentation and classification framework of COVID-19 images based on deep ***,the classification process is employed to discriminate between COVID-19,non-COVID,and pneumonia by Convolutional Neural Network(CNN).Then,the segmentation process is applied for COVID-19 and pneumonia CT ***,the resulting segmented images are used to identify the infected region,whether COVID-19 or *** proposed CNN consists of four Convolutional(Conv)layers,four batch normalization layers,and four Rectified Linear Units(ReLUs).The sizes of Conv layer used filters are 8,16,32,and *** maxpooling layers are employed with a stride of 2 and a 2×2 *** classification layer comprises a Fully-Connected(FC)layer and a soft-max activation function used to take the classification decision.A novel saliencybased region detection algorithm and an active contour segmentation strategy are applied to segment COVID-19 and pneumonia CT *** acquired findings substantiate the efficacy of the proposed framework for helping the specialists in automated diagnosis applications.

Early detection of hepatocellular carcinoma co-occurring with hepatitis C virus infection: A mathematical model

AIM: To develop a mathematical model for the early detection of hepatocellular carcinoma (HCC) with a panel of serum proteins in combination with α-fetoprotein (AFP).METHODs: serum levels of interleukin (IL)-8, soluble intercellular adhesion molecule-1 (sICAM-1), soluble tumor necrosis factor receptor II (sTNF-RII), proteasome, and β-catenin were measured in 479 subjects categorized into four groups: (1) HCC concurrent with hepatitis C virus (HCV) infection (n = 192); (2) HCV related liver cirrhosis (LC) (n = 96); (3) Chronic hepatitis C (CHC) (n = 96); and (4) Healthy controls (n = 95). The R package and different modules for binary and multi-class classifiers based on generalized linear models were used to model the data. Predictive power was used to evaluate the performance of the model. Receiver operating characteristic curve analysis over pairs of groups was used to identify the best cutoffs differentiating the different ***: We revealed mathematical models, based on a binary classifier, made up of a unique panel of serum proteins that improved the individual performance of AFP in discriminating HCC patients from patients with chronic liver disease either with or without cirrhosis. We discriminated the HCC group from the cirrhotic liver group using a mathematical model (-11.3 + 7.38 × Prot + 0.00108 × sICAM + 0.2574 × β-catenin + 0.01597 × AFP) with a cutoff of 0.6552, which achieved 98.8% specificity and 89.1% sensitivity. For the discrimination of the HCC group from the CHC group, we used a mathematical model [-10.40 + 1.416 × proteasome + 0.002024 × IL + 0.004096 × sICAM-1 + (4.251 × 10sup>-4sup>) × sTNF + 0.02567 × β-catenin + 0.02442 × AFP] with a cutoff 0.744 and achieved 96.8% specificity and 89.7% sensitivity. Power Dependent of Nonlinear Absorption Coefficient of Cdse-Au Hybrid Nanocomposites

Power dependent study of optical nonlinear absorption coefficient of semiconductor-plasmonic hybrid nanocomposites of Cdse:Au was investigated using Z-scan technique, provided with nanosecond pulses of second harmonic Nd:YAG laser. The prepared materials were characterized with linear absorption and High Resolution Transmission Electron Microscope (HRTEM) which confirm the quantum confinement of Cdse as well as the formation of Cdse:Au core shell quantum dots. An analysis of power dependence open aperture Z-scan was discussed. A change from saturable absorption (sA) to reverse saturable absorption (RsA) is observed with increasing the laser power. also, an enhancement of the nonlinear absorption coefficient of Cdse:Au of larger sizes compared to Au nanoparticles was detected.

