Background Surgical site infections （SSI’s） are still form a large health problem even in thedeveloped countries which are the2ndmost common hospitals acquired infections.（SSI’s）are so important because they contribute substantially to patient morbidity, mortality,prolonged hospitalization and prolonged therapy.（SSI’s） are also widely present in publichospitals of Hodeidah city especially in surgical wards reported seldomly. Objectives The objective of this study is to explore （SSI’s） from clinical features （signs＆symptoms） of the wound and associated factors in public hospitals of Hudeidah city, Yemenfor evidence to clinical decision. Methods The study have been conducted using cross-sectional study and the sample comprisedof600subjects among which300was patients and the other300was nurses. Structuredquestionnaire was used to collect data from patients as well as nurses in two publichospitals in Hodeidah city during the period of the1stof July,2012through the30thofSeptember,2012and the questionnaire was prepared in Arabic language. SPSS versions12have been used to analyze the data and univariate and multiplelogistic regressions was used to reflect the subjects’demographic characteristics and factorsassociated with surgical site infections. Results The results in this study revealed that102（34%） of the subjects suffered frominfection, and the rest198（66%） not suffered from infection. And in the other sidecomparison of gender on the （SSI’s） showed that among114（38%） males patients27（26.47%） suffered from （SSI’s） and87（43.93%） not suffered from （SSI’s）. And among186（62%） females patients75（73.53%） suffered from SSI and111（56.07%） not suffered from（SSI’s）. Surgery for Aneurysmal Right Coronary Fistula and Constrictive Pericarditis in an Adult: Case Report

Coronary artery fistula (CAF) is a direct communication between a coronary artery and the lumen of any of the cardiac chambers, i.e. the coronary sinus, the pulmonary artery, the superior vena cava or the proximal pulmonary veins. The majority of these fistulas are congenital in origin although they may occasionally be detected after cardiac surgery. Congenital CAF is a rare anomaly and aneurysmal formation in the fistula is even rarer. Majority of CAFs are isolated lesions, however, congenital or acquired heart diseases may co-exist. Herein, we report a case of huge congenital aneurysmal right CAF connected to the right atrium in an Iraqi man of 62 associated with tuberculous effusive-constrictive pericarditis to whom off pump pericardiectomy was performed followed by ligation of right coronary artery and vein graft implantation to its posterior descending branch under cardiopulmonary bypass. To the best of our knowledge, such association was not previously reported. CAF can be repaired surgically with minimum risk and excellent outcome. Surgery is advised whenever coronary fistula is diagnosed unless it is very small to avoid the potential complications.

Investigating Improved Oil Recovery in Heavy Oil Reservoirs

Primary production mechanisms do not recover an appreciable fraction of the hydrocarbon initially in place (HIIP). Practical knowledge has shown that, at the point when the natural energy in a heavy oil reservoir is nearly or altogether depleted, the recovery factor does not exceed about 20%. Some heavy oil reservoirs do not produce at all by natural drive mechanisms. This often necessitates adopting a production improvement strategy to augment recovery. Prior to implementing an improved oil recovery method (either secondary or tertiary) in the field, it is very important to investigate its potential for success. Reservoir simulation is a part of a continuous learning process used to gain insight into the feasibility and applicability of improved oil recovery methods. In this project, GEM compositional reservoir simulator has been used to study the efficiencies of different improved oil recovery strategies, ranging from waterflooding to solvent injection. The drainage volume investigated is a hypothetical box-shaped heavy oil reservoir composed of three distinct permeable layers.

Evaluation of Low Saline “Smart Water” Enhanced Oil Recovery in Light Oil Reservoirs

