Prevalence and clinical features of colonic diverticulosis in a middle Eastern population

AIm: To determine the prevalence, location, associations and clinical features of colonic-diverticulosis and its role as a cause of lower-gastroenterology-bleeding. mETHODS: We retrospectively reviewed the medical records of 3649 consecutive patients who under-went a colonoscopy for all indications between 2007 and 2011 at King Khalid University Hospital, Riyadh, Saudi Arabia. The demographic data were collected retrospectively through the hospital's information system, electronic file system, endoscopic e-reports, and manual review of the files by two research assistants. The demographic information included the age, sex, comorbidities and indication for the colonoscopy. The association among colonic polyps, comorbidities and diverticular disease was also ***: A total of 270 patients out of 3649 were diagnosed with colonic diverticulosis, with a prevalence of 7.4%. The mean age was 60.82 years ± 0.833, (range 12-110). Females comprised 38.89% (95%CI: 33-44.7) of the study population. The major symp-toms were rectal bleeding in 33.6%, abdominal pain in 19.3%, constipation in 12.8% and anemia in 6%. Diverticula were predominantly left-sided (sigmoid and descending colon) in 62%, right-sided in 13% and in multiple locations in 25%. There was an association between the presence of diverticulosis and adenomatous polyps (Pvalue mellitus (P-value most common cause of lower gastrointestinal bleeding, in 33.6% (95%CI: 27.7-39.4), after internal hemor-rhoids, in 44.6% (95%CI: 40.3-48.9). On multivariable logistic regression, hypertension (OR = 2.30; 95%CI: 1.29-4.10), rectal bleeding (OR = 2.57; 95%CI: 1.50-4.38), and per year increment in age (OR = 1.05; 95%CI: 1.03-1.07) were associated with diverticulosis but not with bleeding diverticular disease. AIm: To analyze experiences to identify treatment outcomes and prognostic factors in a Saudi ***: medical records of patients with brainstem gliomas treated from July 2001 to December 2012 were reviewed to identify treatment outcomes of surgery, radiation therapy and chemotherapy and associated prognostic factors in a Saudi ***: We analyzed 49 brain stem glioma(BSG) patients from July 2001 to December 2012; 31 of them were males(63.3%) with a median age of 12.6 years(range: 8-64 mo). Twenty-two patients(44.9%) had diffuse intrinsic pontine gliomas(DIPG) and 15(30.6%) presented with focal/tectal BSG. Histopathology was available in 30 patients(61.2%). median survival time for the whole cohort was 1.5 years. One and two year OS rates were 51.1% and 41.9% respectively. Two year OS rates for focal/tectal, dorsally exophytic, cervicomedullary and DIPG tumors were 60%, 33.3%, 33.3% and 13.6% respectively(P 18 years) P = 0.01, KPS mptoms(maximal resection) P = 0.002, and concurrent chemotherapy with radiation therapy in DIPG(better if given) P = ***: BSG, especially the DIPG subgroup, had a dismal prognosis, needing more aggressive neurosurgical, radiation and chemotherapy techniques, while focal and tectal tumors were found to have a better prognosis.

Fasting in Ramadan Affects Cognitive and Physiological Function in Normal Subjects (Pilot Study)

Purpose: Our study aimed to investigate the effects of Ramadan fasting on cognitive and physiological function in a group of healthy muslim subjects. methods: We measured demographic, physiological (blood pressure), and cognitive function including attention switching task (AST) and delayed matching to sample (DmS) using the Cambridge Neuropsychological Test Automated Battery (CANTAB) at two periods of the study (before Ramadan, at the second week in Ramadan) in 15 subjects. Results: During fasting, performance on the AST test improved significantly for switching cost (p = 0.030) and for congruent condition of AST task (p = 0.043), for diastolic pressure decreased (p = 0.069) in healthy subjects. There was no significant effect for incongruent condition and number of errors and percent to complete task for DmS test and systolic blood pressure. Conclusions: The results suggest that Ramadan fasting is associated with significant changes in cognition, and causes a drop in diastolic blood pressure in healthy subject.

Recurrent Pregnancy Loss in Patients with Polycystic Ovary Syndrome: A Case Control Study

Objectives: To identify different biophysical and hormonal factors that could contribute to recurrent pregnancy loss in Saudi women who have been diagnosed with polycystic ovary syndrome. Study Design: Case-control study. Settings: Infertility clinic in Dammam maternity and Children Hospital. Cases: PCOS patients with recurrent pregnancy loss. Controls: PCOS patients without recurrent pregnancy loss. Results: The prevalence of recurrent abortion in PCOS patients was 31%. There was no significant difference between the cases and the controls in the age, 33.1 years versus 31.9 years, BmI, 31.4 kg/m2 versus 31.6 kg/m2 respectively. There was no significant difference in the successful pregnancies outcome in cases and controls, term deliveries 84% versus 90%, preterm deliveries 10% versus 5%, twin pregnancy 6% versus 5% respectively. There was no significant difference in the hormonal profile (prolactin, LH, FSH, LH/FSH ratio, TSH, progesterone, testosterone) between cases and controls. Serum estradiol level was significantly lower in the cases compared to the controls, 55.5 pg/mL versus 83.9 pg/mL respectively (p Conclusion: PCOS patients with recurrent abortions had no significant increase in the age, BmI, preterm pregnancy, multiple gestation and ectopic pregnancy. The hormonal profile (prolactin, LH, FSH, LH/FSH ratio, TSH, progesterone, testosterone) was comparable between the cases and the controls, except for serum estradiol level, which was significantly lower in PCOS patients with recurrent abortions.