UGT1A1*28 Polymorphism in Advanced Colorectal Cancer: The story Is Not Yet Ended

Background: UGT1A1*28 polymorphism is associated with neutropenia and diarrhea in previous reports, while this study tried to investigate correlation with other toxicities like vomiting. Patients and Methods: This is a prospective case control study including all eligible cases of advanced colorectal cancer. The genotypes of UGT1A1*28 was assessed in the peripheral blood and/or in tissues by PCR. Patients were divided into two groups, Group 1: patients with no mutation, Group 2: patients with homo or hetero mutation. all patients received standard IFL regimen. Primary objectives were: 1) comparison between the 2 groups as regard vomiting, 2) assessment of the incidence of UGT1A1*28 polymorphism. secondary objectives were: comparison between the 2 groups as regard: neutropenia, diarrhea, treatment delay, progressive diseases (PD), progression free survival (PFs) and overall survival (Os). Results: 46 cases of advanced colorectal cancer present to National Cancer Institute, Cairo University, aged between 19 and 71 years with a median age of 45 years were included and followed up during the period from september 2010 to January 2013 with a median follow-up of 9 months. UGT1A1*28 polymorphism was present in 20 patients (43%), of whom 15% are homozygous. Grade (II-IV) vomiting was found in 8.3% of Group 1 versus 52.5% of Group 2 (P = 0.01). Grade (II-IV) neutropenia was found in 20.8% of Group 1 versus 64.7% of Group 2 (P = 0.03). Grade (II-IV) diarrhea was found in 37.5% of patients of Group 1 and 27.5% of patients with Group 2. (P = 0.75). Treatment delay occurred in 29.16% of Group 1 versus 72.4% of Group 2 (P = 0.02). 25% of Group 1 showed PD versus 25% of Group 2 (P = 0.8). 1-year PFs was 19% in Group 1 versus 23% in Group 2 (P = 0.8) while there was a trend towards better Os in Group 1 (47% versus 35%) (P = 0.07). sphincter saving surgeries for Locally Advanced Low Rectal Cancer after Neoadjuvant Chemoradiation

Background: Rectal cancer accounts for the largest distribution within one anatomical region of the large bowel, with approximately one third of all CRC located within the rectum. The Golden standard treatment of primary rectal cancer is curative surgical resection;however, a fine balance remains between disease cure and restoration of gastrointestinal continuity. Combined modality has proven efficacy in many malignant tumors with advantage of organ preservation. Methods: Forty nine (49) patients with low rectal carcinoma were included in a prospective study, between Jan 2007 and Jan 2012. Preoperative chemoradiation was administrated to all patients and subjected to different techniques of sphincter saving surgery. stage I and stage IV disease at diagnosis were excluded from the study. Results: Forty nine patients were included in the study. 27 (55%) patients were male and 22 (45%) were female;the age ranges from 23 years to 70 years with the median age 46 years. The main presenting symptoms were bleeding per rectum and tenesmus, stage II 18 patients (36.7%), stage III 31 patients (63.3%). Complete clinical and pathological response in 3 patients (6%), and complete clinical response with only microscopically residual carcinoma in 20 patients (41%), partial response in 18 patients (36.7%), and no significant response in 8 patients (16%) 7 from 8 were mucoid carcinoma. Low anterior resection (LAR) in 22 patients (44.9%), Hartman’s procedure in 4 patients (8.1%), Coloanal pull-through (COP) was done in 19 patients (38.9%) and perineal colostomy in 4 patients (8.1%). For patients with colo-anal pull-through technique complete dehiscent and retraction observed in 2 cases, Major leakage in one case, stenosis in 4 cases. Conclusion: There is tendency of colorectal cancers to affect younger groups. Most patients presented in advanced stage. Neadjuvant chemo radiation is an excellent tool in sphincter saving surgery. In response to a stressful unexpected experience, the brain activates a complex stress system that involves the organism in an adaptive response to the threatening situation. This stress system acts on several peripheral tissues and feeds back to the brain. One of its key players is oxytocin hormone. The neuropeptide, oxytocin (OT), has well-established roles during parturition and lactation. In addition to its peripheral actions, OT is released within multiple areas of the brain and influences behavioural and neuroendocrine responses to stress. several studies suggest that oxytocin is implicated in the central control of responses to stress through modulation of corticotrophin releasing hormone (CRH). Intranasal OT application was associated with an inhibitory effect on adrenocorticotrophic hormone (ACTH) secretion and subsequent impairment of corticosterone secretion. This may be of importance for understanding and perhaps suggesting its utility to buffer stress. synthesis and release of OT depend to a great extent on steroid hormones particularly on estradiol and corticosterone. Estrogens stimulate synthesis and release of OT and increase the number of OT receptors in some areas of the brain. However, the role of OT in mediating stress is variable and may also depend on gender and on external factors.

