Fatal complication, rescue therapy;covered stent for coronary artery perforation

Introduction: Although coronary perforation is a rare complication observed during intervenetional procedures, it has a considerably high mortality rate. The prevelance of coronary perforation has been reported to be 0.20%-0.6%. Its sudden development, the patient’s agitation and development of rapid collapse render intervention difficult. Materials and Method: The presence of perfusion balloon and covered stent in clinics is life-saving. In the present study, we retrospectively reviewed 17 cases with coronary artery perforation that were treated between 2009 and 2012. Of these patients, 10 (58.8%) were men and 7 (41.2%) were women;the median age was 62.8 ± 8.3 years. The coronary artery perforation resulted from guide wire in 23.5%, balloon dilatation in 58.8% and stent implantation in 17.6%. All the lesions were either type B or C lesions. Results: The extensiveness of perforation was Ellis grade I in 23.5%, grade II in 47.1% and grade III in 29.4% of the cases. In the treatment of the perforation, polytetrafluoroethylene-covered stent graft was implemented in 9 (52.9%) patients, whilst conventional and emergency surgical therapy was performed in 8 (47.1%) patients. Grade I perforations occurred due to the guide wire and were managed with conventional therapy (p < 0.05). Grade II and III perforations resulted from balloon and stent. The majority of these patients were inserted Graft Stent (stent graft in 52.9% and conventional therapy in 23.5% of the cases (p < 0.05). Although all the stent grafts were successfully implanted, the complete control of bleeding was achieved only in 77.7% of the patients. Mortality was not observed in grade I perforation, whilst all cases resulted in mortality had grade III perforation. Conclusion: These data indicate that there is a need for further advanced technology in the coronary artery perforation despite of currently available therapeutic options.

To the editor： Communication between normal coronary arteries （ICC） is a substantially rare anomaly. The reason of the fact that two-directional coronary connection causes coronary ischemia in the absence of obstrtictive coronary artery disease is not clear. This coronary anomaly which is thought to be congenital can be readily visualized on coronary angiography.

Under optimal process conditions,pyrolysis of polyolefins can yield ca.90 wt%of liquid product,i.e.,combination of light oil fraction and heavier *** this work,the experimental findings reported in a selected group of publications concerning the non-catalytic pyrolysis of polyolefins were collected,reviewed,and compared with the ones obtained in a continuously operated bench-scale pyrolysis *** process parameters were used for the pyrolysis of waste and virgin counterparts of high-density polyethylene,low-density polyethylene,polypropylene and a defined mixture of those(i.e.,25:25:50 wt%,respectively).To mitigate temperature drops and enhance heat transfer,an increased feed intake is employed to create a hot melt plastic *** 1.5 g·min^(-1) feed intake,1.1 L·min^(-1) nitrogen flow rate,and a moderate pyrolysis temperature of 450℃,the formation of light hydrocarbons was favored,while wax formation was limited for polypropylene-rich *** of virgin plastics yielded more liquid(maximum 73.3 wt%)than that of waste plastics(maximum 66 wt%).Blending polyethylenes with polypropylene favored the production of liquids and increased the formation of gasoline-range *** products were mainly composed of C3 hydrocarbons,and no hydrogen production was detected due to moderate pyrolysis temperature.

How has the COVID-19 pandemic changed treatment preferences of patients with proximal ureteral stones?

Background:The impact of the coronavirus disease 2019(COVID-19)pandemic on patient decision making remains *** study aimed to investigate the effect of the pandemic on treatment preferences of patients with proximal ureteral stones.Materials and methods:Retrospective data regarding treatment preferences of patients diagnosed with symptomatic proximal ure-teral stones between July 2018 and November 2021 at a single center were *** from 493 patients were analyzed according to 2 groups,including patients diagnosed during the COVID-19 pandemic and those diagnosed during an equivalent period of time be-fore the pandemic.Results:Preference for conservative treatment increased during the COVID-19 pandemic(p=0.009).In patients who had previously undergone shock wave lithotripsy(SWL),the preference for SWL decreased and the preference for conservative treatment increased during the COVID-19 pandemic(p=0.042).Multiple logistic regression analysis revealed a significant correlation between a preference for conservative treatment during the pandemic and no prior spontaneous stone passage(p=0.003;odds ratio[OR],2.48;95％con-fidence interval[CI],1.45-4.23),no hydronephrosis(p=0.035;OR,3.57;95％Cl,1.34-9.49),and a visual analog scale score of 4 or less(p=0.018;OR,1.97;95％Cl,1.15-3.38).Conclusions:A significant increase in the preference for conservative treatment was observed among patients diagnosed during the pandemic,and patients with a history of SWL demonstrated a preference shift from SWL to conservative treatment.

