Epilepsy is a chronic and the fourth most common neurological disorder which affects people of all age groups. Recently research and awareness on epilepsy-related deaths have rapidly grown over the past two decades. Many previous studies are attributed to the guidelines that apprise health care professionals in handling these deaths, but there is a relative scarcity of information accessible for clinicians and pharmacists who are responsible for manufacturing or preparing the extemporaneous anti-epileptic suspensions in the hospitals. Mostly in partial seizures, phenytoin is one of the first-choice drugs. In Saudi Arabian hospitals, the extemporaneous preparation of phenytoin suspension is common, but the hot climatic weather in Saudi Arabia possesses stability problems that should be tackled as the prepared suspension should pass all the stability tests to ensure uniform dosage of the extemporaneous formulation. In the current study, the commercial capsules were used to prepare the oral phenytoin sodium extemporaneous suspension. The physical, chemical and microbiological stability of phenytoin sodium suspension is analyzed at various temperatures.

Trends in the Annual Incidence of Carbapenem Resistant among Gram Negative Bacilli in a Large Teaching Hospital in makah City, Saudi Arabia

Objective: To detect the prevalence of carbapenem resistance among Gram negative bacilli at the Al-Noor Specialist Hospital in the western part of Saudi Arabia. materials and methods: This study was conducted in the microbiology department, Al-Noor Specialist Hospital, makah city, Saudi Arabia. The clinical samples were collected from admitted patients in the wards, Intensive Care Units (ICUs), urology unit and surgery unit, these samples included blood, urine, sputum, pus and wound. The identification and antibiotic susceptibility testing of all bacterial isolates were performed via the Vitek 2 Compact System. Results: Out of 4803 Gram negative bacterial bacilli were isolated, the prevalence rate of carbapenem resistance among these isolates was 2797/4803 (58.23%), and sensitivity rate was 2006/4803 (41.77%) of the total isolates. The rate of resistance to carbapenem was among Acinetobacter baumannii 1710 (99.13%), followed by Pseudomonas aeruginosa 575 (62.4%), Klebsiella pneumoniae 459 (38%) and Escherichia coli 56 (5.59%). The trend of carbapenem resistance among Gram negative bacterial isolates from 2013-2015 was 53.99%, 59.88% and 61.43% respectively. Conclusion: A. baumannii rec-orded, the highest rate of carbapenem resistance through three year, 98.19%, 99.48% and 99.81% respectively and the lowest rate of resistance recorded with E. coli, 4.63%, 5.79% and 6.31% respectively.

The main role of this article is to describe dental assistants and the dental department’s role in the dental delivery system for Special health care needs (SCHN) by creating a structure to educate the dental assisting profession and project factors that have a significant impact on the dental assistant. The dental assisting management of SCHN patient’s profession including dental Assistance general responsibilities, regulation credentialing, and education. The SCHN patient’s management creates a challenge in all area of the dental department. Handling patient with a SCHN is underexplored throughout the dental staff especially when there is not enough theoretical foundation, education and training to deal with this category of patients. SCHN patients are defined as individuals who have abnormal behavioral impairments, mental conditions or/and chronic disease or abnormal laboratory results. Thus, the aim of this article is to guide all dental assistance staff in the best dental management choice for SCHN patients. All health care teams are part of patient care in most medical settings. A work environment supportive ensures positive outcomes for patient care. This article was considered as those categories of patients required more care and special way to deal with, moreover language, age, gender and education level of the patient are also considered significant parries. Not many studies were found to support the care of SCHN patients in dentistry department. Taking experience from other area help to design a system to handle the SCHN even if it requires hiring a Dentist who is specialized in that filed.

Plank quantum and classical string energy relations seem to be uncorrelated. This work correlated them. The relativistic energy-momentum relation has been used together with plank and de Brogglie hypothesis to prove that the wave group velocity is equal to the particle velocity in both ordinary and curved space. The plank energy relation is shown also to be related to the classical energy relation of an oscillating string. Starting from plank energy relation for n photons and performing integration, the expression of classical string energy was obtained. This means that one can treat electromagnetic waves as a collection of continuous photons having frequencies ranging from zero to w. Conversely, starting from classical string energy relation by differentiating it with respect to angular frequency, the plank quantum energy for n photons has been found. This means that the quanta results from separation of electromagnetic waves to single isolated waves. Each wave consists of n photons or quanta.