With the depletion of natural driving forces responsible for pushing the oil from reservoirs & declination of oil recovery after secondary stage, the emphasis is now on EOR techniques. The low saline flooding is a type of EOR which gains the attention of researchers due to its easiness to use implications, less cost & environment-friendly nature. Though the low salinity effect has been seen in various labscale core flooding experiments as well as field pilot projects, the mechanism which actually leads to this enhancement in recovery is still the area of research among researchers which is wide open and needs to gain consensus. Seeing the wide mechanisms taking place under different scenarios, it is certain that more than one mechanism is actually supplementing each other in reducing the residual oil saturation while LSW flooding. This study has been undertaken investigations on the low saline flooding in unconsolidated Ottawa sandpack cores with two different, Weyburn & Pelican crude oil, to find out the optimum salinity, LSW Slug Size & underlying mechanisms during LSW flooding. Several core flooding experiments were performed under secondary as well as tertiary recovery stage by unsteady state method. With the reduction of brine salinity from 5000 PPM to 1500 PPM, the oil recovery increased in secondary stage & decreased further upon reduction in salinity to 500 PPM gaining the peak at 1500 PPM. Small enhancement in tertiary recovery of 2.24% observed upon switching to 1500 NaCl PPM brine after injection of formation brine in secondary stage for Weyburn Oil, though large tertiary recoveries of the order of 9.95% for effective oil viscosity of 4 cP and 7.32% for 29.7 cP were observed for n-dodecane diluted Pelican Oil. LSW slug size of 25% pore volume was found to be effective in producing Weyburn Oil in secondary stage.

Stylomastoid Foramen Osteoma: Unique Challenges for Appropriate Management

Osteoma of the temporal bone is a rare and slow-growing benign tumor. It is reported to affect almost all portions of the temporal bone. However, osteoma involving the stylomastoid foramen has never been reported in the literature. We report a case of an osteoma extending from the stylomastoid foramen and occluding the external auditory canal in a young female. Although the osteoma has very close relationship with the facial nerve, it was removed because of the severity of its associated symptomatology;the osteoma was completely removed without any postoperative complications. This report is of interest as it highlights the challenges in the management of osteomas in such localization.

Effects of Using Different Percentages of Fenugreek Flour to Improve the Sensory, Rheological Properties and Keeping Quality in Maize Dough to Produces Gluten-free Breads

Adding different percentages 1%, 3% and 5% of fenugreek flour depending on maize flour base to gluten free bread recipe for making tanoor bread was used to improve the tanoor bread quality, in term of sensory properties like volume, crust, color, symmetry, bake uniformity, texture, grain, aroma and taste. Also some rheological properties like gelatinization temperature and maximum viscosity of dough with bread keeping quality were used. The aim of this work is to produce suitable bread with maintaining the bread quality by selecting the suitable percentage to produce gluten free bread for people who suffering from celiac diseases and its contribution to health benefits. Significant differences were observed by using 5% of fenugreek flour in term of gelatinization temperature, maximum viscosity, breed keeping quality and volume. While using 1% of fenugreek flour significantly improved symmetry value, bread texture, crumb color, aroma and taste.

Outcome of Haemodialysis Arteriovenous Fistula in Baghdad, Iraq

Background: Vascular access (VA) is the life line for end stage renal disease (ESRD). Though there are many methods of VA, native arteriovenous fistula (AVF) is the oldest and the best. AVFs are prone to develop some complications. The aim of this combined prospective and retrospective clinical study was to assess the outcome of native haemodialysis AVF in Baghdad Medical City Teaching Hospital, Baghdad, Iraq. Materials and Methods: Sixty-seven patients (43 males and 24 females) with ESRD were studied over 6 months, from April 1 to September 30, 2013. Demographic and clinical features as well as co-morbidities were checked. Allen’s test and examination of upper limb superficial veins were performed. Radio-cephalic or brachio-cephalic AVFs were created mostly under local anaesthesia using artery-side to vein-end anastamoses. Complications were noted during a follow-up period of 2 weeks to 6 months. Results: The mean age was 51.2 ± 14.4 years. Fistulas (n = 81) were mostly brachio-cephalic (n = 74, 91.4%). One fifth of patients were diabetics and 58.2% were hypertensive. End of the vein to side of the artery was used in 92.5%. All fistulae functioned primarily. Significant complications were thrombosis (n = 18, 22.2%), aneurysms (n = 3, 4.5%) and steal syndrome (n = 3, 13.6%). Distal oedema, venous congestion, wound infection and seroma were managed conservatively. Three surgical revisions were required, one for a large aneurysmal dilation (aneurysmectomy and vessel ligation) and two for an evacuation of seroma. Conclusion: AVF initial success was good. Late complications such as aneurysms and steal syndrome were almost within the reported rates whilst thrombosis was high.