Efficacy of intragastric balloon on weight reduction:Saudi perspective

AIm To evaluate the safety and efficacy of intragastric balloon(IGB) in weight reduction in obese patients referred to a tertiary hospital in the Kingdom of Saudi *** Three hundred and one consecutive obese individuals, who underwent IGB placement during January 2009 to may 2015, were analyzed. The subjects aged 18 to 60 years and had a minimum body mass index(BmI) of 27 kg/m^2. The IGB was placed under conscious sedation and kept for 6 mo. Anthropometric measurements were recorded during and after 6 mo of IGB *** The body weight, excess body weight, and BmI were significantly reduced at the time of IGB removal and 6 mo later. Body weight loss > 10% was achieved in 224 subjects at removal of IGB. End of treatment success and long-term success were both significantly observed in women(70 vs 11)(71 vs 12.5) respectively. Excess BmI loss was significantly higher in subjects retaining the IGB for over 6 mo both at the removal [43.44 ± 19.46(n = 221) vs 55.60 ± 28.69(n = 80); t = 4.19, P = 0.0001] as well as at the end of 6 mo' follow-up [46.57 ± 24.89(n = 221) vs 63.52 ± 31.08(n = 80); t = 4.87, P = 0.0001]. Within 3 d of IGB placement, two subjects developed pancreatitis and one subject developed cardiac arrhythmia. Intestinal obstruction due to displacement of IGB occurred in two subjects. Allthese subjects recovered uneventfully after immediate removal of the IGB. CONCLUSION IGB was effective in our cohorts. The observed weight reduction was maintained for at least 6 mo post IGB removal. IGB placement was safe with a satisfactory tolerance rate.

Availability, Performance and Reliability Evaluation for PV Distributed Generation

Nowadays, renewable energy resources play an important role in replacing conventional energy resources such as fossil fuel by integrating solar, wind and geothermal energy. Photovoltaic energy is one?of?the very promising renewable energy resources which grew rapidly in the past few years, it can be used to produce electric energy through photovoltaic process. The primary objective of the research proposed in this paper is to facilitate the increasing penetration levels of PV systems in the electric distribution networks. In this work, the PV module electrical model is presented based on the mathematical equations and was implemented on mATLAB to simulate the non-linear characteristics I-V and P-V curves with variable input parameters which are irradiance and temperature based on Riyadh region. In addition, the reliability evaluation of distribution networks, including distributed generators of solar photovoltaic (PV) with varying output power capacity is presented also. The monte Carlo simulation algorithm is applied to test the distribution network which is RBTS Bus 2 and the same has been conducted on the original case of distribution network substation 7029 which is located at KSA Riyadh. The two distribution networks have been modified to include the PV’s distributed generators. The distributed generators contribute to supply a part of the load during normal mode and supply the entire load during component failure or failure of grid operation supply. The PV stochastic models have been used to simulate the randomness of these resources. moreover, the study shows that the implementation and integration of renewable resources as distributed generations have improved the reliability of the distribution networks.

Robots in Cancer Surgery: A Boon or Bane

It is an ongoing task to keep exploring and applying the best available technology to alleviate the pain and sufferings of the cancer patients. Since the discovery of robotic surgery, da Vinci surgical systems have played a special and significant role in cancer surgeries worldwide, however, surgeons are still skeptical with the clinical and oncological outcomes which are almost comparable to the laparoscopic approach in several cancers. many meta-analyses using mostly retrospective studies indicated significant advantage of robotic surgery over laparoscopic or open surgery approaches for various cancers, however, scarcity of technically sound robot savvy surgeons and quality multicentered, multinational, coordinated, random clinical trials had not done justice to the positives of robotic surgery which were quite often suppressed by the negative factors like operative cost and oncological outcomes. Nevertheless, robotic surgery approach has been clinically accepted for hysterectomy and prostatectomy. This overview briefly discusses the comparative approaches (open, laparoscopic, robotic assisted) and their clinical outcomes in the surgery of various cancers.

An Improved Convolutional Neural Network model for DNA Classification

Recently,deep learning(DL)became one of the essential tools in bioinformatics.A modified convolutional neural network(CNN)is employed in this paper for building an integratedmodel for deoxyribonucleic acid(DNA)*** any CNN model,convolutional layers are used to extract features followed by max-pooling layers to reduce the dimensionality of features.A novel method based on downsampling and CNNs is introduced for feature *** downsampling is an improved form of the existing pooling layer to obtain better classification *** two-dimensional discrete transform(2D DT)and two-dimensional random projection(2D RP)methods are applied for *** convert the high-dimensional data to low-dimensional data and transform the data to the most significant feature ***,there are parameters which directly affect how a CNN model is *** this paper,some issues concerned with the training of CNNs have been *** CNNs are examined by changing some hyperparameters such as the learning rate,size of minibatch,and the number of *** and assessment of the performance of CNNs are carried out on 16S rRNA bacterial *** results indicate that the utilization of a CNN based on wavelet subsampling yields the best trade-off between processing time and accuracy with a learning rate equal to 0.0001,a size of minibatch equal to 64,and a number of epochs equal to 20.