Impact of Presence or Absence of Trehalose during Vitrification on Viability and Development of Vitrified/Warmed Immature Dromedary Camel Oocytes

Vitrification of immature oocytes at the germinal vesicle (GV) stage is important to preserve female gametes. The standard formula for vitrification solutions has long been a debate. Herein, we investigated the effect of the presence or absence of trehalose in vitrification solution on viability, in vitro maturation (IVm) rates, and development of vitrified/warmed immature dromedary camel oocytes. Cumulus oocyte complexes (COCs) obtained at slaughter from the ovaries of mature she-camels were randomly allocated into three groups;namely, control group, oocytes were directly subjected to IVm without vitrification, vitrification solution 1 (VS1) group, oocytes were vitrified in a solution composed of 25% ethylene glycol (EG) plus 25% dimethyl sulfoxide (DmSO) + 0.5 m trehalose;and vitrification solution 2 (VS2) group, oocytes were vitrified in a solution composed of 25% EG plus 25% DmSO. Vitrification of COCs was conducted by open pulled straws (OPS) method. Following vitrification and warming, morphologically viable oocytes were matured in vitro for 36 h. COCs were then fertilized and cultured in vitro for 7 days. The percentage of viable oocytes was significantly higher (P 0.05) in VS2 than VS1 group (80.0% vs. 63.3%, respectively). Nuclear maturation, cleavage (48 h post-insemination;pi), and blastocyst rates (7 days pi) were significantly higher (P maturation and development rates between VS2 and control groups. In conclusion, vitrification of immature dromedary camel oocytes in trehalose-free solution (VS2) was more advantageous than that in trehalose supplemented media since it did not reduce viability and development.

Genetic Analysis of Familial mediterranean Fever among Egyptian Patients

Background: Familial mediterranean fever (FmF) is an autoinflammatory genetic disorder that associated with different genetic mutations. Frequency of clinical manifestation differs according to age group, geographic region and ethnic population. Objectives: To study the clinical manifestation of FmF in relation to genotype (m680I, m694V, m694I and V726A). Result: The main presentation of studied group was abdominal pain 65.9% (203), followed by fever 60.4% (186) patients. (mutation m694V) was the commonest 47.6% (297), followed by (mutation V726A) in 32.8% (169%), then (mutation m6802) in 23.4% (121) lastly (mutation m6941) was in 22.1% (114) patients. Fever was highly associated with mutation (V729A) and it was statistically significant (*p value 0.047). Conclusion: Abdominal pain and fever were the most common manifestation of FmF patients. (mutation m694V), (mutation V726A) were the most detected mutation. Third age group;fever was associated with genetic mutation (V726A), abdominal pain with (m6941).

表面活性剂-聚合物（SP）复合驱是一种重要的化学驱提高采收率技术,特别适用于不宜碱驱的油藏。在SP复合体系中,表面活性剂能降低油水界面张力并改变岩石润湿性,而聚合物可降低水油流度比。由于溶液中表面活性剂与聚合物存在相互作用,表面活性剂-聚合物混合物溶液的性质与含有单个组分的溶液性质有所不同。本研究测定了30oC及90oC下含有不同浓度的两性表面活性剂及磺化聚合物的SP复合体系与沙特原油的界面张力（IFT）。在30oC下,虽然加入聚合物的体系的IFT高于不含聚合物的体系,但大部分仍处于10-3 mN/m的数量级。在90oC下,随着溶液中表面活性剂与聚合物浓度的改变,聚合物的添加对IFT的影响显示出正负差异。结果表明,在特定的浓度组合之下,添加聚合物有利于降低IFT。IFT等高线图阐明了IFT值的分布,相比30oC而言,90oC下超低IFT（10-3 mN/m）区域的面积更小。此外,当温度从30oC升高至90oC后,超低IFT区域的位置也有所改变。本研究为盐水中表面活性剂-聚合物相互作用提供了见解。

maximal androgen blockade versus castration alone in patients with metastatic prostate cancer

maximum androgen blockade （mAB）, consisting of an antiandrogen plus either a luteinizing hormone- releasing hormone agonist （LHRHA） or orchiectomy, is a standard care for patients with prostate cancer. Although, clinical trial results have been equivocal, none has shown a significant advantage in favor of mAB over castration alone in metastatic prostate cancer and mAB has been the subject of considerable controversy. The aim of this study was to compare mAB （orchiectomy or LHRHA＂Goserelin＂） and anti-androgen ＂Bicalutamide＂ with castration alone （orchiectomy or LHRHA） in previ- ously untreated metastatic prostate cancer patients. methods： Hundred eligible patients with adequate performance status and adequate hematologic, hepatic and renal functions were included. mAB arm, fifty patients underwent castration either surgically by orchiectomy or medically by receiving Goserelin （3.6 rag） depot, which was injected subcutaneously every 28 days plus bicalutamide 50 mg once daily. Castration alone arm, fifty patients underwent castration alone either surgically by orchiectomy or medically by receiving Goserelin （3.6 mg） depot. Results： During the period from January 2011 to January 2013, with a median follow up of 18 months （range 6 to 24 months）, there were eight deaths （16%）, in mAB arm and ten deaths （20%） in castration alone arm. At three months, there were 35 patients （70%） with prostate specific antigen （PSA） normalization （-〈 4 mg/dL） in mAB arm versus 17 patients （34%） with PSA normalization in castration alone arm （P = 0.001）. The median progression free survival （PFS） times were 22.18 months （95% CI, 19.7 to 24.2 months） for mAB arm versus 22 months in castration alone arm （95% CI, 18 to 25.9 months; P = 0.045）. The survival rates for mAB arm were 82% at 18 months and 70.6% at 24 months versus 78.7% at 18 months and 75.1% at 24 months in castration alone arm （P 〉 0.05）.