Protective Effect of Ketamine against Acetic Acid-Induced Ulcerative Colitis in Rats

Objective: Inflammatory bowel diseases (IBD), including Crohn’s disease and ulcerative colitis (UC), are chronic and recurrent disorders of the gastrointestinal tract with unknown etiology. Considering the adverse effects and incomplete efficacy of currently administered drugs, it is crucial to explore new drugs with more desirable therapeutic profiles. As non-competitive N-methyl-D-aspartate (NMDA) receptor antagonists have shown analgesic and anti-inflammatory properties in vitro and in vivo, this study aims to investigate the role of ketamine, a noncompetitive NMDA receptor antagonist, in acetic acid-induced rat colitis. Methods: Ketamine (10, 50 mg/kg), and dexamethasone (1 mg/kg) were given intraperitoneally 30 min before induction of colitis which was done by instillation of 2 mL of 4% acetic acid (vol/vol). At the 4sup>thsup> day of colitis induction, animals were sacrificed and dis.al colons were assessed macroscopically and microscopically. Furthermore, the mucosal contents of lipid peroxidation (LPO), reduced glutathione (GsH), nitric oxide (NO) and tumor necrosis factor-α (TNF-α) were assessed. Results: Ketamine (50 mg/kg) and dexamethasone significantly (p α and elevated GsH levels. Conclusion: Our data suggest that ketamine has valuable protective effects in acetic acid colitis and it may be a new therapy target in ulcerative colitis patients, possibly by regulating antioxidants and inflammatory mediators.

Characteristics of escape mutations from occult hepatitis B virus infected patients with hematological malignancies in south Egypt

AIM To investigate the prevalence and virological characteristics of occult hepatitis B virus(HBV) infections in patients with hematological malignancies in south *** serum samples were collected from 165 patients with hematological malignancies to monitor titers of HBV DNA, hepatitis B surface antigen(HBs Ag), and antibodies to HBV core(anti-HBc) and surface antigens. serum samples negative for HBs Ag and positive for anti-HBc were subjected to nucleic acid extraction and HBV DNA detection by real-time polymerase chain reaction. DNA sequences spanning the s region were analyzed in cases with occult HBV infection. In vitro comparative study of constructed 1.24-fold wild type and s protein mutant HBV genotype D clones was further performed. REsULTs HBV DNA was detected in 23(42.6%) of 54 patients with hematological malignancies who were HBsA g negative, but anti-HBc positive, suggesting the presence of occult HBV infection. The complete HBV genome was retrieved from 6 occult HBV patients, and P120 T and s143 L were detected in 3 and 2 cases, respectively. site directed mutagenesis was done to produce 1.24-fold genotype D clones with amino acid mutations T120 and L143. The in vitro analyses revealed that a lower level of extracellular HBsA g was detected by chemiluminescence enzyme immunoassay(CLEIA) with the clone containing T120 mutation, compared with the wild type or the clone with s143 L mutation despite the similar levels of extracellular and intracellular HBs Ag detected by Western blot. southern blot experiments showed that the levels of intracellular HBV DNA were not different between these clones. CONCLUsION Occult HBV infection is common in patients with hematological malignancies and associated with P120 T and s143 L mutations. 120 T mutation impairs the detection of HBsA g by CLEIA.

The insulin-like growth factor 1（IGF1） gene is a member of the group of somatotropin axis genes that play a significant role in cell proliferation and growth of muscles. Here, we searched for polymorphisms in buffalo IGF1 and found two novel single nucleotide polymorphisms（sNPs）, G64 A and G280A, in the noncoding sequences of exon 1 and exon 4, respectively. statistical analysis of different genotypes showed that the individuals with GG genotypes had significantly（P〈0.05） higher body weight（BW） and average daily gain（ADG） than those with other genotypes at ages of 3–6 months in G64A sNP and 6–9 months in G280A sNP. The combined genotypes of these two sNPs produced three haplotypes, GG/GG, AG/AG, and AA/AA, which were significantly associated（P〈0.0001） with BW and ADG at an age from 3 to 12 months. Buffaloes with the homozygous GG/GG haplotype showed higher growth performance than other buffaloes. The two sNPs were correlated with m RNA levels of IGF1 and IGF1 receptor（IGF1 R） in semitendinosus muscle as well as with the serum concentration level of IGF1. also, buffaloes with GG/GG haplotype showed higher m RNA and serum concentration levels. The data revealed that these two sNPs could be valuable genetic markers for selection of Egyptian buffaloes for better performance in the population.