Microbial Degradation of Organic Contaminants in Streambed/Floodplain Sediments in Passaic River—New Jersey Area

This paper is intended to explore soil organic matter and carbon isotope fractionation at three locations of the Passaic River to determine if microbial degradation of organic contaminants in soil is correlated to the surrounding physical environment. Microbial degradation of organic contaminants is important for the detoxification of toxic substances thereby minimizing stagnation in the environment and accumulating in the food chain. Since organic contaminants are not easily dissolved in water, they will penetrate sediment and end up enriching the adjacent soil. The hypothesis that we are testing is microbial activity and carbon isotope fractionation will be greater in preserved soils than urban soils. The reason why this is expected to be the case is the expectation of higher microbial activity in preserved environments due to less exposure to pollutants, better soil structure, higher organic matter content, and more favorable conditions for microbial growth. This is contrasted with urban soils, which are impacted by pollutants and disturbances, potentially inhibiting microbial activity. We wish to collect soil samples adjacent to the Passaic River at a pristine location, Great Swamp Wildlife Refuge, a suburban location, Goffle Brook Park, Hawthorne NJ, and an urban location, Paterson NJ. These soil samples will be weighed for soil organic matter (SOM) and weighed for isotope ratio mass spectrometry (IRMS) to test organic carbon isotopes. High SOM and δ13C depletion activity indicate microbial growth based on the characteristics of the soil horizon rather than the location of the soil sample which results in degradation of organic compounds.

Numerical Prediction of Form Factor and Wave Interference of A Trimaran for Different Outrigger Positions

Two-Dimensional Electrophoretic Analysis of Soluble Leaf Proteins of a Salt-sensitive （Triticum aestivum） and a Salt-tolerant （T. durum） Cultivar in Response to NaCI Stress

In this research, 3-day-old etiolated wheat seedlings of Triticum aestivum L. cv. Ceyhan-99 （salt-sensitive） and T. durum Desf. cv. Firat-93 （salt-tolerant） were grown in control and salt （150 mmol/L NaCl） treatments at a 15/25℃ temperature regime in the light for 12 days. Soluble proteins extracted from the first leaf tissues of two cultivars were analyzed by twodimensional （2-D） electrophoresis in order to detect NaCl-induced changes. The soluble leaf protein profiles of cultivars were observed to be similar. However, quantitative differences in 74 proteins were detected in the salt treatment group, compared to the control. Among the 74 protein spots, 14 were common for two cultivars. As a result of NaCl treatment, two low-molecular-weight （LMW） proteins （28.9 and 30.0 kDa） and one intermediate-molecular-weight （IMW） protein （44.3 kDa） in cv. Ceyhan-99 and six LMW proteins （18.6, 19.4, 25.7, 25.9, 26 and 27.6 kDa） in cv. Firat-93 were newly synthesized. The newly synthesized proteins were specific to each cultivar. In the Firat-93 cultivar, four proteins with LMW （24.8-27.9 kDa） were completely lost in NaCl treatment. Moreover, these four protein spots were not observed in both protein profiles of cv. Ceyhan-99. Most of these proteins were in acidic character （pl 〈6.0-6.9） and low molecular weight （〈31.6 kDa）. It is suggested that the newly synthesized or completely lost LMW proteins may be important for cultivars differing in sensitivity towards NaCl.

Effect of cryogenic cooling in milling process of AISI 304 stainless steel

The effects of cryogenic cooling on cutting forces in the milling process of AISI 304 stainless steel were investigated *** cooling was achieved by spraying liquid nitrogen to tool,chips and material interfaces using a pipe with an internal diameter of 1 mm;the flow rate of liquid nitrogen was 5.2 L/min;two cutting directions（climbing and conventional milling）,two machining conditions（dry and cryogenic cooling） and four cutting speeds（80,120,160 and 200 m/min） were used in the milling *** cooling and cutting speed are found to be effective on cutting *** forces and torque in cryogenic milling are higher than those in dry *** force is increased as the cutting speed is *** fritter around insert nose radius is the main problem of climb milling method in cryogenic cooling at low cutting speeds.