Awareness and perception of assisted reproductive technology practice amongst women with infertility in Northern Nigeria

Background: Involuntary infertility is a tragedy in most African setting. This is due to premium placed on fertility as a result of roles children fulfill in the family and the society. Aetiologic factors of infertility in sub-Saharan Africa are mostly infection related and they are mainly associated with poor treatment outcome to conventional non assisted conception technique. Objective: To evaluate the level of awareness and perception of assisted conception treatment among women attending fertility clinic. Methods: A descriptive cross-sectional study. Results: One hundred and ninety six women attending fertility clinic were interviewed. Mean age was 34.8year and mean duration of infertility of 4.1 years. Of the 196 infertile women interviewed, 150 (76.5%) have heard of Assisted Reproductive Technology treatment. Sources of information were mainly family relation (46%) and friends (28.7%). Knowledge on some of Assisted Reproductive Technology practices showed that 50.7% were aware that the treatment could fail, 36.8% knew it could be applied for male infertility treatment, 9.3% and 18.7% respectively are aware that donor oocyte and sperm could be used for treatment. Perception on babies conceived from assisted conception treatment revealed that 52% of patients interviewed could not comment if they are normal and natural babies. Majority of patients could not affirm if they will agree to the use of donor gamete or zygote for their treatment. Conclusion: Awareness of assisted conception treatment was high, however knowledge on specifics of treatment was low and perception on some of the practices was unfavorable. Sensitization of the public will help overcome some beliefs that may be at tangent to some practices of assisted conception.

Conversion Rate of Laparoscopic Cholecystectomy to Open Surgery at Al Karamah Teaching Hospital, Iraq

Background: Laparoscopic cholecystectomy has become the standard treatment for symptomatic gall bladder disease. However, there still a substantial proportion of patients in whom Laparoscopic cholecystectomy cannot be successfully performed, and for whom conversion to open surgery is required. Method: In this study, 1600 laparoscopic cholecystectomy performed at Al karamah teaching hospital from January 2010 to January 2015, were prospectively analyzed. The patients studied included 1600, 1245 where females (78.4%) and 346 where males (21.6%) with a mean age of 41.2 years. From the data collected, only factors available to surgeon preoperatively were considered for analysis. These factors included: age, gender, history of acute Cholecystitis, jaundice, previous abdominal surgery, obesity and concomitant disease, ultrasound preoperatively & ERCP. Results: Of the 1600 patients in whom Laparoscopic cholecystectomy was attempted 56 patients (3.5%) required conversion to open surgery. The most common reason for conversion was difficult to define anatomy in patients with inflamed, contracted gall bladder (n = 42). Significant predictor factors for conversion were male gender, previous abdominal surgery, acute Cholecystitis, and obesity. Conclusion: An appreciation for these predictors for conversion will allow appropriate planning by the patient, the institution and the surgeon.

Efficiency of Tamsulosin after Shockwave Lithotripsy for the Treatment of Lower Ureteric Stone: Alkaramah Teaching Hospital Experience, Iraq

Background: Stone disease is a wide health problem;certain drugs have been used as supplement with ESWL for the treatment of symptomatic uncomplicated lower ureteric stone like Nefidipine, Alfuzosin and Tamsulosin. The purpose of this study is to evaluate the efficacy of Tamsulosin for the treatment of lower ureteric stone of 5 - 10 mm diameter after ESWL. Methods: A total of 105 patients with lower ureteric stone of less than 10 mm diameters were divided into two groups. Group 1 including 48 patients received 0.4 mg Tamsulosin and diclofenac sodium analgesia as needed immediately after ESWL and continued until stone expulsion was confirmed up to maximum of 30 days. Group 2 including 47 patients was given diclofenac sodium only as needed after ESWL. Result: 46 of 49 patients in group 1 and 44 of 48 patients in group 2 ultimately passed stones. The number of ESWL sessions was 1.4 ± 0.6 in group 1 & 1.42 ± 0.75 in group 2. There were 10 patients who needed analgesia after ESWL in group 1 and 17 patients in group 2. Visual analogue scale pain severity score was 5.32 ± 1.23 and 6.41 ± 1.26 in group 1 and 2, respectively. The time to stone expulsion in group 1 and 2 was 8 ± 5.4 days and 12 ± 10.3 days, respectively. No significant complications or side effects occurred. Conclusion: The use of Tamsulosin in combination with ESWL in patients with lower ureteric stone of 5 - 10 mm was effective for reducing the time to stone expulsion, decreasing need for analgesia, and facilitating stone passage without significant improvement in stone free rate.
